Deficiency of dominant response formation in hebephrenic schizophrenia

Cognitive deficits, characteristic for schizophrenia, are often associated with an impairment of executive functions, especially the inability of volitional behavior control. On the contrary, many patients with hebephrenic schizophrenia exhibit successful supervisional activity control in their behavior as well as test results. By this, they even compensate for their basic deficit. With evidence from presented cases, we show that they have a deficit in the formation of the internal context which directs the current action. The context itself is established on the basis of dominant responses, gained previously and from experience. We hypothesize that the inability to discount the dominant response in a given situation represents the original failure. Results we have obtained from neuropsychological tests confirm this hypothesis, e.g. on Stroop’s color-word tasks these patients acquire supra-normal results, and in Hayling’s trials, where the completion of phrases with inappropriate words is required, their reaction times are shorter and the degree of semantic integration is lower. In contrast, on tests, where taking context into account is necessary for better achievements, these patients are less successful. Further empirical research is needed to support our preliminary findings.
