Background Studies have shown that medical students and residents believe that their ethics preparation has been inadequate for handling ethical conflicts. The objective of this study was to determine the self-perceived comfort level of medical students and residents in confronting clinical ethics issues. Methods Clinical medical students and residents at the University of Maryland School of Medicine completed a web-based survey between September 2009 and February 2010. The survey consisted of a demographic section, questions regarding the respondents’ sense of comfort (...) in handling a variety of clinical ethics issues, and a set of knowledge-type questions in ethics. Results Survey respondents included 129 medical students (response rate of 40.7%) and 207 residents (response rate of 52.7%). There were only a few clinical ethics issues with which more than 70% of the respondents felt comfortable in addressing. Only a slight majority (60.8%) felt prepared, in general, to handle clinical situations involving ethics issues, and only 44.1% and 53.2% agreed that medical school and residency training, respectively, helped prepare them to handle such issues. Prior ethics training was not associated with these responses, but there was an association between the level of training (medical students vs residents) and the comfort level with many of the clinical ethics issues. Conclusions Medical educators should include ethics educational methods within the context of real-time exposure to medical ethics dilemmas experienced by physicians-in-training. (shrink)

The emotions are at the centre of our lives and, for better or worse, imbue them with much of their significance. The philosophical problems stirred up by the existence of the emotions, over which many great philosophers of the past have laboured, revolve around attempts to understand what this significance amounts to. Are emotions feelings, thoughts, or experiences? If they are experiences, what are they experiences of? Are emotions rational? In what sense do emotions give meaning to what surrounds us? One puzzle concerning highly idealized models is whether they explain. Some suggest they provide so-called ‘how-possibly explanations’. However, this raises an important question about the nature of how-possibly explanations, namely what distinguishes them from ‘normal’, or how-actually, explanations? I provide an account of how-possibly explanations that clarifies their nature in the context of solving the puzzle of model-based explanation. I argue that the modal notions of actuality and possibility provide the relevant dividing lines between how-possibly and how-actually explanations. Whereas how-possibly (...) explanations establish claims of possible explanations, how-actually explanations establish claims of actual ones. Whenever the problem of the relationship between culture and politics is addressed, Julien Benda undoubtedly remains the most frequently mentioned author at the international level. His indictment of the intellectuals’ betrayal is as famous as his speeches to the European nation, published in 1933, about five years later than his widely diffused La Trahison des clercs. Throughout the Discours à la nation européenne, the author explicitly addresses the intellectuals already mentioned in his previous essay and asks them to assume (...) responsibility – becoming protagonists of a new moral revolution. His intention was to reply to Fichte’s well-known Reden an die deutsche Nation. The idea of Europe could be built to transcend nations, manifesting itself as the individual’s renunciation of himself. In this way, the idea of Europe might appear as a ‘moral act’: renouncing the distinct and the finite and turning to unity and infinity. Benda was aware of the possible rise of a type of ‘Europhile nationalism’. To prevent this, he saw it necessary to avoid the closure of the nation and prolong this movement of association so that it might increase its tendency to be inclusive. The political vision of Europe that Julien Benda hoped for should not generate European sovereignty. But, by prompting intellectuals to follow practical and political methods, he consciously exploits their role – stated in La Trahison des clercs – with the hope of building the moral and political framework of a united Europe. This book takes a phenomenological approach to feminist issues in medical ethics: AIDS and reproductive technology.

Argumentation has been an important topic in knowledge representation, reasoning and multi-agent systems during the last twenty years. In this paper, we propose a new abstract framework where arguments are associated with a strength, namely a quantitative information which is used to determine whether an attack between arguments succeeds or not. Our Strength-based Argumentation Framework combines ideas of Preference-based and Weighted Argumentation Frameworks in an original way, which permits to define acceptability semantics sensitive to the existence of accruals between arguments. The question of accruals arises in situations where several arguments defending the same position against another argument are unable to individually defeat this argument, but could do it collectively if they combine their strengths. We investigate some of the theoretical and computational properties of our new framework and semantics, and present a reasoning algorithm that is based on a translation of the problem into pseudo-boolean constraint satisfaction. This paper proposes an intuitive framework which allows strength compensations in an argumentation context where attacks may not succeed, completed by an approach which detects accruals throughout the reasoning process without requiring the elicitation of all compensatory combinations of arguments as an input. Is shame social? Is it superficial? Is it a morally problematic emotion? Researchers in disciplines as different as psychology, philosophy, and anthropology have thought so. But what is the nature of shame and why are claims regarding its social nature and moral standing interesting and important? Do they tell us anything worthwhile about the value of shame and its potential legal and political applications? -/- In this book, Julien Deonna, Raffaele Rodogno, and Fabrice Teroni propose an original philosophical account (...) of shame aimed at answering these questions. The book begins with a detailed examination of the evidence and arguments that are taken to support what they call the two dogmas about shame: its alleged social nature and its morally dubious character. Their analysis is conducted against the backdrop of a novel account of shame and ultimately leads to the rejection of these two dogmas. On this account, shame involves a specific form of negative evaluation that the subject takes towards herself: a verdict of incapacity with regard to values to which she is attached. En 1993, disparaissait Julien Freund. Onze ans après, en dehors de son ouvrage incontournable L'essence du politique, son œuvre demeure méconnue en France, en dépit de sa grande richesse.

