Oral health is a critical part of overall health. The current COVID-19 pandemic has highlighted the importance of oral health. In this article, we describe how dental practice has been impacted by COVID-19, identify the public health response to COVID-19, and explain the gradual resumption of dental care after the initial disruption due to the pandemic. Finally, we discuss how long-standing health disparities in oral health have been exacerbated by the current pandemic.

In this paper, we will consider the role of oaths and codes of ethics in undergraduate medical education. Studies of ethics syllabi suggest that ethics educators typically use well-known bioethics texts such as Beauchamp and Childress. Yet, many issues that medical students will face are addressed by codes of ethics and oaths. We will first provide a historical survey of oaths and codes and then address how these sources of ethical guidance can be effectively used in ethics education of medical (...) students. Oaths and codes can be engagingly taught using a range of techniques including visual narrative. Excerpts from television and film can be used to highlight challenging ethical dilemmas in a variety of settings, taking the learning from the theoretical to the more applied while offering context. Pressure is mounting to hold researchers and research institutions accountable for the protection of human subjects. When subjects or their family members believe they have been injured, they are increasingly willing to file lawsuits. Recent cases indicate that institutional review boards and their members may be pulled more and more into the legal fray.On September 17, 1999, 18-year-old Jesse Gelsinger died while participating in research conducted by the University of Pennsylvania's Institute for Gene Therapy. Gelsinger was involved in a Phase (...) I clinical trial testing a new approach to treatment of ornithine transcarbamylase deficiency, a rare metabolic disorder. Although infants affected by the severe form of OTC die shortly after birth, Gelsinger suffered from a relatively mild form. It appears undisputed that the cause of Gelsinger's death was not the disorder itself, but multiple organ system failure triggered by the virus used to carry new genetic material into his system. Tantrums don’t vanish after the age of three. Aggression is a way of life. Simple communication is a constant challenge. And dreams of a child’s (...) future have to be altered and adapted. In the narratives that accompany this commentary, we see that the challenges of raising a child with autism do not necessarily diminish with age; they may, however, change because children with autism become adults with autism and this presents a new set of both challenges and opportunities.The current epidemic of young children with autism is giving rise to yet another epidemic—the rising number of young adults who are autistic. As one of the authors here mentioned, “the system is in for a tsunami of new individuals needing medical care of all types, needing continued supports for housing and vocational training, communication, occupational and physical therapy” (Cornell). According to a recent study published in Pediatrics, “[y]outh with an ASD have poor postsecondary employment and education outcomes, especially in the first 2 years after high school” (Shattuck, 2012). With the rising number of children being diagnosed with autism, this employment disparity is likely to be exacerbated until or unless the workforce adapts to and accommodates the needs and skills of persons with autism.The needs of young adults with autism are only recently beginning to receive more attention from the media. But for these children’s parents, this issue has always been personal. The emotions on display in these narratives are heartfelt—even raw at times—and seem to swell like the wave of a tsunami itself as parents are now confronting what should be the independence of adulthood. With barriers to employment, health care and housing, young adults with autism remain largely dependent upon their parents. While the topic of transition to adulthood for those with autism is a vast one, implicating the need for wide scale policy change, certain themes emerge in these personal narratives that we wish to focus on here, which may well serve as a guide for that change.The Advocate MotherThe Advocate Mother is a common theme in many narratives of autism and the ones presented here are not different. In fact, nearly all of the narratives presented are written by mothers. Mothers in these narratives exemplify sacrifice, dashed hopes, and even heartbreak. As Laura Shumaker recounts the story of taking her adult son to Washington, DC, she experiences great anxiety when her son is mocked. Take, for example, the powerful narrative of Nancy Coughlin. She confesses that her daughter’s autism took over her life and the life of her family: “my own mental health was disintegrating, as was my marriage, and in our family’s last–ditch effort to ease the burden we bought a second home; a cheap little place just a few blocks down the road from where we lived. We called it our ‘respite house’.”Others confess that raising a child with autism becomes a “full time job.” (Van der Poel). And the demands of this job do not diminish simply because the child reaches adulthood; it may, in fact, become not only a personal job for some parents but their vocation as well. I discuss recent work by Ernst Fehr and his collaborators on cooperation and reciprocity. (i) Their work demonstrates conclusively the reality and importance of non-self-interested motivations. (ii) It allows for a useful distinction between trust and blind trust. (iii) It points to a category of quasi-moral norms, distinct both from social norms and moral norms. (iv) It demonstrates how social interactions can generate irrational belief formation. (v) It shows the potential of punishment for sustaining social norms and for overcoming the (...) second-order free rider problem as well as obstacles to group selection. (vi) It offers a provocative experimental basis for the ‘warm-glow’ explanation of altruistic behavior. I conclude by suggesting some experiments that might allow for further developments of the theory. (shrink)

