In this brief reply to Fraser MacBride's critical examination of the four‐category ontology and the place within it of the particular ‐ universal distinction, it is argued that the prospects for identifying the four basic ontological categories in terms of the characteristic patterns of ontological dependency between entities belonging to the different categories are rather more promising than MacBride suggests.

Jonathan Lowe argues that metaphysics should be restored to a central position in philosophy, as the most fundamental form of rational inquiry, whose findings underpin those of all other disciplines. He portrays metaphysics as charting the possibilities of existence, by idetifying the categories of being and the relations of ontological dependency between entities of different categories. He proceeds to set out a unified and original metaphysical system: he defends a substance ontology, according to which the existence of the world s (...) one world in time depends upon the existence of persisting things which retain their identity over time and through processes of qualitative change. And he contends that even necessary beings, such as the abstract objects of mathematics, depend ultimately for their existence upon there being a concrete world of enduring substances. Within his system of metaphysics Lowe seeks to answer many of the deepest and most challenging questions in philosophy. (shrink)

Meat is a multi-billion-dollar industry that relies on people performing risky physical work inside meat-processing facilities over long shifts in close proximity. These workers are socially disempowered, and many are members of groups beset by historic and ongoing structural discrimination. The combination of working conditions and worker characteristics facilitate the spread of SARS-CoV-2, the virus that causes COVID-19. Workers have been expected to put their health and lives at risk during the pandemic because of government and industry pressures to keep (...) this “essential industry” producing. Numerous interventions can significantly reduce the risks to workers and their communities; however, the industry’s implementation has been sporadic and inconsistent. With a focus on the U.S. context, this paper offers an ethical framework for infection prevention and control recommendations grounded in public health values of health and safety, interdependence and solidarity, and health equity and justice, with particular attention to considerations of reciprocity, equitable burden sharing, harm reduction, and health promotion. Meat-processing workers are owed an approach that protects their health relative to the risks of harms to them, their families, and their communities. Sacrifices from businesses benefitting financially from essential industry status are ethically warranted and should acknowledge the risks assumed by workers in the context of existing structural inequities. Until quite recently, mind-body dualism has been regarded with deep suspicion by both philosophers and scientists. This has largely been due to the widespread identification of dualism in general with one particular version of it: the interactionist substance dualism of Réné Descartes. This traditional form of dualism has, ever since its first formulation in the seventeenth century, attracted numerous philosophical objections and is now almost universally rejected in scientific circles as empirically inadequate. During the last few years, however, renewed attention (...) has begun to be paid to the dualistic point of view, as a result of increasing discontent with the prevailing materialism and reductionism of contemporary scientific and philosophical thought. Awareness has grown that dualism need not be restricted to its traditional form and that other varieties of dualism are not subject to the difficulties commonly raised against Descartes' own version of it. -/- Interest in these alternative versions of dualism is growing fast today, because it seems that they are capable of capturing deep-seated philosophical intuitions, while also being fully consistent with the methodological assumptions and empirical findings of modern scientific work on the human mind and brain. The object of this book is to provide philosophers, scientists, their students, and the wider general public with an up-to-date overview of current developments in dualistic conceptions of the mind in contemporary philosophy and science. During the century before its final evacuation in 1930 the population of St Kilda declined from over 100 to 36. While undoubtedly emigration and natural disasters played a part in this depopulation, ongoing processes were also important. In particular, replacement levels were never sufficient to maintain a constant population size. In the early part of this period the main factor responsible was heavy neonatal mortality, almost all from tetanus (), but latterly the fertility of those who survived was low, even (...) though mortality rates had declined. In part at least, this decline in fertility could be ascribed to a lower frequency of marriage—a phenomenon seen elsewhere in the Outer Hebrides during the latter part of the 19th century. (shrink)

This paper calls into question the assumptions implicit in many traditional theories of moral agency; namely, the assumption that moral agency requires the agent to be disinterested, disengaged, and psychically distant in order to be a good moral agent, an agent worthy of moral praise. I explore the nature of what it means to be a moral agent and, more broadly, what it means to live well as a human being and apply this analysis to education. The arguments made are (...) grounded in a naturalistic-transactional understanding of how the human being comes to be and continues to be in this world and spells out some of the implications of this understanding of the human individual for education. In addition, I suggest specifi c strategies for capitalizing on the transactional potential of the educational relationship between student and teacher. I argue that, if we take the human being to be a transactional being, i.e., an entity that comes into existence and continues to exist necessarily in and through co-constitutional exchanges with both human and non-human entities, then the role of education becomes central to this development of the individual. Further, it also becomes clear that education must be of a particular nature if it is going to foster the immediate and ongoing development of that individual as a fl ourishing human being. However, we must fi rst explore briefl y what is problematic about our traditional conception of education in order to see that change is required. (shrink)

