Background: Although fair distribution of healthcare services for older patients is an important challenge, qualitative research exploring clinicians’ considerations in clinical prioritisation within this field is scarce. Objectives: To explore how clinicians understand their professional role in clinical prioritisations in healthcare services for old patients. Design: A semi-structured interview-guide was employed to interview 45 clinicians working with older patients. The interviews were analysed qualitatively using hermeneutical content analysis. Participants: 20 physicians and 25 nurses working in public hospitals and nursing homes (...) in different parts of Norway. Results and interpretations: The clinicians struggle with not being able to attend to the comprehensive needs of older patients, and being unfaithful to professional ideals and expectations. There is a tendency towards lowering the standards and narrowing the role of the clinician. This is done in order to secure the vital needs of the patient, but is at the expense of good practice and holistic role modelling. Increased specialisation, advances and increase in medical interventions, economical incentives, organisational structures, and biomedical paradigms, may all contribute to a narrowing of the clinicians’ role. Conclusion: Distributing healthcare services in a fair way is generally not described as integral to the clinicians’ role in clinical prioritisations. If considerations of justice are not included in clinicians’ role, it is likely that others will shape major parts of their roles and responsibilities in clinical prioritisations. Fair distribution of healthcare services for older patients is possible only if clinicians accept responsibility in these questions. (shrink)

Antiphon's speech on the murder of Herodes has been variously dated by several scholars, but all seem to agree that it was delivered a good many years after the revolt and recapture of Mytilene. According to this opinion the speaker in § 74 declares himself too young to know much of what happened in those days. Before going into this more carefully, it seems necessary to visualize the situation of the accused man. In order to achieve this the best we (...) can do is to follow his own words in the Sirjyrjcris, §§ 20–24: ‘I travelled from Mytilene on the same ship as the lost man; we were on our journey to Ainos, I to see my father—he happened to be there at that time—he in order to sell some slaves to Thracian natives. These natives, who were to pay the ransom, and the slaves were on board too.—All this I will prove by witnesses.—As we, however, met with stormy weather, we were compelled to put in to a harbour in the territory of Methymna; there we found that* ship into which Herodes changed; in that ship, according to the accusers, he died. Now all this was mere chance: obviously I had not persuaded him to travel with me, no, he was travelling on his own business. Then I, personally, had a very good reason for travelling to Ainos; our putting in to that harbour was not premeditatd, we were compelled to. And again, when we were lying at anchor, the change of ship took place without any trickery or fraud: again, it was done because of necessity. The study of anosognosia has witnessed an unprecedented increase in interest over the last 20 years. This has resulted in numerous empirical investigations as well as theoretical writings on the nature of human consciousness and how disorders of the brain may influence the person's subjective awareness of a disturbed neurological or neuropsychological function. This edited text summarizes many of the advances that have taken place in the field of anosognosia. It reviews research findings on anosognosia for hemiplegia following stroke, Anton's (...) syndrome, and a variety of disorders in which impaired self-awareness is common. La « théorie » de la prédication de Barth comme « théologie de la Parole de Dieu » a été remise en question au début des années 60 alors qu’elle était constituée depuis les années 20. Qu’en est-il exactement de cette conception trop peu connue dans son exactitude, en France notamment ? Soucieux de pratique, Barth conçoit d’abord la prédication comme une Theologia viatorum et semble affirmer le caractère divin de la parole de la prédication. Or, la difficulté, voire l’échec, (...) de celle-ci en rappelle le caractère de terrible humanité. Ainsi passe-t-il « d’une attestation indirecte du Christ » dans la prédicaiton de l’église, à une « prédication, service de la parole de Dieu », compte tenu de ce que seul le Christ, en définitive, est Parole de Dieu, et que la charge de prédication appartient à toute la « communauté de témoins ».Barth's « theory » of preaching as « theology of the World of God » was put into question at the beginning of the 60's, although it goes back to the 20’s. What is it exactly about this conception, too little known in its details, especillaly in France ? Concerned with practice, Barth first conceived preaching as a Theologia viatorum and seems to have confirmed the divine character of the preached word. But the difficulty, indeed the failure, of the latter recalls the character of terrible humanity. Thus there evolves from « an indirect attestation of Christ « in the preaching of the Church, to a « preaching service of God’s word », taking into account that only Christ, finally is Word of God, and that the role of preaching belongs to all the « community of witnesses». In a recent article on the Vettius affair Professor Lily Ross Taylor has tried to show that this letter should be dated to mid-July 59, and that it is therefore antecedent to 2. 20, 21, and 22. According to the hitherto accepted view the letters 2. Lentulus, one of the persons Vettius implicated, was then a candidate for the consulship and that the letter is therefore antecedent to the consular elections, postponed by Bibulus' edict to 18 October . Commentators have noted that the issue stands taken by each side of the American “culture war” lack conceptual consistency and can even seem contradictory. We sought to understand the psychological underpinnings of culture war attitudes using Moral Foundations Theory. In two studies involving 24,739 participants and 20 such issues, we found that endorsement of five moral foundations predicted judgments about these issues over and above ideology, age, gender, religious attendance, and interest in politics. Our results suggest that dispositional tendencies, particularly (...) a person’s moral intuitions, may underlie, motivate, and unite ideological positions across a variety of issues and offer new insights into the multiple “moral threads” connecting disparate political positions. This article tries to create a bridge of understanding between cognitive scientists and phenomenologists who work on attention. In light of a phenomenology of attention and current psychological and neuropsychological literature on attention, I translate and interpret into phenomenological terms 20 key cognitive science concepts as examined in the laboratory and used in leading journals. As a preface to the lexicon, I outline a phenomenology of attention, especially as a dynamic three-part structure, which I have freely amended from the work (...) of phenomenologist and Gestalt philosopher Aron Gurwitsch (1901–1973). As a conclusion, I discuss the nature of subjectivity in attention and attention research, and whether attention might be the same as consciousness. (shrink)
