Background Despite its negative impact on patients and nurses, the use of restraint in somatic health care continues in many settings. Understanding the reasons and justifications for the use of restraint among nurses is crucial in order to manage this challenge. Aim To understand nurses’ justifications for restraint use in neurosurgical care. Research design A qualitative, descriptive design was used. Data were analysed with inductive qualitative content analysis. Participants and research context Semi-structured interviews with 15 nurses working in three neurosurgical (...) departments in Sweden. Ethical considerations Approved by The Regional Ethics Committee, Stockholm, Sweden. Findings The analysis resulted in three categories. The category Patient factors influencing restraint use describes patient factors that trigger restraint, such as a diminished decision-making competence, restlessness, and need for invasive devices. The category Specific reasons for justifying restraint describes reasons for restraining patients, such as restraint being used for the sake of the patient or for the sake of others. The category General reasoning in justifying restraint describes how nurses reason when using restraint, and the decision to use restraint was often based on a consequentialist approach where the nurses’ weighed the pros and cons of different alternatives. Discussion Nurses with experience of restraint use were engaged in a constant process of justifying and balancing different options and actions. Restraint was considered legitimate if the benefit exceeded the suffering, but decisions on which restraint measures to use and when to use them depended on the values of the individual nurse. Conclusion How nurses reason when justifying restraint, why they use restraint, and who they use restraint on must be considered when creating programs and guidelines to reduce the use of restraint and to ensure that when it is used it is used carefully, appropriately, and with respect. The present volume explores how and why Averroes, a Muslim philosopher and jurist, became one of the most important figures in the history of Jewish philosophy.

The aim of this article is (1) to investigate the ‘neurosciences’ as an object of study for historical and genealogical approaches and (2) to characterize what we identify as a particular ‘style of thought’ that consolidated with the birth of this new thought community and that we term the ‘neuromolecular gaze’. This article argues that while there is a long history of research on the brain, the neurosciences formed in the 1960s, in a socio-historical context characterized by political change, faith (...) in scientific and technological progress, and the rise of a molecular gaze in the life sciences. They flourished in part because these epistemological and technological developments were accompanied by multiple projects of institution-building. An array of stakeholders was mobilized around the belief that breakthroughs in understanding the brain were not only crucial, they were possible by means of collaborative efforts, cross-disciplinary approaches and the use of a predominantly reductionist neuromolecular method. The first part of the article considers some of the different approaches that have been adopted to writing the history of the brain sciences. After a brief outline of our own approach, the second part of the article uses this in a preliminary exploration of the birth of the neurosciences in three contexts. We conclude by arguing that the 1960s constitute an important ‘break’ in the long path of the history of the brain sciences that needs further analysis. We believe this epistemological shift we term the ‘neuromolecular gaze’ will shape the future intellectual development and social role of the neurosciences. (shrink)

Faith development theory has evolved as a prominent theoretical perspective during the past three decades to explain different ways of relating to religious beliefs and worldviews. Recent revisions of the theory have elaborated on these characteristic ways as religious styles namely the fundamentalist, mutual, individuative-systemic, and dialogical. The present study developed an Urdu version of its principal measure, i.e., Faith Development Interview, to analyze twelve cases of Muslims of various religious affiliations within Islam in Pakistan. Four case studies representative of (...) each faith style are presented in detail. The cases are compared to analyze Islamic faith in terms of faith development theory and to understand fundamentalism in a Muslim context. The findings support faith development theory as a comprehensive paradigm to address the varieties of faith orientations in Islam. Implications for future research with larger samples in highly religious and collectivistic cultures are discussed. (shrink)

Restraint is regularly used in somatic healthcare settings, and countries have chosen different paths to regulate restraint in somatic healthcare. One overarching problem when regulating restraint is to ensure that patients with reduced decision-making capacity receive the care they need and at the same time ensure that patients with a sufficient degree of decision-making capacity are not forced into care that they do not want. Here, arguments of justice, trust in the healthcare system, minimising harm and respecting autonomy are contrasted (...) with different national regulations. We conclude that a regulation that incorporates an assessment of patients’ decision-making capacity and considers the patient’s best interests is preferable, in contrast to regulations based on psychiatric diagnoses or regulations where there are no legal possibilities to exercise restraint at all in somatic care. Wadud analyzes citizenship in theocratic states governed by Shari’ah, or Islamic law, states which do not traditionally envisage the equality of women. Against such theocratic systems, Wadud argues that the canonical Islamic tradition of jurisprudence is open to reinterpretation in light of changing conditions in Muslim societies. According to Wadud, the Qur’an does not restrict agency on the grounds of gender. The gender disparity that has developed in Islamic tradition and theory denies women the means of completing their duties before (...) Allah. Wadud calls upon all Muslims, female and male, to reform their societies so as to implement the equality inherent in the “tawhidic” Islamic paradigm, which stresses unity and harmony. (shrink)

