It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

This paper considers an approach to teaching ethics in bioengineering based on the How People Learn (HPL) framework. Curricula based on this framework have been effective in mathematics and science instruction from the kindergarten to the college levels. This framework is well suited to teaching bioengineering ethics because it helps learners develop “adaptive expertise”. Adaptive expertise refers to the ability to use knowledge and experience in a domain to learn in unanticipated situations. It differs from routine expertise, which requires using (...) knowledge appropriately to solve routine problems. Adaptive expertise is an important educational objective for bioengineers because the regulations and knowledge base in the discipline are likely to change significantly over the course of their careers. This study compares the performance of undergraduate bioengineering students who learned about ethics for stem cell research using the HPL method of instruction to the performance of students who learned following a standard lecture sequence. Both groups learned the factual material equally well, but the HPL group was more prepared to act adaptively when presented with a novel situation. (shrink)

Background Advance Care Planning via Group Visits (ACP-GV) is a patient-centered intervention facilitated by a clinician using a group modality to promote healthcare decision-making among veterans. Participants in the group document a “Next Step” to use in planning for their future care needs. The next step may include documentation of preferences in an advance directive, discussing plans with family, or anything else to fulfill their ACP needs. This evaluation seeks to determine whether there are identifiable subgroups of group participants with (...) differing needs prior to delivery of the ACP-GV program and, if so, to use information about the subgroups to enhance the program offered to veterans in United States Department of Veterans Affairs (VA) healthcare settings. Methods We conducted a secondary analysis of national data from a quality improvement evaluation. Patient- and provider-level data from administrative healthcare records for VA users in all 50 states, territories, and the District of Columbia provides data on veterans attending ACP-GV during federal fiscal years 2018–2022 (N = 26,857). Latent class analysis seeks to identify the various subgroups of veterans based on their level of ACP self-efficacy before attending ACP-GV and any demographic differences across the resulting subgroups of veterans attending ACP-GV. ACP self-efficacy is derived from seven items obtained from a participant worksheet used during the group. Results Analysis revealed two distinct groups of veterans, distinguishable by their pre-ACP-GV levels of one aspect of ACP self-efficacy: prior knowledge of ACP. Veterans with higher prior knowledge of ACP are associated with an identified next step focused on checking their current AD status and updating it, and veterans with lower ACP prior knowledge are associated with identifying a next step to discuss ACP more fully with family. Differences in age, sex, race, ethnicity, and marital status exist across subgroups of veterans. Conclusion Greater attention must be paid to ACP and veterans’ prior knowledge of ACP to consistently encourage annual review and status updates. (shrink)

Jack Miles, a former member of the Society of Jesus (Jesuits), and Mark Taylor, a philosophical atheist, have both in different ways brought religious and philosophical concerns into the wider world. Approaching the end of their careers as well as the end of their lives, they were prompted by the advent of a deadly pandemic amid worldwide political crises to think through matters of "ultimate concern": what is the human self, embedded as it is in a cosmos of (...) nonhuman and artificial intelligences? Within this larger ecology, what is the meaning of individual death? And can philosophy help us intellectually, emotionally, and spiritually understand and accept our fundamental impermanence? The authors' sense of urgency about their impending mortality, the worth of a life, the grief of loss and what was left undone, the fragility of existence, the uncertainty of the ending permeate their conversations. Readers will be drawn into the inner lives, the hidden fears and emotions, the existential dread experienced by these two exceptional philosophers facing life's close in extraordinary times when both annihilation and revolutionary new beginnings seem equally possible. The authors glean insight from Kierkegaard and Weil, Dickinson and Baldwin, Nagarjuna and Mahavira, Bateson and El Greco, leavened with a little Gluck, Auden, Borges, Ella Fitzgerald, and Hamilton. Reminiscent of the intimate conversations in "My Dinner with Andre," they confront an unknown future. Combining the diary's closeness to the self with the reflection of the personal letter much as Karl Ove Knausgaard combines diary with novelistic realism, these two distinctive voices are uncompromising, immediate, and raw. At the book's end they decide that their dialogue "will continue to change us even after one of us, perhaps both of us, dies." Ghosts may not be holy spirits, but even an atheist can believe that they are real. (shrink)

