Assuming Martin's Axiom, we compute the value of the cofinality of the symmetric group on the natural numbers. We also show that Martin's Axiom does not decide the value of the covering number of a related Mycielski ideal.

• We define a notion of order of indiscernibility type of a structure by analogy with Mitchell order on measures; we use this to define a hierarchy of strong axioms of infinity defined through normal filters, the α-weakly Erdős hierarchy. The filters in this hierarchy can be seen to be generated by sets of ordinals where these indiscernibility orders on structures dominate the canonical functions.• The limit axiom of this is that of greatly Erdős and we use it to calibrate (...) some strengthenings of the Chang property, one of which, CC+, is equiconsistent with a Ramsey cardinal, and implies that where K is the core model built with non-overlapping extenders — if it is rigid, and others which are a little weaker. Following up on a 1989 paper on the subject, this essay revisits the question of ethical expertise in the court room. Informed by recent developments in the use of ethics experts, the authors argue 1) that the adversarial nature of court proceedings challenges the integrity of the ethicist's pedagogical role; 2) that the use of ethics experts as normative authorities remains dubious; 3) that clarification of the State's interest in “protecting the ethical integrity of the medical profession” is urgently required; (...) and 4) that the expertise of the ethicist may be more appropriately used in advising the legislature than in influencing the court. Surveys and routine clinical procedures applied in research protocols are typically considered only minimally risky to participants. The apparent benign nature of "minimal risk" tasks increases the chance that investigators and Institutional Review Boards (IRBs) will overlook the probability that clinical tools will identify signs, symptoms, or definitive test results that are clinically-relevant to subjects' welfare. "Minimal risk" procedures may also pose a particular hazard to participants in clinical research by increasing the therapeutic misconception because the tasks mimic clinical care (...) and are often conducted in clinical settings. Investigators should anticipate which measures could yield clinically-important findings and should describe explicit plans for data monitoring, disclosure, and follow-up. Protocols that include reliable and valid clinical measures should prompt a more detailed risk assessment by the IRB, even when the tasks meet the regulatory criteria for minimal physical, psychological, or emotional risk. Non-communicable diseases now account for the majority of the global burden of disease and an international campaign has emerged to raise their priority on the post-2015 development agenda. We argue, to the contrary, that there remain strong reasons to prioritize maternal and child health. Policy-makers ought to assign highest priority to the health conditions that afflict the worst off. In virtue of how little healthy life they have had, children who die young are among the globally worst off. Moreover, many (...) interventions to deal with the conditions that cause mortality in the young are low-cost and provide great benefits to their recipients. Consistent with the original Millennium Development Goals, the international community should continue to prioritize reductions in communicable diseases, neonatal conditions, and maternal health despite the shifts in the global burden of disease. Arthur Fine's use of prism models to provide local and deterministic accounts of quantum correlation experiments is presented and analyzed in some detail. Fine's claim that "there is... no question of the consistency of prism models... with the quantum theory" (forthcoming, p. 16) is disputed. Our criticisms are threefold: (1) consideration of the possibility of additional analyzer positions shows that prism models entail unacceptably high rejection rates in the relevant experiments; (2) similar considerations show that the models are at best (...) only superficially local and deterministic; and (3) in any case, Fine extracts the quantum correlation statistics from prism models only by resurrecting conceptual problems similar to those that his models were to designed to solve. Background: Despite the expansion of ethics consultation services, questions remain about the aims of clinical ethics consultation, its methods and the expertise of those who provide such services.Objective: To describe physicians’ expectations regarding the training and skills necessary for ethics consultants to contribute effectively to the care of patients in intensive care unit .Design: Mailed survey.Participants: Physicians responsible for the care of at least 10 patients in ICU over a 6-month period at a 921-bed private teaching hospital with an established (...) ethics consultation service. 69 of 92 eligible physicians responded.Measurements: Importance of specialised knowledge and skills for ethics consultants contributing to the care of patients in ICU; need for advanced disciplinary training; expectations regarding formal-training programmes for ethics consultants.Results: Expertise in ethics was described most often as important for ethics consultants taking part in the care of patients in ICU, compared with expertise in law , religious traditions , medicine and conflict-mediation techniques . When asked about the formal training consultants should possess, however, physicians involved in the care of patients in ICU most often identified advanced medical training as important.Conclusions: Although many physicians caring for patients in ICU believe ethics consultants must possess non-medical expertise in ethics and law if they are to contribute effectively to patient care, these physicians place a very high value on medical training as well, suggesting a “medicine plus one” view of the training of an ideal ethics consultant. As ethics consultation services expand, clear expectations regarding the training of ethics consultants should be established. (shrink)

The basic concepts appropriate for anS matrix theory for classical nonlinear physics are formulated here. These concepts are illustrated by a discussion of shock wave diffraction patterns. Other information concerning solutions of non-linear conservation laws is surveyed, so that a coherent picture of this theory can be seen. Within thisS matrix framework, a number of open problems as well as a few solved ones will be discussed.

