Background: Nurses are the largest professional group in healthcare and those who make more decisions. In 2014, the Committee on Bioethics of the Council of Europe launched the “Guide on the decision-making process regarding medical treatment in end-of-life situations”, aiming at improving decision-making processes and empowering professionals in making end-of-life decisions. The Guide does not mention nurses explicitly. Objectives: To analyze the ethical principles most valued by nurses working in palliative care when making end-of-life decisions and investigate if they are (...) consistent with the framework and recommendations of the Guide; to identify what disputed/controversial issues are more frequent in these nurses’ current end-of-life care practices. Design: Qualitative secondary analysis. Participants/context: Three qualitative datasets including 32 interviews from previous studies with nurses working in palliative care in Portugal. Ethical consideration: Ethical approval was obtained from the Ethics Research Lab of the Instituto de Bioética. Ethical procedures are thoroughly described. Findings: All participant nurses referred to autonomy as an ethical principle paramount in end-of-life decision-making. They were commonly involved in end-of-life decision-making. Palliative sedation and communication were the most mentioned disputed/controversial issues. Discussion: Autonomy was highly valued in end-of-life care and decision-making. Nurses expressed major concerns in assessing patients’ preferences, wishes, and promoting advance care planning. Nurses working in palliative care in Portugal were highly involved in end-of-life decision-making. These processes embraced a collective, inclusive approach. Palliative sedation was the most mentioned disputed issue, which is aligned with previous findings. Communication also emerged as a sensitive ethical issue; it is surprising, however, that only three nurses referred to it. Conclusion: While the Guide does not explicitly mention nurses in its content, this study shows that nurses working in palliative care in Portugal are involved in these processes. Further research is needed on nurses’ involvement and practices in end-of-life decision-making. (shrink)

Background The expressions of a “wish to hasten death” or “wish to die” raise ethical concerns and challenges. These expressions are related to ethical principles intertwined within the field of medical ethics, particularly in end-of-life care. Although some reviews were conducted about this topic, none of them provides an in-depth analysis of the meanings behind the “wish to hasten death/die” based specifically on the ethical principles of autonomy, dignity, and vulnerability. The aim of this review is to understand if and (...) how the meanings behind the “wish to hasten death/die” relate to and are interpreted in light of ethical principles in palliative care. Methods We conducted a meta-ethnographic review according to the PRISMA guidelines and aligned with Noblit and Hare’s framework. Searches were performed in three databases, Web of Science, PubMed, CINAHL, with no time restrictions. Original qualitative studies exploring the meanings given by patients, family caregivers and healthcare professionals in any context of palliative and end-of-life care were included. A narrative synthesis was undertaken. PROSPERO registration CRD42023360330. Results Out of 893 retrieved articles, 26 were included in the analysis, accounting for the meanings of a total of 2,398 participants. Several factors and meanings associated with the “wish to hasten death” and/or “wish to die” were identified and are mainly of a psychosocial and spiritual nature. The ethical principles of autonomy and dignity were the ones mostly associated with the “wish to hasten death”. Ethical principles were essentially inferred from the content of included articles, although not explicitly stated as bioethical principles. Conclusions This meta-ethnographic review shows a reduced number of qualitative studies on the “wish to hasten death” and/or “wish to die” explicitly stating ethical principles. This suggests a lack of bioethical reflection and reasoning in the empirical end-of-life literature and a lack of embedded ethics in clinical practice. There is a need for healthcare professionals to address these topics compassionately and ethically, taking into account the unique perspectives of patients and family members. More qualitative studies on the meanings behind a wish to hasten death, their ethical contours, ethical reasoning, and implications for clinical practice are needed. (shrink)

