Artykuł prezentuje krytyczny szkic historyczny i charakterystykę biomedycznego modelu psychopatologii, koncentrując się na drugiej połowie XX w. Wskazuje i opisuje także kluczowe problemy związane z jego dominacją we współczesnej praktyce klinicznej i badawczej. Problemy te dotyczą, m. in., trafności i rzetelności diagnoz psychiatrycznych, metodologii badań, skuteczności leczenia, czy też wpływów koncernów farmaceutycznych na działalność naukową i terapeutyczną. Poważne konceptualne problemy całego modelu stawiają pod znakiem zapytania fundamenty współczesnej psychiatrii oraz opartą na nich wiedzę. Nie udało się stworzyć przekonujących biologicznych wyjaśnień (...) zaburzeń psychicznych ani zidentyfikować mechanizmów odpowiadających za ich powstawanie. Uzasadnione wątpliwości istnieją także w odniesieniu do najczęściej przepisywanych współcześnie leków: przeciwpsychotycznych i antydepresyjnych – pomimo ich coraz szerszego stosowania, przybywa ludzi przewlekle chorych. Rodzi to poważne dylematy etyczne i uzasadnione pytania o nadmierną medykalizację kwestii wiązanych ze zdrowiem psychicznym. Konieczna wydaje się reforma dominującego modelu i wzrost nakładów na badania i praktykę uwzględniającą czynniki psychologiczne, społeczne i kulturowe. (shrink)

Next SectionBackground Scientific papers are retracted for many reasons including fraud (data fabrication or falsification) or error (plagiarism, scientific mistake, ethical problems). Growing attention to fraud in the lay press suggests that the incidence of fraud is increasing. Methods The reasons for retracting 742 English language research papers retracted from the PubMed database between 2000 and 2010 were evaluated. Reasons for retraction were initially dichotomised as fraud or error and then analysed to determine specific reasons for retraction. Results Error was (...) more common than fraud (73.5% of papers were retracted for error (or an undisclosed reason) vs 26.6% retracted for fraud). Eight reasons for retraction were identified; the most common reason was scientific mistake in 234 papers (31.5%), but 134 papers (18.1%) were retracted for ambiguous reasons. Fabrication (including data plagiarism) was more common than text plagiarism. Total papers retracted per year have increased sharply over the decade (r=0.96; p<0.001), as have retractions specifically for fraud (r=0.89; p<0.001). Journals now reach farther back in time to retract, both for fraud (r=0.87; p<0.001) and for scientific mistakes (r=0.95; p<0.001). Journals often fail to alert the naïve reader; 31.8% of retracted papers were not noted as retracted in any way. Conclusions Levels of misconduct appear to be higher than in the past. This may reflect either a real increase in the incidence of fraud or a greater effort on the part of journals to police the literature. However, research bias is rarely cited as a reason for retraction. (shrink)

Background Continued advances in human microbiome research and technologies raise a number of ethical, legal, and social challenges. These challenges are associated not only with the conduct of the research, but also with broader implications, such as the production and distribution of commercial products promising maintenance or restoration of good physical health and disease prevention. In this article, we document several ethical, legal, and social challenges associated with the commercialization of human microbiome research, focusing particularly on how this research is (...) mobilized within economic markets for new public health uses. Methods We conducted in-depth, semi-structured interviews (2009–2010) with 63 scientists, researchers, and National Institutes of Health project leaders (“investigators”) involved with human microbiome research. Interviews explored a range of ethical, legal, and social dimensions of human microbiome research, including investigators’ perspectives on commercialization. Using thematic content analysis, we identified and analyzed emergent themes and patterns. Results Investigators discussed the commercialization of human microbiome research in terms of (1) commercialization, probiotics, and issues of safety, (2) public awareness of the benefits and risks of dietary supplements, and (3) regulation. Conclusion The prevailing theme of ethical, legal, social concern focused on the need to find a balance between the marketplace, scientific research, and the public’s health. The themes we identified are intended to serve as points for discussions about the relationship between scientific research and the manufacture and distribution of over-the-counter dietary supplements in the United States. In this paper, we outline a framework for understanding the different kinds of knowledge required for medical practice and use this framework to show how scientism undermines aspects of this knowledge. The framework is based on Michael Polanyi’s claim that knowledge is primarily the product of the contemplations and convictions of persons and yet at the same time carries a sense of universality because it grasps at reality. Building on Polanyi’s ideas, we propose that knowledge can be described along two (...) intersecting “dimensions”: the tacit–explicit and the particular–general. These dimensions supersede the familiar “objective−subjective” dichotomy, as they more accurately describe the relationship between medical science and medical practice. Scientism, we argue, excludes tacit and particular knowledge and thereby distorts “clinical reality” and impairs medical practice and medical ethics. (shrink)
