Defensive medicine is defined as a doctor’s deviation from standard practice to reduce or prevent complaints or criticism. The objectives of this study were to assess the prevalence of the practice of defensive medicine in the UK among hospital doctors and the factors affecting it.

Pervasive digitality reveals us as analogue creatures that are unprepared for a world and a logic generated increasingly through automation. Promulgated by capitalism, digitality has created a new form of alienation, one far more powerful and comprehensive than that envisaged by either Marx or Lukács in the analogue-industrial age. Digital alienation-through-automation is the central process in our digital post-modernity. The effects reach increasing registers and spheres of culture, economy and politics. This essay considers the effects within the production of knowledge (...) within higher education. Through a critique of Clayton Christensen’s concept of ‘disruptive technologies’, it develops the argument that knowledge produced in our higher institutions of learning suffer from two forms of ‘knowledge capture’. The first and most comprehensive is knowledge increasingly oriented toward instrumental outcomes, generated in vocationally oriented and market-pressured contexts, and produced by the general-sector institutions; second is knowledge, much of it profound and potentially world-changing—in economics and climate science in the cases discussed here. These are generated in both prestigious general-sector universities, and in the elite Ivy League and Russell Group universities but are unable to gain traction with an ascendant and alien post-Westphalian, post-modern and post-Enlightenment political context—created by a globalising digitality. I argue that unless we recognise our analogue essence, and that this has been alienated by digital automation, then the aporias of digitality, with knowledge production an important element, then our institution of higher learning will continue to produce much valuable knowledge that has diminishing practical (political) effect. This article presents the first results of a study of the decisions made by health professionals in South Australia concerning the management of death, dying, and euthanasia, and focuses on the findings concerning the attitudes and practices of medical practitioners. Mail-back, self-administered questionnaires were posted in August 1991 to a ten per cent sample of 494 medical practitioners in South Australia randomly selected from the list published by the Medical Board of South Australia. A total response rate of 68 per (...) cent was obtained, 60 per cent of which (298) were usable returns. It was found that forty-seven per cent had received requests from patients to hasten their deaths. Nineteen per cent had taken active steps which had brought about the death of a patient. Sixty-eight per cent thought that guidelines for withholding and withdrawal of treatment should be established. Forty-five per cent were in favour of legalisation of active euthanasia under certain circumstances. (shrink)

Through a systematic foregrounding of temporality as a framework of analysis, the dynamics of neo-liberal globalization and the revolution in ICTs constitute a new epistemological context. From this perspective the world as an economic, social, cultural, and political postmodernity becomes apparent. The article argues that liberal democracy was created and evolved in a specific context too. It was one formed through the interactions of Enlightenment thought and capitalist action - both of which were suffused by the temporality of the clock. (...) For nearly three-hundred years we have taken for granted its meter of the clock as the measure of temporal reality, and have similarly accepted that liberal democracy functions on the same temporal basis. This has blinded us to a "social acceleration" that has developed through the convergence of neo-liberal economics and ICTs to the point where liberal democracy and its clock time functioning simply cannot synchronize with the times of the network society. Classical liberal democracy has been rendered ineffectual as a means of democratic action, and neo-liberal globalization offers nothing to replace it. (shrink)