The complex hexaploid wheat genome offers many challenges for genomics research. Expressed sequence tags facilitate the analysis of gene-coding regions and provide a rich source of molecular markers for mapping and comparison with model organisms. The objectives of this study were to construct a high-density EST chromosome bin map of wheat homoeologous group 2 chromosomes to determine the distribution of ESTs, construct a consensus map of group 2 ESTs, investigate synteny, examine patterns of duplication, and assess the colinearity with rice (...) of ESTs assigned to the group 2 consensus bin map. A total of 2600 loci generated from 1110 ESTs were mapped to group 2 chromosomes by Southern hybridization onto wheat aneuploid chromosome and deletion stocks. A consensus map was constructed of 552 ESTs mapping to more than one group 2 chromosome. Regions of high gene density in distal bins and low gene density in proximal bins were found. Two interstitial gene-rich islands flanked by relatively gene-poor regions on both the short and long arms and having good synteny with rice were discovered. The map locations of two ESTs indicated the possible presence of a small pericentric inversion on chromosome 2B. Wheat chromosome group 2 was shown to share syntenous blocks with rice chromosomes 4 and 7. (shrink)

Without the "acid-stability" requirement in low pH environments such as that of proton exchange membrane fuel cells, a much wider range of materials including noble metals, non-noble transition metals, and even metal-free electrocatalysts for the oxygen reduction reaction in alkaline media have been developed due to both thermodynamic and kinetic reasons. As compared to the rapidly (...) increasing number of reports on the development of novel catalyst materials, the understanding of the reaction mechanisms of the various ORR electrocatalysts is quite insufficient, and the application and investigation in real alkaline anion exchange membrane fuel cells is even scarcer. By reviewing the compositions, preparation methods, physiochemical properties and ORR performance of different categories of cathodic electrocatalysts that have emerged in the past few years, some common and intrinsic properties and factors that account for the superior activity of these materials may be extracted and summarized, which may further help to identify the reasons for the kinetic facility of the ORR in alkaline media. Some practical issues of utilization of the promising novel replacement materials for the state-of-the-art Pt-based cathodic electrocatalysts in AAEMFCs are pointed out. In addition to the progress on the development of novel materials with outstanding ORR activity, many and varied compositions and morphologies in one, two and three dimensions, scalable preparation technologies, low cost, and other unique properties, some feedback on the performance and especially the problems of their use as cathodes in AAEMFCs is urgently needed. Such feedback should provide guidelines for the design and manufacture of next-generation electrocatalysts and accelerate the application of AAEMFCs. (shrink)

Initial responses to questionnaires used to assess participants' understanding of informed consent for malaria vaccine trials conducted in the United States and Mali were tallied. Total scores were analyzed by age, sex, literacy (if known), and location. Ninety-two percent (92%) of answers by United States participants and 85% of answers by Malian participants were correct. Questions more likely to be answered incorrectly in Mali related to risk, and to the type of vaccine. For adult participants, independent predictors of higher scores (...) were younger age and female sex in the United States, and male sex in Mali. Scores in the United States were higher than in Mali (P = 0.005). Despite this difference participants at both sites were well informed overall. Although interpretation must be qualified because questionnaires were not intended as research tools and were not standardized among sites, these results do not support concerns about systematic low understanding among research participants in developing versus developed countries. While the ICD-DSM paradigm has been a major advance in clinical psychiatry, its usefulness for biological psychiatry is debated. By defining consensus-based disorders rather than empirically driven phenotypes, consensus classifications were not an implementation of the biomedical paradigm. In the field of endogenous psychoses, the Wernicke-Kleist-Leonhard (WKL) pathway has optimized the descriptions of 35 major phenotypes using common medical heuristics on lifelong diachronic observations. Regarding their construct validity, WKL phenotypes have good reliability and predictive and face validity. WKL phenotypes come (...) with remarkable evidence for differential validity on age of onset, familiality, pregnancy complications, precipitating factors, and treatment response. Most impressive is the replicated separation of high- and low-familiality phenotypes. Created in the purest tradition of the biomedical paradigm, the WKL phenotypes deserve to be contrasted as credible alternatives with other approaches currently under discussion.. (shrink)

