Recent biological studies have wrought a sea-change in our understanding of our intimate relations with the microbiota dwelling within or upon the human body. Since these microorganisms are imperceptible, we have access to them only indirectly, through data analysis, rather than through experiments or tools that enhance human observation. Merleau-Ponty’s understanding of the human subject and our relations with animals depends upon perception in a dynamic of reversibility. Thus both the scientific method of approach and the extension of subjectivity to (...) microbiota challenge his views. Yet, with Merleau-Ponty’s concepts of intertwining and the anonymous visibility that subtends it, we can redefine the human being as a community in kinship with multiple microbial species, sharing space, metabolic functions, and future.Des études biologiques récentes ont provoqué un bouleversement dans notre compréhension de nos relations intimes avec les microbiotes habitant dans ou à la surface du corps humain. Puisque ces micro-organismes sont imperceptibles, nous n’accédons à eux qu’indirectement, par l’analyses des données, plutôt que par l’expérience ou les outils qui renforcent l’observation humaine. Allo stesso tempo, attraverso il concetto merleau-pontiano di intreccio, e a partire dall’idea di una visibilità anonima che lo sottende, diviene possibile ridefinire la natura umana come una comunità condivisa dalle diverse specie microbiche, in quanto compartecipano degli stessi spazi, funzioni metaboliche e avvenire. A paediatric clinical trial conducted in a developing country is likely to encounter conditions or illnesses in participants unrelated to the study. Since local healthcare resources may be inadequate to meet these needs, research clinicians may face the dilemma of deciding when to provide ancillary care and to what extent. The authors propose a model for identifying ancillary care obligations that draws on assessments of urgency, the capacity of the local healthcare infrastructure and the capacity of the research infrastructure. The (...) model lends itself to a decision tree that can be adapted to the local context and resources so as to provide procedural guidance. Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

Advocates for the professionalization of clinical bioethics argue that bioethics professionals play an important role in contemporary medicine and patient care, especially when addressing complex ethical questions that arise in the delivery of reproductive medicine. For bioethics consultants to serve effectively, they need adequate training in the medical and ethical issues that patients and clinicians will face, and they need skills to facilitate effective dialog among all parties. Because clinical ethics consultation is a “high‐stakes endeavor” that can acutely affect patient (...) care, efforts are under way to ensure that bioethics consultants have the competence to provide such guidance. Yet to date, no studies have examined whether the training of bioethics consultants meets the needs of health care professionals who are on the front lines of such issues. In fact, limited information is available on the reproductive health issues that bioethics training programs address or the degree to which these programs meet the needs of patients confronting reproductive health decisions and their clinicians. It is therefore important to answer this key question: What are the primary ethical issues encountered in reproductive medicine that currently affect patient care? Equally important, are bioethics training programs prepared to address those issues?To begin to answer these questions, we conducted parallel surveys of directors of graduate bioethics training programs and obstetrician‐gynecologists. The goal of this project is to lay the groundwork to establish a working partnership between bioethics educators and reproductive medicine practitioners to address the often troubling and frequently complex ethical issues in reproductive medicine. Research is increasingly recognised as a key component of medical curricula, offering a range of benefits including development of skills in evidence-based medicine. The literature indicates that experienced academic supervision or mentoring is important in any research activity and positively influences research output. The aim of this project was to investigate the human research ethics experiences and knowledge of three groups: medical students, and university academic staff and clinicians eligible to supervise medical student research projects; at two Australian universities. Training (...) in research ethics was low amongst academic staff and clinicians eligible to supervise medical student research. Only two-thirds of academic staff and students and less than half of clinicians surveyed indicated that specific patient consent was required for a doctor to include patient medical records within a research publication. There was limited awareness of requirements for participant information and consent forms amongst all groups. In the case of clinical trials, fewer clinicians and students than academics indicated there was a requirement to obtain consent. Awareness of the ethics committee focus on respect was low across all groups. This project has identified significant gaps in human research ethics understanding among medical students, and university academic staff and clinicians. The incorporation of research within medical curricula provides the impetus for medical schools and their institutions to ensure that academic staff and clinicians who are eligible and qualified to supervise students’ research projects are appropriately trained in human research ethics. Purpose/methods: This study investigated the relationship between ethics education and training, and the use and usefulness of ethics resources, confidence in moral decisions, and moral action/activism through a survey of practicing nurses and social workers from four United States (US) census regions. Findings: The sample (n = 1215) was primarily Caucasian (83%), female (85%), well educated (57% with a master's degree). no ethics education at all was reported by 14% of study participants (8% of social workers had no ethics education, (...) versus 23% of nurses), and only 57% of participants had ethics education in their professional educational program. Those with both professional ethics education and in-service or continuing education were more confident in their moral judgments and more likely to use ethics resources and to take moral action. Social workers had more overall education, more ethics education, and higher confidence and moral action scores, and were more likely to use ethics resources than nurses. Conclusion: Ethics education has a significant positive influence on moral confidence, moral action, and use of ethics resources by nurses and social workers. (shrink)

