Background Central to ethically justified clinical trial design is the need for an informed consent process responsive to how potential subjects actually comprehend study participation, especially study goals, risks, and potential benefits. This will be particularly challenging when studying deep brain stimulation and whether it impedes symptom progression in Parkinson’s disease, since potential subjects will be Parkinson’s patients for whom deep brain stimulation will likely have therapeutic value in the future as their disease progresses.Method As part of an expanded informed (...) consent process for a pilot Phase I study of deep brain stimulation in early stage Parkinson’s disease, an ethics questionnaire composed of 13 open-ended questions was distributed to potential subjects. The questionnaire was designed to guide potential subjects in thinking about their potential participation.Results While the purpose of the study was extensively presented during the informed consent process, in returned responses 70 percent focused on effectiveness and 91 percent included personal benefit as potential benefit from enrolling. However, 91 percent also indicated helping other Parkinson’s patients as motivation when considering whether or not to enroll.Conclusions This combination of responses highlights two issues to which investigators need to pay close attention in future trial designs: how, and in what ways, informed consent processes reinforce potential subjects’ preconceived understandings of benefit, and that potential subjects see themselves as part of a community of Parkinson’s sufferers with responsibilities extending beyond self-interest. More importantly, it invites speculation that a different paradigm for informed consent may be needed. (shrink)

Social media particularly Facebook has become a popular platform amongst medical professionals for both social and professional interactions. However, given the nature of such platforms, their use raises ethical concerns including violation of patient privacy and blurring of classical professional and patient-physician relationship boundaries. In order to investigate the pattern of Facebook usage among medical professionals in Pakistan, a mixed method study was conducted at five medical institutions in three different cities including Karachi, Lahore and Islamabad. 806 participants, including 87 (...) faculty members, 239 trainees and 350 medical students, responded to the quantitative survey. Results were analysed using Statistical Package for Social Sciences. For the qualitative arm, focus group discussions and in-depth interviews were conducted among [End Page 272] different groups, and analysed to draw out common themes. Results show that 44% of respondents found professional utility for Facebook. 36% of respondents had never changed their privacy settings and 44% indicated lack of confidence in current privacy settings of the medium. Respondents had posted identifiable patient content and non-identifiable patient content, with students and trainees posting such content more frequently than faculty. (shrink)

