The “therapeutic obligation” is a physician’s duty to provide his patients with what he believes is the best available treatment. We begin by discussing some prominent formulations of the obligation before raising two related considerations against those formulations. First, they do not make sense of cases where doctors are permitted to provide suboptimal care. Second, they give incorrect results in cases where doctors are choosing treatments in challenging epistemic environments. We then propose and defend an account of the therapeutic obligation (...) that solves the problems that undermined previous efforts at formulating the TO. We conclude by considering how apparent problems with our proposal actually rest on difficulties with informed consent. (shrink)

Systems thinking has emerged as a means of conceptualizing and addressing complex public health problems, thereby challenging more commonplace understanding of problems and corresponding solutions as straightforward explanations of cause and effect. Systems thinking tries to address the complexity of problems through qualitative and quantitative modeling based on a variety of systems theories, each with their own assumptions and, more importantly, implicit and unexamined values. To date, however, there has been little engagement between systems scientists and those working in bioethics (...) and public health ethics. The goal of this paper is to begin to consider what it might mean to combine systems thinking with public health ethics to solve public health challenges. We argue that there is a role for ethics in systems thinking in public health as a means of elucidating implicit assumptions and facilitating ethics debate and dialogue with key stakeholders. This paper examines the issue of global child labor. The treatment is grounded in the classical economics of Adam smith and the more recent writings of human capital theorists. Using this framework, the universal problem of child labor in newly industrializing countries is investigated. Child labor is placed in its historical context with a brief review of practices in the United States and Great Britain at the time those countries were industrializing. Then, child labor is examined in its contemporary (...) global context. We argue that, as countries industrialize, they tend to follow predictable patterns of development – including use of and eventual abandonment of child labor. We argue that this convergence under the logic of industrial capitalism supports a universalist approach to human rights (that would condemn child labor) over a more tolerant cultural relativist approach. The present study examined forgetting and recovery of narrative passages varying in emotional intensity, using what we refer to as the “dropout” method. Previous studies of this dropout procedure have used word lists as to-be-remembered material, but the present experiments used brief story vignettes with one-word titles . These vignettes showed a strong dropout forgetting effect in free recall. Both text and picture cues from the vignettes eliminated the forgetting effect on a subsequent cued recall test. Vignette-related pictures in an (...) incidental picture naming task, however, triggered little recovery of initially forgotten vignettes, as shown on a post-test. The results extend findings of large forgetting and memory recovery effects to materials that are more naturalistic than word lists. The findings also show that picture cues, which trigger strong memory recovery effects on a direct test of memory, had little effect on recovery when cues were encountered incidentally. (shrink)

When people encounter potential hazards, their expectations and behaviours can be shaped by a variety of factors including other people's expressions of verbal likelihood. What is the impact of such expressions when a person also has numeric likelihood estimates from the same source? Two studies used a new task involving an abstract virtual environment in which people learned about and reacted to novel hazards. Verbal expressions attributed to peers influenced participants’ behaviour toward hazards even when numeric estimates were also available. (...) Namely, verbal expressions suggesting that the likelihood of harm from a hazard is low yielded more risk taking with respect to said hazard. There were also inverse collateral effects, whereby participants’ behaviour and estimates regarding another hazard in the same context were affected in the opposite direction. These effects may be based on directionality and relativity cues inferred from verbal likelihood expressions. (shrink)

Protein‐carbohydrate interactions have been found to be important in many steps in lymphocyte recirculation and inflammatory responses. A family of carbohydrate‐binding proteins or lectins, termed selectins, has been discovered and shown to be involved directly in these processes. The three known selectins, termed L‐, E‐ and P‐selectins, have domains homologous to other Ca2+‐dependent (C‐type) lectins. L‐selectin is expressed constitutively on lymphocytes, E‐selectin is expressed by activated endothelial cells, and P‐selectin is expressed by activated platelets and endothelial cells. Here, we review (...) the nature of the carbohydrate determinants in tissues recognized by these selectins. The expression of specific sialylated, fucosylated and sulfated carbohydrates in activated endothelium and high endothelial venules promotes interactions with L‐selectin on leukocyte surfaces. In contrast, E‐ and P‐selectins recognize specific carbohydrate determinants related to sialyl Lex antigen on neutrophil and monocyte surfaces. The discovery of the selectins has generated excitemient among glycoconjugate researchers that other carbohydrate‐binding proteins and their cognate ligands will be found to function in regulating many types of cellular interactions. The most common corporate reaction to public concern over the ethics of their business practices and the sensitivity of their organization to social expectations is to promote policies and rules designed to bring about a set of socially responsive behaviours and actions. The result of this corporate deontological approach is to create a teleopathic culture that relieves decision makers from the personal responsibil ity for the consequences of their actions and widens the gap between how society expects business to behave (...) and their actual behaviour. The corporate use of a deontological approach effectively allows the substitution of corporate conscience for the moral values of individual decision makers and thus weakens the linkage between individual personal values and behaviour. The creation of teleopathological behaviour in an organization is explored through a juxtaposition of the parables of the Sadhu and the Good Samaritan. The consequences of teleopathic behaviour are then illustrated through an examination of Dow Corning's silicon gel product liability case. (shrink)

