The aim of this study was to elucidate municipal night registered nurses’ (RNs) experiences of the meaning of caring in nursing. The research context involved all night duty RNs working in municipal care of older people in a medium-sized municipality located in central Sweden. The meaning of caring in nursing was experienced as: caring for by advocacy, superior responsibility in caring, and consultative nursing service. The municipal night RNs’ experience of caring is interpreted as meanings in paradoxes: ‘being close at (...) distance’, the condition of ‘being responsible with insignificant control’, and ‘being interdependently independent’. The RNs’ experience of the meaning of caring involves focusing on the care recipient by advocating their perspectives. The meaning of caring in this context is an endeavour to grasp an overall caring responsibility by responding to vocational and personal demands regarding the issue of being a RN, in guaranteeing ethical, qualitative and competent care for older people. This paper describes the meaning of a father’s presence with a full-term healthy child delivered by caesarean section, as observed during the routine post-operative separation of mother and child. Videotaped observations recorded at a maternity clinic located in the metropolitan area of Stockholm, Sweden formed the basis for the study, in which fifteen fathers with their infants participated within two hours of elective caesarean delivery in the 37th - 40th week of pregnancy. A phenomenological analysis based on Giorgi’s method was (...) conducted on the data. The description of the new father’s experiences that emerged pointed to a process of being and becoming in taking the child to himself. Fatherhood developed gradually as a result of recurrent experiences of the child’s expressions. There was an ebb and flow between taking on the role of being a father and physical withdrawal from the role. The findings of this study not only confirm previous accounts of new fathers’ experiences, but go further in revealing an ebb and flow variation in the fathers’ involvement. What this indicates is that the process of transition to fatherhood requires not only presence but time. The period required for this process thus must not be disturbed, but supported, trusting in the father’s ability to assume his role as a father. It is suggested that, in addition to their relevance in guiding the attitudes and expectations of those professionally involved in postnatal care and community health, these findings could be useful in antenatal courses for parents, and especially in instances when caesarean birth is planned, to highlight the meaning of the role of fathers as caregivers. In an important and timely book, Anneli Jefferson outlines a view according to which a given mental disorder is a brain disorder if it is a (harmful) mental dysfunction realised by a brain dysfunction. Prima facie, Jefferson’s book is a study in the metaphysics of dysfunction: how does mental dysfunction relate to brain dysfunction, and what does this imply for the status of mental disorders and brain disorders? In what follows, I shall argue that Jefferson’s contribution to this debate is (...) better understood as a conceptual explication of how psychiatrists, some philosophers, scientists and clinicians in the field (broadly construed) think of the label ‘brain disorder’. I infer this on the basis that Jefferson’s thesis would not follow from any serious, well-worked out theory of proper function. Despite initial appearances, Jefferson’s book about brain dysfunction is not about function at all. (shrink)

The aim of the study was to describe and compare whether women who used the pill or condoms discussed the choice of contraceptive method with their partner, and their sexual activity and interaction with their partner. It also studied women's thoughts about the attitudes of their partner and close others concerning unplanned pregnancy and abortion. A total of 134 women, aged 23-29, who had a stable partner relationship, answered three questionnaires. The contraceptive pill was used by 94 of the women (...) and 40 used condoms. The results show that most women discussed and planned choice and use of contraceptive method with their partner. However, more women whose partner used the condom discussed and talked about the use of the contraceptive method than women who used the pill. Independent of the contraceptive method, the women stated that non-agreement with the partner about the method resulted in no intercourse. More women who used the pill reported that they sometimes forgot to use it compared to women who used the condom. This latter group also stated to a higher degree that unplanned pregnancy would result in positive attitudes on the part of their partner and close others. (shrink)