In a world surrounded by smart objects from sensors to automated medical devices, the ubiquity of ‘smart’ seems matched only by its lack of clarity. In this article, we use our discussions with expert stakeholders working in areas of implantable medical devices such as cochlear implants, implantable cardiac defibrillators, deep brain stimulators and in vivo biosensors to interrogate the difference facets of smart in ‘implantable smart technologies’, considering also whether regulation needs to respond to the autonomy that such artefacts carry (...) within them. We discover that when smart technology is deconstructed it is a slippery and multi-layered concept. A device’s ability to sense and transmit data and automate medicine can be associated with the ‘sting’ of autonomy being disassociated from human control as well as affecting individual, group, and social environments. Sollicitée au niveau expressif, technique, historique, non moins qu'anthropologique et politique, la musique prend part aux grands concepts deleuzo-guattariens - ritournelle, rhizome, agencements, machines - pour n'en citer que quelques-uns, désormais traduits dans de nombreuses langues. Un état des lieux des relations que la musique entretient avec la pensée de Gilles (...) Deleuze - domaine qui n'a pas encore fait l'objet d'une recherche approfondie en France - est apparu nécessaire. Ce premier ouvrage collectif en français réunit les travaux de musicologues, ethnomusicologues, compositeurs, philosophes et théoriciens de l'art qui font appel au corpus de sa pensée dans le domaine musical. Comment Deleuze s'inspire-t-il et s'instruit-t-il de la pratique musicale qui lui est contemporaine pour créer ses concepts, et en retour, quelle est l'incidence des lectures de ses textes sur la création, la musicologie, l'esthétique, l'ethnomusicologie, les techniques et les technologies musicales? The Baptist pastor John Gill believed the doctrine of eternal generation was vital to the Christian faith. While he firmly held to the doctrine of eternal generation, counting it as indispensable for grounding distinctions between the persons within the Godhead, he denied that the divine essence is communicated in generation. Generation, for Gill, entailed only the begetting of persons, and spoke to the ordering and personal relations between the Trinitarian Persons. As the second Person, the Son is from (...) the Father, but as God, he is of himself. This understanding of eternal generation flowed from Gill’s commitment to the aseity of all the divine Persons. According to Gill, each of the divine Persons fully possesses the essence without any communication of essence and without respect to their ordered subsistence. Each person equally, fully, and eternally partakes of the divine essence of himself. Gill’s affirmation of eternal generation was strengthened and elaborated by his understanding of the Son as the divine Word. Gill’s understanding of the Son as the divine Word incorporated the analogy of the mind, which was further understood by other Scriptural images and was further apprehended by the Son’s identification as Wisdom. Gill understood these analogies and names as mutually defining for understanding the nature of the Son of God. The central theological implications of this divine name, namely, the Son’s deity, eternality, and distinct personality, were all based on Gill’s reading of Scripture, most notably in the Gospel of John. Or, celui-ci a plusieurs visages. Déjà, dans Le Banquet, Pausanias distingue l'Éros "céleste" de celui "vulgaire". Le premier s'adresse à l'âme et aux Idées, en portant un désir alimenté par les formes culturelles propres aux arts libéraux. Le second signifie l'abandon charnel à la transe et à (...) la jouissance. Si le conflit entre Apollon et Dionysos se retrouve alors, Éros révèle aussi un troisième visage, plus primordial que les deux autres et nécessaire à leur opposition. Ces trois visages définissent chacun une esthétique musicale, tout en interrogeant l'élaboration corporelle et symbolique du sujet. Que ce soit dans un cadre éducatif ou thérapeutique, ou dans le contexte de la société postmoderne qui n'a retenu que le seul visage dionysiaque, ces trois Éros se meuvent selon un équilibre tendu, où se donne le sens même de l'humanisation. This paper reconstructs some issues of Deleuze’s theory of institution as they are exposed in two early texts: Empiricism and subjectivity and Instincts and Institutions. The problem of institution is there formulated for the frst time through the reading of Hume. My purpose is to reconnect the idea of institution as an artifcial system of ‘means of satisfaction’ to its theoretical background and to its further developments. The three essential concepts that I employ to defne the limits of this issue (...) are: animality, as institution allows the understanding of both identities and diferences between the human being and the animal; creativity, as, according to Deleuze, the institution is a positive model which is always created by the human being in order to satisfy his tendencies; law, as this conception is built in opposition to all theories which conceive law and limitation as the sources of the social feld. The complex hexaploid wheat genome offers many challenges for genomics research. Expressed sequence tags facilitate the analysis of gene-coding regions and provide a rich source of molecular markers for mapping and comparison with model organisms. The objectives of this study were to construct a high-density EST chromosome bin map of wheat homoeologous group 2 chromosomes to determine the distribution of ESTs, construct a consensus map of group 2 ESTs, investigate synteny, examine patterns of duplication, and assess the colinearity with rice (...) of ESTs assigned to the group 2 consensus bin map. A total of 2600 loci generated from 1110 ESTs were mapped to group 2 chromosomes by Southern hybridization onto wheat aneuploid chromosome and deletion stocks. A consensus map was constructed of 552 ESTs mapping to more than one group 2 chromosome. Regions of high gene density in distal bins and low gene density in proximal bins were found. Two interstitial gene-rich islands flanked by relatively gene-poor regions on both the short and long arms and having good synteny with rice were discovered. The map locations of two ESTs indicated the possible presence of a small pericentric inversion on chromosome 2B. Wheat chromosome group 2 was shown to share syntenous blocks with rice chromosomes 4 and 7. (shrink)