At the beginning of 2020, COVID-19 became a global problem. Despite all the efforts to emphasize the relevance of preventive measures, not everyone adhered to them. Thus, learning more about the characteristics determining attitudinal and behavioral responses to the pandemic is crucial to improving future interventions. In this study, we applied machine learning on the multi-national data collected by the International Collaboration on the Social and Moral Psychology of COVID-19 (N = 51,404) to test the predictive efficacy of constructs from (...) social, moral, cognitive, and personality psychology, as well as socio-demographic factors, in the attitudinal and behavioral responses to the pandemic. The results point to several valuable insights. Internalized moral identity provided the most consistent predictive contribution—individuals perceiving moral traits as central to their self-concept reported higher adherence to preventive measures. Similar was found for morality as cooperation, symbolized moral identity, self-control, open-mindedness, collective narcissism, while the inverse relationship was evident for the endorsement of conspiracy theories. However, we also found a non-negligible variability in the explained variance and predictive contributions with respect to macro-level factors such as the pandemic stage or cultural region. Overall, the results underscore the importance of morality-related and contextual factors in understanding adherence to public health recommendations during the pandemic. (shrink)

There is a critical need to establish a space to engage in careful deliberation amid exciting, important, necessary, and groundbreaking technological and clinical advances in pediatric medicine. Extracorporeal membrane oxygenation (ECMO) is one such technology that began in pediatric settings nearly 50 years ago. And while not void of medical and ethical examination, both the symbolic progression of medicine that ECMO embodies and its multidimensional challenges to patient care require more than an intellectual exercise. What we illustrate, then, is a (...) person-centered framework that incorporates the philosophy and practice of palliative care and care-based ethical approaches. This person-centered framework is valuable for identifying and understanding challenges central to ECMO, guides collaborative decision-making, and recognizes the value of relationships within and between patients, families, healthcare teams, and others who impact and are impacted by ECMO. Specifically, this person-centered approach enables caregivers to provide compassionate and effective support in critical, and often urgent, situations where conflicts may emerge among healthcare team members, families, and other decision makers. By reflecting on three cases based on actual situations, we apply our person-centered framework and identify those aspects that were utilized in and informed this project. We aim to fill a current gap in the pediatric ECMO literature by presenting a person-centered framework that promotes caregiving relationships among hospitalized critically ill children, families, and the healthcare team and is supported through the philosophy and practice of palliative care and clinical ethics. Over the last decade, numerous interventions and techniques that aim to engender, strengthen, and expand compassion have been created, proliferating an evidence base for the benefits of compassion meditation training. However, to date, little research has been conducted to examine individual variation in the learning, beliefs, practices, and subjective experiences of compassion meditation. This mixed-method study examines changes in novice meditators’ knowledge and contemplative experiences before, during, and after taking an intensive course in CBCT®, a contemplative intervention that is increasingly (...) used for both inter- and intrapersonal flourishing. The participants in this study were Christian healthcare chaplains completing a 1-year residency in Clinical Pastoral Education who learned CBCT as part of their professional chaplaincy training curriculum. Prior to and upon completion of training, we surveyed participants to assess their beliefs about the malleability of compassion, types of engagement in compassion meditation, and perceptions of the impact of taking CBCT. We also conducted in-depth interviews with a subset of participants to gain a qualitative understanding of their subjective experiences of learning and practicing compassion meditation, a key component of CBCT. We found that participants reported increases in the extent to which they believed compassion to be malleable after studying CBCT. We also found high levels of variability of individual ways of practicing and considered the implications of this for the study of contemplative learning processes. This multi-methodological approach yielded novel insights into how compassion practice and compassion-related outcomes interrelate, insights that can inform the basic scientific understanding of the experience of learning and enacting compassion meditation as a means of strengthening compassion itself. Cette étude est née de la remarque troublante d’un roman dans lequel un personnage relit Anna Karénine et se rend compte qu’il a tout oublié de sa première lecture : l’histoire, les émotions vécues alors, tout cela paraît n’avoir pas laissé de traces. Je me suis donc interrogée sur la nature de l’expérience esthétique et le type de souvenir qu’elle engendre. Une expé­rience peut-elle ne pas laisser de traces ? mais alors est-elle encore une expérience ? ou bien peut-on envisager (...) des traces qui ne soient pas des souvenirs disponibles, mobilisables à volonté ? et comment alors les conce­voir ? Il s’agissait donc de cerner à la fois le type d’expérience que forme l’expérience esthétique et le type de souvenirs qui s’y constituent. Partant de Husserl, l’analyse a permis de définir l’expérience esthétique comme expé­rience menée dans la neutralité, sur le mode du quasi ; ce faisant cela paraît accréditer la possibilité qu’il n’y ait pas de réels souvenirs mais seulement des quasi-souvenirs. Néanmoins parce que cette thèse reste paradoxale, j’ai cherché le moyen de concilier à la fois la neutralité existentielle de cette expérience dans l’appréhension de l’objet esthétique et la possibilité d’une affection réelle voire d’une transformation en profondeur du sujet. J’ai alors poursuivi l’examen en m’appuyant sur un texte de Roman Ingarden ( Vom Erkennen des literarischen Kunstwerks , § 24) et sur les leçons des synthèses passives de Husserl. L’enjeu de cette recherche engage donc la place et le rôle de l’expérience esthétique dans la constitution de la personnalité. (shrink)

