BackgroundThe proper and ethical inclusion of PWLHIV and their young children in research is paramount to ensure valid evidence is generated to optimize treatment and care. Little empirical data exists to inform ethical considerations deemed most critical to these populations. Our study aimed to systematically review the empiric literature regarding ethical considerations for research participation of PWLHIV and their young children.MethodsWe conducted this systematic review in partnership with a medical librarian. A search strategy was designed and performed within the following (...) electronic databases: Ovid MEDLINE, Embase and CINAHL. We screened titles and abstracts using the following inclusion criteria: (1) a study population of PWLHIV or children under 5 years of age; and (2) collection of qualitative or quantitative data regarding ethics of research participation. Excluded were reviews, commentaries, policy statements, clinical care-related ethics concerns, abstracts, case studies, or studies unrelated to HIV research. Studies were appraised for quality, data were extracted, and studies were qualitatively analyzed using a principle-based ethical framework within the Belmont Report.ResultsOf the 7470 titles identified, 538 full-text articles were reviewed for eligibility and only three articles met full criteria for inclusion within this review. While we allowed for inclusion of studies involving young children born to mothers with HIV, only articles focused on PWLHIV were identified. Within the results of these studies, four themes emerged: (1) adequacy of informed consent; (2) consideration of paternal involvement; (3) balancing risks; and (4) access to research and treatment. A strength of this review is that it included perspectives of international research investigators, community leaders, and male partners. However, only two studies collected empiric data from PWLHIV regarding their experiences participating in researchConclusionResearchers and funding agencies should be aware of these considerations and appreciate the value of and critical need for formative research to ensure clinical trials involving PWLHIV promote ethical, well-informed research participation and, ultimately, improve care outcomes. More research is needed to create a comprehensive ethical framework for researchers when conducting studies with PWLHIV. (shrink)

Objective Adolescents have had very limited access to research on biomedical prevention interventions despite high rates of HIV acquisition. One concern is that adolescents are a vulnerable population, and trials carry a possibility of harm, requiring investigators to take additional precautions. Of particular concern is preventive misconception, or the overestimation of personal protection that is afforded by enrolment in a prevention intervention trial. Methods As part of a larger study of preventive misconception in adolescent HIV vaccine trials, we interviewed 33 (...) male and female 16–19-year-olds who have sex with men. Participants underwent a simulated HIV vaccine trial consent process, and then completed a semistructured interview about their understanding and opinions related to enrolment in a HIV vaccine trial. A grounded theory analysis looked for shared concepts, and focused on the content and process of adolescent participants’ understanding of HIV vaccination and the components of preventive misconception, including experiment, placebo and randomisation. Results Across interviews, adolescents demonstrated active processing of information, in which they questioned the interviewer, verbally worked out their answers based upon information provided, and corrected themselves. We observed a wide variety of understanding of research concepts. While most understood experiment and placebo, fewer understood randomisation. All understood the need for safer sex even if they did not understand the more basic concepts. Conclusions Education about basic concepts related to clinical trials, time to absorb materials and assessment of understanding may be necessary in future biomedical prevention trials. (shrink)

BackgroundStreet-connected children and youth in low- and middle-income countries have multiple vulnerabilities in relation to participation in research. These require additional considerations that are responsive to their needs and the social, cultural, and economic context, while upholding core ethical principles of respect for persons, beneficence, and justice. The objective of this paper is to describe processes and outcomes of adapting ethical guidelines for SCCY’s specific vulnerabilities in LMIC.MethodsAs part of three interrelated research projects in western Kenya, we created procedures to (...) address SCCY’s vulnerabilities related to research participation within the local context. These consisted of identifying ethical considerations and solutions in relation to community engagement, equitable recruitment, informed consent, vulnerability to coercion, and responsibility to report.ResultsSubstantial community engagement provided input on SCCY’s participation in research, recruitment, and consent processes. We designed an assent process to support SCCY to make an informed decision regarding their participation in the research that respected their autonomy and their right to dissent, while safeguarding them in situations where their capacity to make an informed decision was diminished. To address issues related to coercion and access to care, we worked to reduce the unequal power dynamic through street outreach, and provided access to care regardless of research participation.ConclusionsAlthough a vulnerable population, the specific vulnerabilities of SCCY can to some extent be managed using innovative procedures. Engaging SCCY in ethical research is a matter of justice and will assist in reducing inequities and advancing their health and human dignity. (shrink)

