In today’s globalized world, nations cannot be totally isolated from or indifferent to their neighbors, especially in regards to medicine and health. While globalization has brought prosperity to millions, disparities among nations and nationals are growing raising once again the question of justice. Similarly, while medicine has developed dramatically over the past few decades, health disparities at the global level are staggering. Seemingly, what our humanity could achieve in matters of scientific development is not justly distributed to benefit everyone. In (...) this paper, it will be argued that a global theoretical agreement on principles of justice may prove unattainable; however, a grass-roots change is warranted to change the current situation. The UNESCO Declaration on Bioethics and Human Rights will be considered as a starting point to achieve this change through extracting the main values embedded in its principles. These values, namely, respecting human dignity and tending to human vulnerability with a hospitable attitude, should then be revived in medical practice. Medical education will be one possible venue to achieve that, especially through role models. Future physicians will then become the fervent advocates for a global and just distribution of health care. (shrink)

This paper was written to commemorate the 20th anniversary of the University of Nottingham’s Easter School on “Issues in Agricultural Bioethics,” organized by Ben Mepham in 1993. At that time, agricultural ethics was being envisioned as an interdisciplinary sub-discipline comparable to that of medical ethics. Agricultural ethicists would co-operate with other agricultural faculty to produce careful articulation, analysis and critique of norms and values being implicitly assumed by agricultural researchers, practitioners and policy makers. Roughly two factors have conspired to substantially (...) limit the realization of that vision in the intervening 20 years. First, the institutional environment within agricultural universities was far less conducive to such an activity than medical schools. Second, while the rise of a food-oriented social movement might have provided new opportunities for food ethics, the social movement orientation has actually constrained the kind of philosophical work typically undertaken by bioethicists. Accelerated changes to the planet have created novel spaces to re-imagine the boundaries and foci of environmental health research. Climate change, mass species extinction, ocean acidification, biogeochemical disturbance, and other emergent environmental issues have precipitated new population health perspectives, including, but not limited to, one health, ecohealth, and planetary health. These perspectives, while nuanced, all attempt to reconcile broad global challenges with localized health impacts by attending to the reciprocal relationships between the health of ecosystems, animals, and humans. While such (...) innovation is to be encouraged, we argue that a more comprehensive engagement with the ethics of these emerging fields of inquiry will add value in terms of the significance and impact of associated interventions. In this contribution, we highlight how the concept of spatial and temporal scale can be usefully deployed to shed light on a variety of ethical issues common to emerging environmental health perspectives, and that the potential of scalar analysis implicit to van Potter’s conceptualization of bioethics has yet to be fully appreciated. Specifically, we identify how scale interacts with key ethical issues that require consideration and clarification by one health, ecohealth, and planetary health researchers and practitioners to enhance the effectiveness of research and practice, including justice and governance. Islamic religious norms are important for Islamic bioethical deliberations. In Muslim societies religious and cultural norms are sometimes confused but only the former are considered inviolable. I argue that respect for Islamic religious norms is essential for the legitimacy of bioethical standards in the Muslim context. I attribute the legitimating power of these norms, in addition to their purely religious and spiritual underpinnings, to their moral, legal, and communal dimensions. Although diversity within the Islamic ethical tradition defies any reductionist or (...) essentialist reconstruction, legitimacy is secured mainly by approximation of Islamic ethical ideals believed to be inherent in the scriptural texts, rather than by the adoption of particular dogmatic or creedal views. With these characteristics, Islamic (bio) ethics may provide useful insights for comparative ethics and global bioethics. (shrink)

The carbon emissions of global health care activities make up 4–5% of total world emissions, placing it on par with the food sector. Carbon emissions are particularly relevant for health care because of climate change health hazards. Doctors and health care professionals must connect their health care delivery with carbon emissions and minimize resource use when possible as a part of their obligation to do no harm. Given that reducing carbon is a global ethical priority, the informed consent process in (...) health care delivery must change. I argue that the expanded role of bioethicists in this climate crisis is to promote and support “green informed consent:” the sharing of climate information with patients, offering options for lower-carbon health care, and accepting the patient’s right to decline treatments which are deemed too carbon intensive