Zusammenfassung
Das Recht von Patienten, selbst über ihre Behandlung zu bestimmen, erfährt in der Radioonkologie eine zunehmende Bedeutung. Damit verknüpft ist die Diskussion um Patientenverfügungen, die trotz großer allgemeiner Akzeptanz in der klinischen Praxis nur selten verwendet werden. Ziel dieser qualitativen Studie war es, die Einstellungen von Patienten, Ärzten und dem Pflegepersonal zur Mitbestimmung von Patienten bei aktuellen Therapieentscheidungen und gegenüber Patientenverfügungen zu untersuchen.
Es wurden 15 Patienten, 7 Ärzte und 13 Pflegende einer radioonkologischen Universitätsklinik in halboffenen, leitfadengestützten Interviews zu folgenden Themen befragt: Kenntnis von Patientenverfügungen, Vor- und Nachteile von Patientenverfügungen, Beteiligung von Patienten an aktuellen Therapieentscheidungen, Gründe für eine Nicht-Beteiligung an Therapieentscheidungen. Die Interviews wurden mit der Methode der qualitativen Inhaltsanalyse ausgewertet.
Bei Patienten kann hinsichtlich ihres Wunsches nach Beteiligung bei Therapieentscheidungen zwischen einer „aktiven“ und einer „passiven“ Gruppe differenziert werden. Ärzte und Pflegepersonal betonen die hohe psychische und körperliche Belastung der Patienten durch Diagnosemitteilung und Therapie. Patientenverfügungen werden generell positiv bewertet und als Mittel zur Ausübung der Selbstbestimmung von Patienten verstanden. Jedoch sehen Patienten selbst im fortgeschrittenen Krankheitsverlauf nur selten die Notwendigkeit, eine Patientenverfügung zu erstellen. In der radioonkologischen Praxis kommunizieren Patienten und Ärzte nur selten über Patientenverfügungen.
Aussagefähige Patientenverfügungen setzen eine rechtzeitige und angemessene Kommunikation über Krankheitsverlauf und Therapieoptionen zwischen Arzt und Patient voraus. In dem untersuchten radioonkologischen Behandlungssetting haben Patientenverfügungen nur eine begrenzte praktische Bedeutung.
Abstract
Background
The right of patients to decide autonomously about their treatment is gaining increasing importance in radiooncology. Associated with this is the discussion about advance directives, which are used infrequently in the clinical setting despite their general acceptance. This qualitative study intended to analyse the attitudes of patients, physicians and nurses towards participation in actual treatment decisions as well as towards advance directives.
Patients and methods
Semi-structured interviews were conducted with 15 patients, 7 physicians and 13 nurses at a German university department of radiation therapy and analysed by content analysis. The questions focused on the knowledge about advance directives, advantages and disadvantages that are associated with advance directives, the participation of patients in treatment decisions, and reasons for being reluctant to participate in treatment decisions.
Results
An “active” versus a “passive” subgroup of patients can be differentiated in regard to their willingness to participate in treatment decisions. Physicians and nurses stress the role of psychological and physical stress in patients with acute cancer. Advance directives are generally regarded as useful because they may strengthen a patient’s autonomy. However, many patients, even in an advanced stage of their disease, do not consider it necessary to make an advance directive. In everyday practice doctors and patients communicate little about advance directives.
Conclusion
Meaningful advance directives need intensive communication about treatment options between patients and doctors in advance. In the radiooncological treatment setting, examined in this study, advance directives play only a limited role in hospital practice.
