In the debate surrounding the introduction of non-invasive prenatal testing in prenatal screening programmes, the concept of routinisation is often used to refer to concerns and potential negative consequences of the test. A literature analysis shows that routinisation has many different meanings, which can be distinguished in three major versions of the concept. Each of these versions comprises several inter-related fears and concerns regarding prenatal screening and particularly regarding NIPT in three areas: informed choice, freedom to choose and consequences for (...) people with a disability. Three of the strongest arguments raised under the flag of routinisation are assessed for their validity: the threat that NIPT poses to informed choice, the potential increase in uptake of first-trimester prenatal screening and its consequences for social pressure to participate in screening or terminate affected pregnancies, and the negative consequences for disabled people. These routinisation arguments lack empirical or normative ground. However, the results of this analysis do not imply that no attention should be paid to possible problems surrounding the introduction of NIPT. At least two problems remain and should be addressed: there should be an ongoing debate about the requirements of informed choice, particularly related to an expanded scope of prenatal screening. Also, reproductive autonomy can only be achieved when expecting parents’ options are variegated, real and valuable, so that they can continue to choose whether or not to screen or to terminate a pregnancy. The introduction of non-invasive prenatal testing in healthcare systems around the world offers an opportunity to reconsider funding policies for prenatal screening. In some countries with universal access healthcare systems, pregnant women and their partners are asked to pay for NIPT. In this paper, we discuss two important rationales for charging women for NIPT: to prevent increased uptake of NIPT and to promote informed choice. First, given the aim of prenatal screening, high or low uptake rates are not intrinsically desirable (...) or undesirable. Using funding policies to negatively affect uptake, however, is at odds with the aim of screening. Furthermore, copayment disproportionally affects those of lower socioeconomic status, which conflicts with justice requirements and impedes equal access to prenatal screening. Second, we argue that although payment models may influence pregnant women’s choice behaviours and perceptions of the relevance of NIPT, the copayment requirement does not necessarily lead to better-informed choices. On the contrary, external influences on women’s personal choices for or against prenatal screening should ideally be avoided. To improve informed decision-making, healthcare systems should instead invest in adequate non-directive, value-focused pretest counselling. This paper concludes that requiring copayments for NIPT in universal access healthcare systems fails to promote reproductive autonomy and is unfair. (shrink)

We argue for a three-step counselling model in which three decision moments are distinguished and differently addressed: (1) professionals explore women’s values concerning whether and why they wish to know whether their baby has a genetic disorder; (2) women receive layered medical-technical information and are asked to make a decision about screening; (3) during post-test counselling, women are supported in decision-making about the continuation or termination of their pregnancy. This model might also be applicable in other fields of genetic (pre-test) counselling, where techniques for expanding genome analysis and burdensome test-outcomes challenge counselling of patients. Asking pregnant women to pay for non-invasive prenatal testing out of pocket leads to unequal access across socioeconomic strata. To avoid these social justice issues, first-trimester prenatal screening should be publicly funded in countries such as the Netherlands, with universal coverage healthcare systems that offer all other antenatal care services and screening programmes free of charge. In this reply, we offer three additional reasons for public funding of NIPT. First, NIPT may not primarily have medical utility for women and children, (...) but rather offers relevant information and reproductive options, and thus serves important autonomy interests of women. Second, public funding of NIPT can be justified because it results in a reduction of collectively borne costs associated with care and support for children with chromosomal abnormalities. It is important to note that this is not an argument for individual women to take part in screening or to terminate an affected pregnancy. However, it is a legitimate argument in policy making regarding funding arrangements for screening programmes. Finally, public funding would help to amend current misunderstandings among pregnant women, and thus to support informed consent for first-trimester prenatal screening. (shrink)

Rationale In Belgium, several policies regulating reimbursement of acid suppressant drugs and evidence-based recommendations for clinical practice were issued in a short period of time, creating a unique opportunity to observe their effect on prescribing. Aims and objectives To describe the evolution of prescriptions for acid suppressants and explore the interaction of policies and practice recommendations with prescribing patterns. Method Monthly claims-based data for proton pump inhibitors (PPIs) and H-2-antihistamines by general practitioners, internists and "astroenterologists were obtained from the Belgian (...) national health insurance database (1997-2005). The evolution of reimbursed defined daily doses and expenses after introduction of reimbursement regulations and dissemination of practice recommendations was explored. Results Recommendations had no impact on prescribing. All changes can be related to concomitant policies. Lifting reimbursement restrictions for cheaper products did not control growth or save costs in the Iona term. Only restricting reimbursement of all PPIs managed to curb the growth. We observed an unintended increase of non-omeprazole PPIs by gastroenterologists. Conclusions Reimbursement policies influence prescribing, but their effect can be unintended. A dialogue between policymakers and guideline developers, and evidence-based policies that are linked to clinical guidelines, could be an effective way to pursue both cost-containment and quality of care. Transition to a sustainable energy regime is one of the key global societal challenges for the coming decades. Many technological innovations are in the pipeline, but an uncritical appraisal of anything and everything called green innovation lacks methods for testing both the necessity and the sufficiency of these developments. We propose to develop a philosophy of energy to fill this lacuna. Its task is to explore and clarify the space in which the so-called energy transition is taking place. This article (...) sketches the fundaments of such a philosophy and suggests how it might be built upon the work of twentieth century critics of the functioning of energy in society, including Mumford, Bataille, and Heidegger; but not without empirical analysis of contemporary energy systems. Via the example of flux and potentiality - two apparently opposing conceptions of energy - we propose that a philosophy of energy allows for a broader perspective on specific problems in energy transition, and illuminates implicit and problematic assumptions behind these problems. As a way to mitigate climate change, ways to reduce electricity consump­tion are being explored. I claim Briggle and Mitcham’s experiential gap offers a useful framework to understand the workings of our environment regarding this consumption. Via Foucauldian ethics, which holds people need to relate to their environment through ‘self practices’ in order to make moral choices, I argue that the complex and opaque electrical network makes it particularly difficult to consciously curb consumption. Efforts to make the network simpler and (...) more transparent could enable engagement and ‘ethical consumption,’ but at the cost of decreased usability. (shrink)