There is no generally accepted paradigm, or even as well-defined set of problems that constitute it as a subdiscipline within economics. Some questions are, however, emerging as foci of interest for a small but increasing number of writers, not just in economics, but also in psychology and philosophy. When we raise children what we are typically aiming for is a kind of flourishing; we want childrento live well as children, and to grow to become adults who live well too. Undoubtedly, part of what we are aiming forwhen we aim for a child’s flourishing is that they meet their developmental milestones well, and that they succeedamong their peers. We are also generally interested in how a child regards themselves; we want children tobelieve that they have value, and that (...) what they care about has value too. We also typically want children to havean appreciation for their relative value among others; that they are important among many other important beings,and we want them to act well in that knowledge. Of course, not all persons who raise children aim for those ends.Some do not care for how a child conceives of themselves, or whether the child flourishes at all. Others care fortheir children, and how they conceive of themselves, but aim poorly. As result, some children come to believe thatthey only deserve love and kindness when they succeed at meeting a caregiver’s ends. Some children come tobelieve that they have no worth at all. And yet other children, rather than believing they are unworthy, come toregard themselves as of superior worth; as children, for example, who bully and sneer at other children for being adifferent race, a different class, gender, ability, etc. may. When a child comes to understand themselves in theseterms, we typically see it at best as morally problematic, and at worst as a tragedy. Despite our interest inchildren’s self-conceptions, and the vast literature on how to conceive of and regard children, there is relatively littleregarding how children should conceive of and regard themselves. In order to explain how children shouldconceive of themselves it is my contention that a new account of self-respect is needed for the context of childhood;one that is responsive to the moral value of children, as well as their developmental capacities to recognize thatvalue. Given that developmental psychology is complex and subject to significant variation as children develop, myaim in this paper is to offer a snapshot of what I take self-respect to amount to in early childhood. ABSTRACT A necessary condition for news to be produced under conditions of ‘negative’ freedom is for newspapers to become ‘reporter codetermined newspapers’, where reporters, along with editors and publishers, have a collective ‘positive’ freedom to vote on news‐policy, select editors and hire reporters. ‘Publisher‐controlled’ newspapers systematically prevent reporters from reporting some news‐stories and coerce and manipulate reporters into reporting others. It is argued here that all newspapers should be legally required to become reporter codetermined newspapers. This change is also required (...) by Rawls’ and Mill's arguments for freedom of the press. On the basis of empirical data it is unlikely that the demands of freedom or justice can be met within existing institutional structures. (shrink)

With each of these encounters, new subjects are brought into focus as others recede into the background, ushering a change not only in how questions are tackled but also in what questions are thought worth asking.

Introduction to the special cluster of articles by feminists from Eastern Europe and the former Soviet Union.

