It is widely thought that genetic counselors should work with parents “nondirectively”: they should keep parents informed and support their decisions. But this view misconceives human decisionmaking by failing to recognize that value choices are constructed within and constrained by a community. Acknowledging that decisions involve interaction with and responsibility toward others leads to a “dialogical” model of counseling, in which genetic counselors may question and guide parents’ decisions.

The increase in the scope of international collaborative medical research involving human subjects is raising the problem of whether and how to maintain Western ethical standards when research is conducted in countries with very different social and ethical values. Existing international ethical guidelines for research largely reflect Western concepts of human rights, focusing on the bioethical principles of respect for persons, beneficence, and justice. Nondirective genetic counseling developed as a means of promoting informed and independent decision-making. To the extent that it minimizes risks of coercion, this counseling approach effectively respects client autonomy. However, it also permits clients to make partially informed, poorly reasoned or ethically questionable choices, and denies counselors a means of demonstrating accountability for the use of their services. These practical and ethical tensions result from an excessive focus on noncoercion while neglecting the contribution of adequate information and deliberative competence to (...) autonomous decision-making. A counseling approach that emphasizes the role of deliberation may more reliably produce thoroughly reasoned decisions. In such an approach, characterized by dialogue, counselors are responsible for ensuring that decisions are fully informed and carefully deliberated. Counseling remains nonprescriptive, but in the course of discussion counselors may introduce unsolicited information and/or challenge what they believe are questionable choices. By this means clients can be better assured that the decisions they make are fully considered, while counselors demonstrate a limited degree of professional accountability. (shrink)

Background:Moving into the last phase of life comprises a developmental transition with specific needs and risks. Facilitating transitions is an important component of the work of nurses. When curative interventions are no longer helpful, nurses enact key roles in caring for patients and families.Aim:The aim of this study was to examine the experiences of registered nurses in acute care settings as they worked with patients and families to facilitate transition to comfort-focused care.Research design:Sampling, data collection, and data analysis were guided (...) by constructivist grounded theory, chosen because of its strength in identifying and explicating social processes.Participants and context:A purposeful sample of 26 registered nurses working in acute care hospitals in one community in the northeastern United States participated in this study through semi-structured interviews.Ethical considerations:The study received approval from the university’s Institutional Review Board for the Protection of Human Subjects. Participants provided informed consent.Findings:Nurses facilitated transition to comfort-focused care by enacting their moral commitments to patients and families. They focused on building relationships, honoring patient self-determination, and maintaining respect for personhood. In this context, they discerned a need for transition, opened a discussion, and used diverse strategies to facilitate achieving consensus on the part of patients, family members, and care providers. Regardless of how the process unfolded, nurses offered support throughout.Discussion:Achievement of consensus by all stakeholders is critical in the transition to comfort-focused care. This study deepens our understanding of how nurses as moral agents utilize specific strategies to assist progress toward consensus. It also offers an example of recognizing the moral agency of nurses through listening to their voices.Conclusion:Increased understanding of effective nursing strategies for facilitating transition to comfort-focused care is essential for developing needed evidence for excellent care and strengthening end-of-life nursing education. A series of structural constitutional arguments lodged in multiple cases against Centers for Medicare and Medicaid Services’ (CMS) authorities to negotiate prescription drug prices via the 2022 Inflation Reduction Act threaten the legitimacy of CMS program and federal agency powers.

Respect for patients’ self-determination has long been considered central to efforts to improve end-of-life care, yet efforts to promote advance directives or engage patients in end-of-life discussions are often unsuccessful. In this article, I contend that this is because the shared decision-making approach typically used in healthcare assumes patients’ capacity to make rational choices, which is not always possible in end-of-life decisions. Drawing on decision theory, behavioral psychology, and related studies of endof-life care, I present a growing body of evidence (...) that suggests the novelty, complexity, and uncertainty of end-of-life circumstances make rational and stable preferences difficult to establish. I argue that an effective decision-making approach for the terminally ill must recognize and respond to the unique characteristics of endof-life choices, including their nonrational dimensions. I conclude with a description of an initiative that appears to do so, resulting in increased patients’ satisfaction. (shrink)

This paper describes the development of two annual lantern parades as case examples of arts in community health, which the authors define as a distinct area of activity operating mainly outside of acute healthcare settings, characterised by the use of participatory arts to promote health. The parades took place in Gateshead 1994–2006 and later in Stockton-on-Tees from 2009 to the present, and the paper reflects on the factors that made for the success of the Gateshead parade and also the problems (...) that led to its demise. It then describes and assesses the Stockton parade, and the benefits and challenges of a workshop ethos of ‘positive regard’ with reference to interview data gathered from adult volunteers and school staff. It considers the potential of this annual ‘tradition’ to shape communal memories that identify with place, and it sets out its aspirations for future programme and research. (shrink)

