Interoception can be broadly defined as the sense of signals originating within the body. As such, interoception is critical for our sense of embodiment, motivation, and well-being. And yet, despite its importance, interoception remains poorly understood within modern science. This paper reviews interdisciplinary perspectives on interoception, with the goal of presenting a unified perspective from diverse fields such as neuroscience, clinical practice, and contemplative studies. It is hoped that this integrative effort will advance our understanding of how interoception determines well-being, (...) and identify the central challenges to such understanding. To this end, we introduce an expanded taxonomy of interoceptive processes, arguing that many of these processes can be understood through an emerging predictive coding model for mind–body integration. The model, which describes the tension between expected and felt body sensation, parallels contemplative theories, and implicates interoception in a variety of affective and psychosomatic disorders. We conclude that maladaptive construal of bodily sensations may lie at the heart of many contemporary maladies, and that contemplative practices may attenuate these interpretative biases, restoring a person’s sense of presence and agency in the world. In a recent paper, Richard Rorty begins by telling us why pragmatists such as himself are inclined to identify truth with justification: ‘Pragmatists think that if something makes no difference to practice, it should make no difference to philosophy. This conviction makes them suspicious of the distinction between justification and truth, for that distinction makes no difference to my decisions about what to do.’ (1995, p. 19) Rorty goes on to discuss the claim, defended by Crispin Wright, that truth is (...) a normative constraint on assertion. He argues that this claim runs foul of this principle of no difference without a practical difference: ‘The need to justify our beliefs to ourselves and our fellow agents subjects us to norms, and obedience to these norms produces a behavioural pattern that we must detect in others before confidently attributing beliefs to them. But there seems to be no occasion to look for obedience to an additional norm – the commandment to seek the truth. For – to return to the pragmatist doubt with which I began – obedience to that commandment will produce no behaviour not produced by the need to offer justification.’ (1995, p. 26) Again, then, Rorty appeals to the claim that a commitment to a norm of truth rather than a norm of justification makes no behavioural difference. This is an empirical claim, testable in principle by comparing the behaviour of a community of realists (in Rorty’s sense) to that of a community of pragmatists. In my view, the experiment would show that the claim is unjustified, indeed false. The comorbidity of chronic pain and opioid addiction is a serious problem that has been growing with the practice of prescribing opioids for chronic pain. Neuroimaging research has shown that chronic pain and opioid dependence both affect brain structure and function, but this is the first study to evaluate the neurophysiological alterations in patients with comorbid chronic pain and addiction. Eighteen participants with chronic low back pain and opioid addiction were compared with eighteen age- and sex-matched healthy individuals in a (...) pain-induction fMRI task. Unified structural equation modeling (SEM) with Lagrange multiplier (LM) testing yielded a network model of pain processing for patient and control groups based on 19 a priori defined regions. Tests of differences between groups on specific regression parameters were determined on a path-by-path basis using z-tests corrected for the number of comparisons. Patients with the chronic pain and addiction comorbidity had increased connection strengths; many of these connections were interhemispheric and spanned regions involved in sensory, affective, and cognitive processes. The affected regions included those that are commonly altered in chronic pain or addiction alone, indicating that this comorbidity manifests with neurological symptoms of both disorders. Understanding the neural mechanisms involved in the comorbidity is crucial to finding a comprehensive treatment, rather than treating the symptoms individually. (shrink)

Introduction: Schools have become a primary setting for providing mental health care to youths in the U.S. School-based interventions have proliferated, but their effects on mental health and academic outcomes remain understudied. In this study we will implement and evaluate the effects of a flexible multidiagnostic treatment called Modular Approach to Therapy for Children with Anxiety, Depression, Trauma, or Conduct Problems on students' mental health and academic outcomes.Methods and Analysis: This is an assessor-blind randomized controlled effectiveness trial conducted across five (...) school districts. School clinicians are randomized to either MATCH or usual care treatment conditions. The target sample includes 168 youths referred for mental health services and presenting with elevated symptoms of anxiety, depression, trauma, and/or conduct problems. Clinicians randomly assigned to MATCH or UC treat the youths who are assigned to them through normal school referral procedures. The project will evaluate the effectiveness of MATCH compared to UC on youths' mental health and school related outcomes and assess whether changes in school outcomes are mediated by changes in youth mental health.Ethics and Dissemination: This study was approved by the Harvard University Institutional Review Board. We must begin by asking; What exactly is common sense? No doubt the word was originally used as a translation of Aristotle's; κοί⋯νη αἴσθησις but that is not its modern meaning. When Reid or more recent philosophers speak of common sense, they clearly have something else in view. At the present day, it is perhaps most often used to mean a quality of a mind, as when we say that jurymen or Members of Parliament should be men of common sense, (...) meaning that they should show intelligence in the ordinary affairs of life; or again, we say that a little common sense would enable us to solve this or that political problem. But we are not concerned with that meaning either, though it would be interesting to discuss it. Common sense, as we are concerned with it here, means rather a body of very general principles commonly accepted by ordinary non-philosophical men in the ordinary affairs of life. These principles are really philosophical—that is to say, they belong to the proper subject-matter of philosophy—but, of course, the plain man who accepts or applies them at every moment of his life is far from being aware of this. Some of them are metaphysical, or epistemological, others are ethical; and whether or not there can be a common-sense theology, as a recent writer has asserted, it seems quite possible that there may be one or two among them which properly belong to the theologian's province. All these principles, taken together, make up what is usually called the common-sense view of the world. But in this discussion we shall confine ourselves to the metaphysical or epistemological ones, which are more frequently appealed to than the others, and which, besides, seem to be more interesting. But our conclusions, if valid, will apply to the others also. (shrink)

Thus we cannot simply dismiss the principles of common sense as invalid or fictitious. What attitude, then, ought we to adopt towards them? There are two obvious suggestions.

This article points to the highly centralized nature of the British tax system. A first section shows how all tax law derives from Parliament, the “onlie begetter” of legally enforceable instruments. It is suggested that this system is not democratically accountable at sub-national levels of government. Reforms of the Thatcher era have resulted in the privatization of many public services, leading to the stabilization of State expenditure as a proportion of GDP. However, at the same time, both tax revenue and (...) expenditure have become increasingly centralized. Public service tasks are determined and financed by central government, and local government is for the most part merely the agent through which government policy is realized. The creation of the Scottish and Welsh Assemblies does not alter this overall assessment. Given the high level of centralization, taxes are uniform throughout the country and there is no scope for meaningful regional fiscal competition. Since regional economic development in Britain is notoriously uneven, the author notes that fiscal uniformity does not lead to economic convergence. A final section is devoted to showing why this is not surprising and why fiscal harmonisation at a European level would be a source of growing, not reduced, disparities.Cet article met l’accent sur la nature hautement centralisée du système fiscal britannique. Les taches du service public sont définies et financées par le gouvernement central et le gouvernement local est dans la majeure partie des cas l’agent à travers lequel la politique du gouvernement s’exerce. La creation des Assemblées Ecossaises et galloises ne modifie pas cette evaluation d’ensemble. Etant donné le niveau élevé de centralisation, les impôts sont uniformes à travers le pays et il n’y a pas de possibilité pour une concurrence fiscale régionale significative. Alors que le développement économique regional en Grande Bretagne est notoirement inégal, l’auteur remarque que l’uniformité fiscale ne conduit pas à uneconvergence économique.