The paper argues that Spinoza may have deepened his conception of poetry as not only a resource for the understanding but as the highest peak of the understanding. I begin by reviewing selected literature on Spinoza’s views on language and show how Spinoza’s presentation of his philosophy builds on a conception of what language can do. John Stuart Mill has often been charged with inconsistency in his social thinking. The reason given is usually that he tries to combine too many different traditions of thought into an ideological whole. Too deeply affected by his father and his severely purposeful early education ever to repudiate utilitarianism, he was yet too sensitive to disregard criticism of his inherited creed, and too open-minded to ignore areas of thought and experience generally allen to the utilitarian mind. Professor Robson, whose editing (...) of Mill’s works has already put social theorists and historians of ideas in his debt, disputes this view. He argues that, beneath Mill’s eclecticism, beneath his perhaps over-generous acknow-ledgements of his obligations to others, there is a basic unity of ideas. His discussion of the influences on Mill’s thought covers some fairly well-trodden ground but he brings a good deal of common sense to bear on an area where the excitement of the academic chase for sources has sometimes obscured the obvious. On the whole, he claims, the influence of Coleridge, the Suint-Simonians, Carlyle and Comte was limited; some of their specific ideas Mill accepted and used, but he was clearly aware of the differences of emphasis and context that did exist. Harriet Taylor and Tocqueville contributed more, the first by extending Mill’s vision of desirable social ends and by emphasising the difficulties of applying social theory in the world of practice, the second by his insights into democratic society and his demonstration of a proper sociological method. The deepest influence was, however, his early Benthamite upbringing and it is this life-long commitment to a utilitarian ethic that constitutes, for Professor Robson, the still, calm centre of his mind. There are other unifying ideas—his bias towards practicality and his constant concern with evolving a satisfactory methodology for the social sciences—but even these centred on a continual examination of the adequacies and inadequacies of his father’s doctrine. This essay suggests the possible form of an orthodox historicism. The essay begins by examining the historicism of Heidegger and Gadamer. It then proposes how a theology might appear which places the faith in conversation with this historicism.

Biology lacks a central organism concept that unambiguously marks the distinction between organism and non-organism because the most important questions about organisms do not depend on this concept. I argue that the two main ways to discover useful biological generalizations about multicellular organization--the study of homology within multicellular lineages and of convergent evolution across lineages in which multicellularity has been independently established--do not require what would have to be a stipulative sharpening of an organism concept.

That Piagetian epistemology has the dynamics of knowledge growth as its core consideration predetermines a need to consider it as potentially applicable to teaching. This paper addresses that need by first outlining the Piagetian theory of equilibration and then applying it to the construction of methods of teaching science.

It is obvious that technology is rapidly changing the world around us. Nowhere is that change more evident than in the revolution occurring for those with physical and mental limitations-their portrayal in the media, their use of the media to achieve group aims and their use of the new on-line media to communicate with others who have limitations and the non-disabled world. It is a great pleasure to introduce this collection of papers on the use of introspective evidence in cognitive science. Our task as guest editors has been tremendously stimulating. We have received an outstanding number of contributions, in terms of quantity and quality, from academics across a wide disciplinary span, both from younger researchers and from the most experienced scholars in the field. We therefore had to redraw the plans for this project a number of times. It quickly became clear (...) to us that the collection would expand beyond the scheduled double issue of the Journal of Consciousness Studies. A triple issue was then drafted, but the number of excellent contributions continued to grow. We therefore had to reconsider the publication plans again, and the decision was made to publish an extended collection of papers in discrete instalments. At present substantial progress has been made towards determining the content of a second double issue of the Journal of Consciousness Studies, due summer 2004. A third instalment now appears to be a real possibility. We welcome enquiries from authors interested in submitting to later instalments, especially those offering a novel perspective that is not otherwise represented. However, we do not intend continue this collection indefinitely. In putting together the first major interdisciplinary collection on this topic, we view our task as that of providing a starting point. Sufficient outlets exist to support ongoing debate. (shrink)