for their values. ABSTRACT There is a growing interest in various forms of naturalism in bioethics, but there is a clear need for further clarification. In an effort to address this situation, I present three epistemological stances: anti‐naturalism, strong naturalism, and moderate pragmatic naturalism. I argue that the dominant paradigm within philosophical ethics has been a form of anti‐naturalism mainly supported by a strong ‘is’ and ‘ought’ distinction. This fundamental epistemological commitment has contributed to the estrangement of academic philosophical ethics from major social (...) problems and explains partially why, in the early 1980s, ‘medicine saved the life of ethics’. Rejection of anti‐naturalism, however, is often associated with strong forms of naturalism that commit the naturalistic fallacy and threaten to reduce the normative dimensions of ethics to biological imperatives. This move is rightly dismissed as a pitfall since ethics is, in part, a struggle against the course of nature. Rejection of naturalism has drawbacks, however, such as detering bioethicists from acknowledging the implicit naturalistic epistemological commitments of bioethics. I argue that a moderate pragmatic form of naturalism represents an epistemological position that best embraces the tension of anti‐naturalism and strong naturalism: bioethics is neither disconnected from empirical knowledge nor subjugated to it. Common understandings of neuroethics, i.e., of its distinctive nature, are premised on two distinct sets of claims: (1) neuroscience can change views about the nature of ethics itself and neuroethics is dedicated to reaping such an understanding of ethics; (2) neuroscience poses challenges distinct from other areas of medicine and science and neuroethics tackles those issues. Critiques have rightfully challenged both claims, stressing how the first may lead to problematic forms of reductionism while the second relies on debatable assumptions about (...) the nature of bioethics specialization and development. Informed by philosophical pragmatism and our experience in neuroethics, we argue that these claims are ill-founded and should give way to pragmatist reconstructions. Namely, neuroscience, much like other areas of empirical research on morality, can provide useful information about the nature of morally problematic situations but it does not need to promise radical and sweeping changes to ethics based on neuroscientism. Furthermore, the rationale for the development of neuroethics as a specialized field need not to be premised on the distinctive nature of the issues it tackles or of neurotechnologies. Rather, it can espouse an understanding of neuroethics as both a scholarly and a practical endeavor dedicated to resolving a series of problematic situations raised by neurological and psychiatric conditions. Millions of people in various parts of the world and within each country are presently surviving in categories described as “mere”, “miserable”, “idealistic”, “irresponsible”, and “acceptable”. The term “acceptable survival” is proposed as a bioethical goal of global survival, looking beyond the 21st century to the year 3000 and beyond. The frequently used alternative term is “sustainable development”, but in most contexts this is an economic concept and does not imply any moral or ethical constraints, except where these are spelled (...) out. Acceptable survival, broadly defined, means acceptable to a universal sense of what is morally right and good and what will continue in the long term. O presente e o futuro da bioética Bioethics is going through a rough patch in the midst of a society that seems to run away very fast from true ethical and humanistic values. Post-humanism presents a new model of the human being, one that dispenses with the principles and concepts that have been employed so (...) far by humanity. The discourse in bioethics presents this model without any relationship to the anthropology that has been used for centuries and does so in a way that is totally lacking in ethical references. These dissimilarities reflect a misrepresented academic perspective that belongs to a post-truth era. Concepts such as dignity, human nature, quality of life, respect for life and vulnerability are not well understood. When it comes to decision-making on the so-called bioethical dilemmas, it lays out a new biotechnologically improved version of man that prevails over real health and biological concerns that need to be solved. The present article analyzes and outlines significant changes in ideas about the object of scientific knowledge in modern science. Special attention is paid to the transition to the paradigm of complexity, within which the object of scientific knowledge acquires a complex systemic character and remains in the same complex connections with systems of different levels. It is marked that such changes entail a number of methodological requirements, which are especially clearly manifested in modern theories of bioethics and its real practices. (...) A special role in this case is assigned to issues of transdisciplinary approach, complementary methodology, and values of the subject of bioethical cognition. Зокрема вірусами, бактеріями, а також пріонами. Наголошується на необхідності прийняття рішень політичного, економічного та філософського характеру, що включають посилення міжнародного контролю за роботою з особливо небезпечними збудниками, а також розробку конкретних програм протидії біотероризму. Підкреслюється, що на даний час вирішення проблем, пов'язаних