We examine ethical considerations in access to facial transplantation (FT), with implications for promoting health equity. As a form of vascularised composite allotransplantation, FT is still considered innovative with a relatively low volume of procedures performed to date by a small number of active FT programmes worldwide. However, as numbers continue to increase and institutions look to establish new FT programmes, we anticipate that attention will shift from feasibility towards ensuring the benefits of FT are equitably available to those in (...) need. This manuscript assesses barriers to care and their ethical implications across a number of considerations, with the intent of mapping various factors relating to health equity and fair access to FT. Evidence is drawn from an evolving clinical experience as well as published scholarship addressing several dimensions of access to FT. Abstract:Facial transplantation is emerging as a therapeutic option for self-inflicted gunshot wounds. The self-inflicted nature of this injury raises questions about the appropriate role of self-harm in determining patient eligibility. Potential candidates for facial transplantation undergo extensive psychosocial screening. The presence of a self-inflicted gunshot wound warrants special attention to ensure that a patient is prepared to undergo a demanding procedure that poses significant risk, as well as stringent lifelong management. Herein, we explore the ethics of considering mechanism of injury (...) in the patient selection process, referring to the precedent set forth in solid organ transplantation. We also consider the available evidence regarding outcomes of individuals transplanted for self-inflicted mechanisms of injury in both solid organ and facial transplantation. We conclude that while the presence of a self-inflicted gunshot wound is significant in the overall evaluation of the candidate, it does not on its own warrant exclusion from consideration for a facial transplantation. As innovations in the field of vascular composite allotransplantation (VCA) progress, whole-eye transplantation (WET) is poised to transition from non-human mammalian models to living human recipients. Present treatment options for vision loss are generally considered suboptimal, and attendant concerns ranging from aesthetics and prosthesis maintenance to social stigma may be mitigated by WET. Potential benefits to WET recipients may also include partial vision restoration, psychosocial benefits related to identity and social integration, improvements in physical comfort and function, and reduced surgical (...) risk associated with a biologic eye compared to a prosthesis. Perioperative and postoperative risks of WET are expected to be comparable to those of facial transplantation (FT), and may be similarly mitigated by immunosuppressive protocols, adequate psychosocial support, and a thorough selection process for both the recipient and donor. To minimize the risks associated with immunosuppressive medications, the first attempts in human recipients will likely be performed in conjunction with a FT. If first-in-human attempts at combined FT-WET prove successful and the biologic eye survives, this opens the door for further advancement in the field of vision restoration by means of a viable surgical option. This analysis integrates recent innovations in WET research with the existing discourse on the ethics of surgical innovation and offers preliminary guidance to VCA programs considering undertaking WET in human recipients. Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

Most aspects of agriculture in Cuba prior to 1989 were comparable to California: a high energy input, conventional agriculture (based on what the Cubans now call the “classical model”) in which little was done to protect the nation's soils from erosion, loss of fertility, salinization, and other forms of degradation. In stark contrast the new “Alternative Model,” which has been rapidly replacing the previous model since 1989, emphasizes soil conservation and rehabilitation and the general improvement of the nation's soils as (...) the key to sustaining low-input production and attainment of food security. One of the first steps in implementing the new model was the launching of an ambitious program to reclassify, evaluate, and map the nation's soils in great detail, and to interpret the maps for management of sustainable production. A main feature of this program is coordinated fertility trials to determine, for each combination of crop and soil, the minimum quantity of plant nutrients needed to produce the crop. The build up and maintenance of soil fertility and productivity is being accomplished with various organic and mineral amendments and biofertilizers, produced or mined within the country (locally, where possible) and through rational management utilizing cover crops, green manures, crop successions (intercropping and rotations), and other appropriate technologies. Rehabilitation of degraded soils, tillage reduction, reforestation, vermiculture, vermicomposting, and other forms of waste cycling are other features of the new model that are important to soil conservation and maintenance for sustainable production. The research presented in this paper focuses on business ethical values inChina, a country in which the process of institutional transformation has left cultural values in a state of flux. A survey was conducted in China and the U.S. by using five business scenarios. Survey results show similarities between the Chinese and American decision choices for three out of five scenarios. However, the results reveal significant differences in rationales, even forsimilar decisions. This paper examines faculty perceptions regarding ethical behavior among colleagues and students, and faculty practices with regard to teaching ethics in three institutions over a 4-year period. Faculty reported an uneven pattern of unethical behavior among colleagues over the period. A majority of business courses included ethics, however as both a specific topic on the syllabus and within course discussions. The percentage of courses with ethics discussions increased in 2006, however, the time allocated to these discussions decreased. These results suggest (...) that faculty are approaching ethics instruction less formally, raising concerns over the success of curriculum integration. (shrink)