Lithium/sulfur cells are receiving significant attention as an alternative power source for zero-emission vehicles and advanced electronic devices due to the very high theoretical specific capacity of the sulfur cathode. However, the poor cycle life and rate capability have remained a grand challenge, preventing the practical application of this attractive technology. Here, we report that a Li/S cell employing a cetyltrimethyl ammonium bromide -modified sulfur-graphene oxide nanocomposite cathode can be discharged at rates as high as 6C and charged at rates (...) as high as 3C while still maintaining high specific capacity, with a long cycle life exceeding 1500 cycles and an extremely low decay rate, perhaps the best performance demonstrated so far for a Li/S cell. The initial estimated cell-level specific energy of our cell was ∼500 W·h/kg, which is much higher than that of current Li-ion cells. Even after 1500 cycles, we demonstrate a very high specific capacity, which corresponds to ∼414 mA·h/g of electrode: still higher than state-of-the-art Li-ion cells. Moreover, these Li/S cells with lithium metal electrodes can be cycled with an excellent Coulombic efficiency of 96.3% after 1500 cycles, which was enabled by our new formulation of the ionic liquid-based electrolyte. The performance we demonstrate herein suggests that Li/S cells may already be suitable for high-power applications such as power tools. Li/S cells may now provide a substantial opportunity for the development of zero-emission vehicles with a driving range similar to that of gasoline vehicles. © 2013 American Chemical Society. He has also argued, however, that this scene contains substantial Plautine expansion, comparable with that which has been recognised in the cook scene of the Aulularia. I wish to suggest that Dohm is largely right but that the Plautine expansion is even more extensive than he supposes. In 790–838 Plautus is probably for the most part following his Greek model (...) fairly closely. One can trace a logical sequence of ideas, as follows. Ballio: ‘I couldn't have found a worse cook’ . Cook: ‘Why did you hire me then?’ . Ballio: ‘You were the only one left. And why was that?’ . Cook: ‘I am expensive, but nowadays people look for cheap cooks, who produce only concoctions of seasoned vegetables. That is why men are so short-lived’ . Ballio: ‘You can make men live longer then?’ . Cook: ‘Certainly, for 200 years’ . The cook then proceeds to give a list of his fantastic sauces for fish and meat , until he is cut short by Ballio's ‘Damn your lies’ . We have here a typical comic mageiros: he is loquacious and boastful , claims magic powers , denigrates his rivals , reels off lists of foods, real and fictitious , and uses grandiose language . This article discusses common ethical and practical considerations in psychosocial and behavioral research in healthcare. Issues such as appropriate objectives and intent, risk-benefit ratios, research design, and human subject protection are explored. The burden of ethical research design and implementation is placed on the investigator, rather than relying solely on institutional review boards to judge individual projects. The benefit of acquisition of knowledge must be balanced against the burdens of the research on society in general and human subjects specifically. Scientific (...) replication of research is encouraged, unnecessary duplication defined and discouraged, and benefits of true collaboration outlined. Investigators are advised to consider the context, intent, purpose, implementation, and use of information when developing research. The concept of “researcher myopia” is defined as a common stumbling block. It is suggested that academic researchers also look to other disciplines, such as industry, for examples of research that is concise, cost-effective, and reliable. (shrink)