While talking about comparative philosophy, we should presumably know what philosophy is. In this paper, the concepts of interpenetration, intensity, and juxtaposition are proposed as philosophical tools to analyse phenomena, including philosophy itself. The axis of interpenetration-juxtaposition is discussed in three domains: embodiment, speaking, and thinking. The role of philosophy is seen, on different levels of interpenetration, as redistribution of concepts, resonance of intensities, and thawing of actualities. In our essay we trace the evolution of the critical theory of religion, or comparative dialectical religiology, out of the critical theory of society of the Institute for Social Research or the Frankfurt School. For us, the history of religions reflects the history of humanity’s intellectual and spiritual evolution. As we developed our critical theory of religion, we have tried to supersede concretely the great accomplishments of three generations of critical theorists, particularly in the field of religion. One main theme (...) of our critical theory of religion is the dialectic of two modes of existence: having and being; egoism and altruism. As we determinately negated the critical theory of society into our comparative dialectical religiology, we also penetrated deeper into its roots in the works of Kant and Hegel, Marx and Freud, Judaism and Christianity, particularly into the mystical theology of the great dialectician Meister Eckhart and of his many modern followers. The ribosomal polypeptide tunnel exit is the site where a variety of factors interact with newly synthesized proteins to guide them through the early steps of their biogenesis. In mitochondrial ribosomes, this site has been considerably modified in the course of evolution. In contrast to all other translation systems, mitochondrial ribosomes are responsible for the synthesis of only a few hydrophobic membrane proteins that are essential subunits of the mitochondrial respiratory chain. Membrane insertion of these proteins occurs co‐translationally and is (...) connected to a sophisticated assembly process that not only includes the assembly of the different subunits but also the acquisition of redox co‐factors. Here, we describe how mitochondrial translation is organized in the context of respiratory chain assembly and speculate how alteration of the ribosomal tunnel exit might allow the establishment of a subset of specialized ribosomes that individually organize the early steps in the biogenesis of distinct mitochondrially‐encoded proteins. This book features writing by 17 authors from Germany and from African and Latin American countries on highly diverse aesthetic phenomena as seen from their own different points of view. The texts in this volume all deal with the imperative of ‘decolonization’: they try to highlight aesthetic strategies for the (re)discovery of unthematized, misappropriated, transcultural and even transcontinental histories and memories and aesthetic practices that are absent from or too little perceived within national consciousnesses. Novels, poems and musical performances from (...) the East African region are analysed as intertwined histories of the Indian Ocean and its different languages. Artworks of the Black Atlantic and perceptions of Africa are discussed from, for example, Brazilian perspectives. Within the German context, decolonisation strategies in exhibition practices in ethnological or art museums developed by Nigerian artists are evaluated; new terms such as ‘dividuation’ are proposed to describe these contemporary composite-cultural entanglements, and so on. I discuss Extended Cognition theory in relation to Confucius’s Analects, the Mozi, and the Zhuangzi. Extendedness is treated as part of an approach that sees cognition as embodied, embedded, enacted, extended and affective. A common feature of extended cognition views is their resistance to the Cartesian split between extension and thinking and as squeezed between two other capacities that involve extension, namely perception and action. This creates the “classical sandwich”: extensive input —unextended symbolic processing —extensive output. In the Chinese tradition, (...) an embodied understanding of knowledge is dominant. Yet, we can discern two directions for its development by thinkers, either towards contextualization and engagement, or towards decontextualization and disengagement. I shall illustrate the two positions using examples from the Analects and the Mozi. They imply two different directions in furthering or extending knowledge, towards intensity and towards extensity. In the final part, the Zhuangzi is presented as an excellent example of an embodied and extended view of cognition, but which at the same time also takes the decontextualising direction to the utmost. To highlight the interdependencies of persons, cultures, social, ecological, and artistic entities as a precondition for a planetary thinking or a world philosophy, this essay offers a short reconstruction of the coinage and transfer of the term “culture” in the European-African-Antillean context. It underlines that a world philosophy can no longer be executed on ideas of individual entities and corresponding opposites such as “European vs. African” and so forth. The author cites cultural understandings of different authors of the Global South (...) as examples of affirmed cultural mixtures and of their mutual participations to bring about a philosophy of relation and dis-individuation. The argument is this: the world of today needs new terms to be conceived adequately in its cultural, social, eco­logical, and artistic interdependencies. The old term, “the individual,” must be replaced by the new term, “dividual” or “dividuation,” thereby underlining the processuality and intermixing of all sorts of entities, helping to move toward a decolonized philosophy of the world. The publication aims to make suggestions for a 'decolonisation of aesthetics' within an Afro-European framework. The texts (whose authors come from different cultural contexts between Germany, France, Senegal, Benin, Nigeria and Tunesia) do not only refer to heterogenous aesthetic practices understood as subversive and decolonial strategies, but also discuss philosophical questions of a renewed (non-in)dividual humanism. This book analyses some of the seminal texts of the Chinese tradition and shows how they exemplify aspects of embodiment theory: the Analects of Confucius, the Zhuangzi, and the Treatise on Music. Margus Ott also develops far-reaching possibilities of an embodied philosophy. The embodied understanding did not go unchallenged in Ancient China. There were important counter-currents, most notably the Mohists and the so-called Legalists. By using embodiment theory Ott demonstrates how these ideas can be seen as a decontextualizing tendency of (...) thought that plays an important role in human affairs. (shrink)