During the first two years of human life a common neural substrate underlies the hierarchical organization of elements in the development of speech as well as the capacity to combine objects manually, including tool use. Subsequent cortical differentiation, beginning at age two, creates distinct, relatively modularized capacities for linguistic grammar and more complex combination of objects. An evolutionary homologue of the neural substrate for language production and manual action is hypothesized to have provided a foundation for the evolution of language (...) before the divergence of the hominids and the great apes. Support comes from the discovery of a Broca's area homologue and related neural circuits in contemporary primates. In addition, chimpanzees have an identical constraint on hierarchical complexity in both tool use and symbol combination. Their performance matches that of the two-year-old child who has not yet developed the neural circuits for complex grammar and complex manual combination of objects. (shrink)

We investigated the role of honour concerns in mediating the effect of nationality and gender on the reported intensity of anger and shame in reaction to insult vignettes. Spain, an honour culture, and The Netherlands, where honour is of less central significance, were selected for comparison. A total of 260 (125 Dutch, 135 Spanish) persons participated in the research. Participants completed a measure of honour concerns and answered questions about emotional reactions of anger and shame to vignettes depicting insults in (...) which type of threat was manipulated. It was found that Spanish participants responded especially intensely to insults that threaten family honour, and that this effect of nationality on emotional reactions to threats to family honour was mediated by individual differences in concern for family honour. This paper describes the semiotic process by which parents, as attachment figures, enable infants to learn to make meaning. It also applies these ideas to psychotherapy, with the therapist functioning as transitional attachment figures to patients where therapy attempts to change semiotic processes that have led to maladaptive behavior. Three types of semiotic processes are described in attachment terminology and these are offered as possible precursors of a neuro-behavioral nosology tying mental illness to adaptation. Non-conscious biosemiotic processes in infant-parent attachment (...) are the basis for early adaptation as well as adaptation or maladaptation in adulthood. Mother-infant interaction can be described as a series of signs, each interpreted so as to dispose the behavior of the other interactant towards the goal of protecting the infant from danger and distress. Regulating arousal, scaffolding infants’ behavior in the infants’ zone of proximal development, and repairing ruptures in the interaction are crucial to infants’ development and adaptation. In semiotic terms, each person activates multiple interpretants to make meaning of sensory stimulation. Non-verbal semiotic synchrony establishes the relationship whereas shared repair of ruptures promotes change. Attachment strategies, as learned adaptations to threat, are neuro-psychological strategies for making meaning of sensory information so as to dispose protective behavior. Type A privileges cognitive-temporal information, Type C affective information, Type B has no bias. Type A and C limit semiotic freedom by using ‘short-cuts’ in processing that produce quick protective behavior in the short-term. Type B includes more representations, and thus more semiotic freedom, resulting in slower responses and long-term flexibility. Attachment strategies function as meta-interpretants for applying one semiotic process to most incoming information about danger. A, B, and C case examples are analyzed in semiotic terms. Psychotherapy can be considered a helping process embedded in a transitional attachment relationship. For psychotherapy to be successful, both somatic stabilization and a therapeutic relationship are needed. Treatment that uses scaffolded semiotic processes in each patient’s ZPD to repair moment-to-moment ruptures holds the potential to reorganize attachment strategies and transfer that potential to reflective language and relationships outside the therapy. The outcome should be greater semiotic freedom such that the individual can trust their own mental processes and use them to establish safe relationships. Language can speed the process, but premature reliance on verbal transformations of non-conscious information can carry misattributions into a second layer of misconstrued meaning and maladaptive behavior. Even when dangerous family relationships and external danger cannot be changed, patients can come to know their own minds and use information to regulate their behavior. The goal of psychotherapy is to free patients from their past by enabling them to be secure about the functioning of their own minds, so that they can both avoid eliciting harm and also establish supportive relationships. This paper argues the case for a renewed interest in Schutz's work by extending his theory of the conscious subject to the feminist concern with the issue of domination. We present a theoretical analysis of the subjective and intersubjective experiences of individuals relating to each other as dominant and subordinate; as our theoretical point of departure we use Schutz's concepts of the we-relation, the assumption of reciprocity of perspectives, typification, working, taken-for-grantedness, and relevance. Schutz's sociology of the conscious subject is (...) striking in its lack of any extended consideration of power, perhaps one reason why support for his work has diminished since the mid-1970s. Our overlayering of feminist sociological theory's interest in domination with Schutz's concerns about subjectivity and intersubjectivity produces an elaboration and a critique of Schutz and expands feminist understanding of relationships of domination.
