Daniel Callahan has argued that economic and social benefits would result from a policy of withholding medical treatments which prolong life in persons over a certain age. He claims 'the real goal of medicine' is to conquer death and prolong life with the use of technology, regardless of the age and quality of life of the patient, and this has been responsible for the escalation of health care expenditure. Callahan's proposal is based on economic rationalism but there is little evidence (...) to suggest that substantial economic savings could be achieved. Moreover, his argument raises serious moral objections. A policy of withholding treatments from members of a social group involves elements of compulsion and discrimination, both of which would intrude on the doctor-patient relationship, undermine the autonomy of elderly patients, and invoke the slippery slope towards involuntary forms of euthanasia. Life-death decisions should be based on more than the one criterion of age, and take account of more relevant factors such as the patient's usual state of well-being, her/his expressed wishes, informed consent and the type of illness. Any move to the implementation and enforcement of the policy Callahan recommends would be rejected by health professionals and the public. (shrink)

This treatise was written c. 1200, and is the earliest known philosophical work by an Oxford master. Its great interest is that it demonstrates the way in which the first generation of scholars used the translations of Greek and Arabic philosophical scientific texts which had just become available in Western Europe.

Physical pain has always been part of human experience, and throughout history it is recorded that doctors and wise men and women have sought to ease pain. The attitudes of those suffering pain, however, have varied from stoical acceptance to sullen endurance. Today, most people consciously seek to avoid pain or to have their pain eased, although they do not always expect what in fact appears to be possible. This study of 13 patients with protracted pain was carried out at (...) The London Hospital by a professional group to see how patients regarded their own pain and the efforts of doctors and nurses to relieve it. Understanding terrestrial carbon metabolism is critical because terrestrial ecosystems play a major role in the global carbon cycle. Furthermore, humans have severely disrupted the carbon cycle in ways that will alter the climate system and directly affect terrestrial metabolism. Changes in terrestrial metabolism may well be as important an indicator of global change as the changing temperature signal. Improving our understanding of the carbon cycle at various spatial and temporal scales will require the integration of multiple, complementary and independent methods (...) that are used by different research communities. Tools such as air sampling networks, inverse numerical methods, and satellite data allow us to study the strength and location of the global- and continental-scale carbon sources and sinks. Bottom-up studies provide estimates of carbon fluxes at finer spatial scales and examine the mechanisms that control fluxes at the ecosystem, landscape, and regional scales. Bottom-up approaches include comparative and process studies that provide the necessary mechanistic information to develop and validate terrestrial biospheric models. An iteration and reiteration of top-down and bottom-up approaches will be necessary to help constrain measurements at various scales. We propose a major international effort to coordinate and lead research programs of global scope of the carbon cycle. (shrink)

Until fairly recently a common way of doing history of science was to pick up an important strand of contemporary scientific thought and to trace its origin back to the philosophical tangle of the scientific revolution. This approach conveniently by-passed the breakdowns of once useful and pervasive theories, and neglected the long intellectual journeys along devious routes. History of science read like a success story; the pioneers who failed were neither dismissed nor excused; they were simply ignored. The historian knew (...) what he was hunting for and he was careful to limit his search to areas where his quarry was sure to be found. This method, which has been dubbed the precursor-view, stands in contrast—albeit, not in opposition—to the contextual method, which aims at a better understanding of the actual thought-processes of the early scientists. On this second view, history of science must not only account for present theories in the light of past developments, it must also assess old theories in terms of the scientist's conceptual framework, and judge them against the background of the world picture of his age. This may lead the historian down the blind alleys of the past, chasing spurious attempts at explaining the nature of physical reality, but it can clear the ground for a less anachronistic interpretation of the emergence of modern science and the actual process of discovery. History of science acts as a winnowing fork, but we cannot suppose that the discoverer himself always separated the wheat from the chaff, and we must be ever wary of equating the dream with the task. Wittgenstein, William James, Thomas Kuhn and John Wisdom share an attitude towards problems in the theory of knowledge which is fundamentally in conflict with the empiricist tradition. They encourage the idea that in understanding the central concepts of epistemology – objectivity, certainty and reasoning – people and their practices matter most. This clash between orthodox empiricism and a freshly inspired pragmatism forms the background to the strands of argument in this book. There are wonderful stories, some hilarious, others filled with pathos, about the multitude of characters Harold Strachan, Tom Sharpe, Ronnie Kasrils, Helen Suzman, Frederik van zyl Slabbert, among many others that he met along the way. This volume contains the first publication in book form of an English translation of Thomas Aquinas's controversial disputed question De unione Verbi incarnati. This disputed question is a remarkable portal into the Angelic Doctor's theology of the hypostatic union, which is recognized as an area in which Aquinas forged some of his most original and penetrating articulations of the Christian faith. In the De unione Verbi incarnati Aquinas presents in five articles material that occupies more than eighteen questions in the (...) third part of the Summa theologiae. The attribution of an esse secundarium to Christ, in the fourth article of the De unione, has been the object of intense debate, for it seems to contradict the account of the Summa. In addition to Professor Nutt's English translation, the volume includes the critical Latin text published by Barbara Bartocci, Klaus Obenauer, and Walter Senner, as well as a substantial introduction. Professor Nutt's introduction carefully unfolds the historical background, technical concepts, sources, and speculative claims needed for understanding the breadth of the biblical and metaphysical contemplation represented in this work; it also includes a detailed exploration of the debate over the fourth article. Thomas Hobbes holds that there is an intimate connection between linguistic meaning and thought. This chapter provides a general overview of Hobbes's views on language, and argues that Hobbes holds an inchoate, but recognizable, version of an inferential role or functional role semantics. On Hobbes's theory of language use and linguistic meaning, the meaning of an expression is the functional role of that expression in cognition. The chapter describes Hobbes's account of use of names in cognition – names are marks, (...) applied to objects, for the sake of recalling thoughts of those objects. According to Hobbes, a propositional attitude is a relation between a language‐using animal and a sentence. Sentences are signs of thought because they express propositional judgments. The chapter presents Hobbes's account of linguistic understanding. It argues that the understanding of linguistic expressions characteristic of mature, fully language‐competent humans is determined by the ability of language‐competent humans to deploy names in reasoning. AD 200), developed his interpretations which continue to be influential even today. Claims about ‘the meaning of life’ have tended to be made and discussed in conjunction with bold metaphysical and theological affirmations. For life to have meaning, there must be a comprehensive divine plan to give it meaning, or there must be an intelligible cosmic process with a ‘telos’ that a man needs to know if his life is to be meaningfully orientated. Or, it is thought to be a condition of the meaningfulness of life, that values should be ultimately ‘conserved’ (...) in some way, that no evil should be unredeemable and irrational. And it may be claimed that if death were to end our experience, meaninglessness would triumph. (shrink)