Käesoleva artikli eesmärgiks on välja töötada olenditeooria mõningad põhimõisted. Olendit määratletakse teatavate eristumisprotsesside lõimimisena ehk "läbivaatusena". Nendel protsessidel on kaks külge, "esi-" ja "isekülg". Esikülg on kõrvutuvuslik, st. paljusus, mille elemendid on üksteise kõrval; isekülg on läbistuvuslik, st. paljusus, mille elemendid läbistavad üksteist. Need kaks külge ei sarnane teineteisega ja nende vahel liikumine kätkeb taandamatut ajalisust, ontogeneesi. Artiklis laiendatakse olendi mõistet "vertikaalis" erinevatele keerukustasanditele (eluline, molekuliline, atomaarne jt) ja "horisontaalis" erinevatele individualiseeritusastmetele. Isekülge käsitletakse liigendatuna piki afekteerimis- ja afekteeritavusviise, mis tähistavad (...) seda, mida olend suudab tajuda ja teostada, ehk seda, milleks ta on võimeline. Lõpuks käsitletakse esi- ja isekülge olemise kui protsessi kahe aspektina, esinemise ja isenemisena. Esinemise ja isenemise ringluste arvukus ja tempo variatiivsus määravad olendi väehulga. A Theory of Beings The paper proposes to work out concepts for describing the individualization of beings on various levels of organization. A being with a certain degree of individuality comes into being when certain processes of differenciation are being integrated together. It is conceived of as having two aspects, self-relation (isekülg) and other-relation (esikülg). The former is a multiplicity where the elements interpenetrate each other and the latter is a multiplicity where the elements are exterior to each other (juxtaposition). The articulations of interpenetrating multiplicity determine how a being can affect its surroundings and be affected by it, and the modes of this articulation can be seen as attractors of the "phase-space" of a being. Ontogenesis is on the one hand the actualization of this virtual phase-space in an actual form with its characteristic ways of spatialization and temporalization. On the other hand, ontogenesis means the revirtualization of juxtaposing elements and processes. The number and intensity of these circuits determines the degree of power of a being. The paper proposes to extend all these notions also to pre-biotic systems.  . (shrink)

In my paper I tackle the question of whether the term “philosophy” should be restricted to a specific tradition or whether it could be used also for other traditions. I argue in favor of the latter solution from extrinsic and intrinsic viewpoints. From extrinsic viewpoint, more liberal use of the term could facilitate exchanges between scholars of different traditions of thought. From intrinsic viewpoint, I propose an understanding of philosophy as creation of concepts and concepts as contractions and embodiment of (...) reality, not as abstractions or representations of it. Throughout his mature writings, Berkeley speaks of minds as substances that underlie or support ideas. After initially flirting with a Humean account, according to which minds are nothing but ‘congeries of Perceptions’, Berkeley went on to claim that a mind is a ‘perceiving, active being... entirely distinct’ from its ideas. Despite his immaterialism, Berkeley retains the traditional category of substance and gives it pride of place in his ontology. Ideas, by contrast, are ‘fleeting and dependent beings’ that must be supported (...) by a mental substance. There is no doubt that Berkeley’s conception of the relationship between minds and ideas is non-traditional, but that fact does not undercut his commitment to the traditional conception of substance. A robust literature has grown up around the thorny issue of reconciling Berkeley’s endorsement of mental substance with his rejection of its material counterpart. (shrink)
