We analyzed the patients’ perception of prenatal diagnosis of fetal cardiac pathology, and the reasons for choosing to continue with pregnancy despite being eligible to receive a medical termination of pregnancy. We also identified the challenges, the motives interfering in decision-making, and the consequences of the decisions on pregnancy, child and mother. This descriptive, prospective and longitudinal study was conducted in France, amongst pregnant women who wished to continue their pregnancy despite an unfavorable medical advice. Socio-demographic data were collected through (...) a questionnaire. Such questionnaire covered information assessing the parents/mother’s perception of prenatal diagnosis, and medical termination of pregnancy, their interpretation of the established diagnosis and their motives for not considering pregnancy termination. 72 eligible patients were analyzed over one year: mean age 33 ± 6.89 years, 47 patients had already given birth to ≥1 healthy child. Mean gestational age at the detection of fetal cardiac pathologies was 30 ± 4.37 weeks of amenorrhea. Patients decided to keep the child after 3 ± 1.25 consultations. 56 patients made their decision with their husbands and 16 made their decision alone. Reasons for declining the medical termination were culpability and responsibility, ideologies and convictions, mistrust and hope. Newborns of 67 patients died with a mean survival duration of 38 days. Patient informed consent should be sought before any decision in neonatology, even if conflicting with the medical team’s knowledge and the pregnant mother’s benefits. Decisions to accept or decline pregnancy termination depend on the patients’ psychological character, ideologies, convictions, and mistrust in the diagnosis/prognosis, or hope in the fetus survival. As Medicine shifts from a paternalistic practice to a patient-centered approach, the concept of medical informed consent (IC) has evolved to safeguard patient autonomy. However, its current implementation still presents many challenges in clinical practice. We assessed the knowledge and attitudes of the general Lebanese population regarding the IC process as well as their sociodemographic and medical correlates. An anonymous online survey was distributed to the Lebanese population using social media channels. A sample of 500 adults with an average age (...) of 36.2 ± 13.5 years, including 319 females and 181 males, was recruited. Most of the respondents had a university degree (85.8%), reported previous hospital admissions (75.9%) and had signed an IC for surgical procedures (40.7%). Few participants were knowledgeable about IC Lebanese law. Variability in knowledge level was significantly related to gender and a previous hospitalization history. Positive attitudes toward patient autonomy (53.1%) and shared decision-making (57.5%) correlated with older age, female gender, graduate education, and a previous history of signing an IC document. Males were more likely to believe that IC has positive effects on health than females. This is the first study that provides novel findings regarding Lebanese peoples’ awareness of the ethico-legal components of medical IC. (shrink)

Deidealization as a topic in its own right has attracted remarkably little philosophical interest despite the extensive literature on idealization. One reason for this is the often implicit assumption that idealization and deidealization are, potentially at least, reversible processes. We question this assumption by analyzing the challenges of deidealization within a menu of four broad categories: deidealizing as recomposing, deidealizing as reformulating, deidealizing as concretizing, and deidealizing as situating. On closer inspection, models turn out much more inflexible than the reversal (...) thesis would have us believe, and deidealization emerges as a creative part of modeling. (shrink)

Este trabalho procura documentar as espécies e os usos de plantas vendidas por ervatários numa feira semanal do bairro da Tijuca na cidade do Rio de Janeiro. Foi realizado entre os meses de agosto/98 e agosto/99, e participaram da pesquisa quatro vendedores, com média de 15 anos de experiência no mercado, fornecendo as plantas e informações sobre seus nomes vulgares, usos e o preparo dos remédios. A feira foi visitada regularmente e os espécimes encontrados foram coletados, fotografados, herborizados e identificados (...) através da consulta de livros, chaves e especialistas, e comparação com exemplares de herbário. A coleta resultou em 151 espécies distribuídas em 59 famílias, a melhor representada sendo Asteraceae (21), Lamiaceae (13), Solanaceae (9), Leguminosae (6), e as restantes representadas por até 5 espécies. As plantas foram classificadas em 4 categorias segundo a procedência: comprada de terceiros (16%), cultivada nos jardins particulares dos vendedores (23%), ruderal (21%), e coletada da Mata Atlântica (40%). Foram documentadas 60 utilidades medicinais e rituais, e 15 formas de preparar as ervas, o número de indicações sendo 30% para uso externo e 70% para uso interno, e a forma mais comum, o chá. Além dos dados obtidos na feira, uma descrição botânica e um levantamento bibliográfico dos usos na medicina popular e em rituais afro-brasileiros são apresentados para cada espécie. Analisa-se o papel das plantas na saúde e na vida religiosa das pessoas do bairro, o conhecimento dos ervatários e a preocupante coleta das plantas da Mata Atlântica. (shrink)

BackgroundAberrant functioning of the autonomic nervous system is an important factor in the occurrence of antisocial behavior. Baseline autonomic functioning and the responsivity of the ANS have been related to psychopathic traits and aggression. Here we investigated whether a naturalistic sample of male multi-problem young adults present with similar autonomic deficits in relation to their psychopathy and aggression as previous studies observed in clinical samples.MethodsIn a sample of 112 multi-problem young adults, baseline autonomic functioning and autonomic responsivity to emotional stimuli (...) were assessed through four physiological measures: heart rate, respiratory sinus arrhythmia, pre-ejection period, and skin conductance. 27 control participants were included primarily to assess whether the task worked appropriately. Participants watched a neutral 5 min video to assess baseline autonomic functioning and watched two sad clips to assess autonomic reactivity to sadness. We investigated the association between autonomic functioning and self-reported psychopathic traits and aggression within the multi-problem group.ResultsWe found no significant associations between autonomic functioning and psychopathy and aggression.ConclusionThese null-findings highlight the importance of research in naturalistic samples in addition to research in clinical and general populations samples and underscore the complexity of translating research findings into practical and clinical implications. La Iglesia ha dado por zanjado el caso Galileo en más de una ocasion. No obstante, la polémica ha continuado. Aquí se argumenta que las distintas iniciativas de la Iglesia respecto al caso Galileo -la revision de la condena dei copernicanismo a partir de 1820; la utilización de los documentos dei dossier inquisitorial de Galileo a partir de 1850 y la polémica suscitada; el caso Paschini (1942-1965); y las conclusiones de Juan Pablo II en 1992-1993- ponen de manifiesto la misma (...) actitud de la Iglesia y la persistencia de los intereses básicos de partida, que hacen muy improbable que el “caso de Galileo”, al margen de los problemas genuinamente históricos, pueda cerrarse.Althoght the Catholic Church has setlled “Galileo’s case” several times, the controverse goes on. I argue that Church’s initatives on this matter -the revision of the condenmation of copernicanism from 1820; the use of documents coming from Galileo’s inquisitorial dossier from 1850 on and the controversy raised by this use; Paschini case (1942-1965); and the conclusions drawn by pope John Paul II in 1992-1993- make evident the identical actitude of the Church as well as the persistence of his basic interests, which make very unlikely that Galileo’s case, regardless of genuine historical problems, call be considered as closed.
