Background In May 2020, England moved to an opt-out organ donation system, meaning adults are presumed to be an organ donor unless within an excluded group or have opted-out. This change aims to improve organ donation rates following brain or circulatory death. Healthcare staff in the UK are supportive of organ donation, however, both healthcare staff and the public have raised concerns and ethical issues regarding the change. The #options survey was completed by NHS organisations with the aim of understanding (...) awareness and support of the change. This paper analyses the free-text responses from the survey. Methods The #options survey was registered as a National Institute of Health Research (NIHR) portfolio trial [IRAS 275992] 14 February 2020, and was completed between July and December 2020 across NHS organisations in the North-East and North Cumbria, and North Thames. The survey contained 16 questions of which three were free-text, covering reasons against, additional information required and family discussions. The responses to these questions were thematically analysed. Results The #options survey received 5789 responses from NHS staff with 1404 individuals leaving 1657 free-text responses for analysis. The family discussion question elicited the largest number of responses (66%), followed by those against the legislation (19%), and those requiring more information (15%). Analysis revealed six main themes with 22 sub-themes. Conclusions The overall #options survey indicated NHS staff are supportive of the legislative change. Analysis of the free-text responses indicates that the views of the NHS staff who are against the change reflect the reasons, misconceptions, and misunderstandings of the public. Additional concerns included the rationale for the change, informed decision making, easy access to information and information regarding organ donation processes. Educational materials and interventions need to be developed for NHS staff to address the concepts of autonomy and consent, organ donation processes, and promote family conversations. Wider public awareness campaigns should continue to promote the positives and refute the negatives thus reducing misconceptions and misunderstandings. Brains, it has recently been argued, are essentially prediction machines. They are bundles of cells that support perception and action by constantly attempting to match incoming sensory inputs with top-down expectations or predictions. This is achieved using a hierarchical generative model that aims to minimize prediction error within a bidirectional cascade of cortical processing. Such accounts offer a unifying model of perception and action, illuminate the functional role of attention, and may neatly capture the special contribution of cortical processing to (...) adaptive success. This target article critically examines this approach, concluding that it offers the best clue yet to the shape of a unified science of mind and action. Sections 1 and 2 lay out the key elements and implications of the approach. Section 3 explores a variety of pitfalls and challenges, spanning the evidential, the methodological, and the more properly conceptual. In "Mathematical Truth", Paul Benacerraf articulated an epistemological problem for mathematical realism. His formulation of the problem relied on a causal theory of knowledge which is now widely rejected. But it is generally agreed that Benacerraf was onto a genuine problem for mathematical realism nevertheless. Hartry Field describes it as the problem of explaining the reliability of our mathematical beliefs, realistically construed. In this paper, I argue that the Benacerraf Problem cannot be made out. There simply is no intelligible problem (...) that satisfies all of the constraints which have been placed on the Benacerraf Problem. The point generalizes to all arguments with the structure of the Benacerraf Problem aimed at realism about a domain meeting certain conditions. Such arguments include so-called "Evolutionary Debunking Arguments" aimed at moral realism. I conclude with some suggestions about the relationship between the Benacerraf Problem and the Gettier Problem. (shrink)

To what extent are the subjects of our thoughts and talk real? This is the question of realism. In this book, Justin Clarke-Doane explores arguments for and against moral realism and mathematical realism, how they interact, and what they can tell us about areas of philosophical interest more generally. He argues that, contrary to widespread belief, our mathematical beliefs have no better claim to being self-evident or provable than our moral beliefs. Nor do our mathematical beliefs have better claim to (...) being empirically justified than our moral beliefs. It is also incorrect that reflection on the "genealogy" of our moral beliefs establishes a lack of parity between the cases. In general, if one is a moral antirealist on the basis of epistemological considerations, then one ought to be a mathematical antirealist as well. And, yet, Clarke-Doane shows that moral realism and mathematical realism do not stand or fall together -- and for a surprising reason. Moral questions, insofar as they are practical, are objective in a sense that mathematical questions are not. Moreover, the sense in which they are objective can be explained only by assuming practical anti-realism. One upshot of the discussion is that the concepts of realism and objectivity, which are widely identified, are actually in tension. Another is that the objective questions in the neighborhood of questions of logic, modality, grounding, and nature are practical questions too. The diagnostic category of borderline personality disorder (BPD) has come under increasing criticism in recent years. In this paper, we analyze the role and impact of epistemic injustice, specifically testimonial injustice, in relation to the diagnosis of BPD. We first offer a critical sociological and historical account, detailing and expanding a range of arguments that BPD is problematic nosologically. We then turn to explore the epistemic injustices that can result from a BPD diagnosis, showing how they can lead to experiences (...) of testimonial injustice which impede patient engagement in meaning-making activities, thereby undermining standard therapeutic goals. We conclude by showing how our arguments bolster ongoing efforts to replace the diagnostic category of BPD with alternatives such as complex post-traumatic stress disorder. (shrink)