Tomando como ejemplo la producción de las artes visuales centroamericanas del período inmediatamente posterior a la firma de los acuerdos de paz en la región se analizan las relaciones entre imagen y palabra en las obras del arte visual en que se experimenta con diversas dimensiones de la palabra escrita. This study examines the relationship between cooperative business relations in small businesses and environmental protection, one of the most important policies of social responsibility in manufacturing. We reviewed the literature and carried out an empirical study of 930 small manufacturing firms in Spain. Results indicate that small businesses that maintain and improve their cooperative relations through business networking with universities, competitors, suppliers and customers spend more on environmental protection. This paper analyses empirical evidence of efforts to enable Spanish micro and small manufacturing companies to boost their labour productivity rates through the development of the main pillars of their corporate social responsibility policies. This study aims to develop new approaches and sensibilities towards work from an ethical, values and CSR perspective, showing how internal dimensions of CSR, such those related to relationships with employees and responsibility in processes and product quality, can improve labour performance and labour efficiency, thereby contributing (...) to a better society. The results of a sample of 929 small businesses indicate that the social responsibility policies that most contributed to a short-term increase in labour productivity are those related to internal aspects of the company, in particular its involvement in the quality of processes and products, promotion of innovation and employee care. However, the impact on labour productivity of CSR policies related to external factors, such as relationship with stakeholders and environmental concern, could not be empirically proven in this paper. In recent years, Julien Deonna and Fabrice Teroni have proposed to understand emotions as embodied evaluative attitudes we take towards objects that figure in nonevaluative representational states. Although their account nicely explains some of the key features that emotions are widely taken to have, it runs into a version of what I call the problem of integration. In the case of the attitudinal view, the integration problem takes the form of explaining how, from the point of view of the subject, (...) the bodily responses that make up the attitude part of the emotion and the representational states that provide the particular object of the emotion come to form an intentionally structured unitary experience, that is, one in which the bodily responses are intentionally directed towards the object. I argue that what explains this integration is the way in which the experience of bodily responses and the experience of the representational states interact. This, I propose, produces what I call an experience of convergence. I also suggest that understanding emotional experience in this way not only solves the problem of the integration but also provides a more solid ground for the claim that emotions qua embodied attitudes are evaluative. RESUMEN Con frecuencia el amor y la racionalidad se ven como capacidades que fácilmente entran en conflicto, cuando no se consideran por principio opuestas. En este artículo desarrollo algunos puntos esbozados por Harry Frankfurt con el fin de proponer que la relación entre amor y racionalidad no es de simple oposición. Tras ofrecer una caracterización del amor como disposición híbrida de seguimiento múltiple, defenderé que el amor es racional en el siguiente sentido: el amor no es justificable, pero a pesar (...) de ello es una fuente de razones básicas y a veces irresistibles que en buena medida informan el campo de nuestra racionalidad. Esto no significa que el amor sea una base irracional o arracional, o que sea completamente blindado a la razón, sino que en circunstancias específicas tiene sentido someter el amor a escrutinio racional. ABSTRACT Love and rationality are often considered as capacities which easily come into conflict, or are even opposed to one another. In the paper I elaborate some points suggested by Harry Frankfurt in order to propose that the relation between love and rationality is not one of opposition. After offering a characterization of love as a hybrid multi-track disposition, I will argue that love is rational in the following sense: although love is not justifiable, it is nevertheless a source of basic and sometimes irresistible reasons which to a large extent shape the field of our rationality. This does not mean that love is an irrational or arrational foundation, or that it is impervious to reason; it only means that in certain circumstances it makes sense to subject love to rational scrutiny. Despite the fact that their disagreement concerns the most basic metaethical and metaphysical questions regarding our relation to nature, it has become apparent that many anthropocentrists share with nonanthropocentrists a concern for the environment for its own sake, that is to say, a noninstrumental concern for nature. This concern is also present in practical spheres of environmental engagement. With regard to the philosophical task of justifying the claim that we ought to protect nature, this concern imposes on those that share (...) it at least three conditions: priority, independence of future interaction, and universality. Reasonably specified, these conditions are neutral between anthropocentrism and nonanthropocentrism and should be attractive to both camps. Although there are reasons to think it would be difficult to meet all three conditions at the same time, with some modification a promising way to do it becomes apparent. (shrink)

