Clinical ethics consultations exist to support patients, families and clinicians who are facing ethical or moral challenges related to patient care. They provide a forum for open communication, where all stakeholders are encouraged to express their concerns and articulate their viewpoints. Ethics consultations can be requested by patients, caregivers or members of a patient’s clinical or supportive team. Although patients and by extension their families are the common denominators in most ethics consultations, these constituents are the least likely to request (...) them. At many healthcare organisations in the USA, ethics consultations are overwhelmingly requested by physicians and other clinicians. We believe it is vital that healthcare institutions bridge the knowledge gaps and power imbalances over access to ethics consultation services through augmented policies, procedures and infrastructure. With enhanced education and support, patients and families may use ethics consultation to elevate their voices and prioritise their unique characteristics and preferences in the delivery of their healthcare. Empowering patients and families to request ethics consultation can only strengthen the patient/family–clinician relationship, enhance the shared decision-making model of care and ultimately lead to improved patient-centred care. In a healthcare setting, a multitude of ethical and moral challenges are often present when patients and families direct uncivil behavior toward clinicians and staff. These negative interactions may elicit strong social and emotional reactions among staff, other patients, and visitors; and they may impede the normal functioning of an institution. Ethics Committees and Clinical Ethics Consultation Services (CECSs) can meaningfully contribute to organizational efforts to effectively manage incivility through two distinct, yet inter-related channels. First, given their responsibility to promote (...) a humane, respectful, and professional climate, many CECSs and Ethics Committees may assist institutional leadership in evaluating and monitoring incivility policies and procedures. Second, when confronted with individual incidents of patient/family incivility, Ethics Consultants can and often do work with all stakeholders to address and mitigate potentially deleterious impacts. This manuscript presents an overview of the multifaceted ethical implications of incivility in the healthcare environment, discusses the inherent qualifications of Ethics Consultants for assisting in the management of incivility, and proposes specific mitigating actions within the purview of CECSs and Ethics Committees. We also invite healthcare organizations to harness the skills and reputation of their CECSs and Ethics Committees in confronting incivility through comprehensive policies, procedures, and training. It is a widely held belief that one can will to believe, disbelieve, and withhold belief concerning propositions. It is sometimes said that we have a duty to believe certain propositions. These theses have had a long and respected history. In one form or another they receive the support of a large number of philosophers and theologians who have written on the relationship of the will to believing. In the New Testament Jesus holds his disciples responsible for their beliefs, reprimands (...) them for doubting, and speaks of the ability to believe as if it were optional. Paul makes it clear that he thinks propositional belief is a necessary condition for salvation. If a man confesses Christ as Lord with his lips and believes in his heart that God has raised him from the dead, he shall be saved . The writer of Hebrews implies that unless we have certain propositional beliefs we cannot please God . In the New Testament most cases of pistis involve more than a propositional attitude. They involve the idea of trust and faithfulness. Nevertheless, a prima facie case for saying that the volitional theses can be found in the New Testament can be made. Forms of volitionalism can be found stated more explicitly in the writings of the early Church, in the writings of Irenaeus, in the Athanasian Creed, and in Augustine. Acquinas describes faith as an act of the intellect moved by the will. Descartes is perhaps the classic example of a volitionalist, holding that if we were not responsible for our beliefs , then God would be - which is tantamount to blasphemy in that it makes God into a deceiver. In debate on faith and reason two opposing positions have dominated the field. The first position asserts that faith and reason are commensurable and the second position denies that assertion. Those holding to the first position differ among themselves as to the extent of the compatibility between faith and reason, most adherents relegating the compatibility to the ‘preambles of faith’ over against the ‘articles of faith’ . Few have maintained complete harmony between reason and faith, i.e. a religious belief within (...) the realm of reason alone. The second position divides into two sub-positions: that which asserts that faith is opposed to reason , placing faith in the area of irrationality; and that which asserts that faith is higher than reason, is transrational. Calvin and Barth assert that a natural theology is inappropriate because it seeks to meet unbelief on its own ground . Revelation, however, is ‘self-authenticating’, ‘carrying with it its own evidence’. 1 We may call this position the ‘transrationalist’ view of faith. Faith is not so much against reason as above it and beyond its proper domain. Actually, Kierkegaard shows that the two sub-positions are compatible. He holds both that faith is above reason and against reason . The irrationalist and transrationalist positions are sometimes hard to separate in the incommensurabilist's arguments. At least, it seems that faith gets such a high value that reason comes off looking not simply inadequate but culpable. To use reason where faith claims the field is not only inappropriate but irreverent or faithless. (shrink)