Large-scale sequencing tests, including whole-exome and whole-genome sequencing, are rapidly moving into clinical use. Sequencing is already being used clinically to identify therapeutic opportunities for cancer patients who have run out of conventional treatment options, to help diagnose children with puzzling neurodevelopmental conditions, and to clarify appropriate drug choices and dosing in individuals. To evaluate and support clinical applications of these technologies, the National Human Genome Research Institute and National Cancer Institute have funded studies on clinical and research sequencing under (...) the Clinical Sequencing Exploratory Research program as well as studies on return of results. Most of these studies use sequencing in real-world clinical settings and collect data on both the application of sequencing and the impact of receiving genomic findings on study participants. They are occurring in the context of controversy over how to obtain consent for exome and genome sequencing. (shrink)

Among bioethicists and members of the public, genetics is often regarded as unique in its ethical challenges. As medical researchers and clinicians increasingly combine genetic information with a range of non-genetic information in the study and clinical management of patients with common diseases, the unique ethical challenges attributed to genetics must be re-examined. A process of genetic routinisation that will have implications for research and clinical ethics, as well as for public conceptions of genetic information, is constituted by the emergence (...) of new forms of genetic medicine, in which genetic information is interpreted in a multifactorial frame of reference. Although the integration of genetics in medical research and treatment may be a helpful corrective to the mistaken assumptions of genetic essentialism or determinism, the routinisation of genetics may have unintended consequences for the protection of genetic information, perceptions of non-genetic information and the loss of genetic research as a laboratory for exploring issues in research and clinical ethics. Consequently, new ethical challenges are presented by the increasing routinisation of genetic information in both biomedical and public spheres. (shrink)

