OBJECTIVE: How outside factors affect physician decision making remains an open question of vital importance. We sought to investigate the importance of various influences on physician decision making when clinical guidelines differ from patient preference. -/- METHODS: An online survey asking 469 primary care providers (PCPs) across four practice sites whether they would order magnetic resonance imaging for a patient with uncomplicated back pain. Participants were randomized to one of four scenarios: a patient's preference for imaging (control), a patient's preference (...) plus a colleague's opinion against imaging (colleague), a patient's preference plus a professional society's recommendation against imaging (profession), or a patient's preference plus an accountable care organization's quality metric that measures physician use of imaging (ACO). Demographic information and the reasoning behind participants' decisions also were obtained. -/- RESULTS: A total of 168 PCPs completed the survey, yielding a 36% completion rate. A majority chose not to pursue imaging: control 68%, colleague 85%, profession 87%, and ACO 78%. Multivariate logistic regression revealed that participants were more likely not to order advanced imaging only when reminded of a professional society recommendation (P = 0.017). Regression also suggested that practice site exerted an effect on the primary outcome. Evidence-based medicine and clinical judgment were the most cited reasons for the decision. -/- CONCLUSIONS: Our results reinforce the potential to leverage professional societies to advance evidence-based medicine and reduce unnecessary testing. At the same time, practice site appeared to exert influence, suggesting that these recommendations must be part of local institutional culture to be effective. (shrink)

In essence, it is altogether unclear how to simultaneously substantiate, on the one hand, the claim that unsaturated entities must be recognized in order to account for unity and, on the other, the claim that unsaturatedness is incompatible with objecthood. Are there ethics justifying anti-colonial violence? How and why did the violence and visions of nationalist movements become incorporated by colonial and neo-colonial rule? Using the insurrection by the Malayan Communist Party (1948–1960) as an example, this book argues that resorting to violence sped up the decolonisation of British Malaya by forcing its colonial administration to invent Malay nationalism and pursue ameliorative social policy among the Chinese diaspora community in a manner clearly derived from the Party’s platform. Yet this was (...) not the same as giving the country economic emancipation from the expectations of neo-colonial rule. Violence and Emancipation in Colonial Ideology entertains no warm colonial memories of the cold war years. Confirming Price’s reputation as a plain speaking critic of Empire apologia, this book asks how colonial ideology was considered to be beneath Europe yet desperately needed by it. He faces down nostalgic communities defending an outdated view that “might was right” in South East Asia and that communism failed to contribute to the world that came to be. Using an Althusserian assumption, the book begs the question: if a late colonial state was subjective, then how did it claim a sufficiently objective mantle to rule and how did ideological techniques enable this? Concepts employed in folk descriptions of the world often turn out to be more perspectival than they seem at first sight, involving previously unrecognised sensitivity to the viewpoint or 'situation' of the user of the concept in question. Often, it is progress in science that reveals such perspectivity, and the deciding factor is that we realise that other creatures would apply the same concepts with different extension, in virtue of differences between their circumstances and ours. In this paper I argue (...) that causal concepts are perspectival in this way, and describe the 'situation' on which they depend in terms of an abstract characterisation of the viewpoint of a deliberating agent. I argue that this approach makes better sense than rivals of the apparent asymmetry and temporal orientation of the causal relation. (shrink)