Although there is now a substantial body of literature exploring the effects of language diversity in international management contexts, little attention has been paid to the ethical dimensions of language diversity at work. This conceptual paper draws on the concept of epistemic injustice in order to explore how language, and in particular corporate language policies, may act as a source of epistemic injustice within the workplace. It demonstrates how language competence affects credibility judgements about a speaker, and also considers how (...) corporate language policies can create situations of hermeneutic injustice, in which marginalised groups are denied the vocabularies to understand their own experiences. Finally, ways in which such epistemic harms can be reduced are discussed, and the possibilities for management education to create epistemically responsible managers are highlighted. (shrink)

Background:Moral distress is a complex phenomenon frequently experienced by critical care nurses. Ethical conflicts in this practice area are related to technological advancement, high intensity work environments, and end-of-life decisions.Objectives:An exploration of contemporary moral distress literature was undertaken to determine measurement, contributing factors, impact, and interventions.Review Methods:This state of the science review focused on moral distress research in critical care nursing from 2009 to 2015, and included 12 qualitative, 24 quantitative, and 6 mixed methods studies.Results:Synthesis of the scientific literature revealed (...) inconsistencies in measurement, conflicting findings of moral distress and nurse demographics, problems with the professional practice environment, difficulties with communication during end-of-life decisions, compromised nursing care as a consequence of moral distress, and few effective interventions.Conclusion:Providing compassionate care is a professional nursing value and an inability to meet this goal due to moral distress may have devastating effects on care quality. Further study of patient and family outcomes related to nurse moral distress is recommended. (shrink)

Galen Strawson has published several versions of an argument to the effect that moral responsibility is impossible, whether determinism is true or not. Few philosophers have been persuaded by the argument, which Strawson remarks is often dismissed “as wrong, or irrelevant, or fatuous, or too rapid, or an expression of metaphysical megalomania.” I offer here a two-part explanation of why Strawson’s argument has impressed so few. First, as he usually states it, the argument is lacking at least one key premise. (...) The premise in question concerns the very point on which Strawson and many of his contemporary opponents disagree. Strawson will persuade these opponents only when he convinces them of the truth of this crucial premise. Second, Strawson employs a striking conception of responsibility that has not generally been remarked upon. Many might accept that responsibility so conceived is impossible. But there is very different conception of responsibility that appears of great importance to us. To be thoroughly convincing, Strawson will have to show either that this second conception is inadequate or that his argument goes through even given this alternative conception. (shrink)

This book discusses the problem of mathematical knowledge, and its broader philosophical ramifications. It argues that the problem of explaining the (defeasible) justification of our mathematical beliefs (‘the justificatory challenge’), arises insofar as disagreement over axioms bottoms out in disagreement over intuitions. And it argues that the problem of explaining their reliability (‘the reliability challenge’), arises to the extent that we could have easily had different beliefs. The book shows that mathematical facts are not, in general, empirically accessible, contra Quine, (...) and that they cannot be dispensed with, contra Field. However, it argues that they might be so plentiful that our knowledge of them is intelligible. The book concludes with a complementary ‘pluralism’ about modality, logic and normative theory, highlighting its revisionary implications. Metaphysically, pluralism engenders a kind of perspectivalism and indeterminacy. Methodologically, it vindicates Carnap’s pragmatism, transposed to the key of realism. (shrink)

It is commonly suggested that evolutionary considerations generate an epistemological challenge for moral realism. At first approximation, the challenge for the moral realist is to explain our having many true moral beliefs, given that those beliefs are the products of evolutionary forces that would be indifferent to the moral truth. An important question surrounding this challenge is the extent to which it generalizes. In particular, it is of interest whether the Evolutionary Challenge for moral realism is equally a challenge for (...) mathematical realism. It is widely thought not to be. In this paper, I argue that the Evolutionary Challenge for moral realism is equally a challenge for mathematical realism. Along the way, I substantially clarify the Evolutionary Challenge, discuss its relation to more familiar epistemological challenges, and broach the problem of moral disagreement. The