We are printing, by kind permission of the Law Commission, two sections of the report of the Law Commission on injuries to unborn children. This report was the result of a request to the Law Commission by the Lord Chancellor at the time (Lord Hailsham of Saint Marylebone) to advise on `what the nature and extent of civil liability for antenatal injury should be'. The Law Commission followed its usual practice in such circumstances of consulting various bodies and obtaining expert (...) advice on the subject and then embodying the results in a working paper (Working Paper No. 47 - injuries to unborn children) published on 19 January 1973, which preceded their report (Cmnd 5709). Meanwhile a Royal Commission is considering much wider issues of civil liability for injury (including antenatal injury) but the terms of reference for the Law Commission were much narrower and confined to the position of children injured before birth. In the section relating to the present law the report makes it clear that it is probable that liability under the common law already exists. The Scottish Law Commission has also issued a report (Cmnd 5371). They were given different terms of reference and came to somewhat different conclusions. We are printing from this long report the paragraphs discussing the medical background and the summary of recommendations. As will be evident on reading the paragraphs on the medical background to injuries to the unborn child, events are moving very rapidly, particularly in the study of congenital defects and the effects of drugs but the problems of proof present great difficulty. Other causes of injury to the unborn child are better known to the general public: for example, those following the illness, infection and disease of the mother during pregnancy, injury caused in attempted termination of pregnancy and the risks resulting from the mother's condition. The summary of the recommendations sets out very clearly the legal position of the unborn child, as the Law Commission sees it, arising from injury before birth, the final conclusion being that `legislation is desirable'. These extracts from the report, apart from their intrinsic interest, lead on to the paper by Mr Kennedy and Dr Edwards in which they set out their criticisms of it, and provide quick references to the original document. The death of Roger Caillois was vividly felt by writers and intellectuals all over the world. Not only in France, where his work in sociology, surrealism, criticism and literature brought him into the Académie française, but also in Japan, Brazil (whose Academy elected him to the seat previously occupied by André Malraux), in Argentina (where he counted numerous friends whose works and thought he had made known in Europe) his passing profoundly saddened literary and intellectual circles. Struck to the heart (...) by his death also is the review Diogenes, which he founded and which has been formed by his perpetually needle-sharp curiosity, his creative imagination and his uncompromising precision. We take up her book’s call for a “problematizing” history which challenges “taken-for-granted” preconceptions in order to contest Allen’s own representation of the thought of the enlightenment. Allen accepts that all the enlighteners agreed upon a stadial, progressive account of history, which she critiques epistemically and normatively. But we show in Part 2, drawing on the work of Henri Vyverberg and (...) other historians of eighteenth century ideas, that a cyclical, rise and fall account of historical succession was more prominent than the progressive narrative in leading enlighteners such as Montesquieu, Voltaire, Diderot, D’Alembert, Condillac, Jancourt, Grimm, and Raynal, all of whom Allen does not mention. In Part 3, we show that not all thinkers of the enlightenment were pro-colonial or pro-imperialist, as Allen also presupposes in The End of Progress. By examining Abbé Raynal’s History of The Two Indies in Part 3, and notably its Diderotian interpolations, we show that many enlighteners propounded fierce criticisms of European colonialism and the slave trade, even calling directly for armed resistance against European infractions. In critical theorists’ search for chastened normative foundations, our concluding remarks contend, there is a need to develop more accurate, balanced, post-postmodern reckonings of the enlightenment. Carol Gilligan has identified two orientations to moral understanding; the dominant justice orientation and the under-valued care orientation. Based on her discernment of a voice of care, Gilligan challenges the adequacy of a deontological liberal framework for moral development and moral theory. This paper examines how the orientations of justice and care are played out in medical ethical theory. Specifically, I question whether the medical moral domain is adequately described by the norms of impartiality, universality, and equality that characterize the (...) liberal ideal. My analysis of justice -oriented medical ethics, focuses on the libertarian theory of H.T. Engelhardt and the contractarian theory of R.M. Veatch. I suggest that in the work of E.D. Pellegrino and D.C. Thomasma we find not only a more authentic representation of medical morality but also a project that is compatible with the care orientation's emphasis on human need and responsiveness to particular others. Albert Camus repeatedly denied the label “existentialist,” and pointed to his formative experiences of natural beauty and his early introduction to classical Greek thought and culture as determinative of his philosophy. Pierre Hadot, famous for his post-1970 work on philosophy as a way of life in classical antiquity, continued throughout his life to work on the history of Western conceptions of nature. In Le voile d’Isis, Hadot excavated a second strain of Western attitudes towards nature, alongside the instrumental or “Promethean” (...) approach dominant in modernity: this is that of the contemplative Orphic perspective, closely tied in antiquity to philosophical regimens of spiritual exercises to transform philosophers’ ways of seeing. This paper will argue that Camus and Hadot should be read as two twentieth century “Orphic” figures in this sense, in a way that at once singles them out against almost all other European contemporaries, as well as speaking to our ecological concerns today. Yet the only comparative piece on the two thinkers to date, by Matthew Lamb, misses this shared contemplative, Orphic core to their positions. This paper aims to redress this shortcoming in the reception of the two figures. (shrink)

Chefs have been recognized as potentially important partners in efforts to promote local food systems. Drawing on the diffusion of innovation framework we (a) examine the characteristics of chefs and restaurants that have adopted local foods; (b) identified local food attributes valued by restaurants; (c) examine how restaurants function as opinion leaders promoting local foods; (d) explored network linkages between culinary and production organizations; and (e) finally, we consider some of the barriers to more widespread adoption of local foods in (...) the culinary community. Analyzing quantitative and qualitative data collected from interviews with individuals from 71 restaurants, we compare and contrast restaurants that utilize relatively large amounts of locally-produced ingredients with restaurants using few, if any, local products. Results reveal that chefs are most interested in intrinsic food qualities, such as taste and freshness, and less interested in production standards. As opinion leaders, chefs utilize signage, wait staff, and cooking classes to promote local foods; however, the diffusion process across restaurants, and between restaurants and producers, is limited by network associations. Structural barriers such as distribution problems and lack of convenience were identified as limiting more widespread use of locally-grown foods. We offer several implications of this research for further work that seeks to engage chefs as opinion leaders who are important to building greater support for local food systems. (shrink)

Many medical ethicists accept the thesis that there is no moral difference between withholding and withdrawing life-sustaining therapy. In this paper, we offer an interesting counterexample which shows that this thesis is not always true. Withholding is distinguished from withdrawing by the simple fact that therapy must have already been initiated in order to speak coherently about withdrawal. Provided that there is a genuine need and that therapy is biomedically effective, the historical fact that therapy has been initiated entails a (...) claim to continue therapy that cannot be attributed to patients who have not yet received therapy. This intrinsic difference between withholding and withdrawing therapy is of moral importance. In many instances, patients will waive this claim. But when one considers withdrawing therapy from one patient to help another in a setting of scarce resources, this intrinsic moral difference comes into sharp focus. In an era of shrinking medical resources, this difference cannot be ignored. (shrink)

A comprehensive formal system is developed that amalgamates the operational and the realistic approaches to quantum mechanics. In this formalism, for example, a sharp distinction is made between events, operational propositions, and the properties of physical systems.

