“HTA is a multidisciplinary process that summarizes information about the medical, social, economic and ethical issues related to the use of a health technology in a systematic, transparent, unbiased, robust manner. Its aim is to inform the formulation of safe, effective, health policies that are patient focused, and seek to achieve best value” (EUnetHTA 2007). Even though the assessment of ethical aspects of a health technology is listed as one of the objectives of a HTA process, in practice, the integration (...) of these dimensions into reports remains limited. El artículo pone en evidencia la dimensión ético-antropológica de la historia de la medicina. En el pasado podemos encontrar aquellos elementos de relevancia ética que están en estrecha continuidad con el presente. Desde los orígenes el médico ha experimentado el conflicto entre el mundo del deseo y el mundo del límite. El cuidado de los enfermos comienza desde ahí, de la consciencia de compartir el mismo deseo, el mismo límite, el mismo destino. El artículo mira la historia de la medicina (...) como historia de la humanidad; dentro está toda la vida humana, según la definición del historiador Fielding Garrison . En esta perspectiva la dimensión ético-antropológica emerge sobre todo en las múltiples figuras de médicos ejemplares caracterizados por valores como el valor, la dedicación, la empatía, pero sobre todo por un gran sentido de humanidad y de solidaridad por sus enfermos. En fin, una lectura ético-social puede emerger del arte y de la literatura. Ellas son no sólo testimonio de una época sino también un sismógrafo de las dimensiones éticas de la medicina.This article highlights the ethical-anthropological level of the history of medicine. It explores the close connection between past and present regarding those elements of ethical relevance in medicine. Since the beginning, the physician experienced the conflict between hopes and limits. Medical care springs exactly from the awareness of sharing the same desire, the same limit, and the same destiny. This article regards the history of medicine as history of mankind; according to the historian Fielding Garrison, the history of medicine embodies the entire human life . In this perspective, the ethical-anthropological dimension emerges particularly in many exemplary figures of physicians, distinguished for courage, commitment, empathy, humanity and solidarity towards their patients. Finally, arts and literature can be regarded as instruments to get a cultural perspective, as well as guidelines for social and ethical key of interpretation. They are not only historical documents, but also a seismograph, registering the fundamental historical dimension of medicine. (shrink)

The provision of standardized hearing aids is now considered to be a crucial part of the UK National Health Service. Yet this is only explicable through reference to the career of a woman who has, until now, been entirely forgotten. Dr Phyllis Margaret Tookey Kerridge was an authoritative figure in a variety of fields: medicine, physiology, otology and the construction of scientific apparatus. The astounding breadth of her professional qualifications allowed her to combine features of these fields and, later in (...) her career, to position herself as a specialist to shape the discipline of audiometry. Rather than framing Kerridge in the classic ‘heroic-woman’ narrative, in this article we draw out the complexities of her career by focusing on her pursuit of standardization of hearing tests. Collaboration afforded her the necessary networks to explore the intricacies of accuracy in the measurement of hearing acuity, but her influence was enhanced by her ownership of Britain's first Western Electric audiometer, which she placed in a specially designed and unique ‘silence room’. The room became the centre of Kerridge's hearing aid clinic that, for the first time, allowed people to access free and impartial advice on hearing aid prescription. In becoming the guardian expert and advocate of the audiometer, Kerridge achieved an objectively quantified approach to hearing loss that eventually made the latter an object of technocratic intervention. Nanomedicine has the potential to transform medical therapy and diagnosis. Its technologies predict improved drug delivery systems with site-specific treatment, precise new surgical techniques that would reduce patient trauma and treatment cause, and even cellular repair that would make age-related conditions such as Alzheimer’s disease a thing of the past. Currently, nanomedicine products are reaching the world market with an annual growth rate of twenty-five percent. However, like any emerging new technology, along with doomsday scenarios of nanoparticles gone amuck, nanomedicine (...) raises serious ethical, moral, and social issues that may potentially limit its technological developments. In this essay, I briefly evaluate some of these issues, and argue that ethical and epistemic issues should be considered prior to research and development. By examining three crucial epistemic challenges in nanomedicine—toxicity, nanodrugs and drug delivery systems, and clinical trials—I argue that by implementing a multi-criterion decision analysis framework as outlined by Linkov, Satterstorm and Corey, these challenges can bridge the knowledge gap between R&D and the introduction of nanomedicine technologies into the market. Editors Fritz Allhoff and Patrick Lin are among the leading philosophers in nanoethics and founders of The Nanoethics Group. While their first anthology, Nanoethics: The Ethical and Social Implications of Nanotechnology, presented