As many as two million people in the United Kingdom repeatedly see people, animals, and objects that have no objective reality. Hallucinations on the border of sleep, dementing illnesses, delirium, eye disease, and schizophrenia account for 90% of these. The remainder have rarer disorders. We review existing models of recurrent complex visual hallucinations (RCVH) in the awake person, including cortical irritation, cortical hyperexcitability and cortical release, top-down activation, misperception, dream intrusion, and interactive models. We provide evidence that these can neither (...) fully account for the phenomenology of RCVH, nor for variations in the frequency of RCVH in different disorders. We propose a novel Perception and Attention Deficit (PAD) model for RCVH. A combination of impaired attentional binding and poor sensory activation of a correct proto-object, in conjunction with a relatively intact scene representation, bias perception to allow the intrusion of a hallucinatory proto-object into a scene perception. Incorporation of this image into a context-specific hallucinatory scene representation accounts for repetitive hallucinations. We suggest that these impairments are underpinned by disturbances in a lateral frontal cortex–ventral visual stream system. We show how the frequency of RCVH in different diseases is related to the coexistence of attentional and visual perceptual impairments; how attentional and perceptual processes can account for their phenomenology; and that diseases and other states with high rates of RCVH have cholinergic dysfunction in both frontal cortex and the ventral visual stream. Several tests of the model are indicated, together with a number of treatment options that it generates. Comparing the phenomenology, neurochemical pathology, and psychopharmacology of hallucinations and dreaming is limited by the available data. Evidence to date reveals no simple correspondence between the two states. Differences in the phenomenology of visual hallucinations and the visual component of dreams may reflect variations in visual context acting on the same underlying mechanism – the minimal visual input during dreaming contrasts with the more substantial perceived context in hallucinations. Variations in cholinergic, dopaminergic and serotonergic neurotransmitter function during sleep and during (...) hallucinations in Lewy body dementias, together with relevant drug effects suggest that, on the whole, different, potentially opposite, changes characterise the two states. A similar analysis of other psychotic features in Lewy body dementia and other disorders suggests that, in contrast to hallucinations, there may be more convincing parallels between dreaming and delusional states. Merker's core idea, that the experience of being conscious reflects the interactions of actions, targets, and motivations in the upper brainstem, with cortex providing the content of the conscious experience, merits serious consideration. However, we have two areas of concern: first, that his definition of consciousness is so broad that it is difficult to find any organisms with a brain that could be non-conscious; second, that the focus on one cortical–subcortical system neglects other systems (e.g., basal forebrain and brainstem cholinergic (...) systems and their cortical and thalamic target areas) which may be of at least equal significance. Commentators agree that the Perception and Attention Deficit (PAD) model is a promising model for accounting for recurrent complex visual hallucinations (RCVH) across several disorders, though with varying detailed criticisms. Its central tenets are not modified, but further consideration of generative models of visual processing and the relationship of proto-objects and memory systems allows the PAD model to deal with variations in phenomenology. The commentaries suggest new ways to generate evidence that will test the model.

Applying Behrendt & Young's (B&Y's) model of thalamocortical synchrony to complex visual hallucinations in neurodegenerative disorders, such as dementia with Lewy bodies and progressive supranuclear palsy, leads us to propose that the primary pathology may be cortical rather than thalamic. Additionally, the extinction of active hallucinations by eye closure challenges their conception of the role of reduced sensory input.

Matthew Walker's article has prompted us to consider neuropsychiatric disorders and pharmacological effects associated with sleep alterations, and aspects of memory affected. Not all disorders involving insomnia show memory impairment, and hypersomnias can be associated with memory deficits. The use of cholinergic medication in dementia indicates that consideration of the link between sleep and memory is more than academic.