This article explores the concept of internationally acceptable codes of ethics within the context of an Egyptian nurse’s PhD studies. Theoretical work, including gaining ethical approval for the project, took place in the UK, while the data collection phase of the study was done in Egypt. This highlighted areas where the Arab Muslim interpretation of some ethical principles, especially around the issue of gaining informed consent, differed from that currently accepted in British research ethics. The authors argue that it may (...) not be possible, or even desirable, to standardize codes of ethics globally in areas such as academic research. Ethical principles develop from a unique mix of culture and religion. It may be more important to develop cultural competence that includes the ability to understand and respect the way in which ethical principles are interpreted by various societies. (shrink)

The positive role of religion in reducing prejudice has remained a neglected theme in Psychology of religion, concerning itself mostly with prejudice and fundamentalism. Recently, noting the absence of a positive antithesis to prejudice and fundamentalism, faith development theory presents xenosophia as going beyond mere tolerance to a creative engagement with other religious faiths to develop new insights and broaden one’s own worldview. The current research undertakes a study of Muslim faith contents to get insights into how these beliefs shape (...) construction of self in relation to other faith communities. Conducting inductive thematic analysis of faith development interviews from 12 Muslim participants from three major religious affiliations in Pakistan in an earlier research, the research analyzes a range of xenological patterns from xenophobia to xenosophia with associated potentials for inter-faith dialogue. Focusing on residents of a country with a dense Muslim population, the study carries implications for religious socialization and religious education in a globalized world. (shrink)

The following review summarizes and examines Mark Solms's article Dreaming and REM Sleep are controlled by different brain mechanisms, which argues why the understanding of REM sleep as the physiological equivalent of dreaming needs to be re-analyzed. An analysis of Solms's article demonstrates that he makes a convincing argument against the paradigmatic activation-synthesis model proposed by Hobson and McCarley and provides provocative evidence to support his claim that REM and dreaming are dissociable states. John Locke is one of the key figures in the history of early modern philosophy. He had a great influence on the development of philosophy, which is based on the empirical method, and on the political systems of countries. And even though his works have not been deprived of attention, some questions of his philosophy remain insufficiently disclosed. The author's deals with the religious turn in studies of John Loke. Attention is paid to the role of religion in the philosophy (...) of the thinker. The issue of religious influences on the issue of identity is being discussed. The essence of the term “modern” is highlighted and the criteria according to which John Locke's ideas should be considered modern are indicated. The author notes that some researchers considered John Locke one of the founders of modern philosophy, while others believed that the role of the English philosopher in the development of modern philosophy was overestimated. A characteristic feature of modern philosophy is the emphasis on research and exploration. The author names the identity of John Locke a subjective identity. The main attributes of identity are: reflection, consciousness and memory. Self-reflection and meaningful self-perception allows each person to choose their own way of worshiping God. Consciousness allows a person to evaluate his actions, forms an idea of morality, the concept of sin and so on. The article deals with different meanings of the concept of identity in the religious and secular aspects. The difference - is in memory. Personal responsibility is an integral part of identity. The author of the article notes that we should make a difference between responsibility in religious and political field. Religious responsibility requires observance not only of legal law, but also of moral rules and biblical commandments. (shrink)

Intersectionality has become a significant intellectual approach for those thinking about the ways that race, gender, and other social identities converge in order to create unique forms of oppression. Although the initial work on intersectionality addressed the unique position of black women relative to both black men and white women, the concept has since been expanded to address a range of social identities. Here we consider how to apply some of the theoretical tools provided by intersectionality to the clinical context. (...) We begin with a brief discussion of intersectionality and how it might be useful in a clinical context. We then discuss two clinical scenarios that highlight how we think considering intersectionality could lead to more successful patient–clinician interactions. Finally, we extrapolate general strategies for applying intersectionality to the clinical context before considering objections and replies. (shrink)