a general introduction to critical issues in nanoethics, in this new book Allhoff and Lin recognize nanotechnology’s “strange schizophrenia”—as a brave new science filled with unlimited (...) futuristic vision contrasted with the exaggerated hype of research progress—and strive to foster a solid foundation for nanoethics. In some peripheral parts of Europe, prehistoric menhirs, dolmens, henges, and other megalithic constructions made such strong marks on the terrain that they became integral to the landscape. In the Middle Ages, such monuments were often used as landmarks or boundary markers and, indeed, ended up in coeval descriptions of the territory. In some instances, prehistoric monuments were Christianized to promote the conversion of “pagan” populations; in other cases, they stimulated the creation of stone artifacts - stelae, crosses, standing stones (...) - that might have been used as well as instruments of conversion. The investigation in this article revolves around analysis of the landscape of Sardinia, medieval descriptions of Sardinia’s terrain, and examination of some early medieval stone artifacts there. Parallels found in similar contexts in the British Isles are also considered. (shrink)

During the early twentieth century, otologists began collaborating with organizers of the New York League for the Hard of Hearing to build a bridge to “adjust the economic ratio” of deafness and create new research avenues for alleviating or curing hearing loss. This collegiality not only defined the medical discourse surrounding hearing impairment, anchoring it in hearing tests and hearing aid prescription, but, in so doing, solidified the notion that deafness was a “problem” in dire need of a “solution.” Public (...) health campaigns thus became pivotal for spreading this message on local and national levels. This paper focuses on how, from the 1920s to 1950s, as otologists became more involved with social projects for the deaf and hard of hearing — advocating lip-reading, community work, and welfare programs — at the same time, they also mandated for greater therapeutic regulation, control of hearing aid distribution, and standardization of hearing tests. The seemingly paradoxical nature of their roles continued to reinforce the stigmatization of deafness: with widespread availability of effective help, the hearing impaired were expected to seek out therapeutic or technological measures rather than live with their affliction. Describing how the study of artifacts is greatly enhanced by an understanding of the history of museums, Ken Arnold remarks that there is “an implicit faith in the power of objects to tell, or at least ask, historians things that the written word alone cannot” (1999, p. 145). Rather than remaining mute objects or passive accessories to textual descriptions, artifacts (and the museums that house them) are tangible incarnations of the culture from which they emerged, providing unique information on the (...) attitudes and behaviors of the past. In practice, studying and learning from artifacts can sometimes pose methodological problems, as a text-oriented historian may have no idea of how to “read” an object in order to reveal its secrets of the past. Historians and philosophers are trained almost exclusively to work with written and oral documents, limiting their analysis by neglecting such a valuable group of sources. However, as outlined in a special issue of Studies in the History and Philosophy of Science (2007, vol. 38, no. 2), it is apparent that a new historiographical tide has swept over scholars, encouraging new studies and methodologies for working with artifacts, objects, and images. The introduction to the topic of this issue is an overview of the research articles authored by Russian, Lithuanian, and Indian scholars on various problems of Mahayana Buddhist philosophy. While explaining the status of the terms “Mahāyāna” and “Hīnayāna,” the author emphasizes that since they are represent the apologetic conceptualizations of Mahayanists, the appellation “Hīnayāna” (“Lesser Vehicle”, etc.) is not recognized either by those Buddhists who are supposed to be characterized by it, or by scholars striving for a neutral appellation. (...) This creates difficulties, including the need for a generally accepted designation for this Buddhist tradition. However, despite the apologetic nature of the Mahāyāna - Hīnayāna opposition, the difference between the two is captured very plausibly. The first one teaches about the individual way of achieving liberation from the cycle of endless rebirths (saṃsāra) through a personal, individual self-perfection (the path of the arhat) leading to enlightenment (bodhi) and nirvāṇa (extinction of passions). The second one develops a full-fledged religion with its own pantheon and rituals, in which nirvāṇa and individual liberation, while remaining, more or less theoretically, the ultimate goal, are pushed to a distant plane. At the same time, the idea of compassion and the ideal of bodhisattva who, having taken a vow to help all living beings to get rid of sufferings, continues to remain in saṃsāra, is put forward in the center. However, despite this major difference a Buddhist discipline known as Abhidharma which consists in analysis and classification of discrete states of consciousness (dharmas), identified in meditation, remains a reference point in both traditions. Three authors touch upon it (Helen Ostrovskaya, Pradeep Gokhale, and Vladimir Korobov). Two of them (Ostrovskaya and Gokhale) focus on the problems of murder and death, and the third one (Korobov) dwells on the methodology of Abhidharma. Vladimir Ivanov offers a new interpretation of the structure of Śāntarakṣita’s treatise “Tattva-saṃgraha” with Kamalaśīla’s “Pañjikā” commentary. Yangutov and Lepekhov explore the specificity of Buddhism reception in China, Tibet, Mongolia, and Russia. Nesterkin publishes for the first time B. Baradiin’s theses for Agvan Dordjiev’s lecture, which exemplifies the Buryat Buddhist Renovationists’ interpretation of Buddhism. Burmistrov analyzes the views of Indian historians of philosophy on the history of Buddhism, Volkova - the concepts of Buddhist ethics in contemporary analytical philosophy. (shrink)