Lack of attribution and plagiarism can create a special problem for journalists. As numerous examples indicate, there is confusion about the sometimes fine line between lack of attribution and plagiarism. But there is even more confusion over how to solve the problem. Short of restructuring the journalism profession to create an overall governing body similar to the law bar, there is no way to create a set of national guidelines on when lack of attribution might become plagiarism. This article describes the spiritual formation training program for counseling students at Richmont Graduate University, an evangelical institution providing Master’s-level instruction for counselors and ministers. This model of spiritual formation has a dual foundation which includes the centrality of love to the Christian life and the importance of attachment to the development of persons. The training is intentionally designed to invite students to pursue a more secure attachment to God, healthier relationships with others, and a more grace-based self-awareness. Integrative and (...) clinical instruction, and experiences that foster establishment of secure attachment are described. Co-curricular efforts in research and student advisement focus on grace and wellness, as well as opportunities for service, and these serve to further contribute to a supportive environment for spiritual formation. Background: Nurses and midwives have a professional obligation to promote health and prevent disease, and therefore they have an essential role to play in vaccination. Despite this, some nurses and midwives have been found to take an anti-vaccination stance and promulgate misinformation about vaccines, often using Facebook as a platform to do so. Research question: This article reports on one component and dataset from a larger study – ‘the positives, perils and pitfalls of Facebook for nurses’. It explores the specific (...) issue of nurses and midwives who take an anti-vaccination stance, deemed to be unprofessional by crossing professional boundaries and by providing medical information on Facebook that is not within their scope of practice. Participants: Data were collected via an online worldwide survey from nurse and midwife participants, distributed and ‘snowballed’ through relevant nursing and midwifery groups on Facebook. In total, 1644 Registered Nurses and Midwives, and Enrolled Nurses worldwide attempted the online survey. There were 1100 completed surveys and 54 partially completed surveys. Semi-structured interviews were also conducted online using Skype® with 17 participants in Australia. Ethical considerations: Ethical processes and procedures have been adhered to relating to privacy, confidentiality and anonymity of the participants. Findings/results: A mixed-methods approach was used, including descriptive and content analysis of the quantitative survey data and thematic analysis of the qualitative interview data. The main theme ‘blurred boundaries’ was generated, which comprised three sub-themes: ‘follow the science, ‘abuse of power and erosion of trust’ and ‘the moral and ethical responsibility to safeguard public health’. The results offer an important and unique understanding of how nurses and midwives interpret the conduct of fellow health professionals as unprofessional and crossing the professional boundary if they used Facebook to promulgate anti-vaccination messages and/or give medical advice online. Conclusion: There are many positives and negatives for nurses and midwives associated with using Facebook for personal and professional communication, which is in keeping with the results of the larger study from which this article is taken. Professional behaviour is a key theme in the larger research as is the ethical construct of ‘every act has a consequence’; however, in this article, the theme ‘blurred boundaries’ offers an overall understanding of how nurses and midwives interpret the behaviour of their colleagues who espouse anti-vaccination sentiment and/or give medical advice online that is outside their scope of practice and education. The Monographs produced by the International Agency for Research on Cancer (IARC) apply rigorous procedures for the scientific review and evaluation of carcinogenic hazards by independent experts. The Preamble to the IARC Monographs, which outlines these procedures, was updated in 2019, following recommendations of a 2018 expert Advisory Group. This article presents the key features of the updated Preamble, a major milestone that will enable IARC to take advantage of recent scientific and procedural advances made during the 12 years since (...) the last Preamble amendments. The updated Preamble formalizes important developments already being pioneered in the Monographs Programme. These developments were taken forward in a clarified and strengthened process for identifying, reviewing, evaluating and integrating evidence to identify causes of human cancer. The advancements adopted include strengthening of systematic review methodologies; greater emphasis on mechanistic evidence, based on key characteristics of carcinogens; greater consideration of quality and informativeness in the critical evaluation of epidemiological studies, including their exposure assessment methods; improved harmonization of evaluation criteria for the different evidence streams; and a single-step process of integrating evidence on cancer in humans, cancer in experimental animals and mechanisms for reaching overall evaluations. In all, the updated Preamble underpins a stronger and more transparent method for the identification of carcinogenic hazards, the essential first step in cancer prevention. (shrink)