Conversely, when I now say, “Palms are beautiful,” or “I love palms,” it (...) is really individual palms that I have in mind. This quote from Elaine Scarry’s On Beauty and Being Just applies perfectly to an experience I recently had regarding a painting by an artist I had assumed I didn’t much care for. Perhaps it would be best to lead into my epiphany gradually, to recreate the preparation that might have been necessary to have my realization. Actually, I speak of a double epiphany, in that it was shared by the friend I was travelling with when we saw the artwork in question. He and I are both painters, and have much in common, both in terms of our own work and our voracious appetite for seeing museums, so when I planned a trip for late October 2018 to see thirty museums in two weeks in Germany and Austria, I knew just whom to call. David Carbone and I have known each other, and appreciated each other’s work, since leaving graduate school in 1975, meeting at the College Art Association held that year in Washington, DC, where people compete for jobs while trying to maintain a veneer of civility. David made less of an effort than most, and I vividly remember him in saddle shoes he had painted bright blue, wearing a tie featuring a burlesque dancer, sipping champagne in the garden courtyard of the National Gallery. All the nearby aspiring art historians were immaculately dressed and scrupulously behaved, but David finished his drink, and in imitation of a arion 27.1 spring/summer 2019 Russian toast, threw his empty glass against the wall, where it noisily shattered. I like this guy, thought I. Over the next few days, visiting DC’s museums together, I recognized the brilliance of his commentaries on the art we saw. And his wit: He told of studying with his painting professor, who became frustrated with David’s pace in painting and said “David, painting is really very simple!” The reply: “But, what if you’re not simple?” I can’t conjure a single artist about whose work we’ve disagreed in the course of the fortythree years we’ve known each other. Meeting often in New York, where he lives with his wife Donna (who, like my wife, is always perfectly willing to let us go museum schlepping without them), we’ve also travelled in central Italy, and even flew to Fort Worth to see a show of the Le Nain brothers. A very sweet guard there asked us, “Who ARE you?” and when we said we were painters, she said, “Oh, because I’ve never seen anybody look at paintings this long, or talk about them so much!” Berlin is a treasure trove, where we visited ten fine museums, then flew to Vienna, which was hosting an extensive exhibit of Bruegel’s paintings, though feast might be a more apt word. A visual feast, but one that didn’t leave one sated, jaded, or bloated, but rather improved, made less ambivalent about membership in a questionable species. Yes, the viewer consumed a meal of war, famine, foolishness, and pride, but the chef made it not just palatable but beautiful, thought-provoking and humorous. Bruegel and his contemporary Rabelais seem kindred spirits, skeptical but amused, bawdy but tender, creating not just works but micro to macro worlds, an alternate world view, in fact. Each painting mesmerized anyone looking long and carefully enough, discovering that a tiny girl on a sled is able to get a purchase on the ice not just with her diminutive sticks, but by the miniscule iron nails in which they end, painted with a one hair-brush. From this minutia we graduate to a wider and wider view, one that comes to imply infinity. Bruegel’s brilliant guidance doesn’t confront us... (shrink)

This book closely examines our commonly held beliefs about human suffering, and offers unique insights into God's role in why we suffer. Dr. Perri critically examines what it means to be human from a Judeo-Christian perspective, and extrapolates from the work of Carl Gustav Jung showing a deeply complex development of human transcendence in human suffering. On an interpersonal level, Dr. Perri elaborates on the work of Martin Buber and Emanuel Levinas and shows how our suffering can be shared and (...) lessened by our unconditionality and presence for the other. Perceptions of Medieval Manuscripts takes as its starting point an understanding that a medieval book is a whole object at every point of its long history. As such, medieval books can be studied most profitably in a holistic manner as objects-in-the-world. This means readers might profitably account for all aspects of the manuscript in their observations, from the main texts that dominate the codex to the marginal notes, glosses, names, and interventions made through time. This holistic approach allows us to (...) tell the story of the book's life from the moment of its production to its use, collection, breaking-up, and digitization--all aspects of what can be termed 'dynamic architextuality'. The ten chapters include detailed readings of texts that explain the processes of manuscript manufacture and writing, taking in invisible components of the book that show the joy and delight clearly felt by producers and consumers. Chapters investigate the filling of manuscripts' blank spaces, presenting some texts never examined before, and assessing how books were conceived and understood to function. Manuscripts' heft and solidness can be seen, too, in the depictions of miniature books in medieval illustrations. Early manuscripts thus become archives and witnesses to individual and collective memories, best read as 'relics of existence', as Maurice Merleau-Ponty describes things. As such, it is urgent that practices fragmenting the manuscript through book-breaking or digital display are understood in the context of the book's wholeness. Readers of this study will find chapters on multiple aspects of medieval bookness in the distant past, the present, and in the assurance of the future continuity of this most fascinating of cultural artefacts. Much of the discussion surrounding the ethics of abortion has centered around the notion of personhood. This is because many philosophers hold that the morality of abortion is contingent on whether the fetus is a person - though, of course, some famous philosophers have rejected this thesis (e.g. Judith Thomson and Don Marquis). In this article, I construct a novel argument for the immorality of abortion based on the notion of impairment. This argument does not assume that the fetus is (...) a person - indeed, I concede (for the sake of argument) that the fetus is not a person - and hence the morality of abortion is not contingent on whether the fetus is a person. I finish by answering a plethora of objections to my argument, concluding that none of them are successful. (shrink)

Much has been said about intuitionistic and classical logical systems since Gentzen’s seminal work. Recently, Prawitz and others have been discussing how to put together Gentzen’s systems for classical and intuitionistic logic in a single unified system. We call Prawitz’ proposal the Ecumenical System, following the terminology introduced by Pereira and Rodriguez. In this work we present an Ecumenical sequent calculus, as opposed to the original natural deduction version, and state some proof theoretical properties of the system. We reason that (...) sequent calculi are more amenable to extensive investigation using the tools of proof theory, such as cut-elimination and rule invertibility, hence allowing a full analysis of the notion of Ecumenical entailment. We then present some extensions of the Ecumenical sequent system and show that interesting systems arise when restricting such calculi to specific fragments. This approach of a unified system enabling both classical and intuitionistic features sheds some light not only on the logics themselves, but also on their semantical interpretations as well as on the proof theoretical properties that can arise from combining logical systems.