ABSTRACTThirty years ago, the subfields of emotion and cognition operated relatively independently and the associated science reflected the tacit view that they were distinct constructs. Today, questions about the integration of cognition and emotion are among the most interesting questions in the field. I offer a personal view of the key changes that fuelled this shift over time and describe research from my group that unfolded in parallel and led to the identification of the positivity effect.

As the technology for ectogenesis continues to advance, the ethical implications of such developments should be thoroughly and proactively explored. The possibility of full ectogenesis remains hypothetical at present, and myriad concerns regarding the safety and efficacy of the technology must be evaluated and addressed, while pressing moral considerations should be fully deliberated. However, it is conceivable that the technology may become sufficiently well established in the future and that eventually full ectogenesis might be deemed ethically acceptable as a reproductive (...) alternative to gestation within a human womb under certain circumstances. If the safety and efficacy of full ectogenesis are established, if ethical dilemmas are sufficiently well addressed, and if the technology is offered as a reproductive option to cisgender heterosexual individuals or couples desiring to become parents, there is a moral obligation grounded in social justice to ensure that full ectogenesis be made available to individuals or couples identifying as members of sexual‐ or gender‐minority groups who likewise seek to pursue parenthood. We examine the history of access to current family‐building options, including assisted reproductive technology, surrogacy and adoption, for these populations and conclude that in the absence of robust empirical evidence suggesting an increased risk of harm to children of individuals and couples who identify as members of sexual‐ or gender‐minority groups, equitable access to ectogenesis as a pathway to parenthood for sexual and gender minorities must be assured as a matter of reproductive justice. We examined disgust and fear responses to spiders in spider-distressed and nondistressed individuals. Undergraduate participants (N = 134) completed questionnaires concerning responses to spiders and other potentially aversive stimuli, as well as measures of disgust sensitivity, anxious arousal, worry, and anhedonic depression. In addition, we obtained self-report and facial expressions of disgust and fear while participants were exposed to a live tarantula. Both spider distressed and nondistressed individuals reported disgust and exhibited disgust facial expressions in response to a tarantula. Disgust (...) in response to spiders was not found to be part of a general disgust response to all negative stimuli, nor was it due to a general negative emotional response to spiders. Emotional responses to spiders were differentially associated with scores on personality and psychological functioning measures. The results of this study provide evidence that spiders have a specific disgust-evoking status in both distressed and nondistressed populations. (shrink)