Objective Our study sought to (1) describe the practices and preferences of Colombian adolescents in accessing sexual and reproductive health services: accompanied versus alone; (2) compare actual practices with stated preferences; and (3) determine age and gender differences regarding the practice and these stated preferences. -/- Methods 812 participants aged 11–24 years old answered a survey in two Profamilia clinics in the cities of Medellin and Cali in Colombia. A cross-sectional analysis was performed to compare participants’ answers based on the (...) variables of gender and age. -/- Results A quarter of participants visited the clinic alone (25.4%). Females were more likely to go alone in comparison to males (26.3% vs 14.1%; p = 0.031), and older participants went alone more often than younger participants (p < 0.001). Most participants – 72.7% (95 %CI: 69.3–75.9) – expressed a preference in being accompanied to the clinic, and more than 90% had their preferences met. The preferences of older participants were, however, less likely to be met than those of younger participants (p < 0.001), notably, because they predominantly wanted to be accompanied. -/- Conclusion Contemporary public health and bioethics literature advocates in favor of developing health services that better meet the preferences of adolescents. The present research highlights an apparent blind spot related to the role that others (e.g., parents, friends, partners) can or should play in accompanying adolescent patients when they access sexual and reproductive health services. Respecting adolescents’ preferences, and hence their autonomy, is not simply a matter of ensuring freedom from constraints (e.g., their right and ability to go alone). Rather, it should also consider the liberty to choose whether to be accompanied when accessing SHRS and by whom. (shrink)

In this paper, we develop a fresh understanding of the sense in which emotions are evaluations. We argue that we should not follow mainstream accounts in locating the emotion–value connection at the level of content and that we should instead locate it at the level of attitudes or modes. We begin by explaining the contrast between content and attitude, a contrast in the light of which we review the leading contemporary accounts of the emotions. We next offer reasons to think (...) that these accounts face substantial problems since they locate the link emotions bear to values at the level of content. This provides the incentive to pursue an alternative approach according to which emotions qualify as evaluations because they are specific types of attitudes, an approach we substantiate by appealing to felt bodily stances. We conclude by considering two reasons why this approach may be resisted; they respectively pertain to the alleged impossibility of drawing the attitude–content contrast in the case of the emotions and to the suspicion that so doing raises qualia-related worries. There is a traditional conception of knowledge but it is not the Justified True Belief analysis Gettier attacked. On the traditional view, knowledge consists in having a belief that bears a discernible mark of truth. A mark of truth is a truth-entailing property: a property that only true beliefs can have. It is discernible if one can always tell that a belief has it, that is, a sufficiently attentive subject believes that a belief has it if and only if it (...) has it. Requiring a mark of truth makes the view infallibilist. Requiring it to be discernible makes the view internalist. I call the view Classical Infallibilism. (shrink)

The ethics of safe sex in the gay community has, for many years, been focused on debates surrounding the responsibility regarding the use of condoms to prevent HIV transmission, once the only tool available. With the development of Truvada as a pre-exposure prophylaxis for HIV, for the first time in the history of the HIV/AIDS epidemic there is the potential to significantly reduce the risk of HIV transmission during sex without the use of condoms. The introduction of PrEP necessitates a (...) renewed discussion about the politics and ethics of safe sex for men who have sex with men. We present the arguments of authors who hold radically opposite positions with regard to the ethics of condom use by gay men, but who currently both criticize the use of PrEP. We offer a critique of their arguments and advance the position that the use of PrEP, even without condoms, can be acceptable and part of a safe sex ethics framework for MSM. (shrink)

