This paper examines the role of clinical practitioners and clinical researchers internationally in establishing the utility of harm-reduction approaches to substance use. It thus illustrates the potential for clinicians to play a pivotal role in health promoting structural interventions based on harm-reduction goals and public health models. Popular media images of drug use as uniformly damaging, and abstinence as the only acceptable goal of treatment, threaten to distort clinical care away from a basis in evidence, which shows that some ways (...) of using drugs are far more harmful than others and that punitive approaches and insistence on total abstinence as the only goal of treatment often increases the harms of drug use rather than reducing drug use. Therefore the leadership and scientific authority of clinicians who understand the health impact of harm-reduction strategies is needed. Through a review of harm-reduction interventions in Canada, the United Kingdom, the United States, Australia, Switzerland, and the Netherlands, we identify three ways that clinicians have helped to achieve a paradigm shift from punitive approaches to harm-reduction principles in clinical care and in drug policy: through clinical research to provide data establishing the effectiveness and feasibility of harm-reduction approaches, by developing innovative clinical programmes that employ harm reduction, and thereby changing the standard of care to include routine use of these evidence-based approaches in their practices. We argue that through promotion of harm-reduction goals and methods, clinicians have unique opportunities to improve the health outcomes of vulnerable populations. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Locke on the Knowledge of Material Things ROBERT FENDEL ANDERSON IT IS nOT John Locke's intention, in his Essay Concerning Human Understanding, to deal with matter and material substance nor with how these are able to affect the mind. These are considerations for natural philosophy; Locke counts himself rather among the moral philosophers. He does not propose, therefore, to meddle with the physical aspects of the mind, (...) nor with the physical production of sensations through the organs, nor with the production of ideas in the understanding, nor even with whether ideas depend upon matter for their source. He defers to the wisdom of such men as Boyle, Sydenham, Huygens and Newton in these considerations, and finds it "... ambition enough to be employed as an under-labourer in clearing the ground a little, and removing some of the rubbish that lies in the way to knowledge....,, 1 It is true that Locke's Essay is characterized chiefly not by an interest in matter and the mechanics of perception but rather by an interest in epistemology and in the ordering and assessment of our knowledge. Because of the very nature and emphasis of this epistemology, however, Locke finds that he must, from time to time, at least mention the nature of matter and how it affects us. Finally, it becomes apparent, certain changes or ambiguities in his point of view regarding the mode of our knowledge and its certainty are consequences of his understanding of matter. It is a demonstration of these elements of his doctrine, which seem not to have obtained the full attention of such scholars as Aaron, Fraser, and Gibson, that I shall attempt in this paper. Fundamentally, Locke's theory of matter appears to be that of Democritus and Epicurus. Like the Greek atomists, he conceives of matter as consisting of particles or corpuscles infinite in number. At least some of these are so minute as to be individually insensible, and probably he intends that all are, although, to my knowledge, he never states that this is so. In the aggregate, in any case, particles do constitute perceptible bodies. As for the Greeks, so also for Locke, the particles arc all extended, having size and shape. They are said also to possess motion or rest, although he takes care to say that matter cannot produce motion in itself,e Locke frequently mentions other properties as being "primary" or "really in" material things. The ethical theories employed in health care today assume, in the main, a modern Western philosophical framework. Yet the diversity of cultural and religious assumptions regarding human nature, health and illness, life and death, and the status of the individual suggest that a cross-cultural study of health care ethics is needed. A Cross-Cultural Dialogue on Health Care Ethics provides this study. Heyes's literature review of deception, imitation, and self-recognition is inadequate, misleading, and erroneous. The anaesthetic artifact hypothesis of self-recognition is unsupported by the data she herself examines. R. Lanier Anderson presents a new account of Kant's distinction between analytic and synthetic judgments, and provides it with a clear basis within traditional logic. He reconstructs compelling claims about the syntheticity of elementary mathematics, and re-animates Kant's arguments against traditional metaphysics in the Critique of Pure Reason.