з протидією біотероризму, є провідним завданням, яке безпосередньо стосується інтересів забезпечення безпеки не лише окремих країн, а й усієї планети загалом. Mindful of how the history of bioethics has often been presented, we explore the background, contributions, and influence of Van Rensselaer Potter on the roots of bioethics. In the last few decades, dozens of papers have been written and published, including several doctoral theses and defenses on V. R. Potter‘s concept of bioethics. In those works, the context of the emergence of Potter’s bioethics has sometimes been suggested, but never analyzed thoroughly. We identify seven pillars of influence for Van Rensselaer (...) Potter’s bioethical credo, drawing on several facts from the rich cultural heritage of Wisconsin, where Potter had lived and worked for the most part of his life. (shrink)

Bioethics can be considered as a topic, an academic discipline, a field of study, an enterprise in persuasion. The historical specificity of the forms bioethics takes is significant, and raises questions about some of these approaches. Bioethics can also be considered as a governance practice, with distinctive institutions and structures. The forms this practice takes are also to a degree country specific, as the paper illustrates by drawing on the author’s UK experience. However, the UNESCO Universal Declaration on Bioethics can (...) provide a starting point for comparisons provided that this does not exclude sensitivity to the socio-political context. Bioethics governance practices are explained by various legitimating narratives. These include response to scandal, the need to restrain irresponsible science, the accommodation of pluralist views, and the resistance to the relativist idea that all opinions count equally in bioethics. Each approach raises interesting questions and shows that bioethics should be studied as a governance practice as a complement to other approaches. (shrink)

This article enquiries whether normative bioethics can be a science. The article aims to address the conditions of possibility for bioethics to be considered a science, without directly answering the question. The article focuses on two conditions that we typically associate with our common concept of science: truth and knowledge, on the one hand, and naturalization, on the other. Bioethics should be able to provide moral truths and therefore moral knowledge so that we could consider it as a science. On (...) the other hand, the normative character of bioethics raises the question of whether it is possible to naturalize it and make it compatible with a scientific perspective. With regards to its origin, foundation and development, bioethics is a relatively new discipline, scientific and theoretical field, where different and even contradicting definition models and methodological patterns of its formation and application meet. In some philosophical orientations, bioethics is considered to be a sub-discipline of applied ethics as a traditional philosophical discipline. Yet in biomedical and other sciences, bioethics is designated as a specialist scientific discipline, or a sort of a new medical ethics. The concept of integrative bioethics as (...) an interdisciplinary scholarly and pluriperspectivistic area goes beyond such one-sided determinations, both philosophical and scientistic, and intends to integrate the philosophical approach to bioethics with its particular scientific contents, as well as different cultural dimensions and perspectives. This concept of integrative bioethics has gradually developed at philosophical and interdisciplinary conferences and institutions on the “bioethical islands” of the Croatian Mediterranean. In this paper, the author follows the formation, development and prospects of integrative bioethics in the wider region of the Mediterranean and Southeast Europe. (shrink)

What purpose can be served by empirically unsubstantiated speculation in ethics? In answering that question, we need to distinguish between the major branches of ethics. In foundational moral philosophy, the use of speculative examples is warranted to the extent that ethical principles and theories are assumed to be applicable even under the extreme circumstances referred to in these examples. Such an assumption is in need of justification, and it cannot just be taken for granted. In applied ethics, the use of (...) unrealistic scenarios is more difficult to justify. It can be positively harmful if it diverts our attention from more urgent issues. Neuroethics is one of the areas of applied ethics where speculative scenarios have taken up much of the attention that could instead have been devoted to problems that are relevant for the treatment and care of patients. Speculative ethics has often been defended with mere possibility arguments that may at first hand seem difficult to refute. It is shown with examples how such claims can be defeated with a combination of science and argumentation analysis. (shrink)

O pensamento filosófico na atualidade é caracterizado pela diversidade e pela pluralidade, o que permite interpretações singulares acerca do julgamento moral, como no caso da bioética. Considerando a especificidade das interpretações, o propósito deste artigo é apresentar pressupostos para pensar a bioética a partir da filosofia de Ricœur. Embora o filósofo tenha procurado elaborar uma ética, a bioética esteve sempre em seu horizonte, sendo designada ou de forma fragmentária, pelo conceito de éticas regionais, ou de forma robusta, como sabedoria prática.