Are critique and the “art of governing” antithetical? The aim of this article is to examine this tension that was laid bare by the Covid-19 pandemic by introducing “critical friendship” as a conceptual framework for a constructive interdisciplinary engagement with science in a post-pandemic era. It does so by drawing on several works and insights: (i) Michel Foucault’s notion of “critical attitude” as well as his assessment of philosophy as providing a “diagnosis of the present;” (ii) Bruno Latour and colleagues’ (...) idea of a “critical zone” or what I call a horizontal epistemology of critique; (iii) Aristotle’s notion of friendship as being necessary for the “common good;” and finally (iv) Jacques Derrida’s interpretation of the messianic character of friendship in the constitution of progressive democracies. Whereas critical theory has been described as either “explanatory-diagnostic” or “emancipatory-utopian,” a critical friendship approach aims to be both diagnostic and emancipatory in an age of uncertainty and democratic backsliding. When comparing both GDP loss and mortality across countries, it appears that countries that have managed to save more lives during the Covid-19 pandemic have also managed to save their economies better. What accounts for these stark differences in country performances? In this article, we argue that a salient feature of economic and health performance is the degree of trust populations have in their governments. We set up a heuristic analytical framework that models this relation, under particular assumptions about what (...) drives government and individual behavior, in order to better understand the mechanisms that may be at work. We identify three key roles that trust in government may play in enforcing social distancing policies, conveying credible information for individual decision-making, and shaping government attitudes towards risk. We argue that these implications are consistent with the empirical evidence. We also discuss the relevance of other forms of trust, namely, interpersonal trust and trust in science. (shrink)

In Pakistan, as in many other societies, politico-religious movements or so-called Islamist fundamentalist movements are becoming an important site for women's activism as well as the harnessing of such activism to promote agendas that seem to undermine women's autonomy. This has become a concern for a growing feminist literature which from a variety of political and theoretical positions seeks to understand and explain the subject-position of Muslim women as politico-religious activists. This paper attempts a deconstructive reading of texts by leading (...) Pakistani feminist scholars as they attempt the difficult process of steering between fundamentalism and Orientalism in their accounts of ‘fundamentalist’ women in the political ideological space of Pakistan. Feminist scholarship on women in religious and right-wing social and political movements has moved from a reductive focus on causal or motivational factors to more sophisticated analyses explicating processes of agency and subject formation. With the aim of expanding and deepening this conceptual space, I will discuss some of my interactions with a group of women in the Jamaat-e-Islami in Pakistan, as we attempted to explore the complex meanings of ‘the modern’ that informed the self-understanding of my interviewees. My work (...) corroborates some of the contemporary scholarship on what is referred to as Political Islam in arguing that Islamist movements in Muslim societies are also the catalysts of modernization, rather than simply its interlocutors. This article argues that these processes of social and political organizing entail particular interrogations and the reconstituting of identities in ways that blur the line between ‘the religious’ and ‘the secular’. On the one hand, we need to understand Jamaat women's self-construction as religious or pious women; on the other hand, we must grasp the specificity of their claims to act as modern subjects situated in the time of political and cultural modernity. Whether hailed for transitioning to the ballot box, or condemned for failing to hold elections, Africa’s postcolonial states exhibit profound contradictions in the arena of gender politics. Where reforms have been achieved, implementation remains minimal, as undemocratic state structures and uncivil societies alike lack the political will to change. This article addresses the emergence of feminism as an intellectual and political force for freedom that radically challenges the ongoing exploitation and oppression of women in Africa. It focuses on the contribution (...) of radical intellectuals to the theory and practice of women’s movements, arguing that the research, analysis, and activism they carry out defines them as a radical public intellectual cadre that continuously mobilizes with, by, and for women to pursue liberation for themselves as much as others. This study includes an analytical study of the term "Qiyās al-Adna” among the Scholars of Uṣūl al-Fiqh. It is known that the Qiyās is divided into the following types: an analogy of higher order (Qiyās al-Awla), an analogy of an equivalent order (Qiyās al-Musawi), an analogy of a lower order (Qiyās al-Adna). However, Qiyās al-Adna raised issues and objections among some scholars, in past and present; hence, there is an urgent need to present a comprehensive study that shows the meaning (...) of Qiyās al-Adna for those who advocate it, discusses the objections and issues that have been raised about it, and clarifies its connection to other Qiyās types. Moreover, the study will concern the applied side of Qiyās al-Adna by explaining how jurists used it. The researcher used the analytical method and referred to the most important, ancient and modern Uṣūl al-Fiqh sources and references. The study concluded with results, the most prominent of which is that the Qiyās al-Adna has two meaning: the first is that in which the original case is more deserving of legal ruling than the new or parallel case, so the new case is considered as a lower order, and the second is that the effective cause in the new case is weaker than in the original case. Among the results of the study is that the objections directed to the Qiyās al-Adna are weak, and that it is not limited to the presumptive Qiyās on the most correct opinion, also, that the jurists used the term Qiyās al-Adna in the second meaning, which says that the lower order is in its effective cause, not the legal ruling. We have controversial intuitions about the rightness of retributive punishment, keeping promises for its own sake, and pushing the heavy man off of the bridge in the footbridge trolley case. How do these intuitions relate to ethical theories? Should ethical theories aim to fit with and explain them? Or are only uncontroversial intuitions relevant to explanatory ethical theorising? I argue against several views that we might hold about the relationship between controversial intuitions and ethical theories. I then propose and defend (...) the view that ethical theories should only aim to fit with and explain the intuitions that almost all people (who minimally understand the relevant issues) share. I argue that this view has interesting and important implications for ethical theorising and theorising about justice and equality. (shrink)