This study tests an instructional model designed to empower students in an early childhood classroom as emerging digital storytellers. Educators can use digital storytelling to support students’ learning by encouraging them to organize and express their ideas and knowledge in an individual and meaningful way while developing voice and facility in child-computer interactions. This work also helps develop traditional communication skills, fosters collaboration, and strengthens emergent literacy practices. Students may develop enhanced communications skills by learning to organize their ideas, ask (...) questions, express opinions, and construct narratives as they interact with others and computers in the creation of digital stories. The “Emerging Digital Storytellers” instructional model focuses on social-emotional development and finding student voice through writing and digital content construction in the early childhood educational context. BackgroundJunior medical doctors have a key role in discussions and decisions about treatment and end-of-life care for people with dementia in hospital. Little is known about junior doctors’ decision-making processes when treating people with dementia who have advance care directives, or the factors that influence their decisions. To describe among junior doctors in relation to two hypothetical vignettes involving patients with dementia: their legal compliance and decision-making process related to treatment decisions; the factors influencing their clinical decision-making; and the factors (...) associated with accurate responses to one hypothetical vignette. MethodA cross-sectional survey of junior doctors, including trainees, interns, registrars and residents, on clinical rotation in five public hospitals located in one Australian state. The anonymous, investigator-developed survey was conducted between August 2018 and June 2019. Two hypothetical vignettes describing patients with dementia presenting to hospital with an ACD and either: bacterial pneumonia; or suspected stroke were presented in the survey. Participants were asked to indicate whether they would commence treatment, given the ACD instructions described in each vignette.ResultsOverall, 116 junior doctors responded. In Vignette 1, 58% of respondents selected the legally compliant option. Participants who chose the legally compliant option perceived ‘following patient wishes’ and ‘legal requirements to follow ACDs’ as equally important reasons for complying with the ACD. The most common reason for not selecting the legally compliant option in Vignette 1 was the ‘ACD is relevant in my decision-making process, but other factors are more relevant’. In Vignette 2, 72% of respondents indicated they would commence treatment and 18% selected they would not commence treatment.. Similar reasons influenced participant decision-making in Vignette 2, a less legally certain scenario.ConclusionsThere are critical gaps in junior doctors’ compliance with the law as it relates to the implementation of ACDs. Despite there being differences in relation to the legal answer and its certainty, clinical and ethical factors guided decision-making over and above the law in both vignettes. More education and training to guide junior doctors’ clinical decision-making and ensure compliance with the law is required. (shrink)

Philosophers and geographers have converged on the topic of public space, fascinated and in many ways alarmed by fundamental changes in the way post-industrial societies produce space for public use, and in the way citizens of these same societies perceive and constitute themselves as a public. This volume advances this inquiry, making extensive use of political and social theory, while drawing intimate connections between political principles, social processes, and the commonplaces of our everyday environments.

Four women have been conventionally framed as wives and/or mistresses and/or sexual partners in the biographical reception of Karl Marx and Friedrich Engels as heterosexual men. These women were Jenny Marx, Helene Demuth, Mary Burns, and Lydia Burns. How exactly they appear in the few contemporary texts and rare images that survive is less interesting than the determination of subsequent biographers of the two “great men” to make these women fit a familiar genre, namely intellectual biography. An analysis of (...) Marx–Engels biographies shows how this masculinized genre enforces an incuriosity that makes gendered political partnerships unthinkable and therefore invisible. By contrast, a positive interest in these women, which rethinks what a gendered political partnership is or could be, results in a significantly different view of the two men. As historical figures, they shift from being individualized or paired‐with‐each‐other “great thinkers” to communist/socialist activists working in and through everyday spaces and material practices. Their pamphlets, articles, and books thus appear more as immediate political interventions and less as timeless theorizing or as the raw material for such intellectualizing reconstructions. (shrink)

