BackgroundCaregivers may play a fundamental role in the clinical pathway of cancer patients. They provide emotional, informational, and functional support as well as practical assistance, and they might help mediate the interaction and communication with the oncologists when care options are discussed, or decisions are made. Little is known about the impact of dyadic dynamics on patient-doctor communication, patient's satisfaction, or adherence to the therapies. This study protocol aims to evaluate the efficacy of a psychological support intervention on patients-caregivers relationship (...) and their alignment in the treatment decision-making process and estimate related improvement in patient' compliance/adherence to treatments.MethodsA total of 102 patients-caregivers' dyads will be involved, among breast and prostate cancer patients. The study entails a pre- post- evaluation through psychological questionnaires, with a randomization of participants in two conditions, the experimental one in which subjects participate in a psychological support consultation, and the control one, where dyads do not receive any intervention. A follow up after 6 months from the enrollment is planned.DiscussionA positive impact of the psychological support intervention on patients' anxiety, depression, distress, and perceived social support is expected. Such improvements can directly affect patients' satisfaction and adherence to treatments. Data gathered from this study may inform health care providers, policy makers, and public health managers about the importance of caregiver's involvement in the cancer care pathway, and the best way to manage it. A further impact is to develop a specific intervention protocol to support caregivers' involvement in cancer care pathway, improve patient's wellbeing, the interaction with physicians and the compliance with the cancer treatment. In his play Clizia Machiavelli explores the nature of comedy, indicating the rhetorical difference between his political and his literary works. Comedy, a safe and decent medium that is at the same time subversive and appealing to the young, provides the means for instructing the future prince about the art of love. This education is necessary, according to Machiavelli, because the erotic desires and longings of the future prince guide his princely ambition while posing a danger to its successful (...) accomplishment. The more subversive and indecent aspect to this education, however, is Machiavelli's attempt to undermine the piety of the future prince by raising profound questions about love and the Providence of the God of Love in order to replace it with his own teaching on virtu and Fortuna. In his Epistle to the Pisos, Horace advises aspiring playwrights to use their work to teach and delight,1 a dictum that has resonated down through the ages and has been referred to as the "Horatian platitude."2 In the preface to his comedy Clizia, Niccolò Machiavelli echoes Horace: "Comedies were discovered in order to benefit and to delight the spectators. Truly it is a great benefit to any man, and especially to a youth, to know the avarice of an old (...) man, the passion of a lover, the tricks of a servant, the gluttony of a parasite, the misery of a pauper, the ambition of one who's rich, the flatteries of a whore, the untrustworthiness of all men."3And what of Machiavelli's Mandragola?... Machiavelli is popularly known as a teacher of tyrants, a key proponent of the unscrupulous “Machiavellian” politics laid down in his landmark political treatise The Prince. Others cite the Discourses on Livy to argue that Machiavelli is actually a passionate advocate of republican politics who saw the need for occasional harsh measures to maintain political order. Which best characterizes the teachings of the prolific Italian philosopher? With Machiavelli’s Politics, Catherine H. Zuckert turns this question on its head with a major (...) reinterpretation of Machiavelli’s prose works that reveals a surprisingly cohesive view of politics. Starting with Machiavelli’s two major political works, Zuckert persuasively shows that the moral revolution Machiavelli sets out in The Prince lays the foundation for the new form of democratic republic he proposes in the Discourses. Distrusting ambitious politicians to serve the public interest of their own accord, Machiavelli sought to persuade them in The Prince that the best way to achieve their own ambitions was to secure the desires and ambitions of their subjects and fellow citizens. In the Discourses, he then describes the types of laws and institutions that would balance the conflict between the two in a way that would secure the liberty of most, if not all. In the second half of her book, Zuckert places selected later works—La Mandragola, The Art of War, The Life of Castruccio Castracani, Clizia, and Florentine Histories—under scrutiny, showing how Machiavelli further developed certain aspects of his thought in these works. In The Art of War, for example, he explains more concretely how and to what extent the principles of organization he advanced in The Prince and the Discourses ought to be applied in modern circumstances. Because human beings act primarily on passions, Machiavelli attempts to show readers what those passions are and how they can be guided to have productive rather than destructive results. A stunning and ambitious analysis, Machiavelli’s Politics brilliantly shows how many conflicting perspectives do inform Machiavelli’s teachings, but that one needs to consider all of his works in order to understand how they cohere into a unified political view. This is a magisterial work that cannot be ignored if a comprehensive understanding of the philosopher is to be obtained. (shrink)