Different questions generate different forms of practical reasoning. A contextually unrestricted ‘What shall I do?’ is too open to focus reflection. More determinately, an agent may ask, ‘Shall I do X, or Y?’ To answer that, he may need to weigh things up—as fits the derivation of ‘deliberation’ fromlibra(Latin for ‘scales’). Ubiquitous and indispensable though this is, I mention it only to salute it in passing. Or he may ask how to achieve a proposed end: if his end is to (...) do X, he may ask ‘How shall I do X?’ Or he may ask how to apply a universal rule or particular maxim. William James said that sometimes detailed philosophical argument is irrelevant. Once a current of thought is really under way, trying to oppose it with argument is like planting a stick in a river to try to alter its course: “round your obstacle flows the water and ‘gets there just the same’”. He thought pragmatism was such a river. There is a contemporary river that sometimes calls itself pragmatism, although other titles are probably better. At any rate it is the denial (...) of differences, the celebration of the seamless web of language, the soothing away of distinctions, whether of primary versus secondary, fact versus value, description versus expression, or of any other significant kind. What is left is a smooth, undifferentiated view of language, sometimes a nuanced kind of anthropomorphism or “internal” realism, sometimes the view that no view is possible: minimalism, deflationism, quietism. Wittgenstein is often admired as a high priest of the movement. Planting a stick in this water is probably futile, but having done it before I shall do it again, and—who knows?—enough sticks may make a dam, and the waters of error may subside. How do rational minds make contact with the world? The empiricist tradition sees a gap between mind and world, and takes sensory experience, fallible as it is, to provide our only bridge across that gap. In its crudest form, for example, the traditional idea is that our minds consult an inner realm of sensory experience, which provides us with evidence about the nature of external reality. Notoriously, however, it turns out to be far from clear that there is any viable (...) conception of experience which allows it to do the job. The original problem is to show that thought is rationally constrained by external reality. If sensory experience is to provide the solution--in particular, if it is to provide the answer to sceptical challenges--it must therefore meet two criteria. First, it must itself be `receptive'--i.e., appropriately constrained by external reality. Second, it must be the kind of thing that can enter into a logical or rational relationship with belief--it must already be `conceptual,' in other words. In arguing against the idea that anything could serve both roles, Wilfred Sellars termed this conception of experience "the Myth of the Given.". (shrink)

Soluble components of Notch signalling can be applied to manipulate a central pathway essential for the development of metazoans and often deregulated in illnesses such as stroke, cancer or cardiovascular diseases. Commonly, the Notch cascade is inhibited by small compound inhibitors, which either block the proteolysis of Notch receptors by γ‐secretases or interfere with the transcriptional activity of the Notch intracellular domain. Specific antibodies can also be used to inhibit ligand‐induced activation of Notch receptors. Alternatively, naturally occurring endogenous inhibitors of (...) Notch signalling might offer a specific way to block receptor activation. Examples are the soluble variants of the canonical Notch ligand Jagged1 and the non‐canonical Notch ligand Dlk1, both deprived of their transmembrane regions upon ectodomain shedding, or the bona fide secreted molecule EGFL7. We present frequently used methods to decrease Notch signalling, and we discuss how soluble Notch inhibitors may be used to treat diseases. (shrink)

There is fierce controversy in the United States over whether parents should be able to choose their children's schools and/or curriculum. To discuss the pedagogical arguments inherent in this question, Todd Alan Price and Ruprecht Mattig begin with the classical concept of Bildung as developed by Wilhelm von Humboldt around 1800. Next, they compare Humboldt's ideas with the ideas of John Stuart Mill and Milton Friedman, who stand in the tradition of liberal thought, as Mill was strongly influenced by (...) Humboldt and Friedman, in turn, by Mill. Finally, they consider Friedman's role as a central figure in the school choice movement, and are thus able to trace a line of thought from Humboldt to actual controversies. Price and Mattig's analysis here shows that the liberalism of Humboldt, Mill, and Friedman revolves around diversity and choice in education. Moreover, it traces that in the process of translating, translocating, and transcontextualizing Humboldt's original thought through Mill to Friedman, the complexity of arguments about education and the role of parents and the state in a liberal society has been unduly reduced. The authors conclude the paper by proposing a principle for thinking about the relationship between different educational institutions in democracies. (shrink)

In this paper, I try to develop an account of functions which might be of use to a biologist engaged in classifying and explaining natural phenomena. The most pressing difficulty facing such an account is the need to reconcile the normativity of function statements with their explanatory force. I consider two familiar accounts of function statements, offered by Andrew Woodfield and Larry Wright . I examine both accounts in search of the strongest possible formulation of each type of theory. I (...) fail to find a formulation of the ACT which is completely immune from counterexamples, but I do find a satisfactory formulation of the HT. In particular, I argue that the HT should incorporate a point central to the ACT that functions involve a means/end relationship between two devices. I then consider functional explanations: I argue that an ACT which holds that function statements are intrinsically explanatory cannot offer a satisfactory account. The account offered by Woodfield, who rejects this assumption, is adequate, though less attractive than the account suggested by the HT. I conclude by considering how the HT can account for functional norms. (shrink)