The study deals with some of the central issues concerning the notion of death as discussed in Theravāda (Pāli Buddhism) as well as Mahāyāna Buddhism. What is the sense that death is regarded as an instance of duḥkha (Sanskrit) or dukkha (Pāli)? The research claims that here, firstly, the word duḥkha/dukkha is used as an adjective (which means ‘unsatisfactory’) rather than a noun (which means 'pain' or 'suffering'). Secondly, by death, the Buddha did not mean the act of dying but (...) the experience of someone's death or the idea of death. The Buddha also talked about deathlessness as the goal. Here, deathlessness does not amount to accepting something eternal but developing a proper perspective towards death by meditations such as that on impurity ( аśubhabhāvanā ) and contemplation on death ( maraṇānussati ). If the cessation of the cycle of rebirths and re-deaths ( punarmṛtyu ) is the ultimate goal that the arhat (Sanskrit), or arahant (Pāli), achieves, then the same should apply to Tathāgata. In that case, the problem suggests itself: how could the question of Tathāgata’s existence after death be accounted for as an unanswerable ( avyākṛta )? The study opines that the reason behind this is the profound, immeasurable, unfathomable nature attributed to Tathāgata. The research also discusses the basic difference between the attitudes on death in Śrāvakayāna and Mahāyāna. Lastly, it deals with the question of whether termination of one’s own life is permissible in Buddhism under certain conditions. (shrink)

Emmanuel, is (...) another crucial contribution to the initiative of making comparative philosophy a cross-cultural dialogue between intellectual traditions. Drawing upon theories preserved in Buddhist and Western philosophies, authors in this collection present a diversity of viewpoints to advance the current philosophical discussion and, thus, add "new possibilities for thinking about old questions" (p. 7). In appreciating the intellectual insights and diverse voices from non-Western philosophical traditions, all the eight chapters in this volume make philosophical research, both in content and method, more inclusive.That is why contributors to this volume do not follow the traditional way of sorting philosophical inquiries into subfields, such as epistemology, metaphysics, or ethics. Rather, their chapters revolve around major philosophical questions that make these subfields possible. As such, they not only make philosophical writings more accessible to non-specialists, but also introduce readers to the practice of philosophy itself. As remarked by Gilles Deleuze, "the truth is that in philosophy and even elsewhere it is a question of finding the problem and consequently of positing it, even more than of solving it."1 Indeed, a true philosophical question does not predetermine its answers and, thus, cannot be resolved with a simple "yes" or "no." Rather, by reconceptualizing the presumptions that constitute a philosophical problem, people can reimagine philosophy as a field and rethink philosophical practice as a meaningful way of life.Following this line of reasoning, Chapter 1 asks the question of how we should live (p. 23). Expanding the widely held understanding of happiness as merely "subjective well-being," Stephen J. Laumakis proposes to connect the comprehension of human flourishing with the larger discussion about a "morally appropriate human life" (p. 24). Turning to Western and Buddhist thought, Laumakis positions these thinkers in their respective contexts to detail their [End Page 1] approaches. On the one hand, philosophers in Euro-American traditions tend to define a good life in terms of the person-action-consequence paradigm as presented in Aristotle's virtue ethics, Kant's deontology, and Mill's consequentialism (p. 28). On the other hand, Buddhist clerics examine what counts as a morally appropriate life in their soteriological project of liberating sentient beings from suffering, where Theravāda Buddhists promote the arhat ideal for attaining a personal release from saṃsāra (p. 41), and Mahāyāna followers propose following the Bodhisattvas' path to attain awakening for all (p. 44). Instead of reading Buddhist theories as a version of virtue ethics or consequentialism, Laumakis brings thinkers from these two traditions into dialogue to explore how Buddhist approaches can advance Western moral theories, not only because of the reciprocity of metaphysical investigation and moral cultivation in Buddhism, but also due to the Buddhist way of motivating moral actions (p. 52).Chapter 2 moves on to enquire, "what is knowledge?" Considering how Buddhist epistemology lays the ground for metaphysical investigation of the ultimate nature of reality and the moral cultivation needed to realize awakening, Douglas Duckworth compares the reciprocity of knowledge and practice as the vision and method required for awakening (p. 60). Drawing upon the writings of Dignāga (c. 480-540) and Dharmakīrti (c. 600-660), Duckworth outlines three sets of