La critique que peut porter la philosophie économique se comprend comme un partage bien pensé entre bon et mauvais usage de la raison, comme méthodologie ainsi que comme ontologie, une discussion du rôle des sciences adjacentes (comme la psychologie), de l'usage des normes, des modes de raisonnement, l'explicitation des (...) bases trop souvent dans l'ombre de l'analyse économique vue en ses champs d'application multiples : simulation, analyse institutionnelle, finance et autres enjeux. Les contributeurs du présent ouvrage, connus pour leur expertise dans leurs champs respectifs, en proposent un examen représentatif des tendances actuelles. Le collectif ici réuni s'inscrit dans une tradition, celle des Leçons de philosophie économique coordonnées par Alain Leroux et Pierre Livet, qui avait marqué une étape dans ce domaine. La philosophie économique comprise comme mise en question réflexive de la discipline trouve avec le présent état des lieux un nouveau jalon : au titre d'un regard épistémologique, il offre à nouveaux frais la carte d'un champ en plein essor, et ce avec une ampleur renouvelée par les auteurs francophones ici rassemblés. In Différence et répétition, the transcendental field is presented through the concepts of Idea and intensity. A lot has been said about these concepts, but the relation between them is still a subject of dispute. In this article we aim to shed light upon this knot. First, we will show that intensity is actual, and not virtual nor intermediate like some interpretations suggest. In this point, our reading of the book from 1968 is complemented with Le pli. Leibniz et le (...) Baroque, to show that the actual status of intensity remains the same in the late works of Deleuze. Then, we will explore the relation between intensity and the three characters of the Idea and with the dramatization commanding actualization. After that, the bond between Idea and intensity will be thought through the problem of expression. This results, at last, in a complex articulation between these two planes from the point of view of their order of constitution. In this point, it will be key to put together concepts from both Différence et répétition and Logique du sens. (shrink)

Car la voix risque de disparaître dans l'épaisseur même des lettres ; les textes réifiés de perdre leur pouvoir de dire. Re-susciter la voix au coeur de la lettre, faire retentir son grain, son souffle, son amplitude et ses nuances : telle doit être l'ambition d'une véritable lecture. Pour Benny Lévy, la pensée fut indissociable d'un tel travail de lecture, l'amenant à reprendre à neuf les textes, à en renouveler la compréhension. Deleuzian criticisms of representational thinking converge on the understanding of thought as a recognitive process. To recognize is to think. Among all the problems concerning such a model of thought, there is a fundamental obstacle to recognition: it does not recognize what does not fit into its previously established premises and which delimit the linearity of correct thinking. Thus, as Deleuze warns us, the difference has never been thought for itself. In such a notion, the difference is just an opposition (...) to the equal. In one of his dialogues with the philosophical tradition, the French thinker found a punctual gap in the thinking of the Greek philosopher Aristotle where difference is brought up. A moment when the difference emerged and was almost considered by itself, and which was called "the happy Greek moment" by Deleuze. It is about Aristotelian thought of difference, that Greek moment and the conceptual criticism that Deleuze conducts to compose his theory that the article will deal with. (shrink)

In _Annotations_ Nahum Dimitri Chandler offers a philosophical interpretation of W. E. B. Du Bois’s 1897 American Negro Academy address, “The Conservation of Races.” Chandler approaches Du Bois as a generative and original philosophical thinker-writer on the status and historical implication of matters of human difference, both the fact of and the very idea thereof. Chandler proposes both a close reading of Du Bois’s engagement of the concept of so-called race and a deep meditation on Du Bois’s (...) conceptualization of historicity in general. He elaborates the way Du Bois’s thought in this address can give an account of the organization of the historicity that yields the emergence of something like the African American, at once with its own internal dimensions and yet also as an originary articulation of forces and possibilities that are world historical implication. Chandler refigures Du Bois’s thought as a vital theoretical resource for rethinking our concepts of differences among humans and, so too, our understanding of modern historicity itself. (shrink)