Objective: To present a narrative review of the history of bioethics in Latin America and of scientific output in this interdisciplinary field. Methods: This was a mixed-methods study. Results: A total of 1458 records were retrieved, of which 1167 met the inclusion criteria. According to the Web of Science classification, the predominant topics of study were medical ethics, social sciences and medicine, and environmental and public health topics. Four themes of bioethics output in the Latin American literature have emerged: issues (...) involving the beginning and end of life, ethics in human research, patient–provider relationships, and ethics training for health professionals. Conclusion: Although bioethics is a growing interdisciplinary field in Latin America, its academic impact is still very low, and programmes are highly concentrated in large urban centres in a few countries. Sheep that did have problems wereoften assisted, recorded or marked and thenculled at an appropriate time; thus bothartificial (culling) and natural selection wereused. Secondly, natural selection enabled theimportant traits to be identified and they weresubsequently incorporated into artificialselection programs. Thirdly, the practice wasnecessitated by the impracticality ofsupervising lambing in difficult terrain andthe cost of skilled farm labor. Finally, it wasacknowledged that disturbance at lambingcreated problems and most importantly, theeasy-care approach reduced some of the problemstraditionally associated with lambing.Easy-care lambing systems thus aim to minimizesome of the detrimental effects associated withcarefully supervised lambing in someenvironments, by selecting sheep to suit boththat environment and modern farm management.They overcame pervasive influences our culturallegacy was exerting on the way we interact withanimals, and may have produced a system more inkeeping with the biology of the animal in anextensive environment. This essay addresses a moral and cultural challenge facing health care in the People’s Republic of China: the need to create an understanding of medical professionalism that recognizes the new economic realities of China and that can maintain the integrity of the medical profession. It examines the rich Confucian resources for bioethics and health care policy by focusing on the Confucian tradition’s account of how virtue and human flourishing are compatible with the pursuit of profit. It offers the Confucian account (...) of the division of labor and the financial inequalities this produces with special attention to China’s socialist project of creating the profession of barefoot doctors as egalitarian peasant physicians and why this project failed. It then further develops the Confucian acknowledgement of the unequal value of different services and products and how this conflicts with the current system of payment to physicians which has led to the corruption of medical professionalism through illegal supplementary payments. It further gives an account the oblique intentionality of Confucian moral psychology that shows how virtuous persons can pursue benevolent actions while both foreseeing profit and avoiding defining their character by greed. This account of Confucian virtue offers the basis for a medical professionalism that can function morally within a robustly profit-oriented market economy. The paper concludes with a summary of the characteristics of Confucian medical professionalism and of how it places the profit motive within its account of virtue ethics. (shrink)

This book series deals with religion and its interface with the state and society in Southeast Asia. It examines the multidimensional facets of politics, public policies and social change in relation to contemporary forms of religions, religious communities, thinking, praxis and ethos. All articles in this Book Series were a direct result of a policy-relevant research collaboration conducted by investigators from the participating countries from 2013–2016. The issues under examination in this Series include: state management of diversity, multicultural policies, religious (...) social activism and state-society relations. In this essay, and in his honor, I focus on two of physician-philosopher H. Tristram Engelhardt, Jr.’s many contributions, namely, his view that biomedical ethics cannot offer a singular content-driven theoretical approach and requires an appreciation of epistemologies of knowing in medicine. While these two positions remain controversial, because we all want definitive answers to our questions concerning what we ought to do in medicine and elsewhere, Dr. Engelhardt’s view makes possible discussion and debate in medicine to include diverse, defensible (...) ways of knowing. In the end, Dr. Engelhardt’s approach in biomedical ethics is one of shared decision making and negotiation. The authors introduce and make an attempt to describe the main problems that present and future populations of the underdeveloped world will be facing to provide enough food for themeselves. This essay describes the anachronistic situation where underdeveloped countries grow, with big deal of economical efforts, agricultural products that eventually will be used to grow and feed cattle whose meat does constitutes the principal component of the western world diet. Should this practice be reduced, underdeveloped countries will be able to (...) provide food for themeselves in large quantities. Ironically, meat diet and overfeeding, lead to a number of disease like overweight, heart attack which may lead to death. With the abnormal and speculative increase of oil price and with the “save the world from pollution” philosophy, farmers were induced, hoping to make more profit from their work, to turn their agricultural production into product which will be used to make