Qualitative validation consists in showing that a model is able to mimic available observed data. In population level biological models, the available data frequently represent a group status, such as pool testing, rather than the individual statuses. They are aggregated. Our objective was to explore an approach for qualitative validation of a model with aggregated data and to apply it to validate a stochastic model simulating the bovine viral-diarrhoea virus (BVDV) spread within a dairy cattle herd. Repeated measures of the (...) level of BVDV-specific antibodies in the bulk-tank milk (total milk production of a herd) were used to summarise the BVDV herd status. First, a domain of validation was defined to ensure a comparison restricted to dynamics of pathogen spread well identified among observed aggregated data (new herd infection with a wide BVDV spread). For simulations, scenarios were defined and simulation outputs at the individual animal level were aggregated at the herd level using an aggregation function. Comparison was done only for observed data and simulated aggregated outputs that were in the domain of validation. The validity of our BVDV model was not rejected. Drawbacks and ways of improvement of the approach are discussed. (shrink)

This research questions the prevention and health promotion practices developed for young people, in France, and re-examines the nature of competences enlisted in the classical model of empowerment in health education. Moving away from the epidemiological postulate of evidence based medicine (EBM), it explores another approach of these educational practices. The research uses the perspective of narrative mediation; it tries also to identify issues about a better knowledge of oneself, making more comprehensive the issues of young students’ health.

Texting while walking is a dangerous behavior that can lead to injury and even death. While several studies have examined the relationship between smartphone use and stress, to our knowledge no studies have yet investigated the relationship between stress and TWW. The objective of the present study was to investigate this relationship by examining the effects of stress on TWW, the effects of TWW on subsequent stress, and the effect of stress on multitasking performance. A total of 80 participants completed (...) two sequential tasks in a laboratory while they walked on a treadmill and responded to a biological motion stimulus imitating the movement of another pedestrian. In the unrestricted task, participants were given the choice to use their personal phones. In the controlled task, they carried a text conversation with a research assistant while they walked and responded to the stimulus. Stress was measured via questionnaire and saliva collection for measure of cortisol before and after each task. Results show that greater psychological stress and cortisol variations were associated with a greater number of phone uses during the unrestricted task. Greater phone use during the unrestricted task was associated with lower subsequent psychological stress in women and total time of phone use was correlated with subsequent cortisol levels. Stress measured before the controlled task had no effect on multitasking performance, but participants with moderate performance were those with the highest cortisol levels. Our results suggest that stress could be a precursor to TWW and that it could affect a pedestrian’s ability to stay safe when using their smartphone. (shrink)

Digital communication between a patient and their clinician offers the potential for improved patient care, particularly for young people with long term conditions who are at risk of service disengagement. However, its use raises a number of ethical questions which have not been explored in empirical studies. The objective of this study was to examine, from the patient and clinician perspective, the ethical implications of the use of digital clinical communication in the context of young people living with long-term conditions. (...) A total of 129 semi-structured interviews, 59 with young people and 70 with healthcare professionals, from 20 United Kingdom -based specialist clinics were conducted as part of the LYNC study. Transcripts from five sites were read by a core team to identify explicit and implicit ethical issues and develop descriptive ethical codes. Our subsequent thematic analysis was developed iteratively with reference to professional and ethical norms. Clinician participants saw digital clinical communication as potentially increasing patient empowerment and autonomy; improving trust between patient and healthcare professional; and reducing harm because of rapid access to clinical advice. However, they also described ethical challenges, including: difficulty with defining and maintaining boundaries of confidentiality; uncertainty regarding the level of consent required; and blurring of the limits of a clinician’s duty of care when unlimited access is possible. Paradoxically, the use of digital clinical communication can create dependence rather than promote autonomy in some patients. Patient participants varied in their understanding of, and concern