paper should be of interest to ethicists because it places pressure on anyone who rejects moral realism on the basis of the Evolutionary Challenge to reject mathematical realism as well. And the paper should be of interest to philosophers of mathematics because it presents a new epistemological challenge for mathematical realism that bears, I argue, no simple relation to Paul Benacerraf's familiar challenge. We advance a slightly qualified version of the following thesis: -/- (Reason) F is (at some time) a reason for S to feel guilty (to some extent) for A-ing. -/- Leaving implicit the qualification concerning extent, we claim as well: -/- (Desert) S's having this reason suffices for S’s deserving to feel guilty for A-ing. -/- We also advance a third (...) thesis connecting desert of feeling guilty with the fittingness of this response. -/- In light of our three theses, we address several claims that have been made regarding responsibility and desert. We take issue with the divorce of desert from responsibility. We find acceptable a claim regarding blameworthiness and reason to induce guilt, and we defend the idea that it is noninstrumentally good that one who is blameworthy be subject to a fitting feeling of guilt. Surprise is treated as an affect in Aristotelian philosophy as well as in Cartesian philosophy. In experimental psychology, surprise is considered to be an emotion. In phenomenology, it is only addressed indirectly (Husserl, Heidegger, Levinas), with the important exception of Ricoeur and Maldiney; it is reduced to a break in cognition by cognitivists (Dennett). Only recently was it broached in linguistics, with a focus on lexico-syntactic categories. As for the expression of surprise, it has been studied in connection with evidentiality (...) in languages that encode surprise morphosyntactically. However, how surprise is encoded in languages that lack an evidential morphosyntactic system has been largely unexplored. This book provides new insights into the dynamics of surprise based on a heuristic hypothesis tested against the investigation of time, language and emotion. It is intended to arouse the interest of a multidisciplinary audience keen on crossing the disciplinary borders of phenomenology, cognitive sciences, and pragmatics. The theoretical approaches adopted in this collection of articles rely on experiments and corpus data. Il y a un pari d'envergure, presque une provocation, à montrer l'ampleur et l'acuité des méthodes pratiques qui tissent le propos de Husserl dans un texte qui a été considéré par ses interprètes comme le livre le plus "métaphysique", à savoir celui où l'auteur prend parti pour une thèse philosophique souvent jugée éculée: l'idéalisme. Tout le destin de la phénoménologie s'est joué autour d'une prise de position contre son "tournant idéaliste" en 1913, Heidegger ayant ouvert les hostilités, suivi par Sartre, (...) Ricœur, Derrida et Levinas. A rebours de cette herméneutique constitutive de la critique philosophique, l'auteur cherche ici à faire voir un Husserl "praticien", "épistémologue", c'est-à-dire peu soucieux de défendre une position métaphysique. Cette épistémologie pratique de la phénoménologie peut être caractérisée comme un "empirisme transcendantal". Par là, un sens de la vérité émerge, qui puise sa validité dans l'expérience native du lecteur, c'est-à-dire dans son aptitude à vérifier par lui-même l'accord du concept en relation avec son vécu. In the literature of collective intentions, the ‘we-intentions’ that lie behind cooperative actions are analysed in terms of individual mental states. The core forms of these analyses imply that all Nash equilibrium behaviour is the result of collective intentions, even though not all Nash equilibria are cooperative actions. Unsatisfactorily, the latter cases have to be excluded either by stipulation or by the addition of further, problematic conditions. We contend that the cooperative aspect of collective intentions is not a property of (...) the intentions themselves, but of the mode of reasoning by which they are formed. We analyse collective intentions as the outcome of team reasoning, a mode of practical reasoning used by individuals as members of groups. We describe this mode of reasoning in terms of formal schemata, discuss a range of possible accounts of group agency, and show how existing theories of collective intentions fit into this framework. The preface paradox does not show that it can be rational to have inconsistent beliefs, because preface writers do not have inconsistent beliefs. I argue, first, that a fully satisfactory solution to the preface paradox would have it that the preface writer's beliefs are consistent. The case here is on basic intuitive grounds, not the consequence of a theory of rationality or of belief. Second, I point out that there is an independently motivated theory of belief – sensitivism – which (...) allows such a solution. A good definition of process tracing should highlight what is distinctive about process tracing as a methodology of causal inference. I look at eight criteria that are used to define process tracing in the methodological literature, and I dismiss all eight criteria as unhelpful (some because they are too restrictive, and others because they are vacuous). In place of these criteria, I propose four alternative criteria, and I draw a distinction between process tracing for the ultimate aim of testing a (...) start--end hypothesis versus process tracing as an ultimate end in itself. Although it is clear enough how the former method works, there is still much methodological work to be done in understanding the latter method as a distinctive method of causal inference, I argue. Background:Ethical conflict and subsequent nurse moral distress and burnout are common in the intensive care unit (ICU). There is a gap in our understanding of nurses’ perceptions