Although by no means absent from the comedies of Menander and Terence, it is not nearly so prominent in them as in Plautus. One reason for this difference may be Plautus' choice of Greek models, but there are good grounds for believing that changes made by Plautus to his models also served to emphasize the theme. E. Fraenkel showed that Plautus elevated the (...) scheming slave into a comic hero in a manner alien to Menander and Terence. He identified as an important factor in this heroization of the slave certain features of style, notably the recurring use of military metaphors which equate the slave with a victorious general. He also showed that Plautus probably greatly expanded certain slave roles, this expansion being most evident in elaborate polymetric monodies such as Chrysalus' Troy canticum, Bacch. 925ff. These stylistic effects and the increased bulk of the roles give greater emphasis to the Plautine scheming slave and ipso facto to the element of intrigue in the plot. The omissions which naturally went together with Plautus' insertions could also distort the emphasis of the Greek play in favour of the element of intrigue; a probable example is the Casina, in which it is generally agreed that Plautus cut the original anagnorisis ending. Recent writers have stressed another aspect of Plautus' style which helps to give a particular prominence to the theme of intrigue in his plays, his liking for metatheatrical effects, which remind the spectators that they are watching a stage performance not real life; these effects are pervasive but particularly cluster around scenes of deception, emphasizing that the deception is in a special sense a performance, a play within a play. The focus of this study was to analyze the content, distribution, and comparative genome relationships of 996 chromosome bin-mapped expressed sequence tags accounting for 2266 restriction fragments on the homoeologous group 3 chromosomes of hexaploid wheat. Of these loci, 634, 884, and 748 were mapped on chromosomes 3A, 3B, and 3D, respectively. The individual chromosome bin maps revealed bins with a high density of mapped ESTs in the distal region and bins of low density in the proximal region of the (...) chromosome arms, with the exception of 3DS and 3DL. These distributions were more localized on the higher-resolution group 3 consensus map with intermediate regions of high-mapped-EST density on both chromosome arms. Gene ontology classification of mapped ESTs was not significantly different for homoeologous group 3 chromosomes compared to the other groups. A combined analysis of the individual bin maps using 537 of the mapped ESTs revealed rearrangements between the group 3 chromosomes. Approximately 232 of the consensus mapped ESTs matched sequences on rice chromosome 1 and revealed large- and small-scale differences in gene order. Of the group 3 mapped EST unigenes ∼21 and 32% matched the Arabidopsis coding regions and proteins, respectively, but no chromosome-level gene order conservation was detected. (shrink)

We examine “de Broglie-Bohm” causal trajectories for the two electrons in a nonrelativistic helium atom, taking into account the spin-dependent momentum terms that arise from the Pauli current. Given that this many-body problem is not exactly solvable, we examine approximations to various helium eigenstates provided by a low-dimensional basis comprised of tensor products of one-particle hydrogenic eigenstates.First to be considered are the simplest approximations to the ground and first-excited electronic states found in every introductory quantum mechanics textbook. For example, the (...) trajectories associated with the simple 1s(1)1s(2) approximation to the ground state are, to say the least, nontrivial and nonclassical.We then examine higher-dimensional approximations, i.e., eigenstates Ψ α of the Hamiltonian in this truncated basis, and show that ∇ i S α =0 for both particles, implying that only the spin-dependent momentum term contributes to electronic motion. This result is independent of the size of the truncated basis set, implying that the qualitative features of the trajectories will be the same, regardless of the accuracy of the eigenfunction approximation.The electronic motion associated with these eigenstates is quite specialized due to the condition that the spins of the two electrons comprise a two-spin eigenfunction of the total spin operator. There is ambiguity with regard to what counts as an acceptable level of risk in clinical research in pregnant women and there is no input from stakeholders relative to such research risks. The aim of our paper was to explore what stakeholders who are actively involved in the conduct of clinical research in pregnant women deem an acceptable level of risk for pregnant women in clinical research. Accordingly, we used the APOSTEL VI study, a low-risk obstetrical randomised controlled trial, as (...) a case-study. We conducted a prospective qualitative study using 35 in-depth semi-structured interviews and one focus group. We interviewed healthcare professionals, Research Ethics Committee members and regulators who are actively involved in the conduct of clinical research in pregnant women, in addition to pregnant women recruited for the APOSTEL VI case-study in the Netherlands. Three themes characterise the way stakeholders view risks in clinical research in pregnant women in general. Additionally, one theme characterises the way healthcare professionals and pregnant women view risks with respect to the case-study specifically. First, ideas on what constitutes an acceptable level of risk in general ranged from a preference for zero risk for the foetus up to minimal risk. Second, the desirability of clinical research in pregnant women in general was questioned altogether. Third, stakeholders proposed to establish an upper limit of risk in potentially beneficial clinical research in pregnant women in order to protect the foetus and the pregnant woman from harm. Fourth and finally, the case-study illustrates that healthcare professionals’ individual perception of risk may influence recruitment. Healthcare professionals, RECs, regulators and pregnant women are all risk adverse in practice, possibly explaining the continuing underrepresentation of pregnant women in clinical research. Determining the acceptable levels of risk on a universal level alone is insufficient, because the individual perception of risk also influences behaviour towards pregnant women in clinical research. Therefore, bioethicists and researchers might be interested in changing the perception of risk, which could be achieved by education and awareness about the actual benefits and harms of inclusion and exclusion of pregnant women. The conclusions emphasize the need to involve local professionals in all phases of research and technology transfer in developing countries, avoidance of research that may worsen the situation of the recipients, sustainability of production systems, and the need for detailed assessment of potential impacts of technology on recipients. Although nicotine may have some harmful effects, the detrimental health effects of smoked tobacco are primarily due to non-nicotine constituents. Hence, the health effects of nicotine reduction would likely be determined by changes in behavior that result in changes in smoke exposure. Methods: (...) Herein, we review the current evidence on nicotine reduction and discuss some of the challenges in establishing the empirical basis for regulatory decisions. Results: To date, research suggests that very low nicotine content cigarettes produce a desirable set of outcomes, including reduced exposure to nicotine, reduced smoking, and reduced dependence, without significant safety concerns. However, much is still unknown, including the effects of gradual versus abrupt changes in nicotine content, effects in vulnerable populations, and impact on youth. Discussion: A coordinated effort must be made to provide the best possible scientific basis for regulatory decisions. The outcome of this effort may provide the foundation for a novel approach to tobacco control that dramatically reduces the devastating health consequences of smoked tobacco. (shrink)

