Although moral distress is now a well-recognized phenomenon among all of the healthcare professions, few evidence-based strategies have been published to address it. In morally distressing situations, the “presenting problem” may be a particular patient situation, but most often signals a deeper unit- or system-centered issue. This article describes one institution’s ongoing effort to address moral distress in its providers. The research sets out an organizing framework for the field of social ontology, the study of the of the nature of the social world. Social ontology is also gaining prominence in traditional philosophical discussions. The subject matter of social ontology is clarified, in particular the difference between it and the study of causal relations and the explanation of social phenomena. For any social fact, there are two distinct ontological questions: what is the ground for that fact, why is that fact (...) grounded just the way. Two different inquires are defined and explained: the study of how social categories are “anchored” or “grounded”. The distinction between these inquires is used to clarify prominent programs in social theory, particularly theories of practice and varieties of ontological individualism. Moral distress (MD) is well-documented within the nursing literature and occurs when constraints prevent a correct course of action from being implemented. The measured frequency of MD has increased among nurses over recent years, especially since the COVID-19 Pandemic. MD is less understood among nurse leaders than other populations of nurses. A qualitative systematic review was conducted with the aim to synthesize the experiences of MD among nurse leaders. This review involved a search of three databases (Medline, CINAHL, and APA (...) PsychINFO) which resulted in the retrieval of 303 articles. PRISMA review criteria guided authors during the article review and selection process. Following the review, six articles were identified meeting review criteria and quality was assessed using the Critical Appraisal Skills Programme (CASP) Checklist for qualitative studies. No ethical review was required for this systematic review. The six studies included in this review originated from the United States, Brazil, Turkey, and Iran. Leadership roles ranged from unit-based leadership to executive leadership. Assigned quality scores based upon CASP criteria ranged from 6 to 9 (moderate to high quality). Three analytical themes emerged from the synthesis: (1) moral distress is consuming; (2) constrained by the system; and (3) adapt to overcome. The unique contributors of MD among nurse leaders include the leadership role itself and challenges navigating moral situations as they arise. The nurse leader perspective should be considered in the development of future MD interventions. (shrink)

The article realizes the analysis of B. Epstein’s social ontology. Social ontology is teaching on basic principles of constructing of social reality, founded on interdisciplinary and transdisciplinary strategies of investigations of the social world. There are five leading programs in contemporary social ontology: “CIIF-program” of J. Searle, “Cambridge program” of T. Lawson, “Tufts program” of B. Epstein, “critical realism” and “the other institutionalism”. “Tufts program” is one from them. Social ontology tries to make progress on clarifying all of (...) these in the context of specific topics: group intentions, laws, corporations, property, institutions, social groups. To begin an inquiry in social ontology, we need to choose which entities to work out the ontology of, that is, where to focus our attention in analyzing the social world. B. Epstein supposes his own model of re-conceptualization of framework of social ontology: two concepts play here a leading role, - “grounding” and “anchoring”. “Anchoring” and grounding”: these are two fundamental aspects to the building of the social world. Correspondingly, social ontology consists of two distinct projects. The grounding project is the inquiry into the conditions for the social facts to obtain. There are facts in the world are metaphysically sufficient reasons, - that is, grounds, - but it is more exact social facts of some kind. The anchoring project is the inquiry into what puts those conditions in place. Also it should be realized research concerning the grounding conditions for social facts. The last work of B. Moral distress is a phenomenon increasingly recognized in healthcare that occurs when a clinician is unable to act in a manner consistent with his or her moral requirements due to external constraints. We contend that some experiences of moral distress are self-inflicted due to one’s under-assertion of professional authority, and these are potentially avoidable. In this article we outline causes of self-inflicted moral distress and offer recommendations for mitigation.

The research analyzes the social ontology of the American philosopher B. Epstein. Social ontology studies the nature of the social world: what are its main elements and how they come together. There are different theories in modern social ontology: the theory of structuration, the theory of communicative action, social constructivism, critical realism. B. Epstein opposes psychological theories of social ontology and ontological individualism in explaining the social world. B. Epstein distinguishes between ontological questions about the social world (...) and causal relationships between events. The initial category of social ontology for American philosopher is a social fact. To establish a social fact, two actions are necessary: to give an ontological explanation of the existing social fact and to find facts that determine the conditions necessary for a social fact. Accordingly, B. Epstein distinguishes between two projects: the project of the foundation of a social fact and the project of fixing a social fact. The project of the foundation of a social fact provides an ontological explanation of a social fact, studies the conditions for the presence of social facts. The fixation project explores what gives rise to the conditions of the basis for social facts. The ground relation and the fixation relation are not causal relationships. A social fact must have diachronic constituent elements, which is, in particular, a test of social theory for truth. The article also discusses the theory of social facts by J. Serle. The theory of J. Serle B. Epstein refers to a psychological concept based on the collective acceptance of certain rules by the community. This position does not seem to be entirely correct, since J. Searle, in understanding the nature of a social fact, relies on social institutions, and the fact itself refers to institutional facts. (shrink)

