Antimicrobial resistance (AMR) poses an increasing threat to patient care and population health and there is a growing need for novel therapies to tackle AMR. Bacteriophage (phage) therapy is a re-emerging antimicrobial strategy with the potential to transform how bacterial infections are treated in patients and populations. Currently, in the UK, phages can be used as unlicensed medicinal products on a ‘named-patient’ basis. We make an ethical case for why it is crucially important for the UK to invest in Good (...) Manufacturing Practice (GMP) for both ongoing unlicensed and future licensed phage therapy. Access to phages produced to GMP (GMP phages) will ensure effective patient care and better outcomes as well as health systems benefits. The UK also has the potential to become a global leader in the timely and cost-efficient manufacturing and supply of a therapy that meets internationally recognised standards. (shrink)

_This paper discusses the concept of compassion fatigue in light of the importance that political decisions, especially the application of the concept of public spirit, have had on care and nursing in a Danish hospital context during the COVID-19 pandemic. The paper is based on recent research literature in the field as well as the author’s own participatory observation study. The paper suggests that nurses already show a sense of public spirit due to their authorization and professional ethics, but at (...) the same time they must balance the ambiguity of nursing care. A linguistic-philosophical study of public spirit shows that the concept can have a discursive, double-binding and interpellative effect on nurses, who may therefore have an experience of inadequacy and compassion fatigue. In this context, compassion fatigue must be understood as the fact that nurses cannot provide the care they want or that is expected of them. Public spirit can be said to have had a renaissance and linguistic and moral supremacy during the COVID-19 pandemic. The paper views the concept of __waywardness 1 as a possible response for how to prevent or completely avoid compassion fatigue, so that nurses instead have an experience of compassion / self-compassion. Georg Lukács (1885-1971) was one of the most original Marxist philosophers and literary critics of the twentieth century. His work was a major influence on what we now know as critical theory. The present article offers three critiques of the universal basic income guarantee (BIG) proposal discussed by Standing in this volume. First, there is a fundamental tension between the way income in a monetary production economy is generated, the manner in which BIG wishes to redistribute it, and the subsequent negative impact of this redistribution on the process of income generation itself. The BIG policy is dependent for its existence on the very system it wishes to undermine. Second, the macroeconomic effects (...) of BIG on contemporary economies that use modern money are destabilizing. The job guarantee (JG), by contrast, stabilizes both the macro-economy and the currency while helping transform the nature of work itself. Finally, the employment safety-net in Standing’s piece is not an accurate representation of the modern JG proposals – a confusion which this paper aims to remedy. (shrink)

This book is the authorized political biography of Kenneth Waltz. The theoretical work of Kenneth Neal Waltz (June 8, 1924 - May 12, 2013) during the last half of the twentieth century in international relations theory is known by virtually everyone in this scholarly field. Through a series of interviews with Waltz about his life-starting with his German great-grandparents' immigration to the US in the 19th century-this biography produces personal insights into the development of his theoretical and policy-based contributions. We follow the inner movement of his theory construction, its meaning and coherence, as well as his conceptual logic and personal directions concerning his evolving views of the reciprocal interactions between mind and body, the motivational force of instinctual drives, and the dominant role of sexuality rooted in evolutionary biology in human development, behaviour, and the creation of neurotic disturbances. Cristina Lafont's recent reading of Heidegger offers a powerful formulation of the widespread view that once one recognizes our `facticity' and the role of language in shaping it, there is no room left to talk about transcendental structures of meaning or experience. In this article I challenge this view. I argue that Lafont inaccurately conflates what Heidegger calls our `understanding of being' with that which language discloses. In order to show that the philosophical motivation for this conflation is unsound, I (...) also argue that Lafont's own positive theory of meaning itself tacitly assumes a distinction between factical and transcendental, and so rests on exactly what she finds problematic in Heidegger. This still leaves a puzzle as to how factical individuals are actually able to grasp anything transcendental, so I conclude by sketching Heidegger's method of `formal indication', which is meant to show precisely how this can be done. Two of the most important topics in the field of medical ethics today are insemination by donor (AID) and in-vitro fertilization. The conclusions of a working party set up by the British Association for the Advancement of Science are embodied in a small book reviewed on pate 108 of this issue, but we feel that more discussion than can be set out in the space allocated to book reviews is justified. Also, this journal in its first number has already devoted (...) a group of papers to the issue of abortion, so the conclusions of the working party and our own comments overlap and we hope illuminate further the problems enunciated. The insurance hypothesis is a reasonable explanation for the current obesity epidemic. One alternative explanation is that the marketing of high-sugar foods, especially sugar-sweetened beverages, drives the rise in obesity. Another prominent hypothesis is that obesity spreads through social influence. We offer a framework for estimating the extent to which these different models explain the rise in obesity.