BackgroundPatient advocacy organizations (PAOs) have an increasing influence on health policy and biomedical research, therefore, questions about the specific character of their responsibility arise: Can PAOs bear moral responsibility and, if so, to whom are they responsible, for what and on which normative basis? Although the concept of responsibility in healthcare is strongly discussed, PAOs particularly have rarely been systematically analyzed as morally responsible agents. The aim of the current paper is to analyze the character of PAOs’ responsibility to provide (...) guidance to themselves and to other stakeholders in healthcare.MethodsResponsibility is presented as a concept with four reference points: (1) The subject, (2) the object, (3) the addressee and (4) the underlying normative standard. This four-point relationship is applied to PAOs and the dimensions of collectivity and prospectivity are analyzed in each reference point.ResultsUnderstood as collectives, PAOs are, in principle, capable of intentionality and able to act and, thus, fulfill one prerequisite for the attribution of moral responsibility. Given their common mission to represent those affected, PAOs can be seen as responsible for patients’ representation and advocacy, primarily towards a certain group but secondarily in a broader social context. Various legal and political statements and the bioethical principles of justice, beneficence and empowerment can be used as a normative basis for attributing responsibility to PAOs.ConclusionsThe understanding of responsibility as a four-point relation incorporating collective and forward-looking dimensions helps one to understand the PAOs’ roles and responsibilities better. The analysis, thus, provides a basis for the debate about PAOs’ contribution and cooperation in the healthcare sector. (shrink)

The rising and acceleration of the Shia armed group in Iraq, Syria, Yemen, and Lebanon require a deep understanding of the root of the multi-dimensional conflicts in the Middle East. An appropriate and sufficient approach to the research about these militias will be from an internal conflict rather than an external conflict. The legitimization for the existence of the majority of these militias if not all of them is to fight and struggle against an entity which is the Sunni sect, (...) in this case, that will assimilate them not integrate them peacefully. The study of the chaotic dynamics in fractional-order discrete-time systems has received great attention over the last years. Some efforts have been also devoted to analyze fractional maps with special features. This paper makes a contribution to the topic by introducing a new fractional map that is characterized by both particular dynamic behaviors and specific properties related to the system equilibria. In particular, the conceived one dimensional map is algebraically simpler than all the proposed fractional maps in the literature. Using (...) numerical simulation, we investigate the dynamic and complexity of the fractional map. The results indicate that the new one-dimensional fractional map displays various types of coexisting attractors. The approximate entropy is used to observe the changes in the sequence sequence complexity when the fractional order and system parameter. Finally, the fractional map is applied to the problem of encrypting electrophysiological signals. For the encryption process, random numbers were generated using the values of the fractional map. Some statistical tests are given to show the performance of the encryption. (shrink)

Deborah is a thirty-three-year-old who presented to labor and delivery at thirty-seven weeks gestation with complaints of contractions. Upon arrival, she explained that her fetus, Nathan, had been diagnosed with a “lethal” condition by her primary obstetrician. At twenty-two weeks gestation, an amniocentesis confirmed trisomy 13, a chromosomal abnormality leading to miscarriage or stillbirth in nearly one-half of affected pregnancies. During the admission process, Deborah voices the worry that due to Nathan's brain and heart structure, vaginal delivery could be traumatic (...) and cause him to suffer. Deborah wishes for him to have as painless and as dignified a death as possible; cesarean section, she feels, will achieve this. Yet with her history of three prior vaginal deliveries, normally progressing labor, and poor fetal prognosis that is unlikely to improve with cesarean delivery, there is no maternal or fetal indication for a cesarean section. Should the obstetrician proceed with a cesarean delivery despite knowing that it would expose the mother to surgical risks with little or no corresponding fetal or neonatal benefit? (shrink)