ecological fuel like Ethanol, retrieving, by doing so, a lot of land and products from the food market. Adaptive success and evolution are determined by how we interact with the natural environment and all other forms of life. Yet in our pursuit to dominate the natural world, we have lost sight of this basic premise and continue to exploit natural resources, to contaminate, to consume more than necessary and to misuse our reproductive capacities. For this reason, global bioethics emerged in the 1980s, a culmination of mental resistance on the part of many observers who sought to readdress the (...) balance between humankind and nature – a balance which must be reinstated if we are to survive. Corrective measures are required, which should be free from purely religious or political influences because their ideologies are frequently founded on strategies of power, with little regard for the general well-being of all living species. Global bioethics, as opposed to bioethics, was formulated by myself, Van Rensselaer Potter, Antonio Moroni, Laura Westra and others, to transcend the restraints of science, uniting it with the humanities to create a new expanded consciousness, an alliance between life and the environment in which all factors – environmental, biological, physical, psychological, social and economic – recognize that they are inderdependent. BackgroundThe Western-European concept of libertarian rights-based autonomy, which advocates respect for individual rights, may conflict with African cultural values and norms. African communitarian ethics focuses on the interests of the collective whole or community, rather than rugged individualism. Hence collective decision-making processes take precedence over individual autonomy or consent. This apparent conflict may impact informed consent practice during biomedical research in African communities and may hinder ethical principlism in African bioethics. This study explored African biomedical researchers' perspectives regarding informed consent (...) and potential limitations to the principle of respect for autonomy in African communities.MethodsWe conducted a qualitative study based on in-depth interviews with 12 biomedical researchers, five females and seven males aged 34 to 74 years, currently working at an African university. Interviews lasted 35–40 min each and involved semi-structured open-ended interviews, which allowed participants to offer information about their perceptions and feelings regarding respect for autonomy and informed consent as practised in Africa. Empirical data from the interviews were recorded, transcribed, and analysed using thematic content analysis, together with an interrogation of relevant scientific literature about African communitarian ethics, making evaluations and drawing inferences consistent with the empirical bioethics approach.ResultsBased on these interviews and analysis of relevant literature, we found that informed consent is difficult to apply in an African context because it derives from a Western conception of libertarian rights-based autonomy. Most respondents pointed out that it was challenging to implement informed consent in the African setting. Furthermore, communalism, customary beliefs, spirituality, and relational autonomy are predominant in most African communities, as exemplified by the African moral philosophies of Ubuntu/botho and Ukama, which emphasize communitarianism over individual rights. We also found that language, education, poverty, and cultural beliefs are barriers to obtaining proper informed consent in African communities.ConclusionsWe conclude that there are limitations to applying the principle of respect for autonomy and informed consent in African communities, especially in the context of human biomedical research. We recommend using a more relational approach, such as Ross’s prima facie duties, to implement informed consent in African communities. (shrink)

Scholars, policymakers and organizations in the EU, especially at the level of the European Commission, have turned their attention to the ethics of (trustworthy and human-centric) Artificial Intelligence (AI). However, there has been little reflexivity on (1) the history of the ethics of AI as an institutionalized phenomenon and (2) the comparison to similar episodes of “ethification” in other fields, to highlight common (unresolved) challenges.Contrary to some mainstream narratives, which stress how the increasing attention to ethical aspects of AI is (...) due to the fast pace and increasing risks of technological developments, Science and Technology Studies(STS)-informed perspectives highlight that the rise of institutionalized assessment methods indicates a need for governments to gain more control of scientific research and to bring EU institutions closer to the public on controversies related to emerging technologies.This article analyzes how different approaches of the recent past (i.e. bioethics, technology assessment (TA) and ethical, legal and social (ELS) research, Responsible Research and Innovation (RRI)) followed one another, often “in the name of ethics”, to address previous criticisms and/or to legitimate certain scientific and technological research programs. The focus is on how a brief history of the institutionalization of these approaches can provide insights into present challenges to the ethics of AI related to methodological issues, mobilization of expertise and public participation. The aim of the present paper is to verify the qualitative typologies of “prescriptive, jurisprudential and doctrinal positivization” of universal ethical values by reporting different contemporaneous juridical experiences. Particular attention is devoted to problems such as voluntary interruption of pregnancy, assisted reproduction, euthanasia and refusal of medical treatment.