Annihilationism provides a fruitful point of contact between philosophers and theologians for further reflection on nonexistence. In this paper I articulate a key commitment of annihilationism; namely, that some persons cease to exist. Such a commitment, I argue, amounts to the claim that some persons exist at time t and then do not exist at t+1, become ‘annihilated objects.’ Claims about annihilated objects induct the annihilationist into a wider realism/anti-realism debate about nonexistent objects. I survey some major viewpoints in this (...) debate. I then draw out some implications for each view for the annihilationist’s commitment to annihilated objects. I show that annihilationism is consistent with some forms of realism and anti-realism and inconsistent with others. (shrink)

John Haugeland aimed throughout his career to determine what it is for an entity to count as having intelligence or thought, and at each stage he developed the idea from the phenomenological tradition that genuine thought requires intentionality. His most mature essay to do this, “Authentic Intentionality,” shows how the intentional directedness of thought requires that thinkers understand themselves as responsive to entities they think about, that they be committed to maintaining the socially shared forms of understanding of those entities, (...) and yet that they be self-critically open to the possibility of needing to revise or reject those forms of understanding. In this essay I argue that, while Haugeland’s account of intentionality sheds much light on empirical thought (thought directed at different kinds of things in the world), it doesn’t address what it takes for thought to intend or think its own form—and so it fails to describe the kind of transcendental thought of which the account itself is an instance. Building on Haugeland’s own rich picture of self-understanding, I show how we can remedy this omission, and that when we do, we see that transcendental thought is performed by each of us as concrete individuals and yet takes place from a perspective outside of, and thus free from, the normative demands and existential situations of our empirical lives. It is thus of at most therapeutic use in them, even as it is valuable for its own sake as an exercise of our finite freedom. (shrink)