‘The problem with simulations is that they are doomed to succeed.’ So runs a common criticism of simulations—that they can be used to ‘prove’ anything and are thus of little or no scientific value. While this particular objection represents a minority view, especially among those who work with simulations in a scientific context, it raises a difficult question: what standards should we use to differentiate a simulation that fails from one that succeeds? In this paper we build on a structural (...) analysis of simulation developed in previous work to provide an evaluative account of the variety of ways in which simulations do fail. We expand the structural analysis in terms of the relationship between a simulation and its real-world target emphasizing the important role of aspects intended to correspond and also those specifically intended not to correspond to reality. The result is an outline both of the ways in which simulations can fail and the scientific importance of those various forms of failure. (shrink)

Cells fine‐tune their DNA repair, selecting some regions of the genome in preference to others. In the paradigm case, excision of UV‐induced pyrimidine dimers in mammalian cells, repair is concentrated in transcribed genes, especially in the transcribed strand. This is due both to chromatin structure being looser in transcribing domains, allowing more rapid repair, and to repair enzymes being coupled to RNA polymerases stalled at damage sites; possibly other factors are also involved. Some repair‐defective diseases may involve repair‐transcription coupling: three (...) candidate genes have been suggested.However, preferential excision of pyrimidine dimers is not uniformly linked to transcription. In mammals it varies with species, and with cell differentiation. In Drosophila embryo cells it is absent, and in yeast, the determining factor is nucleosome stability rather than transcription.Repair of other damage departs further from the paradigm, even in some UV‐mimetic lesions. No selectivity is known for repair of the very frequent minor forms of base damage. And the most interesting consequence of selective repair, selective mutagenesis, normally occurs for UV‐induced, but not for spontaneous mutations. The temptation to extrapolate from mammalian UV repair should be resisted. (shrink)

Akira Kurosawa is arguably known as the director who opened up Japanese film to Western audiences and following his death in 1998, a process of reflection has begun about his life's work as whole and its legacy to the cinema and its global audiences. Rashomon has arguably become the best known Japanese film, ever. After this, his twelfth film, Kurosawa's reputation was firmly established in international cinema, and Rashomon continues to be discussed and imitated more than sixty years after its (...) first screening. This volume addresses issues beyond the realm of Rashomon within film studies, and to do with the Rashomon effect which itself has become a widely recognized English term referring to significantly different perspectives and interpretations of different eyewitnesses to the same dramatic event. The dual figures of ripples and circles comprise the organizing image and principle of this book as each chapter addresses either the movement of ripples, representing the continuing and vibrant influence of Rashomon effects into the twenty-first century, or of circles, representing specific events, such as the publication of a new script, a particular production, or a remake. Clinical study of envy and jealousy in the psychotherapy situation indicates that these two states of mind are biopsychosocial response patterns involving the perceptual, cognitive, affective, and intentional mentalfunctions. These response patterns are evoked by perceptual events that inform the individual of one’s relative position vis-à-vis the requirements of one s life. Once these patterns can be discerned in the patient, the clinician is able to hear things in the psychotherapy situation not previously heard and understand, interpret, and work through (...) conflict in a new and useful way. In this paper psychotherapy paradigms and strategies are discussed from the new perspective afforded by the psychology of envy and jealousy. (shrink)

This essay describes possible futures that may result from accelerating technological advances and the challenges these futures present to psychotherapists. In the next 100 years, human beings will be likely to increasingly use computers and artificial intelligence and become extremely dependent on this relationship. Chip and stem cell implants may provide people with greater memory capacity, computational capacity, and skill sets. Genetic engineering, cryonics, and cloning may allow dramatic increases in the human life span these developments occur, they will challenge (...) what it means to be a human being and have a soul. Although psychotherapists will acquire new tools for assessment and intervention, they will face the daunting task of addressing these changes and challenges to personhood with their clients. (shrink)