Julien Tricard tackles the abductive solution to the problem of induction. In order to best explain the regularities that can be observed in nature, should one assume that they necessarily result from natural laws, without which they would be improbable cosmic coincidences? By examining David Armstrong's and John Foster's versions of this inference, Julien Tricard shows that it is based on the confusion of two incompatible concepts of “regularity”. From this he derives a conception of induction that is (...) neither the empiricists’ factual generalization, nor the abduction of nomic necessities from observed regularities, but the operation of constituting particular facts as regular instances of laws. How can we use models to understand real phenomena if models misrepresent the very phenomena we seek to understand? Some accounts suggest that models may afford understanding by providing causal knowledge about phenomena via how-possibly explanations. However, general equilibrium models, for example, pose a challenge to this solution since their contribution appears to be purely mathematical results. Despite this, practitioners widely acknowledge that it improves our understanding of the world. I argue that the Arrow–Debreu model provides a mathematical how-possibly explanation (...) which establishes claims of mathematical dependence. The account developed reveals how mathematical knowledge can inform claims about the world, allow ‘what-if-things-had-been-different’ inferences, and thus improve our understanding. (shrink)

When spelled out properly infallibilism is a viable and even attractive view. Because it has long been summary dismissed, however, we need a guide on how to properly spell it out. The guide has to fulfil four tasks. The first two concern the nature of knowledge: to argue that infallible belief is necessary, and that it is sufficient, for knowledge. The other two concern the norm of belief: to argue that knowledge is necessary, and that it is sufficient, for justified (...) certainty. With such a guide in hand infallibilism can be evaluated on its own merits. The most controversial parts are the first and fourth. The idea that knowledge requires infallible belief is thought to be excessively sceptical. The idea that knowledge warrants certainty is thought to be excessively dogmatic. The present paper addresses the first. It argues that knowledge requires infallible belief. (shrink)

In the early 1930s, Julien Benda provided one of the most uncompromising visions for a united Europe. In line with his rationalist universalism, Benda sought a continent that was cleansed of passion and particularism, and called on European intellectuals to act as a rationalist vanguard in constructing such a Europe. However, Benda fatefully wavered between polity-building strategies of reshaping and redirection. For the most part, Benda seemed to demand nothing less than a comprehensive reshaping of the moral and political (...) psychology of European citizens. However, his universalism faltered frequently, and he conceived of Europe rather as a large nation, in which the ‘passion for reason’ would come to dominate other passions. Such ambiguities - and failures to draw a clear line between normative ideals and the pragmatics of polity-building - persist in many present debates on European unification. (shrink)

Knowledge-first evidentialism combines the view that it is rational to believe what is supported by one's evidence with the view that one's evidence is what one knows. While there is much to be said for the view, it is widely perceived to fail in the face of cases of reasonable error—particularly extreme ones like new Evil Demon scenarios (Wedgwood, 2002). One reply has been to say that even in such cases what one knows supports the target rational belief (Lord, 201x, (...) this volume). I spell out two versions of the strategy. The direct one uses what one knows as the input to principles of rationality such as conditionalization, dominance avoidance, etc. I argue that it fails in hybrid cases that are Good with respect to one belief and Bad with respect to another. The indirect strategy uses what one knows to determine a body of supported propositions that is in turn the input to principles of rationality. I sketch a simple formal implementation of the indirect strategy and show that it avoids the difficulty. I conclude that the indirect strategy offers the most promising way for knowledge-first evidentialists to deal with the New Evil Demon problem. (shrink)

For the past two decades, philosophers of religion have paid close attention to the debates on public reason taking place within the context of political philosophy. Some thinkers claim that religious arguments should play a very limited role in political discourse, as this would amount to a politically sanctioned imposition of religious beliefs on people with different religious or non-religious worldviews. Others claim that excluding religious reasons would lead to an unfair exclusion of religious citizens from democratic processes. Underlying these (...) positions is a highly problematic idea of what it means to be a religious citizen. On the one hand, religious citizens are conceptualized as highly self-reflective theologians. Others instead imagine religious citizens to be wholly incapable of such self-reflection and of distinguishing between public and non-public spheres. In my article I attempt to criticize these underlying assumptions and point towards a more nuanced conception of the religious citizen. (shrink)

Models typically have actual, existing targets. However, some models are viewed as having non-actual targets. I argue that this interpretation comes at various costs and propose an alternative that fares better along two dimensions: (1) agreement with practice and (2) ontological and epistemological parsimony. My proposal is that many of these models actually have actual targets.