about, confidentiality in the context of digital communication. An overarching theme emerging from the data was a shifting of the boundaries of the patient-clinician relationship and the professional duty of care in the context of use of clinical digital communication. The ethical implications of clinical digital communication are complex and go beyond concerns about confidentiality and consent. Any development of this form of communication should consider its impact on the patient-clinician-relationship, and include appropriate safeguards to ensure that professional ethical obligations are adhered to. La modernité, dès ses débuts, attribue un rôle-clef aux passions : qu’elles soient hostiles à la Raison ou au contraire ses alliées, dangereuses ou fascinantes, elles marquent le rôle du corps, du désir, du langage et de l’imagination dans la nature de l’homme. La même époque voit se développer différentes variantes du matérialisme. Presque toutes réévaluent ce que la raison classique avait tendance à réprimer ou à considérer comme révélateur de la faiblesse humaine : le corps et tout ce qui, (...) dans l’âme ou dans la société, porte les traces de l’activité et de la positivité du corps. On peut donc s’attendre à ce que les matérialistes fassent un sort particulier aux passions, à ce qu’ils y reconnaissent des lois et non pas seulement des manques ou des vices, à ce qu’ils essaient d’en repérer l’efficace dans l’ensemble des activités humaines. Encore faut-il se demander comment chaque matérialisme procède, par quelle configuration propre il rend compte de ces phénomènes ou comment il dévie les discours de la théorie classique pour les forcer à passer par les objets qui sont les siens. Plutôt que de supposer l’existence d’une théorie matérialiste unique, une enquête s’impose donc qui prenne en vue la diversité de ces auteurs, les situe dans leur contexte et repère les points d’inflexion que rencontre chez chacun d’entre eux cette problématique générale des passions qui semble avoir gouverné plusieurs siècles. Since the 1980s, there has been an increasing acknowledgement of the importance of recognising the ethical dimension of clinical decision-making. Medical professional regulatory authorities in some countries now include ethical knowledge and practice in their required competencies for undergraduate and post graduate medical training. Educational interventions and clinical ethics support services have been developed to support and improve ethical decision making in clinical practice, but research evaluating the effectiveness of these interventions has been limited. We undertook a systematic review of (...) the published literature on measures or models of evaluation used to assess the impact of interventions to improve ethical decision making in clinical care. We identified a range of measures to evaluate educational interventions, and one tool used to evaluate a clinical ethics support intervention. Most measures did not evaluate the key impact of interest, that is the quality of ethical decision making in real-world clinical practice. We describe the results of our review and reflect on the challenges of assessing ethical decision making in clinical practice that face both developers of educational and support interventions and the regulatory organisations that set and assess competency standards. Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making.Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments.DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system.Trial RegistrationISRCTN89993391. Immortalité de l'âme, intelligence, distinction des substances, vie éternelle: ces questions, qui sont au coeur de la réflexion sur la nature de l'être humain, ont passionné les philosophes et les lettrés du Grand Siècle et du Siècle des Lumières. Un débat animé, polémique, s'est noué dans toute l'Europe savante et, notamment, de part et d'autre de la Manche. Avec pour fil conducteur l'émergence d'une conception laïque et matérielle de l'être humain, Ann Thomson nous fait partager l'effervescence féconde de ces échanges (...) et de ces débats. Elle nous initie aux polémiques théologico-politiques qui ont nourri la "crise de la conscience européenne" et ses prolongements, jusqu'au milieu du XVIIIesiècle. Consacrée à l'Angleterre après la Glorieuse Révolution de 1688, la première partie analyse les luttes idéologiques et religieuses de la fin du XVIIe et du début du XVIIIe siècle, replacées dans leur contexte historique, philosophique et scientifique, avant d'étudier la controverse sur Dieu, sur l'âme et sur la nature humaine. La deuxième partie, centrée sur les "premières Lumières" françaises, suit le cheminement des échos de ces controverses anglaises transmis par les passeurs huguenots et les voies clandestines de la circulation philosophique, puis analyse la spéculation scientifique et philosophique dans la première moitié du XVIIIe siècle. Ces vifs débats ont fait naître de multiples courants qui ont permis l'éclosion et l'épanouissement d'une pensée matérialiste qui s'affirma à partir des années 1760, dans le cadre, notamment, dune réflexion sur la génération. En soulignant le rôle joué par les polémiques