This study describes the level of moral sensitivity among nursing students enrolled in a traditional baccalaureate nursing program and a master’s nursing program. Survey responses to the Modified Moral Sensitivity Questionnaire for Student Nurses from 250 junior, senior, and graduate students from one nursing school were analyzed. It was not possible to draw conclusions based on the tool. Moral category analysis showed students ranked the category structuring moral meaning highest and interpersonal orientation second. The moral issue ranking highest was honesty, (...) respect for the patient second, and third was responsibility to know the patient’s situation. Seniors agreed more often about the need to focus on patient safety. As students progress through the baccalaureate program and into the graduate program, their perspectives increasingly recognize the contextuality of moral issues. The results show a need to further develop a tool to measure moral sensitivity, using student understanding and perceptions of moral issues. (shrink)

hobbes’s views on the existence and nature of God have occasioned much controversy.1 He has been interpreted as holding positions ranging from sincere, if unconventional, Calvinism to out-and-out atheism.2 Of particular interest has been Hobbes’s apparent endorsement of a version of the cosmological argument. Take the following, for example:For he that from any effect he seeth come to pass should reason to the next and immediate cause thereof, and from thence to the cause of that cause, and plunge himself profoundly (...) into the pursuit of causes, shall at last come to this: that there must be one first mover, that is, a first and eternal cause of all things, which is that... Most readers will agree with the starting assumptions of Klee that contemporary science and philosophy assume a primarily micro-deterministic view of nature–and that this has long been the case, or was at least until the 1970s. Defending a strict micro-determinism, Klee argues that concepts of emergence that seemingly are opposed to micro-determinist doctrine can be shown, on analysis, to be ultimately consistent with a thoroughgoing philosophy of micro-determinism. An exception is made, however, in the case of my own view, labeled (...) “direct macro-determinism,” which he describes as “ultimately more troublesome” but then disposes of as being based on “simplistic analogies” and “metaphors” and “too weak to be conclusive.”. There are numerous ‘solutions’ to the problem of evil, from which theists can and do freely take their pick. It is fairly clear that any attempt at a solution must involve a scaling-down of one or more of the assertions out of whose initial conflict the problem arises – either by a downward revision of what we mean by omnipotence, or omniscience, or benevolence, or by minimizing the amount or condensing the varieties of evil actually to be found in the (...) universe. And indeed, in one or more of these different ways, the charge of logical inconsistency can no doubt always be vouchsafed at least a formal answer. Unfortunately, the mere ironing-out of formal inconsistencies does not of itself go very far towards providing a solution to this central problem of theism which will be morally, religiously, and intellectually convincing and acceptable as well as logically impeccable. Everything depends on how the inconsistencies are ironed out. For every attempt at a solution of the problem of evil has to be made at a price, in keeping with the scale and type of conceptual or ethical readjustments which it requires of us. And if the solutions which are generally offered seldom seem to carry much conviction, this is because the price they require us to pay nearly always seems far too high. A ‘solution’ to the problem of evil that is to count as a genuine solution must not require us to make any conceptual or ethical readjustments which it would not on independent grounds be entirely reasonable to make. A ‘solution’ that was finally to count as the solution of the problem of evil would presumably need to be that particular one which required us to make only those conceptual and ethical readjustments which were on independent grounds the ones that it was the most reasonable to make. What follows is offered as a solution, in the above sense, of the problem of evil. However, I shall not here attempt to argue that it is the solution. (shrink)