This article argues that environmental considerations fall within the scope of medical bioethics, and there are implications specific to medical education. It endorses the need to expand the scope and epistemology of contemporary medical bioethics discourse by including themes related to environmental considerations. Our discussion begins by providing a brief history of environmental bioethics. It then offers a critique of three specific health and environmental issues, namely technology, toxics, and consumption, and discusses how these issues are key to articulating moral (...) considerations of human health and subsequently medicine and its teaching. Lastly, it explores criticisms of including environmental issues into the bioethical debate before providing suggestions of how environmental ethics can be included into the medical curriculum. This article concludes by suggesting theoretical possibilities for environmentally inclusive bioethics, such as reorienting bioethical discussions to its original environmental advocacy and supporting environmental bioethics as a competency in medical education. Problems posed by HIV/AIDS differ from those ofpast epidemics by virtue of unique propertiesof the causative agent, dramatic societalchanges of the late 20th century, and thetransition of medical practice from aprofessional ethic to a technology-dependentbusiness ethic. HIV/AIDS struck during thecoming-of-age of molecular biology and also ofbioethics, and the epidemic stimulated thegrowth of both disciplines. The number ofarticles published about AIDS and ethics (asidentified by a MEDLINE search) peaked in 1990,just before the peak incidence of AIDS in theUnited States. The character (...) of ethicaldialogue has now shifted from familiar moralquandaries such as civil liberty versus publicwelfare to concerns about vaccine trials andpublic policy toward the developing world.Physicians and other health care workers whowere involved from the onset endured somethingof an emotional roller coaster. Theircompassion-based work ethic was to a largeextent replaced by a competence-based workethic after the introduction in 1996 of highlyactive antiretroviral therapy. The abundantrecent literature on ``professionalism'' inmedicine makes scant mention of AIDS/HIV. Thedisruptive effect of AIDS/HIV on society wouldhave been substantially greater had relevanttechnology such as the ability to isolateretroviruses and potent therapy againsttuberculosis not been in place. This soberingconsideration, along with such recent events asthe use of bioterrorism against civilianpopulations, suggests new relevance forPotter's definition of ``bioethics'' as a scienceof survival in which the biology of ecosystemsmust be taken into account. In the United States, disturbing concerns pertaining to both how putative bioethicists are perceived and the potential for the abuse of their power in connection with these perceptions compel close examination. This paper addresses these caveats by examining two fundamental and interrelated components in the image-construction of the ethicist: definitional and contextual. Definitional features reveal that perceptions and images of the ethicist are especially subject to distortion due to a lack of clarity as to the nature and qualifications of the (...) ethicist. Furthermore, the clinical, professional, political, academic, and linguistic contexts in which these ethicists are engaged are contexts of disquieting degrees of power. I argue that the lack of definitional clarity as to what constitutes an ethicist combined with the above volatile contexts together set the stage for the abuse of power on the part of ethicists. Throughout, I question the extent of self-critical analyses among ethicists, and, in view of these components in image-construction and their relationship to power, I challenge the degree of integrity within the field. En este trabajo se describen los datos aportados por 40 profesores de Educación Secundaria sobre su conocimiento y docencia en temas de bioética. El libro de texto, el debate y los trabajos de investigación en grupos reducidos son las herramientas habituales. Se comentan sus necesidades de formación y la metodología más adecuada para la enseñanza. La salud humana, la biotecnología y nuevos temas, como el maltrato animal, son los más comentadas entre los estudiantes, sin apreciarse diferencias de género. La bioética (...) puede plantearse como una transversal que fomente el trabajo interdisciplinar y la adquisición de las competencias. (shrink)