The notion that patients’ medical decision-making capacity depends on risk considerations has some acceptance in the bioethical literature. However, it arouses some criticism since it seems to give rise to paternalistic attitudes. In addition, the idea of capacity assessment as a collaborative space in which aid is given to the patient is emphasized so that they can decide about their life. It does not seem ethically acceptable to pose the evaluation as a simple observer report. Capacity assessment can sometimes be (...) impossible or inconsequential. In short, capacity assessment is a procedure that, used wisely, can allow the patient to take charge of their life. Misused, it can be the gateway to unjustified paternalistic actions. (shrink)

Se realizó una investigación cualitativa para comprobar la utilidad del cine como apoyo a la docencia en la asignatura de Psiquiatría del quinto año de la carrera de Medicina en tres subgrupos de estudiantes de la Universidad de Ciencias Médicas "Carlos J. Finlay" de Camagüey. La muestra ascendió a 43 estudiantes de ambos sexos y diferentes nacionalidades, a quienes se les realizaron entrevistas individuales grabadas en audio, explorando criterios personales tras haber presenciado un grupo de películas previamente escogidas por abordar (...) el tema del trastorno mental con profundidad. Las mismas se exhibieron una vez por semana, coincidiendo su tema con los objetivos propuestos según los contenidos, siempre después de la conferencia introductoria y clase taller, y antes del seminario correspondiente. Entre los resultados obtenidos se destaca que la totalidad expresó opiniones positivas respecto a la actividad, y propuso se mantuviera en el resto de las rotaciones de Psiquiatría. La totalidad consideró que la misma representó un enriquecimiento de su cultura general. La mayoría expresó que les fue fácil la identificación de síntomas, síndromes y situaciones causales en las películas utilizadas, y manifestó que existe una estrecha relación entre los contenidos semanales y las películas elegidas. Se recogieron además algunas sugerencias respecto a la actividad. Teniendo en cuenta los resultados obtenidos, se puede considerar como muy positiva la utilización del cine como apoyo a la actividad docente en Psiquiatría en el grupo estudiado. A qualitative research was carried out in three sub-groups of medical students at Carlos J. Finlay University in order to test the usefulness of films as a teaching aid for the 5th year subject of Psychiatry. The sample was composed of 43 students, both male and female, from different nationalities whose individual interviews were recorded in order to analyze personal criteria after watching a series of movies previously chosen since the plot deals with serious mental disorders. These were screened once a week. Their subject matter coincided with the objectives set according to the class content, always after the introductory lecture and workshop class, and before the corresponding seminar. The most outstanding results show that all students expressed positive views with regard to the activity, and suggested to maintain this initiative in the rest of Psychiatry rotations. All of them considered that this activity allowed them to enrich their general culture. Most expressed that it was easy for them to identify symptoms, syndromes and causal situations used in films and that there is a close relationship between the weekly contents and selected films. In addition some suggestions regarding the activity were collected. Taking into account the obtained results, the use of cinema as a teaching aid in Psychiatry can be considered as very positive in the studied group. (shrink)

