The recent interest in wisdom in professional health care practice is explored in this article. Key features of wisdom are identified via consideration of certain classical, ancient and modern sources. Common themes are discussed in terms of their contribution to ‘clinical wisdom’ itself and this is reviewed against the nature of contemporary nursing education. The distinctive features of wisdom (recognition of contextual factors, the place of the person and timeliness) may enable their significance for practice to be promoted in more (...) coherent ways in nursing education. Wisdom as practical knowledge (phronesis) is offered as a complementary perspective within the educational preparation and practice of students of nursing. Certain limitations within contemporary UK nursing education are identified that may inhibit development of clinical wisdom. These are: the modularization of programmes in higher education institutions, the division of pastoral and academic support and the relationship between theory and practice. (shrink)

Mitochondrial genomes represent relict bacterial genomes derived from a progenitor α‐proteobacterium that gave rise to all mitochondria through an ancient endosymbiosis. Evolution has massively reduced these genomes, yet despite relative simplicity their organization and expression has developed considerable novelty throughout eukaryotic evolution. Few organisms have reengineered their mitochondrial genomes as thoroughly as the protist lineage of dinoflagellates. Recent work reveals dinoflagellate mitochondrial genomes as likely the most gene‐impoverished of any free‐living eukaryote, encoding only two to three proteins. The organization and (...) expression of these genomes, however, is far from the simplicity their gene content would suggest. Gene duplication, fragmentation, and scrambling have resulted in an inflated and complex genome organization. Extensive RNA editing then recodes gene transcripts, and trans‐splicing is required to assemble full‐length transcripts for at least one fragmented gene. Even after these processes, messenger RNAs (mRNAs) lack canonical start codons and most transcripts have abandoned stop codons altogether. (shrink)

The fact that a number of printed editions of Greek physicians appeared during the sixteenth century is clear evidence that publishing houses of the time believed that a substantial interest in such texts existed. What is most surprising is that, until the last decade of the fifteenth century, a prevailing shortage of Greek medical manuscripts had not at all troubled the scholarly and medical communities. This essay shows how minor a niche Galen and other Greek medical writers occupied in the (...) Introduction While quizzing during informed consent for research to ensure understanding has become commonplace, it is unclear whether the quizzing itself is problematic for potential participants. In this study, we address this issue in a multinational HIV prevention research trial enrolling injection drug users in China and Thailand. Methods Enrolment procedures included an informed consent comprehension quiz. An informed consent survey followed. Results 525 participants completed the informed consent survey (Heng County, China=255, Xinjiang, China=229, Chiang Mai, Thailand=41). Mean age was (...) 33 and mean educational level was 8 yrs. While quizzing was felt to be a good way to determine if a person understands the nature of clinical trial participation (97%) and participants did not generally find the quiz to be problematic, minorities of respondents felt pressured (6%); anxious (5%); bored (5%); minded (5%); and did not find the questions easy (13%). In multivariate analysis, lower educational level was associated with not minding the quizzing (6–10 yrs vs 0–5 yrs: OR=0.27, p=0.03; more than 11 yrs vs 0–5 yrs: OR=0.18, p=0.03). There were also site differences (Heng County vs Xinjiang) in feeling anxious (OR=0.07; p=<0.01), not minding (OR=0.26; p=0.03), being bored (OR=0.25; p=0.01) and not finding the questions easy (OR=0.10; p=<0.01). Conclusions Quizzing during the informed consent process can be problematic for a minority of participants. These problems may be associated with the setting in which research takes place and educational level. Further research is needed to develop, test and implement alternative methods of ensuring comprehension of informed consent. Trial Registration clinicaltrials.gov number NCT00270257. (shrink)

Background:Moral distress occurs when constraints prevent healthcare providers from acting in accordance with their core moral values to provide good patient care. The experience of moral distress in nurses might be magnified during the current Covid-19 pandemic.Objective:To explore causes of moral distress in nurses caring for Covid-19 patients and identify strategies to enhance their moral resiliency.Research design:A qualitative study using a qualitative content analysis of focus group discussions and in-depth interviews. We purposively sampled 31 nurses caring for Covid-19 patients in (...) the acute care units within large academic medical systems in Maryland and New York City during April to June 2020.Ethical considerations:We obtained approval from the Institutional Review Board at the University of Maryland, Baltimore.Results:We identified themes and sub-themes representative of major causes of moral distress in nurses caring Covid-19 patients. These included (a) lack of knowledge and uncertainty regarding how to treat a new illness; (b) being overwhelmed by the depth and breadth of the Covid-19 illness; (c) fear of exposure to the virus leading to suboptimal care; (d) adopting a team model of nursing care that caused intra-professional tensions and miscommunications; (e) policies to reduce viral transmission (visitation policy and PPE policy) that prevented nurses to assume their caring role; (f) practicing within crisis standards of care; and (g) dealing with medical resource scarcity. Participants discussed their coping mechanisms and suggested future strategies.Discussion/Conclusion:Our study affirms new causes of moral distress related to the Covid-19 pandemic. Institutions need to develop a supportive ethical climate that can restore nurses’ moral resiliency. Such a climate should include non-hierarchical interdisciplinary spaces where all providers can meet together as moral peers to discuss their experiences. (shrink)