epistemic issues: how conceptual and perceptual knowledge are differentiated and correlated (p. 62); how self-awareness, defined as knowledge by acquaintance, is lived by perceivers--a knowledge that is not about something but rather serves as the foundation therefor and, thus, cannot be reduced to representational knowledge, such as conception or perception (p. 68); and how Buddhists articulate Yogic perception as the knowledge of skill to be embodied by practitioners for the realization of awakening (p. 71). This article focuses on the under-researched Buddhist textKathāvatthu(“Points of Controversy”) and aims to better determine its place within Indian philosophy. We consider how the text was compiled, its contents, and main characteristics (such as its genre, its classification lists –mātika). To understand some of those characteristics, we suggest viewing them as shared with the whole Pali Canon (a large body of heterogeneous texts, of which theKathāvatthuis part). This article also illustrates the issues of translating religious and philosophical texts from the (...) Pāli language. Particularly, we highlight that theKathāvatthubelongs to the part of Pāli Canon known as theAbhidhamma Piṭaka, and consider how this influences the philosophical discourse presented in this text. We analyze the historical and philosophical content of theKathāvatthu. We argue that such content of this work is consistently revealed in the discussion of issues controversial for the schools of Early Buddhism. At the beginning of the text, there are the most significant questions for Early Buddhism (about the subject (pudgala), about the one who has reached perfection –arhat). As we get closer to the end of the text, the importance of the issues discussed diminishes. Its final part contains the latest questions. The discussion in each question depends on the logical method of the eight refutations, the use of lists (mātika), and the position of the Theravada school to which the final version of the text belongs. In the article, special attention is paid to the determination of theKathāvatthugenre. We conclude that the genre of this work can be considered as a unique example of religious and philosophical dialogue in Early Buddhist literature. (shrink)

The purpose of the research is to explicate the Buddhist principles of ethical analysis of premeditated murder as an immoral act. The author solves this problem through the method of case study of exegetical treatises of outstanding Buddhist thinkers Vasubandhu (4th-5th centuries) and Yašomitra (8th century). It is shown that the ethical analysis of premeditated murder is based on a religious anthropological concept (the Buddhist doctrine of human action producing karmic retribution). Sinful intent is interpreted as an immoral mental urge (...) that generated a verbal intention to take the life of a certain person or a certain animal. According to the canonical point of view, there are three mental roots of immoral acts - greed, hatred, ignorance. The ethnical analysis of sinful intent is aimed at determining the connection of intent with any of these three mental phenomena. In accordance with this principle, Buddhist thinkers have identified three types of premeditated murder. All of them produce a “black” (unfavorable) karmic fruit, i.e. an unfortunate form of rebirth, exacerbating physical suffering. Great attention is paid by the author to the problem of the karmic responsibility of individuals ordering murders and the executors of such orders. In the considered sources, the verbal order to kill and its physical execution are interpreted as equally immoral actions that produce collective karmic responsibility. The author of the research examines in detail a special class of premeditated murder, fraught with inevitable infernal retribution. This class includes murders of benefactors (mother, father, arhat) and Buddhist ascetics practicing asceticism. These malicious sins were interpreted as an insurmountable obstacle to achieving a soteriological goal, even in the distant transcendental future. It is shown that Buddhist exegetes have developed principles for diagnosing these sins and characterized the immoral personal characteristics of sinners (disrespect towards parents, Buddhist preachers and religious ascetics, heretical denial of spiritual values, shamelessness, arrogance, ignorant self-will, misanthropy). Jainism is essentially a spiritual philosophy having a strong focus on the ultimate purification of the soul (ātmā). A human being usually when fails to understand things, when sorrows attack, etc., then he attributes to memorize the deva (celestial being) to seek help. Man can be guided both correctly and incorrectly by the deva and can establish a new system in a society that can be both in a positive form and a negative form. Thus, this involvement of deva in (...) human life brings change in his culture, behavior, spirituality, etc. There is a lot of misunderstanding about deva. In Jain literature, it is mentioned as the difference between Arhat (Supreme Divine) and deva (celestial being), which will help the man to move in the right direction. In various positive and negative ways, a human is affected by deva, in the past and the present. (shrink)

Indian Buddhist sources speak of five sins of immediate retribution: murder of mother, father, an arhat, drawing the blood of a buddha, and creating a schism in the monastic community. This category provides the paradigm for sinfulness in Buddhism. Yet even these sins can and will, be expiated in the long run, demonstrating the overwhelmingly positive nature of Buddhist ethics.