Two major objections to divine atemporality center on supposed tensions between the claim that God is omniscient and the claim that he is timeless. Since most defenders of divine timelessness are even more firmly committed to omniscience, driving a wedge between the two is intended to convert such persons to a temporal view of God. However, I believe that both arguments fail to demonstrate an incompatibility between omniscience and timelessness, and that the objections themselves rest in large part on misunderstandings (...) regarding both the motivation for and substance of the doctrine of divine timelessness. Deemed consent legislation for deceased organ donation was introduced in England in 2020, and is considered a vital part of the new UK NHS Blood and Transplant’s 10-year strategy to increase consent for organ donation. Despite the legislation containing safeguards to protect the public, the introduction of deemed consent creates ethical, psychological and social un/certainties for healthcare professionals in their practice. In this paper, we offer insights into healthcare professionals’ perspectives on deemed consent, drawn from interview data with 24 healthcare (...) professionals in an NHS Trust in England, prior to the introduction of the legislation. Whilst participants supported deemed consent in principle, they were concerned that it would present a threat to the nature of donation as a ‘gift’; the notion of informed consent (or non-consent); and the autonomy of donors, their relatives, and their own roles as health professionals, posing dilemmas for practice. We argue that healthcare professionals present themselves as guardians of potential (non)donors and thus as having ethics and integrity in their own practice. We draw conclusions around the values and principles that matter to healthcare professionals when contemplating consent in deceased donation which will be useful for organ donation committees and ethics forums. Clinical ethics can be viewed as a practical discipline that provides a structured approach to assist healthcare practitioners in identifying, analysing and resolving ethical issues that arise in practice. Clinical ethics can therefore promote ethically sound clinical and organisational practices and decision-making, thereby contributing to health organisation and system quality improvement. In order to develop students’ decision-making skills, as well as prepare them for practice, we decided to introduce a clinical ethics strand within an undergraduate medical curriculum. We designed a (...) programme of clinical ethics activities for teaching and assessment purposes that involved using ethical frameworks to analyse hypothetical and real-life cases in uni- and inter- professional groups. In this paper, we draw on medical student feedback collected over 6 years to illustrate the appeal to students of learning clinical ethics. We also outline the range of benefits for students, healthcare organisations, and the field of clinical ethics arising from tomorrow’s doctors experiencing clinical ethics early in their training. We conclude by briefly reflecting on how including clinical ethics within tomorrow’s doctors curricular can secure and continue future engagement in clinical ethics support services in the UK, alongside the dangers of preparing students for organisational cultures that might not exist. We anticipate the findings presented in the paper will contribute to wider debates examining the impact of ethics teaching, and its ability to inform future doctors’ practice. (shrink)

Recent research has demonstrated that word learners can determine word-referent mappings by tracking co-occurrences across multiple ambiguous naming events. The current study addresses the mechanisms underlying this capacity to learn words cross-situationally. This replication and extension of Yu and Smith (2007) investigates the factors influencing both successful cross-situational word learning and mis-mappings. Item analysis and error patterns revealed that the co-occurrence structure of the learning environment as well as the context of the testing environment jointly affected learning across observations. Learners (...) also adopted an exclusion strategy, which contributed conjointly with statistical tracking to performance. The proposal that language has evolved to conform to general cognitive and learning constraints inherent in the human brain calls for specification of these mechanisms. We propose that just as cognition appears to be grounded in cross-modal perceptual-motor capabilities, so too must language. Evidence for perceptual-motor grounding comes from non-arbitrary sound-to-meaning correspondences and their role in word learning.

Uterus transplantation is an emerging technology adding to the arsenal of treatments for infertility; specifically the only available treatment for uterine factor infertility. Ethical investigations concerning risks to uteri donors and transplant recipients have been discussed in the literature. However, missing from the discourse is the potential of uterus transplantation in other groups of genetically XY women who experience uterine factor infertility. There have been philosophical inquiries concerning uterus transplantation in genetically XY women, which includes transgender women and women with (...) complete androgen insufficiency syndrome. We discuss the potential medical steps necessary and associated risks for uterus transplantation in genetically XY women. Presently, the medical technology does not exist to make uterus transplantation a safe and effective option for genetically XY women, however this group should not be summarily excluded from participation in trials. Laboratory research is needed to better understand and reduce medical risk and widen the field to all women who face uterine factor infertility. (shrink)

Many theists of a traditional bent have been bothered by the apparent tension between God's essential omnipotence and his essential moral goodness. Nelson Pike draws attention to the conflict between these two attributes in his article ‘Omnipotence and God's Ability to Sin’, and there have been many attempts to respond to it since that time. Most of these responses argue that the essential omnipotence and essential goodness of God are not logically incompatible, so that the traditional conception of God is (...) not incoherent; I think the arguments have been largely successful. However, some theists have found the typical responses to Pike less than convincing, and are tempted to surrender the claim that God has moral perfection essentially in favour of the more modest claim that God is morally perfect in the actual world though in some possible worlds God is morally defective. I argue in this paper that this fall-back position is incoherent. More accurately, I argue that a necessary being who is essentially omniscient and essentially omnipotent cannot be contingently morally perfect or contingently morally defective. Any such being is either essentially good or essentially evil. Since the latter alternative seems unattractive, I argue that theists should embrace the essential moral perfection of God. (shrink)