To reduce bias, previous studies have used a statistical method that directly estimates the SFS from sequencing data by first computing site allele frequency likelihood for each site using a dynamic programming algorithm. Although this method produces an accurate SFS, computing the SAF likelihood is quadratic in the number of samples sequenced. Results: To overcome this computational challenge, we propose an algorithm, 'score-limited DP' algorithm, which is linear in the number of genomes to compute the SAF likelihood. This algorithm works because in a lower triangular matrix that arises in the DP algorithm, all non-negligible values of the SAF likelihood are concentrated on a few cells around the best-guess allele counts. We show that our score-limited DP algorithm has comparable accuracy but is faster than the original DP algorithm. This speed improvement makes SFS estimation practical when using low coverage NGS data from a large number of individuals. (shrink)

The Sarbanes–Oxley Act of 2002 requires audit committees of public companies’ boards of directors to install an anonymous reporting channel to assist in deterring and detecting accounting fraud and control weaknesses. While it is generally accepted that the availability of such a reporting channel may reduce the reporting cost of the observer of a questionable act, there is concern that the addition of such a channel may decrease the overall effectiveness compared to a system employing only non-anonymous reporting options. The (...) rationale underlying this concern involves the would-be reporter’s likelihood of reporting, the seriousness with which the organization treats an anonymous report, and the organization’s ability to thoroughly follow-up the report. Thus, we explore the extent to which the availability of an anonymous reporting channel influences intended use of non-anonymous reporting channels. Further, in response to Sarbanes–Oxley and the environment of financial scandals that led to its passage, many firms are strengthening their internal audit departments, and providing them with greater independence from upper management’s direct control. Accordingly, our examination tests whether the intended use of the internal audit department as an internal reporting channel is greater when the internal audit department is of “high” versus “low” quality. Finally, the study investigates intended reporting behavior across three different cases (e.g., settings). Results show that the existence of an anonymous channel does reduce the likelihood of reporting to non-anonymous channels, that generally the internal audit department quality does not affect reporting to non-anonymous channels, and that case-setting affects the type of channel to be used. Published by Oxford University Press on behalf of the Royal Astronomical Society.We present multiple ultrahigh resolution cosmological hydrodynamic simulations of M* ≃ 104-6.3 M⊙ dwarf galaxies that form within two Mvir = 109.5-10 M⊙ dark matter halo initial conditions. Our simulations rely on the Feedback in Realistic Environments implementation of star formation feedback and were run with high enough force and mass resolution to directly resolve structure on the ~200 pc scales. The resultant galaxies sit on (...) the M* versus Mvir relation required to match the Local Group stellar mass function via abundance matching. They have bursty star formation histories and also form with half-light radii and metallicities that broadly match those observed for local dwarfs at the same stellar mass. We demonstrate that it is possible to create a large constant-density dark matter core in a cosmological simulation of an M* ≃ 106.3 M⊙ dwarf galaxy within a typical Mvir = 1010 M⊙ halo - precisely the scale of interest for resolving the 'too big to fail' problem. However, these large cores are not ubiquitous and appear to correlate closely with the star formation histories of the dwarfs: dark matter cores are largest in systems that form their stars late, after the early epoch of cusp building mergers has ended. Our M* ≃ 104 M⊙ dwarf retains a cuspy dark matter halo density profile that matches that of a dark-matter-only run of the same system. Though ancient, most of the stars in our ultrafaint form after reionization; the ultraviolet field acts mainly to suppress fresh gas accretion, not to boil away gas that is already present in the protodwarf. We report what is to our knowledge the first experimental demonstration and theoretical analysis of an optical laser trap that uses a pair of mutually phase-conjugate beams. A primary trapping beam derived from an argon laser together with its counterpropagating phase-conjugate beam creates a self-aligned dual-beam laser trap that provides stable three-dimensional confinement for micrometer-sized dielectric particles. The transverse trapping efficiency, experimentally measured for low-numerical-aperture objective lenses, is found to be comparable