This article discusses some current research claims on gender and state socialism in Eastern Europe from 1945 to 1989. It raises questions about claims by Revisionist Feminist Scholars that official state socialist women’s organizations were ‘agents’ on behalf of women, or women’s movements, perhaps feminist, and not ‘transmission belts’ of communist parties. State socialist policies are described as ‘friendly towards women’ and ‘pro-women’. In contrast, the author claims that these organizations both were and were not agents on behalf of women, (...) and also prevented women’s agency. Meaningful women’s agency is not actually shown to occur intermittently throughout their history, but in two contexts – before 1955 or in moments of political rupture. Scholars do not distinguish the who, when, and what – Who could be agents? When could they be agents? And what kind of agents could they be? State policies, the author claims, were also at one and the same time ‘friendly’ and ‘unfriendly’ towards women, sometimes harming women. The author explores why this research is happening now, discussing research on Bulgaria, Czechoslovakia, the Democratic Women’s Organization of Germany, the Democratic Association of Hungarian Women, later renamed MNOT, the Romanian National Council of Women and the Yugoslav Anti-Fascist Women’s Organization. This article is a critical examination of Nancy Fraser's contrast of early second-wave feminism and contemporary global feminism in “Feminism, Capitalism and the Cunning of History,” (Fraser ). Fraser contrasts emancipatory early second-wave feminism, strongly critical of capitalism, with feminism in the age of neoliberalism as being in a “dangerous liaison” with neoliberalism. I argue that Fraser's historical account of 1970s mainstream second-wave feminism is inaccurate, that it was not generally anti-capitalist, critical of the welfare system, or challenging the priority (...) of paid labor. I claim Fraser mistakenly takes a minority feminist position as mainstream. I further argue that Fraser's account of feminism today echoes arguments from James Petras and Henry Veltmeyer (2001) to Hester Eisenstein (), but such arguments ignore contemporary feminist minority positions. I challenge Fraser's arguments that feminism legitimates neoliberalism to women, that women's NGOs are simply service-providers enabling the state to withdraw services, and that criticisms of microcredit lending programs can be generalized into criticisms of women's feminism and women's NGOs today. I argue that these claims are vast over-generalizations and ignore countertrends. I give empirical evidence to support my objections by considering women's activities in post-communist European countries, which Fraser discusses. (shrink)

ABSTRACTThe German state permitted about one million refugees to enter Germany in 2015–2016, although many were subsequently denied refugee status. Germany adopted an ‘integration’ and ‘welcome’ politics, an important, if imperfect, model for a European refugee policy. The integration of refugees required the joint activity of state, of civil society, of the public sphere and of refugees themselves. Civil society initiated a vast amount of essential care work and solidarity with refugees pursued especially, but not only, by women, yet civil (...) society and refugees had no say in refugee policies. German refugee policy and practice raised many normative issues, producing an intense and vigorous national debate about them, in which philosophers and theorists were active. Among the questions debated were whether Germany had accepted ‘too many’ refugees, the acceptability of the costs of the policy, the risks of harm to the least well off, the purported risks to safety, security and the social order and the purported threats to German culture. I first describe German state policies, practices and civil society efforts and then turn to normative debates. I discuss the limits of the German discourse and suggest how it needs to be expanded. This article explores witchcraft-related violence against elderly women in the AmaZizi chiefdom of kwaZangashe in Eastern Cape, South Africa. The potential causes that have promoted such violence form the central subject of the study. The study includes a research design that combines questionnaires, focus groups and follow-on interviews. The findings have revealed a prevalence of witchcraft beliefs in the region and have pointed to elderly women as the likely victims of witchcraft violence. This has resulted in AmaZizi’s elderly women being (...) socially isolated, verbally abused and at risk of physical violence and even murder. This study concludes with several interventions organised to combat future witchcraft-related violence and to support the elderly women in the chiefdom.Contribution: By giving voice to the lives and stories of a community of people rendered invisible in the wider public sphere, this article introduces accounts on witchcraft and healthcare that might otherwise have gone unarticulated. This book was first published in 1984, as the revised edition of a 1979 original. The text is composed of studies in a descending sequence from perfect rationality, through imperfect and problematical rationality, to irrationality. Specifically human rationality is characterized by its capacity to relate strategically to the future, in contrast to the myopic 'gradient climbing' of natural selection. There is trenchant analysis of some of the parallels proposed in this connection between the biological and the social sciences. In the (...) chapter on imperfect rationality the crucial notion is that of 'binding oneself', as Ulysses did before setting out to the Sirens, when weakness of will may prevent us from using our capacity for perfect rationality. The second half of the book deals with rational-actor theory, comparing its logical power and success to rival approaches, and with the varieties of irrationality expressed in contradictory beliefs and desires. Jon Elster has written a comprehensive, wide-ranging book on the emotions in which he considers the full range of theoretical approaches. Drawing on history, literature, philosophy and psychology, Elster presents a complete account of the role of the emotions in human behaviour. While acknowledging the importance of neurophysiology and laboratory experiment for the study of emotions, Elster argues that the serious student of the emotions can learn more from the great thinkers and writers of the past, from (...) Aristotle to Jane Austen. He attaches particular importance to the work of the French moralists, notably La Rochefoucauld, who demonstrated the way esteem and self-esteem shape human motivation. The book also maintains a running dialogue with economists and rational-choice theorists.