Casey seems to want to suggest that our moral responses that do not fit well with the tradition of the virtues are simply the last remnants of a particular religion. But his own men· tion of the Stoics as one important source for the ' Christian ' tradition suggests that the commitments that Casey traces to Christianity-for example, to some version (...) of equality-may be more deeply rooted in the wider Western culture than he allows. And on the other hand, it is not obvious that everything that he associates with ' pagan ' virtues is necessarily incompatible with Christianity; after all, Aquinas, in addition to his extensive appropriation of the framework of the cardinal virtues, also argues that self-love is the first injunction of charity, after love of God (Summa theologiae 11-11, 26, 4). Even so, Casey's work raises important questions for contemporary moral theory. He succeeds in challenging too easy an appropriation of the tradition of the virtues by Christian ethics, and his challenge must he answered by anyone who would attempt to defend a contemporary Christian theory of the virtues. Moreover, by calling attention to the complexity and inner tensions that characterize our moral reflection, Casey reminds us that the category of ' the moral ' is not as straightforward as we sometimes take it to be. Wallace is a careful scholar and clear teacher of the history and philosophy of science. This volume is dedicated to him; the sixteen contributors were chosen because they have all been influenced by him, directly or indirectly. A comprehensive chronological list of Father Wallace's works, provided at the end of the book, reveals his two primary interests: contemporary issues in the philosophy of science and studies in the history of science. To reflect this, the first part of this book consists of seven essays on various contemporary issues in the philosophy of science; the second part is nine historical studies. This review will address each part in turn. Part I, "Contemporary Issues," suffers from a notable lack of unity. 352 BOOK REVIEWS Some authors directly and appropriately acknowledge the influence or inspiration of Wallace upon their contributions, the sole factor that might provide cohesion to the book as a whole. Other contributions are surprisingly devoid of any explicit connection to Wallace, save a tangential one. Wallace has provided the scholarly community with consistent and well-grounded work in the philosophy of science from a realist perspective. This realist position is shared by Rom Harre, the author of the opening essay, "Causality and Reality." Harre argues that the classical concept of causality, at least in part, survives as an ineliminable element of contemporary physics. To accomplish this, Harre distin· guishes weak from strong causality, the former as the expression of a causal mechanism, the latter as that connection stimulated by an event. This distinction is central to his Varieties of Realism. Realism is of two kinds: that which can be defined in terms of truth and falsity, and that which he terms " policy realism," which remains open to unlimited revisability of concepts, while preserving ontological categories. Having argued for these distinctions, Harre uses quantum field theory in his pursuit of a post-Humean conception of causality that transcends strict event ontology. Laudable as this pursuit may be, I wonder whether it can be achieved without reference to any conception of nature (s), a concept lamentably absent in Harre's more recent work. The realist theme is admirably pursued in Robert Sokolowski's " Explaining." Sokolowski argues for the irreducibility of names and explanations and their obvious correlates, things and causes. Form is in things; natural kinds exist; "ones" come in a splended variety reflecting the diversity of forms. Such unpopular claims are expertly defended in this essay, one of the best contributions in the book. Sokolowski also paints a fine picture of the mind... (shrink)