théologiques protestantes mais également par la recherche médicale, cet essai d'histoire intellectuelle, transnationale, nous transporte au coeur de la philosophie des Lumières, et des "Lumières radicales". En même temps, il s'inscrit clans une histoire de longue durée, car les questions soulevées par ce livre continuent aujourd'hui d'interpeller les théologiens, les philosophes, les anthropologues, les neurobiologistes. Les penseurs des Lumières furent aux avant-postes d'une vertigineuse interrogation sur l'identité et la nature de l'homme. Michel Foucault’s _History of Sexuality_ has been one of the most influential books of the last two decades. It has had an enormous impact on cultural studies and work across many disciplines on gender, sexuality, and the body. Bringing a new set of questions to this key work, Ann Laura Stoler examines volume one of _History of Sexuality_ in an unexplored light. She asks why there has been such a muted engagement with this work among students of colonialism for whom (...) issues of sexuality and power are so essential. Why is the colonial context absent from Foucault’s history of a European sexual discourse that for him defined the bourgeois self? In _Race and the Education of Desire, _Stoler challenges Foucault’s tunnel vision of the West and his marginalization of empire. She also argues that this first volume of _History of Sexuality_ contains a suggestive if not studied treatment of race. Drawing on Foucault’s little-known 1976 College de France lectures, Stoler addresses his treatment of the relationship between biopower, bourgeois sexuality, and what he identified as “racisms of the state.” In this critical and historically grounded analysis based on cultural theory and her own extensive research in Dutch and French colonial archives, Stoler suggests how Foucault’s insights have in the past constrained—and in the future may help shape—the ways we trace the genealogies of race. _Race and the Education of Desire_ will revise current notions of the connections between European and colonial historiography and between the European bourgeois order and the colonial treatment of sexuality. Arguing that a history of European nineteenth-century sexuality must also be a history of race, it will change the way we think about Foucault. (shrink)

Geography and Empire re-examines the role of geography in imperialism and reinterprets the geography of empire. It brings together new work by eighteen geographers from ten countries. The book is divided into five parts. Part I considers the early engagement of geographers with the imperial adventures of England and France. Part II focuses on the links between nineteenth-century European imperial expansion and the establishment of the first geographical institutions. Dégénérescence de la culture, mépris pour les grandes œuvres, délire interprétatif, amphigouri linguistique, danger politique, confusion sexuelle, licence morale : à en croire les ennemis de la déconstruction, tout ce qui va' (...) mal dans le monde lui est imputable. The purpose of this article is to show that in Merleau-Ponty’s lesser known works, one can find a path leading toward a phenomenology of the body that would not risk the “ambiguity of the flesh,” as The Visible and the Invisible is often charged with, but instead would sustain the ontology of nature that one finds in the “Working Notes” added to Merleau-Ponty’s last writings. Analyzing first his concept of nature, as it was developed in his courses at the Collège (...) de France, the proposed aim of this text is to question anthropological difference by focusing on Merleau-Ponty’s interpretation of the contemporary sciences, whereby the Ineinander between the physical, living, and human orders is re-founded by a new, negative concept of nature. By deconstructing classical ontology and its understanding of space, time, and objectivity, and by returning to the domain of perception, this new concept offers a novel way to reveal the advent of life in nature, its institution, and the emergence of a new corporeity in life itself. The model of Merleau-Ponty’s essential concept of institution becomes birth, and therefore the human body will no longer be interpreted as “one’s own body,” but rather will be understood, in a radically a-subjective way, as from, and based on, the participation of our perception with animality. Thereby, the concept of “ownness” becomes secondary; however, the notion of institution, established against the lexicon of consciousness and subjectivity, opens another sense of anthropological difference, one that is based only on the experiences of corporeity. The American term Bioethics has been adopted over the last ten years and the development of Bioethics committees on the American model testifies this influence, even before the official appointment of a National Committee in 1983. This phenomenon acknowledged as the emergence of French bioethics is in fact the final outcome of a long-lasting crisis in the medical profession, in quest for a new style of ethics, breaking with the traditional professional ethics (French Déontologie, through the Ordre des Médecins). Among (...) other factors of conceptual and institutional change, the increase of biomedical research comes first: a major consequence is the sharing of moral responsibilities in decision-making with outsider scientists and finally the involvement of the whole population as potential moral subjects.The designation of these events as the emergence of French bioethics is hardly appropriate for an account of this dramatic shift in ethical norms and roles in medicine. This paper attempts to review the intellectual roots of the recent evolution and to summarize present and prospective trends. (shrink)