During replication, the topology of DNA changes continuously in response to well‐known activities of DNA helicases, polymerases, and topoisomerases. However, replisomes do not always progress at a constant speed and can slow‐down and even stall at precise sites. The way these changes in the rate of replisome progression affect DNA topology is not yet well understood. The interplay of DNA topology and replication in several cases where progression of replication forks reacts differently to changes in DNA topology ahead is discussed (...) here. It is proposed, there are at least two types of replication fork barriers: those that behave also as topological barriers and those that do not. Esta obra se concentra en cuatro temas cruciales de la formación de docentes, tanto antes como durante el servicio y en la enseñanza en diferentes áreas y niveles educativos. En primer lugar, se aborda el asunto de las creencias que los docentes tienen sobre el proceso educativo, las cuales parecen influir en la práctica profesional que estos desarrollan y, por lo tanto, deberían recibir la atención explícita de los procesos de formación de docentes que deseen promover prácticas específicas. El segundo (...) tema de la obra es la relación entre la formación de docentes y la política pública que la rige. En particular, se analizan las reformas curriculares implementadas por varias licenciaturas en universidades privadas colombianas, en respuesta a reformas a la normatividad sobre las condiciones de calidad que a las que deben ajustarse todas las licenciaturas del país. El tercer tema analizado es en qué medida los docentes en educación superior van más allá del dominio de su saber disciplinar atribuyendo importancia a la pedagogía, la didáctica y la práctica reflexiva en su propia práctica docente. Finalmente, se discuten aspectos de la formación de docentes relacionados con los procesos vividos y los retos encontrados por los docentes al entrar en la profesión, su socialización en la comunidad docente, sus identidades como maestros y su motivación para ser docentes. Vale la pena señalar que las contribuciones a este libro comparten el uso de metodologías cualitativas y presentan los resultados de historias de vida, entrevistas, observaciones, y análisis documentales, entre otros. The objective of this research is to identify the main weaknesses of corporate social responsibility in the textile sector and to propose procedures that help to overcome them. We start from the assumption that the asymmetries and necessary relationships generated to manufacture textile garments in the international context are not attributable to a single circumstance. Equally, it is reasonable to assume that the solutions to these asymmetries are affected by the identification and transversality of such circumstances. The present research was (...) carried out by gathering the opinions and solutions of participants in a Delphi analysis to uncover the critical points and vulnerabilities of the sector. The weaknesses of the sector were then identified following the analysis of responses from two expert panels to two multidisciplinary questionnaires, the questions for which were developed from the stated hypotheses.El objetivo de esta investigación es detectar las principales debilidades del sector textil en relación con la responsabilidad social empresarial y proponer medidas que contribuyan a superarlas. Partimos de que las asimetrías y relaciones generadas necesarias para fabricar prendas textiles en el contexto internacional no son imputables a una sola circunstancia. Es igualmente entendible que sus soluciones pasen por la identificación y la transversalidad de dichas circunstancias. En la presente investigación, lo llevamos a cabo mediante la de la recolección de opiniones y soluciones expresadas por diferentes participantes, y a través de un análisis Delphi, para conocer cuáles son los puntos críticos y vulnerabilidades del sector, y la posterior identificación de sus debilidades tras el análisis de las respuestas de dos paneles de expertos a dos cuestionarios multidisciplinares con preguntas desarrolladas sobre las hipótesis planteadas. The topology of DNA duplexes changes during replication and also after deproteinization in vitro. Here we describe these changes and then discuss for the first time how the distribution of superhelical stress affects the DNA topology of replication intermediates, taking into account the progression of replication forks. The high processivity of Topo IV to relax the left‐handed (+) supercoiling that transiently accumulates ahead of the forks is not essential, since DNA gyrase and swiveling of the forks cooperate with Topo IV (...) to accomplish this task in vivo. We conclude that despite Topo IV has a lower processivity to unlink the right‐handed (+) crossings of pre‐catenanes and fully replicated catenanes, this is indeed its main role in vivo. This would explain why in the absence of Topo IV replication goes‐on, but fully replicated sister duplexes remain heavily catenated. (shrink)