Thomas Aquinas studied Aristotle at the newly founded University of Naples and, as a Dominican friar, at Paris and at Cologne under Albert the Great. In addition he was well acquainted with the doctrine of the Fathers of the Church, especially Augustine, who shows elements of Neoplatonism. This dual influence upon Aquinas's thinking is apparent in his doctrine on the presence of the natural elements in a mixed body. He asserts that the elements remain virtually in the mixture by the (...) preservation of their powers of acting . This is possible, he explains, because the proper accidents of an element are caused by the elemental substantial form as formal cause, as final cause, and "in a way" as agent cause. Since efficient causality in the proper sense implies that one thing act on another as other, the substantial form of the element is said to be an efficient cause by emanation of the proper accidents . ;The proper operations of a natural substance follow upon its proper accidents, which flow spontaneously from its substantial form. Every natural substance thus exhibits a certain dynamism: it is able to perform its proper operations by reason of the accidents that emanate from its substantial form. Natural substances are secondary and proximate causes. The heavenly bodies, on the other hand, are higher causes moved by the separated substances and, ultimately, by God, the first Cause. Hence, the dynamism of natural substance, although truly present in them, derives from God by way of the heavenly bodies as effects derive from an artisan through his instruments. ;This dissertation examines Aquinas's doctrine as outlined above. The doctrine is first detailed by him and is adopted by the Thomistic schools that followed, becoming the leading doctrine on mixture until the rise of modern science. Chapter One examines the concepts of nature and of element, Chapter Two those of natural substance and of mixture. Historians influenced by post-structuralism and the linguistic turn and feminist historians concerned to incorporate the category of gender into historical analysis have recently challenged the categories, methodologies, and questions of labor history as it has been practiced in the United States for the past thirty years. Those operating under the influence of the linguistic turn have challenged labor history's foundational assumption of class as both a category of analysis and as a social formation constituted primarily by material and productive relations. (...) They have argued that class and class interest are constituted culturally and discursively rather than materially. Feminist critics have called attention to the importance of gender both as a subject of analysis and as an analytical lens through which to examine the ways in which class and class relations have been constituted on the basis of sexual difference. Their goal is to interrogate the ways in which notions of masculinity and femininity shaped relations of subjugation and domination and governed mechanisms of inclusion and exclusion in work and in the labor movement. These challenges to the field have led some to charge that labor history is undergoing an epistemological crisis. Recent work by labor historians of France in Lenard Berlanstein's collection, Rethinking Labor History: Essays on Discourse and Class Analysis and by feminist American labor historians in Ava Baron's Work Engendered: Towards a New History of American Labor has taken up these challenges and demonstrates the potential of these new analytical frameworks to awaken new debates and produce new knowledge in the field. Essays in both collections suggest that labor history is less in crisis than it is in the process of an epistemological and methodological rebirth. (shrink)

The current study assessed the extent to which the use of referential prosody varies with communicative demand. Speaker–listener dyads completed a referential communication task during which speakers attempted to indicate one of two color swatches (one bright, one dark) to listeners. Speakers' bright sentences were reliably higher pitched than dark sentences for ambiguous (e.g., bright red versus dark red) but not unambiguous (e.g., bright red versus dark purple) trials, suggesting that speakers produced meaningful acoustic cues to brightness when the accompanying (...) linguistic content was underspecified (e.g., “Can you get the red one?”). Listening partners reliably chose the correct corresponding swatch for ambiguous trials when lexical information was insufficient to identify the target, suggesting that listeners recruited prosody to resolve lexical ambiguity. Prosody can thus be conceptualized as a type of vocal gesture that can be recruited to resolve referential ambiguity when there is communicative demand to do so. (shrink)

Paul Helm offers a welcome and stimulating treatment of the problem of God's timelessness from a standpoint which affirms the traditional theistic view that God is without time. Works devoted wholly to divine eternity are rare in any case, and the most well-known recent book on the subject, Nelson Pike's God and Timelessness, attempts to discredit the notion of God's atemporality. Although Pike has been joined in his view by many philosophers of religion in recent years, there seem to be (...) an increasing number of those sympathetic to divine timelessness. Helm demonstrates an excellent grasp of the issues in this debate and the literature on the topic, especially that of analytic philosophers of religion. One of the greatest strengths of Helm's own discussion is that it is very thorough and carefully argued while remaining largely free of technical jargon and formal notation. This is a very clear and accessible book; it would make a good text for a graduate seminar and will certainly be required reading for anyone working in this field. (shrink)