of how organizational resources support them in addressing ethical conflict in the intensive care unit.Research question/objectives/methods:The aim of this qualitative, descriptive study was to explore how nurses experience ethical conflict and use organizational resources to support them as they address ethical conflict in their practice.Participants and research context:Responses to two open-ended questions were collected (...) from critical care nurses working in five intensive care units at a large, academic medical center in the Midwestern region of the United States.Ethical considerations:This study was approved by the Institutional Review Board at the organization where the study took place.Findings:Three main interwoven themes emerged: nurses perceive (1) intensive care unit culture, practices, and organizational priorities contribute to patient suffering; (2) nurses are marginalized during ethical conflict in the intensive care unit; and (3) organizational resources have the potential to reduce nurse moral distress. Nurses identified ethics education, interprofessional dialogue, and greater involvement of nurses as important strategies to improve the management of ethical conflict.Discussion:Ethical conflict related to healthcare system challenges is intrinsic in the daily practice of critical care nurses. Nurses want to be engaged in discussions about their perspectives on ethical conflict and play an active role in addressing ethical conflict in their practice. Organizational resources that support nurses are vital to the resolution of ethical conflict.Conclusion:These findings can inform the development of interventions that aim to proactively and comprehensively address ethical conflict in the intensive care unit to reduce nurse moral distress and improve the delivery of patient and family care. Universities and other research institutions are increasingly providing additional training in research integrity to improve the quality and reliability of research. Various training courses have been developed, with diverse learning goals and content. Despite the importance of training that focuses on moral character and professional virtues, there remains a lack of training that adopts a virtue ethics approach. To address this, we, a European Commission-funded consortium, have designed a train-the-trainer programme for research integrity. The programme is based on (1) virtue (...) ethics, (2) the ethos of science, (3) learning by doing and (4) learner-centred teaching. The blended learning programme combines e-learning modules with participatory group sessions. Trainers are taught how to guide researchers through a series of structured exercises for fostering reflection on scientific virtues, and how to promote understanding and application of the European Code of Conduct for Research Integrity. Trainers are provided with adaptable tools and resources that can be used and combined in different ways. The programme implementation began in Spring 2020 and 470 trainers have participated to date. When trainers were asked to grade – between 0 (very bad) and 10 (excellent) – the e-learning modules and the participatory exercises, 60% scored a grade 8 or higher (median = 8, IQR = 2) for the e-learning modules, whereas 80% scored a grade 8 or higher (median = 9, IQR = 1) for the participatory exercises. A majority felt that the training helped them as a trainer to learn about ways to organise and teach a research integrity course (82%) and would recommend the interactive exercises to others (92%). Trainers have educated over 3300 researchers in Europe using our virtue-based approach. The VIRT2UE train-the-trainer programme fosters research integrity by providing trainers with exercises and tools which enable them to stimulate the development of good researchers across Europe. In this volume, we suggest new perspectives with which critical animal studies (CAS) can contribute to the development of knowledge and praxis in two fields-dehumanization and critical border studies, both concerned with human migration, refuge, and territorial or other borders. CAS is an interdisciplinary field that reflects on the ethics of humans' relationships with other animals, how the oppression of nonhuman animals intersects with other forms of oppression, and how economic interests drive oppression. It is evident, in the face of the COVID-19 pandemic that has physicians confronting death and dying at unprecedented levels along with growing data suggesting that physicians who care for dying patients face complex emotional, psychological and behavioural effects, that there is a need for their better understanding and the implementation of supportive measures. Taking into account data positing that effects of caring for dying patients may impact a physician’s concept of personhood, or “what makes you, ‘you’”, we adopt Radha (...) Krishna’s Ring Theory of Personhood (RToP) to scrutinise the experiences of physicians working in intensive care units (ICU) using a fictional scenario that was inspired by real events. The impact of death and dying, its catalysts, internal constituents, external factors, dyssynchrony, and buffers, specific to ICU physicians, were identified and explored. Such a framework allows for ramifications to be considered holistically and facilitates the curation of strategies for conflict resolution. This evaluation of the RToP acknowledges the experience and wide-ranging effects it has on ICU physicians. As such, our findings provide insight into their specific needs and highlight the importance of support on a personal and organisational level. Although further research needs to be conducted, the RToP could serve as the basis for a longitudinal assessment tool supported by the use of portfolios or mentorship due to their provision of personalised, appropriate, specific, timely, accessible and long-term support.