"You have to come to my wedding," Kavita told me, turning to face me where I sat next to her on the couch. "You can come with the other people from the street. You will get everything you need for your *research* there." "I will come, I will come!" I replied enthusiastically. I had only met Kavita and her two younger sisters, Arthi and Deepti (see Figure 2.1), mere minutes before this invitation was extended. I had initially come to Pulan (...) that day in October 2012 to meet another woman, Heena, whose family rents a room on the third story of Kavita's family's home. Heena and I had been sitting in the furniture refurbishing store she operates with her husband on the main street of Pulan when Deepti, Kavita's youngest sister, passed by. Heena introduced us and told me to go with Deepti to meet her family. When we reached the family's three-story house-the largest in the gali-Deepti led me past the empty rooms on the ground floor, which I would eventually begin renting, to the second-story living room. There, we found Kavita and Arthi organizing clothing and jewelry they had purchased earlier in the day for the upcoming wedding festivities. Kavita made room for me to sit next to her on the couch and began asking me about myself. I immediately warmed to her because of her open, friendly smile and sharp, staccato Hindi, which I delighted in being able to understand. I explained that I had come to India to study how women's lives are different in rural and urban areas, and Kavita assured me that she and her family could help. She noted that her parents had come to Udaipur from Ram Nagar, a large village thirty-five kilometers north of the city, and that the family would be returning for her and her older brother Krishna's weddings the following month. Their weddings would be held five days apart to help reduce the difficulties of family members traveling from outside Udaipur. Prompted by the description of my research, Kavita commented on differences that she recognized between the village and the city. The biggest difference, she suggested, was the experience of caste, namely that in the village, people from different jatis live separately, whereas in the city, people are "mixed." As I would come to learn when visiting Ram Nagar for various functions, there is a fair amount of caste and religious diversity in the village. Although spatial and ritual segregation was rather strictly maintained during religious observances, it is likely more flexible in everyday life. The segregation during ritual functions-the occasions for which Kavita also traveled to the village-likely informed her sense of a lack of "mixing" in the village as. The majority of residents in the area of Ram Nagar where the family maintains a home were also from the Mali (lit: gardener) jati, although Mali was not a majority jati in Pulan. (shrink)

A recently formulated concept of stochastic localizability is shown to be consistent with a concept of stochastic microcausality, which avoids the conclusions of Hegerfeldt's no-go theorem as to the inconsistency of sharp localizability of quantum particles and Einstein causality. The proposed localizability on quantum space-time is shown to lead to strict asymptotic causality. For finite time evolutions, upper bounds on propagation to the exterior of stochastic light cones are derived which show that the resulting probabilities are too small to (...) be actually observable in a realistic context. The recent crisis in a prominent German car manufacturer generated by unethical leadership practices has brought into sharp focus, once again, the need for radical and fundamental ethical transformation among members of capitalism’s leadership elite. The divide between ethics and business needs to be closed and to do this effectively in a globalized world, cross-cultural aspects of moral sentiment need to be better understood. The current paper contributes to the extant literature in this regard by describing and analyzing cross-cultural (...) aspects of German and South African student’s ‘sympathy’ towards business ethical phenomena, using Adam Smith’s ‘Theory of Moral Sentiment’ as a theoretical framework and qualitative methods. The paper constructs a heuristic device based on Adam Smith’s theoretical framework and then proceeds to empirically analyze the theory by investigating German and South African student moral sentiments towards specific ethical leadership behaviors. The study indicates that while there is general cross-cultural homogeneity in moral approbation among students for fundamental aspects of ethical leadership behavior, nuanced custom-based differences emerge from the qualitative analysis. Following Adam Smith, fine grained differences in moral sentiment arising from ‘custom’ are evident. Thus, although ethicality of specific leadership behavior is found to be viewed similarly by both groups of students, significant nuanced differences arise in German students who emphasize intellectual autonomy over the conservatism favored by their South African counterparts. Practical aspects of these findings are briefly discussed. (shrink)