According to the Argument for Autonomous Mental Disorder, mental disorder can occur in the absence of brain disorder, just as software problems can occur in the absence of hardware problems in a computer. This article argues that the AAMD is unsound. I begin by introducing the “natural dysfunction analysis” of disorder, before outlining the AAMD. I then analyze the necessary conditions for realizer autonomous dysfunction. Building on this, I show that software functions disassociate from hardware functions in a way that (...) mental functions do not disassociate from brain functions. Justin Garson has recently advanced a Generalised Selected Effects Theory of biological proper function. According to Garson, his theory spells trouble for the Dysfunction Account of Disorder. This paper argues that Garson’s critique of the Dysfunction Account from the Generalised Theory fails, and that we should reject the Generalised Theory outright. I first show that the Generalised Theory does not, as Garson asserts, imply that neurally selected disorders are not dysfunctional. Rather, it implies that they are both functional and dysfunctional. This paper asks when a natural disease kind is truly 'reactive' and when it is merely associated with a corresponding social kind. I begin with a permissive account of real kinds and their structure, distinguishing natural kinds, indifferent kinds and reactive kinds as varieties of real kind characterised by super-explanatory properties. I then situate disease kinds within this framework, arguing that many disease kinds prima facie are both natural and reactive. I proceed to distinguish ‘simple dependence’, ‘secondary dependence’ and ‘essential (...) dependence’ between a natural kind and its classification, and argue that a natural kind is only really reactive, in an important sense, under conditions of essential dependence. On this basis, I offer a principled hypothesis for why psychiatric kinds tend to be more metaphysically unstable than paradigm somatic disease kinds. (shrink)

Defining Mental Disorder: Jerome Wakefield and His Critics, edited by Denis Forest and Luc Faucher, is essential reading for students and researchers in philosophy of medicine whose work is informed by that of Jerome Wakefield, or the disease debate in general. If you are anything like me, this book will open the door to a new depth of understanding of the harmful dysfunction analysis (HDA) and its methodical underpinnings, and an enriched appreciation of what is at stake in defining medical (...) disorder. To those confident that they understand Wakefield’s view and the surrounding debate, thinking perhaps that they do not need a refresher, I would say: perhaps you do not understand it as well as you think. Defining Mental Disorder provides much more than an overview of previously well-trodden ground. The chapters themselves, and Wakefield’s own detailed responses, help to elucidate a number of methodological nuances and peculiarities that have rarely been covered in such depth before. I was startled to learn how philosophically complex Wakefield’s position actually is, and how shallow my own understanding of it has been. It emerges as a far more nuanced and sophisticated account than I had given it credit for, and one that deserves serious reevaluation. (shrink)

The aim of this study was to illuminate the meaning of professional carers’ experiences in caring situations when a conflict of interest arises with the older person receiving care. The findings reveal the complexity of the carers’ ambivalence when facing a conflict of interest, weighing up between the older persons’ right to self-determination and external demands. The carers are alone in their ambivalence, and the conclusion is that they need help and support to be more present in the encounter. The (...) implication for this study is to focus on care as a person-centered practice and to focus on people as interdependent on support carers to maintain older people’s right to self-determination in the relationship. This article examines Engelhardt’s thesis from the standpoint of liturgical theology. Fagerberg’s previous work has claimed that liturgy gives birth to theology in such a way that liturgy is the ontological condition for theology, as Schmemann said. If we apply this approach to the question at hand, we will understand liturgy to be the source and foundation also for Christian morality. This is no particular surprise, since the Christian tradition has always integrated liturgy, theology, and asceticism, that last named (...) treating morality as a pathway to deification. Liturgical theology views liturgy as a pneumatic exercise that makes theologian souls, and liturgical asceticism understands such theologian souls to be molded by the doings of the body, and this delivers us to the threshold of liturgical morality, for in liturgy the God’s-eye perspective shares its light with us. The disease debate in philosophy of medicine has traditionally been billed as a debate over the correct conceptual analysis of the term “disease.” This paper argues that although the debate’s participants overwhelmingly claim to be in the business of conceptual analysis, they do not tend to argue as if this is the case. In particular, they often show a puzzling disregard for key parameters such as precise terminology, linguistic community, and actual usage. This prima facie strange feature of the debate (...) points to an interesting and potentially instructive hypothesis: the disease debate makes little sense within the paradigm of conceptual analysis but makes good sense on the assumption that pathology is a real kind. Nursing Inquiry 2012; 19: 247–258 Patients’ statements and experiences concerning receiving mechanical ventilation: a prospective video‐recorded studyProspective studies using video‐recordings of patients during mechanical ventilator treatment (MVT) while conscious have not previously been published. The aim was to describe patients’ statements, communication and facial expressions during a video‐recorded interview while undergoing MVT. Content analysis and hermeneutics inspired by the philosophy of Gadamer were used. The patients experienced almost constant difficulties in breathing and lost (...) their voice. The most common types of communication techniques patients used were nodding or shaking the head. Their expressions were interpreted as stiffened facial expression, tense body position and feelings of sadness and sorrow. Nursing care for patients’ conscious during MVT is challenging as it creates new demands regarding the content of the care provided. In caring for patients undergoing MVT while conscious, establishing a caring relationship, making patients feel safe and helping them to communicate seem to be most important for alleviating discomfort and instilling hope. This article examines one of the relevant concepts in the current debate on home birth—autonomy in place of birth—and its uses in general language, ethics, and childbirth health care literature. International discussion on childbirth services. A concept analysis guided by the model of Walker and Avant. The authors suggest that autonomy in the context of choosing place of birth is defined by three main attributes: information, capacity and freedom; given the antecedent of not harming others, and the consequences of accountability (...) for the outcome. Model, borderline and contrary cases of autonomy in place of birth are presented. A woman choosing place of birth is autonomous if she receives all relevant information on available choices, risks and benefits, is capable of understanding and processing the information and choosing place of birth in the absence of coercion, provided she intends no harm to others and is accountable for the outcome. The attributes of the definition can serve as a useful tool for pregnant women, midwives, and other health professionals in contemplating their moral status and discussing place of birth. (shrink)