One can find in the literature two sets of views concerning the relationship between understanding and explanation: that one understands only if 1) one has knowledge of causes and 2) that knowledge is provided by an explanation. Taken together, these tenets characterize what I call the narrow knowledge account of understanding. While the first tenet has recently come under severe attack, the second has been more resistant to change. I argue that we have good reasons to reject it on the (...) basis of theoretical models that provide how-possibly explanations. These models, while they do not explain in the strict sense, afford understanding. In response, I propose an alternative epistemology of understanding, broad KAU, that takes cases of theoretical modelling into account. (shrink)

BackgroundThe literature shows the negative psychological impact of the coronavirus disease 2019 outbreak on frontline healthcare workers. However, few are known about the mental health of physicians and nurses working in general hospitals during the outbreak, caring for patients with COVID-19 or not.ObjectivesThis survey assessed differences in mental health in physicians and nurses working in COVID-19 or non-COVID-19 medical care units.DesignA cross-sectional mixed-mode survey was used to assess burnout, insomnia, depression, anxiety, and stress.SettingA total of 1,244 physicians and nurses from (...) five general hospitals in Belgium, working in COVID-19 care units, non-COVID-19 care units, or both were informed of the study.ParticipantsSix hundred forty-seven healthcare workers participated in the survey.MeasurementsValidated instruments were used to assess the outcomes: the PFI, the ISI, and the DASS-21.ResultsResults showed high prevalence of burnout, insomnia, depression, and anxiety among participants. After adjusting for confounders, multivariate analysis of variance showed no differences between CCU, NCCU, and CCU + NCCU workers. Univariate general linear models showed higher level of burnout, insomnia, and anxiety among nurses in comparison to physicians. Being a nurse, young, isolated, with an increased workload were risk factors for worse mental health outcomes.LimitationsThe mental health of the tested sample, before the outbreak, is unknown. Moreover, this cross-sectional design provides no information on the evolution of the mental health outcomes over time.ConclusionDirectly caring for patients with COVID-19 is not associated with worse mental health outcomes among healthcare workers in general hospitals. Abstract The content of an emotion, unlike the content of a perception, is directly dependent on the motivational set of the subject experiencing the emotion. Given the instability of this motivational set, it might be thought that there is no sense in which emotions can be said to pick up information about the environment in the same way that perception does. Whereas it is admitted that perception tracks for us what is the case in the environment, no such tracking relation, (...) it is argued, holds between one's emotions and what they are about. It is to this worry – that the construal of the emotions as perceptions inevitably raises – that this paper tries to respond. In this paper, I suggest that when it is realized that one dimension of perception itself is directly dependent on the perceiver's perspective on her environment, then emotion, which is also essentially perspectival in this sense, bears the comparison with perception very well. After having clarified the nature and the role that perspective plays in perception, I argue that, in the case of emotions, the same perspectival role can be played by agents’ long-standing evaluative tendencies and character traits. The resulting conception of emotion as perception is then tested against possible objections. (shrink)

According to the fitting attitude (FA) analysis of value concepts, to conceive of an object as having a given value is to conceive of it as being such that a certain evaluative attitude taken towards it would be fitting. Among the challenges that this analysis has to face, two are especially pressing. The first is a psychological challenge: the FA analysis must call upon attitudes that shed light on our value concepts while not presupposing the mastery of these concepts. The (...) second challenge is normative: the FA analysis must account for the fittingness of the relevant attitudes in terms of a kind of normativity intimately related to these attitudes, but again without presupposing the mastery of the relevant value concepts. In this paper, we show that real progress is possible if we pay close attention to the nature of the attitudes recruited by the analysis. More specifically, we claim that an FA analysis that appeals to emotions conceived as evaluative attitudes — as opposed to, for instance, evaluative judgements or evaluative perceptions — has the best prospects of meeting the two challenges and of putting the FA analysis on a strong footing. Two recent contributions have discussed, and disagreed, over whether so-called toy models that attempt to represent dispositions have the power to explain. In this paper, I argue that neither of these positions is completely correct. Toy models may accurately represent, satisfy the veridicality condition, yet fail to provide how-actually explanations. This is because some dispositions remain unmanifested. Instead, the models provide how-possibly explanations; they _possibly_ explain.