with that produced by a single-beam gradient force trap. (...) A theoretical analysis, which compares the performance of the self-aligned dual-beam trap against that of single-beam gradient force and conventional counterpropagating dual-beam laser traps, shows that phase-conjugate trapping provides a slight improvement in axial trapping efficiency over the other trapping geometries. The advantages of combining laser trapping with photorefractive optical phase conjugation for simultaneous sample micromanipulation and optical image processing are discussed. © 1997 Optical Society of America. This study explores the relationship between moral intensity and the use of different sensemaking strategies in military critical incidents. First, narratives of military personnel were used to select prototypical high/low moral intensity critical incidents. In a follow-up, a scenario study was conducted with active duty military personnel to examine the relationship between moral intensity and the use of sensemaking tactics. This study offers three main conclusions. First, the use of sensemaking tactics is strongly tied to the level of moral intensity (...) in the situation. In high-intense situations, the servicemen draw on previous experiences, prediction of consequences, and help of others to recognize and interpret the situation. Less attention goes out to higher level critical thinking. Thus, it seems that in these critical incidents, the servicemen react without giving room for thorough consideration and deliberation. Second, the number of deployments a serviceman experienced influences the perceived seriousness and harmfulness of the situation negatively in low-intense situations. Finally, and in line with earlier studies, the results indicate that the concept of moral intensity is formed out of three rather than the six dimensions originally proposed by Jones. Facing an increasing number and variety of issues with social salience, firms must determine how to engage with issues that likely have a significant impact on them. Integrating issues management and salience theories, the authors find that firms engage with socially contested issues—where there is a high degree of societal disagreement—in a different manner from issues that have social consensus, or high agreement. Examining social issue resolutions filed by shareholders from 1997 to 2009, the study finds that socially contested issues, (...) as well as those issues with social consensus, are both likely to result in engagement by the firm. For social issues with consensus, a firm is more likely to opt for a low level of shareholder engagement whereas resolutions regarding contested issues lead to engaging shareholders at a higher level. These findings shed new light on the IM and issue salience literature streams that have suggested firms will react differently to these types of issues, even while they remain largely untested. Finally, firms become less engaged with perennial issues over time. rather than more, providing new guidance to researchers, shareholder activists, and firms alike. To the authors’ knowledge, such fined-grained insight into expected levels of firm engagement with social issue salience has not been put forth previously. (shrink)

Copyright © 2015 by the American Thoracic Society. Rationale: A low respiratory arousal threshold is a physiological trait involved in obstructive sleep apnea pathogenesis. Trazodone may increase arousal threshold without compromising upper airway muscles, which should improve OSA. Objectives: We aimed to examine how trazodone alters OSA severity and arousal threshold. We hypothesized that trazodone would increase the arousal threshold and improve the apnea/hypopnea index in selected patients with OSA. Methods: Subjects were studied on two separate nights in a randomized (...) crossover design. Fifteen unselected subjects with OSA underwent a standard polysomnogram plus an epiglottic catheter to measure the arousal threshold. Subjects were studied after receiving trazodone and placebo, with 1 week between conditions. The arousal threshold was calculated as the nadir pressure before electrocortical arousal from approximately 20 spontaneous respiratory events selected randomly. Measurements and Main Results: Compared with placebo, trazodone resulted in a significant reduction in AHI, without worsening oxygen saturation or respiratory event duration. Trazodone was not associated with a significant change in the non-REM arousal threshold compared with placebo. In subgroup analysis, responders to trazodone spent less time in N1 sleep and had an accompanying reduction in arousal index, whereas nonresponders were not observed to have a change in sleep parameters. Conclusions: These findings suggest that trazodone could be effective therapy for patients with OSA without worsening hypoxemia. Future studies should focus on underlying mechanisms and combination therapies to eliminate OSA. (shrink)