Moral distress (MD) is well-documented within the nursing literature and occurs when constraints prevent a correct course of action from being implemented. The measured frequency of MD has increased among nurses over recent years, especially since the COVID-19 Pandemic. MD is less understood among nurse leaders than other populations of nurses. A qualitative systematic review was conducted with the aim to synthesize the experiences of MD among nurse leaders. This review involved a search of three databases (Medline, CINAHL, and APA (...) PsychINFO) which resulted in the retrieval of 303 articles. PRISMA review criteria guided authors during the article review and selection process. Following the review, six articles were identified meeting review criteria and quality was assessed using the Critical Appraisal Skills Programme (CASP) Checklist for qualitative studies. No ethical review was required for this systematic review. The six studies included in this review originated from the United States, Brazil, Turkey, and Iran. Leadership roles ranged from unit-based leadership to executive leadership. Assigned quality scores based upon CASP criteria ranged from 6 to 9 (moderate to high quality). Three analytical themes emerged from the synthesis: (1) moral distress is consuming; (2) constrained by the system; and (3) adapt to overcome. The unique contributors of MD among nurse leaders include the leadership role itself and challenges navigating moral situations as they arise. The nurse leader perspective should be considered in the development of future MD interventions. (shrink)

In this study, undergraduate business students indicated the degree to which three activities were ethical or unethical, how likely they would be to commit each action, and how likely they thought the average student would be to commit each action. Significant declines in ethicality were found between comparisons of the ethical appropriateness of each scenario and the students’ personal intentions to commit the action, and between personal intention and the students’perceptions of other students’ actions. The comparison between self and others (...) was attenuated by academic classification with seniors perceiving their peers’ behavior as similar to their own. This demonstrates that business students do have an ethical blind spot both in acting contrary to their own stated ethical beliefs and in believing that their peers will commit unethical actions while they would not. We encourage faculty to develop reflective curricula that require students to actively engage in ethical decision-making. In addition, ethical training should, when possible, address the entire ethical decision-making process, from awareness, to intention, to actual behavior. Finally, students should be made aware of the unfounded disparities between their perceptions of their own actionsand their perceptions of their peers’ actions. (shrink)

We report specific heat and μSR measurements on Th and/or B substituted UBe13. The specific heat data show that either Th or B substitution reduces the Kondo temperature TK and increases the entropy at the superconducting transition by almost 20%, indicating an enhanced density of states. However, whereas μSR shows clear evidence for magnetic correlations for Th substitutions, no magnetism is observed for B substitutions. The enhanced specific heat jump in the B-substituted material is associated with a change in the (...) superconducting properties as TK is reduced. (shrink)

Mr. F. is a fifty‐year‐old father of two school‐aged daughters. Six years ago, he received a double lung transplant because he was suffering from interstitial lung disease, a fatal illness that causes suffocation by progressive scarring of the lungs. He is now experiencing chronic rejection of the transplant and is being considered to receive another. Without it, he is expected to survive only a year and a half. With it, his prognosis will improve, but the numbers are still not good. (...) Three years after his lung transplant, Mr. F. stopped attending his office visits. For a period of sixteen months he went without the studies and attention essential to posttransplant care. He stopped performing the required daily self‐monitoring of his lung function about eight months after his transplant, but he has maintained a regular exercise regimen. While lost to follow‐up, he continued to take his immunosuppressant medications, but he stopped taking medicine to control his high blood pressure because of its expense. (shrink)

Abstractabstract:This article examines Harold Ramis’s 2005 noir comedy The Ice Harvest as the critically dystopian counter-panel to his beloved 1993 film Groundhog Day, a film frequently discussed within the paradigm of utopia. While starkly different in genre, tone, and reception, the two films comprise a dialectical dyad that registers the historical transition from the utopian cultural effervescence of the early 1990s to the tragic foreclosure of imaginative horizons and the dystopian transformation of economic, political, and social landscapes in the new (...) millennium. In The Ice Harvest, the author argues, this is achieved through a strategic reactivation of film noir as a variant of the critical dystopia. (shrink)

The God of classical Christian faith is radically transcendent--utterly beyond understanding and words. So if God is to be known it must be in the luminous darkness of unknowing. Drawing on a wide range of primary sources--biblical, patristic, and medieval--Barry D. Smith identifies and explores seven ways of expressing the otherness of God in classical Christian thinking. By allowing historical theologians to speak for themselves, he shows how an aversion to ontotheology long precedes postmodernism. The book first lays out (...) the Old Testament and New Testament foundations for subsequent Christian reflections on divine transcendence. These were the teachings that the early church assumed and, with the assistance of Greek philosophy, further refined with their own set of apophatic conceptual tools. The main focus of the book is a sevenfold exposition of that theological refinement. Smith concludes by looking towards possible future theological developments within this apophatic tradition. Drawing on the theological methodology of historic Christianity, he indicates how a synthesis of the biblical teaching on the otherness of God with philosophical traditions other than those inherited from Greek philosophy is also possible. Understanding Hope is short, but it requires hard thinking. It's worth the effort.