Models treating the simple properties of social groups have a common shortcoming. Typically, they focus on the local properties of group members and the features of the world with which group members interact. I consider economic models of bureaucratic corruption, to show that (a) simple properties of groups are often constituted by the properties of the wider population, and (b) even sophisticated models are commonly inadequate to account for many simple social properties. Adequate models and social policies must treat certain (...) factors that are not local to individual members of the group, even if those factors are not causally connected to those individuals. We introduce agreement reducibility and highlight its major features. Given subsets A and B of, we write if there is a total computable function satisfying for all,.We shall discuss the central role plays in this reducibility and its connection to strong‐hyper‐hyper‐immunity. We shall also compare agreement reducibility to other well‐known reducibilities, in particular s1‐ and s‐reducibility. We came upon this reducibility while studying the computable reducibility of a class of equivalence relations on based on set‐agreement. We examine the computably enumerable (c.e.) degrees of prime models of complete atomic decidable (CAD) theories. A structure has degree d if d is the degree of its elementary diagram. We show that if a CAD theory T has a prime model of c.e. degree c, then T has a prime model of strictly lower c.e. degree b, where, in addition, b is low (b' = 0'). This extends Csima's result that every CAD theory has a low prime model. We (...) also prove a density result for c.e. degrees of prime models. In particular, if c and d are c.e. degrees with d < c and c not low₂ (c" > 0"), then for any CAD theory T, there exists a c.e. degree b with d < b < c such that T has a prime model of degree b, where b can be chosen so that b' is any degree c.e. in c with d' ≤ b'. As a corollary, we show that for any degree c with 0 < c < 0', every CAD theory has a prime model of low c.e. degree incomparable with c. We show also that every CAD theory has prime models of low c.e. degree that form a minimal pair, extending another result of Csima. We then discuss how these results apply to homogeneous models. In this book, David B. Wong defends an ambitious and important new version of moral relativism. He does not espouse the type of relativism that says anything goes, but he does start with a relativist stance against alternative theories such that there need not be only one universal truth. Wong proposes that there can be a plurality of true moralities existing across different traditions and cultures, all with one core human question as to how we can all live together.

Although death by neurologic criteria is legally recognized throughout the United States, state laws and clinical practice vary concerning three key issues: the medical standards used to determine death by neurologic criteria, management of family objections before determination of death by neurologic criteria, and management of religious objections to declaration of death by neurologic criteria. The American Academy of Neurology and other medical stakeholder organizations involved in the determination of death by neurologic criteria have undertaken concerted action to address variation (...) in clinical practice in order to ensure the integrity of brain death determination. To complement this effort, state policymakers must revise legislation on the use of neurologic criteria to declare death. We review the legal history and current laws regarding neurologic criteria to declare death and offer proposed revisions to the Uniform Determination of Death Act and the rationale for these recommendations. The health care needs of rural populations often differ from those of their urban counterparts. And the ethical dilemmas that caregivers face are distinctively shaped in rural settings, not only by resource constraints, but by the nature of life in small, close-knit communities as well.

It is predicted that the rapid acquisition of new genetic knowledge and related applications during the next decade will have significant implications for virtually all members of society. Currently, most people get exposed to information about genes and genetics only through stories publicized in the media. We sought to understand how individuals in the general population used and understood the concepts of ???genetics??? and ???genes.??? During in-depth one-on-one telephone interviews with adults in the United States, we asked questions exploring their (...) basic understanding of these terms, as well as their belief as to the location of genes in the human body. A wide range of responses was received. Despite conversational familiarity with genetic terminology, many noted frustration or were hesitant when trying to answer these questions. In addition, some responses reflected a lack of understanding about basic genetic science that may have significant implications for broader public education measures in genetic literacy, genetic counseling, public health practices, and even routine health care.
