This article compares health promotion attitudes towards prostate cancer and Alzheimer’s disease. Our aim is to demonstrate that these two apparently distinct conditions of the aging body – one affecting the male reproductive system, the other primarily the brain – are addressed in similar fashion in recent public health activities because of a growing emphasis on a ‘cardiovascular logic’. We suggest that this is a form of reductionism, and argue that it leaves us with a dangerous paradox: while re-transcending, at (...) least partially, the conceptual separation of body and brain, it clouds much-needed discussion and research, such as contingent issues of socio-economic and socio-cultural disease disparities. Definition of the problem: Rationing medical health care has been debated in the industrialized Western hemisphere for at least two decades. Many factors have contributed to the fact that medical care can no longer be provided free of charge, which used to be an explicit political goal in all countries with a well-established welfare system. Considerable shortage of public financial resources is now generally accepted to pertain also to the health care system. Arguments: So far, efforts have been made to (...) rationalize various medical procedures both with regard to specific medical treatment strategies and to administrative support processes. However, these means are limited in their effects to curb medical costs at a significant level. Thus, rationing medical health care is necessary to further enable the reimbursement system to cover expensive diagnostics and treatment that would exceed the individual’s capability to pay. Rationing can occur explicitly or implicitly. Implicit rationing has to be regarded as an unfair procedure and should be rejected on ethical grounds. Conclusion: Distributive justice in medical care can only be accomplished if the reimbursement system is based on a new theoretical foundation. This entails a stronger emphasis on individual responsibility for one’s own life and health and the insight that health is an individual rather than a collective good. Nevertheless, a mutual insurance-based collective health care system should not be abolished as long as it is thoroughly reformed. As a minimum requirement, it must provide incentives to spend money on preventive health measures. (shrink)

BackgroundChildhood adversity is linked to adverse health in adulthood. One posited mechanistic pathway is through physiological responses to acute stress. Childhood adversity has been previously related to both exaggerated and blunted physiological responses to acute stress, however, less is known about the psychological mechanisms which may contribute to patterns of physiological reactivity linked to childhood adversity.ObjectiveIn the current work, we investigated the role of challenge and threat stress appraisals in explaining relationships between childhood adversity and cortisol reactivity in response to (...) an acute stressor.MethodsUndergraduate students completed an online survey that included general demographic information and the Risky Families Questionnaire 24 h before a scheduled lab visit. In the lab, a research assistant collected a baseline salivary cortisol sample. Following the baseline period, participants were read instructions for the Trier Social Stress Test, a validated psychological lab stressor. Next, they completed a challenge vs. threat task appraisal questionnaire and completed the speech and math portion of the TSST. Twenty minutes following the start of the TSST, a second salivary sample was collected to measure changes in salivary cortisol following the TSST.ResultsLinear regression analyses adjusted for age, sex, childhood socioeconomic status, and baseline cortisol levels, showed childhood adversity associated with changes in cortisol levels [B = –0.29 t = –2.35, p = 0.02, R2=0.07]. Linear regression analyses controlling for age, sex, and childhood SES showed childhood adversity associated with both challenge [B = –0.52 t = –5.04, p < 0.001, R2=0.24] and threat [B = 0.55 t = 5.40, p < 0.001, R2=0.27] appraisals. Significant indirect effects of childhood trauma on cortisol reactivity were observed through challenge appraisals [B = –0.01 ], and threat appraisals [B = –0.01 ].ConclusionChildhood adversity may contribute to blunted cortisol reactivity, a pattern of response which is linked to obesity, addiction, and other behavior-related diseases. The study aims at exploring the salience and functions of media and television contents in children’s lives by focusing on their uses as a discursive resource in naturally occurring peer talk. We observed and recorded Israeli children talk in everyday, natural settings in two separate studies, in 1999–2002 and in 2012–2013. Detailed discourse analysis of television-based interactions from an ethnographic, child-centered perspective reveals the enduring centrality of television as an enjoyable, available, and shared cultural resource with valuable social, cognitive, and (...) discursive affordances: it is frequently mentioned during everyday adult-free interactions; utilized as the basis for drawing and negotiating boundaries and hierarchies within the peer community; and facilitates experiencing a variety of discursive literacy skills, ranging between practicing adherence to original texts and creativity and distancing from them. These findings provide further evidence for the mediatization of everyday life, and may have educational implications, which are discussed. (shrink)

This paper discusses the rise and fall of the theory of paternal transmission, drawing attention to the hitherto underresearched debates about the importance and impact of male-mediated harm to the embryo in reproduction that peaked around the turn of the twentieth century. The focus is on the implications of the twin “great social evils,” syphilis and alcohol, which converged at the time of a general transformation of medicine into experimental science and a concomitant rise in new concepts of heredity. Looking (...) at the way in which the issue of time added to profound changes which were linked to particular visions of society and changes in the politics of gender at the turn of the century, I examine the asymmetrical relationship of sociopolitical and epistemological dimensions of time and reproduction. The paper shows how these debates were positioned within the wider context of eugenics and in relation to concepts of male reproduction that involved fundamental political, social and moral dimensions. The first world wide symposium on the topic of gender-specific medicine provided the latest research on differences in sex and/or gender in medicine and medical care. The presentations ranged beyond the topic of reproduction to encompass the entire human organism. This report critically reviews three issues that emerged during the Conference: gender mainstreaming, the concept of sex/gender differences and the issue of men's health. It suggests that the interdisciplinary concept of gender-specific medicine has to be mirrored by the integration of (...) social and cultural studies into medical research and practice. (shrink)