The current study uses a sensemaking model and thinking strategies identified in earlier research to examine ethical decision making. Using a sample of 163 undergraduates, a low-fidelity simulation approach is used to study the effects personal involvement (in causing the problem and personal involvement in experiencing the outcomes of the problem) could have on the use of cognitive reasoning strategies that have been shown to promote ethical decision making. A mediated model is presented which suggests that environmental factors influence reasoning (...) strategies, reasoning strategies influence sensemaking, and sensemaking in turn influences ethical decision making. Findings were mixed but generally supported the hypothesized model. It is interesting to note that framing the outcomes of ethically charged situations in terms of more global organizational outcomes rather than personal outcomes was found to promote the use of pro-ethical cognitive reasoning strategies. Comprehension and recall of informed consent was assessed after the study closure in the parents/guardians of a birth cohort of children participating in an intensive three-year diarrhoeal surveillance. A structured questionnaire was administered by field workers who had not participated in the study's follow-up protocol. Of 368 respondents, 329 (89.4 per cent) stated that the study was adequately explained during enrolment, but only 159 (43.2 per cent) could recall that it was on diarrhoea. Nearly half (45.9 per cent) of the (...) respondents stated that they would not have participated if free medical treatment was not provided, although the free medical clinic was not offered at enrolment. Spousal approval (82.9 per cent) was the most important factor in the decision to participate. In the multivariate analysis, maternal education was associated with increased awareness about the disease under study: mothers without any formal education had the lowest awareness (OR = 3.47, 95 per cent CI = 1.60–7.51). Despite a high compliance with the study protocol, retention of understanding about the research study was low over a long period of time. Although the local government provides free healthcare in the study area, respondents cited free healthcare as an important reason for participation, highlighting the need for true health equity before meaningful informed consent can be obtained. © 2015 Baum et al.; licensee BioMed Central. Background: Malaria is a public health problem in parts of Thailand, where Plasmodium falciparum and Plasmodium vivax are the main causes of infection. In the northwestern border province of Tak parasite prevalence is now estimated to be less than 1% by microscopy. Nonetheless, microscopy is insensitive at low-level parasitaemia. The objective of this study was to assess the current epidemiology of falciparum and vivax malaria in Tak using molecular methods to detect exposure (...) to and infection with parasites; in particular, the prevalence of asymptomatic infections and infections with submicroscopic parasite levels. Methods: Three-hundred microlitres of whole blood from finger-prick were collected into capillary tubes from residents of a sentinel village and from patients at a malaria clinic. Pelleted cellular fractions were screened by quantitative PCR to determine parasite prevalence, while plasma was probed on a protein microarray displaying hundreds of P. falciparum and P. vivax proteins to obtain antibody response profiles in those individuals. Results: Of 219 samples from the village, qPCR detected 25 Plasmodium sp. infections, of which 92% were asymptomatic and 100% were submicroscopic. Of 61 samples from the clinic patients, 27 were positive by qPCR, of which 25.9% had submicroscopic parasite levels. Cryptic mixed infections, misdiagnosed as single-species infections by microscopy, were found in 7 malaria patients. All sample donors, parasitaemic and non-parasitaemic alike, had serological evidence of parasite exposure, with 100% seropositivity to at least 54 antigens. Antigens significantly associated with asymptomatic infections were P. falciparum MSP2, DnaJ protein, putative E1E2 ATPase, and three others. Conclusion: These findings suggest that parasite prevalence is higher than currently estimated by local authorities based on the standard light microscopy. As transmission levels drop in Thailand, it may be necessary to employ higher throughput and sensitivity methods for parasite detection in the phase of malaria elimination. (shrink)