Energy consumption is central to both a number of pressing environmental issues and to people’s attempts to improve their well-being. Although typically understood as essential for people to thrive, this paper sketches a theoretical foundation for the possibility that the form and amount of energy consumption in modern society may inhibit rather than enable human flourishing. It achieves this goal by connecting and critically assessing the writings of Ivan Illich and Albert Borgmann, which offer a number of concepts that enable (...) a qualitative discussion on energy practices. For different reasons, both authors are highly critical of the societal tendency to command ever-increasing amounts of energy. Illich focuses on negative effects of high energy consumption at the societal level, whereas in my particular re-reading of Borgmann, one finds reasons why high energy consumption fails to realize intended positive effects. It is argued that therefore, energy transition should involve a re-appreciation of the function of energy consumption: to support, rather than to be central to life. (shrink)

The article proposes to defend the basic income as a pillar of a socio-cultural revolution. This is done exclusively from a literature review and articulation of different works. The introduction section presents the characteristics of a basic income. Section 2 brings how it would mitigate current capitalism problems, which are: increasing inequality, lack of equality of opportunity, means tested social security and its bureaucracy, narrow definition of paid work and future of labor. On Section 3, the author does a literature (...) review to address two common objections against basic income: the cost, and potential labor decrease. It is intended as a survey of the writers who have taken note of Dewey and his ideas, and is meant to function as a sort of additive inventory, with no claim to comprehensiveness. Some of the articles mentioned were unfortunately unavailable for direct examination and are thus listed merely for purposes of information.Although the educational and philosophical writings of John Dewey (...) are actually indivisible, Dewey’s oeuvre has not been read in France and Europe generally as of a piece, but has largely been registered in terms of those parts which have relevance to education and teaching. Indicative of this is the fact that it took until 1975 forDemocracy and Education -the book which, in Dewey’s own view, most clearly presented his linking of pedagogy and philosophy — to be published in France. Gérard Delledalle, the translator ofDemocracy and Education, is the only person so far in France to have dealt systematically with the whole of Dewey’s writings. He has translated other works by Dewey and has written several books on him, dealing expressly with Dewey’s philosophy of pragmatism as the foundation of his theory of education.It is actually inadequate to restrict the reception of Dewey’s work to France alone. Rather, one should speak of francophone Europe, for the first translations of Dewey’s educational writings into French were made by Adolphe Ferrière, Ovide Decroly and édouard Claparède — a Swiss, a Belgian, and a Frenchman. It was thanks to them that Dewey’s thoughts on education began to make an impact on the francophone movement for school reform in the early twentieth century.Discussion of his theory of education is typified in France as well by a division into proponents of a concept of ‘learning by doing’ indebted to Dewey and representatives of authoritarian forms of education, which reject Dewey. Although French thought has not yet concerned itself closely with pragmatism, Dewey’s opponents believed that they could denounce him and his theories simply by levelling the charge of “pragmatism.” This dualistic mode of thinking which appears to be deeply rooted in France has proved to be an obstacle to the reception of Dewey and has led to neglect and rejection of his theories. Managerialism is not mere ideology, a concatenation of ideas subsisting in an epiphenomenal superstructure (Überbau) that mirrors economic relations (Base) and masks interests, but a set of practices that, as an extreme manifestation of human resources management (HRM), seeks to constitute the life-world (Lebenswelt) of participants in many sectors of society. Increasingly, it is those at the extremes of elite wealth and marginal poverty who may fall outside its remit and become free to think beyond its parameters. As inheritor of (...) personnel management, HRM as practice and ideology fills a societal vacuum and comes to exercise the power of totalizing social construction. In an integral performance culture the Performative Absolute emerges in the imaginary; it is an immanent sublime that instantiates and regulates the social being of the individual. This apotheosis of the 'manager's right to manage' overrides boundaries between public and private spheres and dissolves the separation of powers. As 'human resource', the employee is subject to living sacrifice; this is an oblation that demands ethical and theological critique. (shrink)