This essay argues that through a new and radical relationship with digital technologies that are oriented towards networking and automaticity, humans have become estranged from what philosopher Arnold Gehlen termed the ‘circle of action’ that expressed our ancient adaptation to tool use and constituted the basis for our capacity for reflective consciousness. The objectification of the material and analogue relationship that enabled humans to ‘act’ upon the world and to construct the basis for our collective endeavours, this paper shows, is (...) beginning to redirect the social orientation that tool use enabled, back to a form of individualism I term a ‘post-modern vanity’. Computer-driven automation on a networked and global scale is engendering a new illusion regarding our relationship with technology. Greek philosophy, later religion, and then the Enlightenment all saw our relation to technology as evidence of either our ‘specialness’ or our inherent ‘mastery over nature’. These perspectives resulted in our acquiring a ‘reasoned self-delusion’ wherein for the individual life had meaning and was ontologically tolerable—we thought ourselves to be of a realm beyond raw nature. Automation, however, to an unprecedented degree, objectifies and exteriorizes the processes of technology use, alienating the individual from the ‘circle of action’. This process is throwing us back to another form of what this essay terms a ‘reasoned self-delusion’ where we increasingly believe that we no longer need each other so much. The cult of computer efficiency and the virtual sphere of the network that it has spawned, I argue, is a largely barren sphere of radical individualism, a post-modern vanity, where we exist at a borderland closer to nature and to the animals than we have been for thousands of years. Objectives: To evaluate the perceptions of clinical dental students on the role of online education in providing dental education during the COVID-19 crisis.Materials and Methods: A cross-sectional online survey was sent to four Egyptian dental schools from the 20th of January 2021 to the 3rd of February 2021. Survey questions included the demographics, uses, experiences, perceived benefits, and barriers of distance learning in dentistry during the COVID-19 pandemic. Responses were collected from the clinical dental school students. Categorical data were presented (...) as frequencies and percentages and were analyzed using Fisher’s exact test.Results: Three hundred thirty-seven clinical dental students across four Egyptian dental schools responded. Most students used either Google Classroom or Microsoft Teams to access the online content. The data showed that the COVID-19 pandemic affected the academic performance of most participants with varying degrees. On average, students were neutral when asked to rate the online lectures, but did not find online practical education as effective as online theoretical teaching. The commonly described barriers to online teaching included loss of interaction with educators, inappropriateness in gaining clinical skills, and the instability of the internet connection.Conclusion: Despite the reported benefits, clinical dental students in Egypt preferred the hybrid approach in dental education as distance learning represented a prime challenge to gain adequate clinical dental skills. RESUMO Este artigo tem como objetivo ilustrar a contribuição do dialogismo para o campo de aquisição da linguagem. De acordo com as abordagens dialógicas, as crianças não experienciam unidades e estruturas linguísticas per se; elas experienciam a linguagem em contextos significativos. Mais especificamente, gêneros do discurso, atividades e situações de interação aparecem como mediadores entre o discurso individual, os usos sociais e uma linguagem particular. Para ilustrar as implicações de uma abordagem dialógica, este artigo apresenta uma pesquisa sobre a aquisição (...) de expressões referenciais (projeto DIAREF). As expressões referenciais são particularmente relevantes, já que seu domínio envolve tanto aspectos formais quanto funcionais da língua. Os resultados mostram que os usos de substantivos, pronomes pessoais, pronomes demonstrativos e deslocamentos realizados pelas crianças são determinados em conjunto com os fatores pragmático-discursivos, tais como a posição nos encadeamentos referenciais, e fatores sócio discursivos, tais como gêneros do discurso, atividades e situações interacionais. This book describes the repertoire and uses of referring expressions by French-speaking children and their interlocutors in naturally occurring dialogues at home and at school, in a wide-range of communicative situations and activities. Through the lens of an interactionist and dialogical perspective, it highlights the interaction between the formal aspects of the acquisition of grammatical morphemes, the discourse-pragmatic dimension, and socio-discursive, interactional and dialogical factors. Drawing on this multidimensional theoretical and methodological framework, the first part of the book deals with (...) the relation between reference and grammar, while the second part is devoted to the role of the communicative experience. Progressively, a set of arguments is brought out in favor of a dialogical and interactionist account of children's referential development. This theoretical stance is further discussed in relation to other approaches of reference acquisition. Good governance is important for the sustainability of Waqf institutions in Malaysia. As a part of good governance, the evaluation of internal control and its components are essential to be considered. While reaching the Maqāṣid al-Sharī‘ah, the appreciation of Wasaṭiyyah approach can be utilised in the evaluation of internal control in the Waqf institutions. Based on qualitative research method, this research explores the internal control and its components in Waqf institutions. The conceptual study on Wasaṭiyyah approach is provided in brief, (...) and the appreciation of Wasaṭiyyah approach in relation to internal control and its components are provided.