To prove that membership of R or F for finite S-structures is undecidable, we reduce from a known undecidable problem—here we use the tiling problem, the partial (...) group embedding problem and the partial group finite embedding problem to prove undecidability of finite membership of R or F for various signatures S. It follows that the equational theory of R is undecidable whenever S includes the boolean operators and composition. We give an exposition of the reduction from the tiling problem and the reduction from the group embedding problem, and summarize what we know about the undecidability of finite membership of R and of F for different signatures S. I still believe that this is so and that because of this property induction is the only reasonable way of predicting. This Festschrift in Professor Kristeller’s honor consists of contributions by scholars who have had some connection with Columbia University, his "intellectual home in the United States for three decades." It also includes a Tabula Gratulatoria listing many other friends from the United States and Europe. The editor’s opening essay provides an interesting and informative account of this scholar’s academic career, and should be read together with the complete annotated bibliography of his publications through 1974. The latter lists 149 "major publications" (...) and 220 "minor publications." Kristeller’s contributions to the history of Renaissance philosophy are well known to historians of philosophy, and deservedly so. Here reference should be made to his groundbreaking studies on Marsilio Ficino and Pomponazzi, and on others such as Pico della Mirandola and Petrarch, as well as on Renaissance Platonism, Aristotelianism in the Renaissance, Thomism in the Renaissance, Paduan Averroism, and Alexandrism. But he has also contributed greatly to the fields of medieval and Renaissance history, and especially to our understanding of Renaissance humanism, Renaissance music, and Renaissance art. He is universally recognized as one of the world’s foremost authorities on manuscript research, as is witnessed, for example, by the cooperative project, Catalogus Translationum et Commentariorum, which he founded, and by his Iter ltalicum. In all of these enterprises he has set an enviable example for other scholars by the exacting standards and the breadth of his expertise. It is only fitting, then, that the many essays in this Festschrift should reflect the breadth and depth of the scholarship so evident in the man to whom they are dedicated. Three proponents of the Canberra Plan, namely Jackson, Pettit, and Smith, have developed a collective functionalist program—Canberra Functionalism—spanning from philosophical psychology to ethics. They argue that conceptual analysis is an indispensible tool for research on cognitive processes since it reveals that there are some folk concepts, like belief and desire, whose functional roles must be preserved rather than eliminated by future scientific explanations. Some naturalists have recently challenged this indispensability argument, though the point of that challenge has been blunted by (...) a mutual conflation of metaphysical and methodological strands of naturalism. I argue that the naturalist’s challenge to the indispensability argument, like naturalism itself, ought to be reformulated as a strictly methodological thesis. So understood, the challenge succeeds by showing (1) that we cannot know a priori on the basis of conceptual analysis of folk platitudes that something must occupy the functional roles specified for beliefs and desires, and (2) that proponents of Canberra Functionalism sometimes tacitly concede this point by treating substantive psychological theories as the deliverances of a priori platitudes analysis. One of the most important ongoing debates in the philosophy of mind is the debate over the reality of the first-person character of consciousness.[1] Philosophers on one side of this debate hold that some features of experience are accessible only from a first-person standpoint. Some members of this camp, notably Frank Jackson, have maintained that epiphenomenal properties play roles in consciousness [2]; others, notably John R. Searle, have rejected dualism and regarded mental phenomena as entirely biological.[3] In the opposite camp (...) are philosophers who hold that all mental capacities are in some sense computational - or, more broadly, explainable in terms of features of information processing systems.[4] Consistent with this explanatory agenda, members of this camp normally deny that any aspect of mind is accessible solely from a first-person standpoint. ABSTRACT Necropolitical arrangements of bifurcations delineate those ontological antagonisms that code Blackness as ontological lack (as non-position). In this article, I attempt to think about this evacuation of being in terms of the necropolitical’s fleshy excess, as what Alexander Weheliye’s work names “habeus viscus.” In so doing, I explore the implications, for our understanding of the “repressed proximities” of which the necropolitical consists, of arrangements that always-already include entanglements with their fleshy excess. In other words, if the nonposition of the (...) excluded is always positioned within, then living in/against the death-logics of necropolitics is always happening from that “nonposition within.” Hence, a reading of Achille Mbembe’s account of necropolitics must reckon with what Katherine McKittrick names “the creative consequences of the plot and the plantation,” with the implications of the inextricable proximal entanglements between the killing of life and the living that persists, despite. This article focuses on this “living despite.” Through a constellation of thinkers like Katherine McKittrick, Alexander Weheliye, and Zakiyyah Iman Jackson (to name a few), it aims to show that there are, perhaps, other futures that are already “now,” shattering what Édouard Glissant refers to as the “stone of time,” shattering (necropolitical) history as destiny.