Genetic and early environmental factors are interwoven in the etiology of Borderline Personality Disorder (BPD). Epigenetic mechanisms offer the molecular machinery to adapt to environmental conditions. There are gaps in the knowledge about how epigenetic mechanisms are involved in the effects of early affective environment, development of BPD, and psychotherapy response. We reviewed the available evidence of the effects of psychotherapy on changes in DNA methylation and conducted a pilot study in a sample of 11 female adolescents diagnosed with BPD, (...) exploring for changes in peripheral DNA methylation of FKBP5 gene, which encodes for a stress response protein, in relation to psychotherapy, on symptomatology and underlying psychological processes. For this purpose, measures of early trauma, borderline and depressive symptoms, psychotherapy outcome, mentalization, and emotional regulation were studied. A reduction in the average FKBP5 methylation levels was observed over time. Additionally, the decrease in FKBP5 methylation observed occurred only in those individuals who had early trauma and responded to psychotherapy. The results suggest an effect of psychotherapy on epigenetic mechanisms associated with the stress response. The finding that epigenetic changes were only observed in patients with early trauma suggests a specific molecular mechanism of recovery. The results should be taken with caution given the small sample size. Also, further research is needed to adjust for confounding factors and include endocrinological markers and therapeutic process variables. (shrink)

Introduction: Resilience is a multidimensional and dynamic construct associated with positive growth and the capacity to transform stressful and negative factors into opportunities of personal development and self-improvement when faced with difficult experiences. The resilience process of each individual integrates multiple analysis levels, which range from genetic-environmental interactions to a complex process of adaptation between the individual and his/her family, friends, co-workers, society, and culture.Objective: To determine whether resilience improves in students of occupational therapy when exposed for the first time (...) to practice placement education.Methodology: Quasi-experimental, prospective, observational, multi-center study with a sample composed of students from the Degree of Occupational Therapy of the public universities of Málaga and Castilla-La Mancha. Two weeks prior to the beginning of the practice education period, the participants completed a questionnaire that included sociodemographic data and the area of their internships. They were also given the Spanish version of the Connor-Davidson's resilience scale. All these instruments were also completed 1 week after the end of the clinical practice.Results: There were statistically significant differences between the variables that make up resilience and the different internship areas. On the other hand, there was a significant improvement of global resilience after the clinical practice period, in both women and men, when the internship area was not considered.Conclusions: The results show that resilient students are more optimistic and work to improve a situation beyond doing simply what is expected of them, knowing how to control their feelings. This is beneficial for students in practice education, since, during these, they face difficult situations that require a resilient pattern, which helps reduce stress and the burnout syndrome. Como hipótesis se plantea que la reescritura discute fenómenos sociales vinculados con las fisuras de la nación chilena y los procesos políticos de la actual coyuntura. Como objetivo, se busca establecer un diálogo con la tradición y el campo literario en torno a (...) la épica, por un lado, y con las reconsideraciones críticas acerca de la imaginación poética de la historia, por otro. Abstract The article focuses on the poem Seudoaraucana by Elvira Hernández (2010; 2017) based on the rewriting as an articulating resource of an off-center and amplified look at the source text, La Araucana de Ercilla y Zúñiga. As a hypothesis, it proposes that the rewriting discusses social phenomena linked to the fissures of the Chilean nation and the political processes of the current situation. As an objective, it tries to establish a dialogue with the tradition and the literary field around the epic, on the one hand, and with critical reconsiderations about the poetic imagination of history, on the other. With increased visibility of trans people comes increased philosophical interest in gendered language. This chapter aims to look at the research on gendered language in analytic philosophy of language so far, which has focused on two concerns: (1) determining how to define gender terms like ‘man’ and ‘woman’ such that they are trans inclusive and (2) if, or to what extent, we should use gendered language at all. We argue that the literature has focused too heavily on how gendered language (...) can harm trans people, and has not considered how trans people use gendered language to create meaning and joy for ourselves. Pulling from the literature in sociolinguistics, we look at examples of how trans people use language to make their lives better by gaining recognition, playing with gendered language, finding joy in gendered language, and taking control of definitional power, concluding that