According to Diderot's article "Art" in the Encyclopédie, secrecy is frequently related to guilds and refers to monopolizing practices and "routine". It appears therefore as an obstacle to innovation. But may we consider secrecy exclusively in this negative light? Studies on mining and metallurgy provide us with a different perspective, for they show how the protection of secrets from the gaze of outsiders also allowed for technical exchanges between the craftsmen themselves and thus implied a certain measure of technical openness. (...) From this point of view, it seemed interesting to look also at the urban craftsmen's practices, the so-called "Metiers jurés." A micro-historical study of guilds in a small town of mid-eighteenth century France, presented in this essay, shows that things were indeed in a state of flux. On the one hand, the very ritualistic practice of producing a chef d'aeuvre, which included even the temporary confinement of the candidate, was still in existence. Secrecy still formed a part of the qualifying process of urban artisans. On the other hand, both the ritual and the chef d'aeuvre were undergoing change, the ritual developing more slowly than its content. In the process, secrecy tended to become pure dissimulation, allowing in fact the spread of a practice like subcontracting, which was officially prohibited. (shrink)

"Si le "jargon de l'authenticité" a sans doute rendu nombre de phénoménologies incapables de penser l'histoire et le politique, Merleau-Ponty a su le faire au coeur des textes étrangement les moins commentés, dans leur lien même, ceux des cours au Collège de France de 1954-1955: L'Institution et La Passivité. Des cours que cet ouvrage se propose enfin de relire, en discussion serrée avec l'ensemble de l'oeuvre merleau-pontienne, tant celle qui les précède que celle qui leur succède, pour la dévoiler (...) autrement. Cela, en trois temps, qui sont autant de "lectures de Merleau-Ponty", et de portes d'entrée dans son oeuvre la moins connue. Trois temps formant les parties de cet ouvrage qui suivent l'élaboration du concept merleau-pontien d'institution dans son rapport même à la passivité, sans chronologie fixe, et sans voeu de systématicité, mais en tentant d'être fidèle à la rédaction même de ces notes de cours d'une rare densité. Des cours qui foisonnent de références, tant à son oeuvre propre qu'aux nombreux auteurs avec lesquels Merleau-Ponty dialogue, tissant ainsi de nouveaux liens thématiques, et n'hésitant pas, par exemple, au coeur même d'un chapitre sur le vivant, à évoquer la question de la révolution marxiste. De Ruyer, Gesell, Lévi-Strauss, Alquié, Lachièze-Rey, à Sartre et Freud, etc., ce sont autant de dialogues qui éclairent en effet l'oeuvre et l'auto-critique de Merleau-Ponty, qui se confronte ici, mieux que nulle part ailleurs, à l'échec de son oeuvre passée, comme au contexte de l'époque. For nearly a century, the French have entertained an unshakable conviction that their ability to recognize themselves-to know and transmit the essence of Frenchness-depended on the teaching of the history of France. In effect, history was a discourse on France, and the teaching of history-"la pédagogie centrale du citoyen"-the means by which children were constituted as heirs and carriers of a common collective memory that made them not only citizens, but family. In this essay, I examine the rhetorical (...) and conceptual effects on history writing that emerge out of this preoccupation with the elaboration of a continuous, coherent national identity. Focusing on schoolbooks, I begin by looking at the dominant, nearly hegemonic model of French history created by Ernest Lavisse in the 1890s-a model informed by the dream of a unified, unitary French nation, embodied in and articulated through the history of France-and at the disruption of this paradigm in the aftermath of the Great War. I then consider a text written in the 1990s specifically to repudiate the kind of nationalist narratives that prevailed for most of this century-a new supranational history of Europe. I argue that, in their different experiments with fixing history, both Lavisse and the contemporary textbook authors did not so much repair a deficient history as produce a historical fixation, creating mythicized histories that are complete, closed, predictable, and at bottom ahistorical. Finally, I turn to a recent World War I novel, A Very Long Engagement by Sébastien Japrisot, in order to suggest ways in which the narrative strategies of a fiction writer may be useful to historians in thinking about a different kind of historical project.