This piece argues that Fanon’s Black Skin, White Masks inscribes the social and psychological experience of the African Diaspora within the conceptual purview of the western sciences by the means of psychoanalytical and philosophical concepts. The upshots of Fanon’s goal are twofold. Its first implication is that in employing psychoanalytical and philosophical lingo, Fanon commits to delineating a distinct tenet of self-determination for the African Diaspora. Such tenet of self-determination consists in a set of norms, beliefs, socio-cultural, and political (...) practices. Secondly, besides the stated goal in the Introduction, namely to ‘liberate the black individual from herself,’ Fanon is attempting to alter the European perception of black communities as sexual and biological threats. Accordingly, this piece concludes that Fanon’s successful inscription of the psychological and lived experiences of the African Diaspora in the western sciences, via his psychoanalytical and philosophical rendition, is hampered by the European perception of black bodies which prevents their complete scientific conceptualization. Though I doubt it has put a Rolls Royce in anybody’s garage, the criticism industry is a reality not to be overlooked. Academics have a responsibility to stay self-aware and self-critical about their own and their profession’s interests. All academic activity has a careerist dimension, but it obviously cannot be explained by that dimension alone, and in this sense Fromm’s point is simply reductive. But of course it is not all academic activity that Fromm is objecting to, only some and (...) notably mine.The image of academic colonization suggests one has stepped beyond some legitimate borders and laid claim to territory rightfully inhabited by others. Whose world was invaded by my essay, or by the “Race,” Writing, and Difference issue in general? Mr. Fromm’s, evidently. Fromm wants a world where words stand still and refer, and don’t get changed. In particular, to use his own examples, he wants a world where blacks are blacks, whites are whites, Americans are Americans, knives are knives, brothers are brothers, and Indians are Indians . During much of U.S. history, Black women had higher employment rates than white women. But by the late twentieth century, women in more privileged racial/ethnic, national origin, and education groups were more likely to work for pay. The authors compare the employment of white women to Blacks and three groups of Latinas—Mexicans, Cubans, and Puerto Ricans—and explain racial/ethnic group differences. White women work for pay more weeks per year than Latinas or Black women, although the gaps are (...) small for all groups but Mexicans. In all groups, education encourages and children reduce employment. Having a husband does not reduce employment, and husbands’ earnings have little effect. The higher fertility of Mexicans and the large number of recent immigrants among Mexican women reduce their employment relative to that of white women. The higher education of white women explains large shares of the employment gap with each group of women of color because, in today’s labor market, education strongly predicts employment. (shrink)

Most of (...) the botanists asserted that taxonomy was a practical matter to be kept distinct from phylogenetic speculation, and most of the zoologists insisted that taxonomists must strive to represent evolution if they wished to be scientific. An important theme in the philosophy of education community in recent years has been the way in which philosophy can be brought to illuminate and evaluate research findings from the landscape of policy and practice. Undoubtedly, some of these practices can be based on spurious evidence, yet have mostly been left unchallenged in both philosophical and educational circles. One of the newer practices creeping into schools is that of ‘No best friend’ policies. In some schools, this is interpreted as suggesting (...) that children should not have just one best friend but a group of good friends. In others, it is interpreted as suggesting that children should forgo having best friends altogether and be friends with everyone. What is common to both is that friendship is seen as somehow ‘dangerous’. This article offers a preliminary examination of what has been referred to as this ‘dark side’ of friendship. Whilst philosophers such as Patricia White have previously alluded to its existence, there has been little philosophical scrutiny in any broad terms elsewhere. I examine three common arguments commonly used to justify ‘No best friends’ practices: that children can be friends with everyone; that young children are developmentally incapable of ‘real’ friendship hence best friendship should be avoided until later age; that only good people can be good friends. I indicate why this unreflective adoption of practices matters so much and why we should be prepared to challenge these cases. I identify practices that we have good evidence to support as making a positive difference in this area. (shrink)

The part anger plays in motivating political action is frequently noted, but less is said about ways in which anger continues to be a part of how people do politics. This article critically assesses approaches to emotions that emphasize managing anger in accordance with ‘feeling rules’. It reflects on the utility of Marxist notions of conflict as the engine of change for the understanding of how anger operates in political life. This involves understanding the ambivalence of anger and its operation (...) within particular power relations. Shifting sets of conventions have had some continuity in discouraging women in Western nations (particularly white and middle-class women) from showing anger. But clearly, women do get angry and feminists have drawn on anger in acting politically. New Zealand feminist writings from the ‘second wave’ are taken as illustrative of the common difficulties Western feminists faced in dealing with anger. These difficulties were due to trying to juggle social conventions about femininity, the political ideal of sisterhood, and the realities of dealing with other women in often new and experimental political processes. Anger could be personalized and hurtful, but in certain cases its expression was constructive in producing more respectful relations. Exploring these cases makes it clear how anger produces, and is produced by, relations with others. (shrink)

The task of giving some sort of interpretation of Wittgenstein's Remarks on Colour is an extraordinarily difficult one. The book is exceptionally fragmentary. Many of the remarks seem to raise questions that are then left completely unanswered, or to invite us to imagine various circumstances that are then left without any further comment. Although nearly all the remarks are related in one way or another to the problem of colour, the range of topics that Wittgenstein touches on is extremely wide, (...) and covers areas that are not normally mentioned in contemporary philosophical discussions of colour. For example, apart from the familiar ‘Why can't there be a transparent white?’ and ‘Why can't there be a reddish-green?’, he asks ‘Can a transparent piece of glass have the same colour as an opaque piece of paper?’, ‘Is white always the lightest colour?’, ‘Do I see blond hair in the black and white photograph of a blond youth?’, ‘Does it make sense to point to a colour in the iris of a Rembrandt eye and ask for the walls of my room to be painted the same colour?’, ‘Do the colour-blind have the same concept of colour-blindness as the normally sighted?’, ‘Can normal vision be described?’, and so on.