debates about gendered language need to consider not only how such language harms trans people but also how trans people use it for our own liberation. RESUMEN El objetivo del presente estudio está dirigido a exponer particularidades éticas del trabajo anatomopatológico a la luz de la realidad histórico social. Se emplean métodos teóricos y empíricos y a partir del análisis documental se opera con referentes en los enfoques contemporáneos. El estudio de aspectos generales referidos a la moral y a la ética permitió acceder a definiciones, bases legales y evolución histórica, a la vez que se profundiza en su repercusión en el sector salud y se desarrollan (...) las peculiaridades del trabajo anatomopatológico desde sus manifestaciones en la actividad asistencial, docente e investigativa. Se concluye que los principios éticos generales de la conducta profesional y médica son aplicables al trabajo anatomopatológico en todas sus esferas; al igual que los de la bioética. ABSTRACT The objective of the present study is directed to exhibit ethical peculiarities of the anatomical-pathological work in view of the historical-social reality. Theoretical and empirical methods are used and from the documentary analysis it occurs with modality in the contemporary approaches. The study of general aspects referred to the morality and to the ethics allowed to gain access to definitions, legal bases and historical evolution, simultaneously that deepens in its aftereffect in the sector health and the peculiarities of the anatomical-pathological work develop from its declarations in the welfare, teaching and investigative activity. One concludes that the general ethical beginning of the professional and medical conduct is applicable to the anatomical-pathological work in all its spheres; as those of the bioethics. (shrink)

RESUMEN Se fundamenta la concepción teórica general de la educación axiológica en el contexto del sistema de enseñanza de postgrado para la formación de educadores ambientales. Se aplicaron los métodos del nivel teórico analítico-sintético, inductivo-deductivo, histórico-lógico y ascensión de lo abstracto a lo concreto, con la intención de sistematizar información proveniente de la bibliografía consultada y de la experiencia profesional de los autores. Se defiende la idea de considerar la existencia de una dimensión ambiental de valores universales, en lugar de (...) hacer referencia a valores estrictamente ambientales. Se enfatiza en la importancia que, para la educación, adquiere la estimulación de la actividad valorativa del sujeto, a partir de una noción precisa del sistema de virtudes y fortalezas del carácter. Se identifican cinco valores básicos a desarrollar en el educador ambiental, para asegurar, desde el punto de vista axiológico, su desempeño en la preparación de diferentes sectores sociales y consolidar la necesaria contribución de los mismos a la sostenibilidad ecológica. ABSTRACT In this paper a general theoretical conception of axiological education is systematized in the context of the postgraduate educational system in the training of environmental educators. Methods of the theoretic level to systematize information obtained from the bibliography and from the vocational experience of the author. The category value is analyzed aimed at using it unilaterally from the environmental perspective and it is concluded that it is much more objective, from the theoretical point of view, to assume the existence of an environmental dimension of universal values ​​rather than of strictly environmental values. It emphasizes on the importance of the stimulation of the person’s valuating activity for education, from a precise notion of the system of virtues and character strengths. Five basic values are identified ​​ to be developed in an environmental educator to ensure, from the axiological point of view, his performance in the preparation of different social sectors to strengthen their necessary contribution to ecological sustainability. Resumen En este trabajo se estudia la percepción de un grupo de estudiantes universitarios sevillanos, sin conocimientos especializados sobre la variación geográfica y social del español, hacia las variedades andaluza, canaria y castellana. Los resultados revelan la presencia significativa de juicios negativos hacia su propia modalidad de habla, tanto en lo relativo a la pronunciación como a su asociación con un estatus social bajo. No obstante, se produjo un incremento de consideraciones positivas cuando se les pidió la valoración de los (...) habitantes de su región, los andaluces. La variedad castellana, que se identificó como la forma de hablar más prestigiosa, recibió las valoraciones más positivas: fue considerada una pronunciación clara y sus hablantes se asociaron a una posición social elevada. En tercer lugar, las creencias sobre la variedad canaria reforzaron algunos tópicos como la dulzura del acento. (shrink)