This book is a joint effort of like-minded researchers to define the concept of process within a psychological setting. Although minor differences exist as regards choice of background theory, their common focus is on personality in a broad psychodynamic context. Their definition of personality rests on a series of test instruments that have been validated during decades of thorough and vigorous empirical work. These were originally designed to open up micro-processes underlying the adaptation to or construction of reality, and have (...) subsequently proven diagnostically efficient. Coming from both sides of the Atlantic, the contributors have their background in psychological as well as medical institutions. In this article a case is made for considering the liturgy as theological norm par excellence. The case is built up by relying on an emphatic current of thought within the field of liturgical studies, namely the ‘liturgical theology’ as it was developed by Alexander Schmemann, Aidan Kavanagh, and David W. Fagerberg. After presenting the concept of ‘liturgical theology’ and the context out of which it emerged, its major characteristics are discussed. Particular attention is devoted to the radicalness of (...) their position. It can be called radical because the reversal of the relation between doctrine and liturgy is by no means evident for the vast majority of modern believers and theologians. However, ‘liturgical theology’ claims that it is not doctrine which determines liturgy but liturgy which determines doctrine. According to liturgical theologians, the liturgy is not simply the ritual expression of the content of faith, but itself theology, even theologia prima . Correspondingly, liturgical theologians point to the original wording of the famous adage lex orandi lex credendi , which is the following: ut legem credendi lex statuat supplicandi . Whereas the usual formulation suggests equality and mutual dependence, the original context lays bare a clear priority of the rule of prayer over the rule of faith. In the final part of the article I explore some avenues to the evaluation of ‘liturgical theology’. I argue that there is need for a more profound philosophical underpinning and historical adequacy. But nevertheless the idea that the liturgy constitutes a theological norm stands firmly and should be considered far more broadly and seriously among contemporary systematic theologians. (shrink)

ABSTRACTThis article investigates some salient features of the fascination for the monk in contemporary scholarship. Interestingly, the figure of the monk has attracted the attention of authors engaged in fields as diverse as political philosophy and liturgical theology, clearly without referring to one another. On the one hand, the much talked-about Italian philosopher Giorgio Agamben discusses the liturgical heritage of Western civilization to better understand the mechanisms behind modern politics and economy. In that context, he sees the monk as someone (...) in whom rules and life have become indistinct. On the other hand, liturgical theologian David W. Fagerberg extensively deals with the monk in his groundbreaking study on liturgical asceticism. He seems to make a similar point as Agamben: the monk exemplifies not simply obedience to rules but embodies the very life a Christian is supposed to live. This article compares both frameworks of thought to see whether the one can be used to critically evaluate the other and to propose some reflections about formation. (shrink)