Local food systems can have economic and social benefits by providing income for producers and improving community connections. Ongoing global climate change and the acute COVID-19 pandemic crisis have shown the importance of building equity and resilience in local food systems. We interviewed ten stakeholders from organizations and institutions in a U.S. midwestern city exploring views on past, current, and future conditions to address the following two objectives: 1) Assess how local food system equity and resilience were impacted by the (...) COVID-19 pandemic, and 2) Examine how policy and behavior changes could support greater equity and resilience within urban local food systems. We used the Community Capitals Framework to organize interviewees’ responses for qualitative analyses of equity and resilience. Four types of community capital were emphasized by stakeholders: cultural and social, natural, and political capital. Participants stated that the local food system in this city is small; more weaknesses in food access, land access, and governance were described than were strengths in both pre- and post-pandemic conditions. Stakeholder responses also reflected lack of equity and resilience in the local food system, which was most pronounced for cultural and social, natural and political capitals. However, local producers’ resilience during the pandemic, which we categorized as human capital, was a notable strength. An improved future food system could incorporate changes in infrastructure (e.g., food processing), markets (e.g., values-based markets) and cultural values (e.g., valuing local food through connections between local producers and consumers). These insights could inform policy and enhance community initiatives and behavior changes to build more equitable and resilient local food systems in urban areas throughout the U.S. Midwest. This reply to an ongoing debate between conspiracy theory researchers from different disciplines exposes the conceptual confusions that underlie some of the disagreements in conspiracy theory research. Reconciling these conceptual confusions is important because conspiracy theories are a multidisciplinary topic and a profound understanding of them requires integrative insights from different fields. Specifically, we distinguish research focussing on conspiracy *theories* (and theorizing) from research of conspiracy *belief* (and mindset, theorists) and explain how particularism with regards to conspiracy theories does not (...) mean we cannot define a problematic subclass of conspiracy beliefs, while avoiding the problems of generalism. We hope this reply helps conspiracy theory researchers recognize the differences between studying conspiracy theories and conspiracy beliefs and appreciate the possibilities for fruitful, integrative, and interdisciplinary research. Kierkegaard's leap of faith is one of the most thoroughly explored topics in modern philosophy. What can yet another inquiry into this notion hope to achieve? A number of significant things, I think, of both historical and systematic value. The main contention of this paper is that the leap of faith, often associated with the emergence of existentialism, is Kierkegaard's response to a problem which is essentially Kantian in origin and structure. Kierkegaard wants to accomodate both the Kantian interpretation of (...) morality as rational command and Kant's insistence on morality as the sole point of access to religion, while rejecting the Kantian moralization of religion and rationalization of faith. The leap of faith is not, as existentialism would have it, an absolute beginning in philosophy or in individual reflection but a transition from morality to religion within an essentially Kantian context. Health care professionals often face moral dilemmas. Not dealing constructively with moral dilemmas can cause moral distress and can negatively affect the quality of care. Little research has been documented with methodologies meant to support professionals in care for the homeless in dealing with their dilemmas. Moral case deliberation is a method for systematic reflection on moral dilemmas and is increasingly being used as ethics support for professionals in various health-care domains. This study deals with the question: What is the (...) contribution of MCD in helping professionals in an institution for care for the homeless to deal with their moral dilemmas? A mixed-methods responsive evaluation design was used to answer the research question. Five teams of professionals from a Dutch care institution for the homeless participated in MCD three times. Professionals in care for the homeless value MCD positively. They report that MCD helped them to identify the moral dilemma/question, and that they learned from other people’s perspectives while reflecting and deliberating on the values at stake in the dilemma or moral question. They became aware of the moral dimension of moral dilemmas, of related norms and values, of other perspectives, and learned to formulate a moral standpoint. Some experienced the influence of MCD in the way they dealt with moral dilemmas in daily practice. Half of the professionals expect MCD will influence the way they deal with moral dilemmas in the future. Most of them were in favour of further implementation of MCD in their organization. (shrink)

In The Value Gap, Toni Rønnow-Rasmussen addresses the distinction between what is finally good and what is finally good-for, two value notions that are central to ethics and practical deliberation. The first part of the book argues against views that claim that one of these notions is either faulty, or at best conceptually dependent on the other notion. Whereas these two views disagree on whether it is good or good-for that is the flawed or dependent concept, it is argued, as (...) against both approaches, that goodness and goodness-for are independent value notions that cannot be fully understood in terms of one another. The second part provides an analysis of good and good-for in terms of a fitting-attitude analysis. By elaborating a more nuanced understanding of the key elements of this analysis—reasons and pro-attitudes—Rønnow-Rasmussen challenges the widespread idea that there are no genuine practical and moral dilemmas. The result is that the gap between favouring for a reason what is good and favouring for a reason what is good for someone appears insurmountable. (shrink)

Most readers will agree with the starting assumptions of Klee that contemporary science and philosophy assume a primarily micro-deterministic view of nature–and that this has long been the case, or was at least until the 1970s. Defending a strict micro-determinism, Klee argues that concepts of emergence that seemingly are opposed to micro-determinist doctrine can be shown, on analysis, to be ultimately consistent with a thoroughgoing philosophy of micro-determinism. An exception is made, however, in the case of my own view, labeled (...) “direct macro-determinism,” which he describes as “ultimately more troublesome” but then disposes of as being based on “simplistic analogies” and “metaphors” and “too weak to be conclusive.”. But my immediate goal is not to convert the skeptic about the existence of the fallacy but rather to use his extreme position to make a point about the nature of question-begging. Those who object that argument (A) begs the question against Robinson must do so on behalf of Robinson.