One of the nation's leading military ethicists, Louis P. Pojman argues that globalism and cosmopolitanism motivate the need for greater international cooperation based on enforceable international law. The best way to realize the promises of globalism and cogent moral arguments for cosmopolitanism, Pojman contends, is through the establishment of a World Government.
The purpose of this study was to examine the nature and extent to which cultural differences bear on perceptions of ethical Organizational Development consulting behaviors. U.S. (n=118) and Taiwanese (n=267) business students evaluated eleven vignettes depicting potential ethical dilemmas. Respondents judged the ethicality of each vignette, the likelihood of the event's occurrence and the party responsible for the event's occurrence. Multivariate Analyses of Variance revealed significant cultural differences in perceptions of ethicality, and group differences in perceptions of the events' likelihood (...) of occurrence. U.S. subjects provided higher ethicality ratings than the Taiwanese, and lower ratings on the likelihood of occurrence. Response distributions resulting from the identification of the responsible party were similar for six of the eleven vignettes. When differences did occur, it appeared that the Taiwanese were more inclined than the U.S. subjects to view responsibility as shared by the client and the consultant. The results suggest the need for the incorporation of cultural differences in a code of ethics for the profession and the need for cross-cultural ethics training for partitioners. (shrink)

Affirmative Action is becoming the most controversial social issue of our day. In this essay I examine nine arguments on the moral status of Affirmative Action. I distinguish between weak Affirmative Action, which seeks to provide fair opportunity to all citizens from strong Affirmative Action, which enjoins preferential treatment to groups who have been underrepresented in social positions. I conclude that while weak Affirmative Action is morally required, strong Affirmative Action is morally wrong.
We define ethical system infrastructure as being composed of three major factors – means, motivation, and opportunity. Means are defined as organizational rules, policies, and procedures. Motivation focuses upon the values and the interests being pursued by the position occupant and the organizational value system, while opportunity is discussed in terms of the environment in which the dilemma occurs, proposing that position in the hierarchy presents its own unique set of ethical dilemmas. Ethical breeches are discussed in terms of the (...) interactional processes among means, motivation, and opportunity. Finally, a sequential process is suggested to use the infrastructural components to institutionalize organizational ethics training and subsequent behavior. (shrink)

The concept of desert, which once enjoyed a central place in political and ethical theory, has been relegated to the margins of much of contemporary theory, if not excluded altogether. Recently a renewed interest in the topic has emerged, and several philosophers have argued that the notion merits a more central place in political and ethical theory. Some of these philosophers contend that justice exists to the extent that people receive exactly what they deserve, while others argue that desert should (...) replace such considerations as rights, need, and equality as the basis for distributions. Still others argue that morality involves a fitting match between one's moral character and a degree of happiness. All of these positions have encountered opposition from egalitarians, libertarians, and those who are skeptical about the coherence of the concept of desert. In 1941 Father Maximilian Kolbe, a Polish friar from Warsaw was arrested for publishing anti-Nazi pamphlets and sentenced to Auschwitz. There he was beaten, kicked by shiny leather boots, and whipped by his prison guards. After one prisoner successfully escaped, the prescribed punishment was to select ten other prisoners who were to die by starvation. As ten prisoners were pulled out of line one by one, Fr. Kolbe broke out from the ranks, pleading with he Commandant to be allowed to (...) take the place of one of the prisoners, a Polish worker with a wife and children dependent upon him. "I'm an old man, sir, and good for nothing. My life will serve no purpose," the 45 year old priest pleaded. He was taken, thrown down the stairs into a dank dark basement with the other nine prisoners and left to starve. Usually, prisoners punished like this spent their last days howling, attacking each other and clawing the walls in a frenzy of despair. (shrink)
