Patients who have an oncological disease and are in palliative care belong to a group that is often characterized as highly vulnerable, and their participation in clinical trials poses a number of ethical problems. This study is cross-sectional and analytic. In all, 82% of those who took part consider it ethical to conduct research with patients in palliative care, either to help other patients in the future, in the hope of gaining some improvement or due to confidence in the physician-researcher. (...) The wide acceptance to participate in these research studies shows the potential benefits appear to exceed the merely therapeutic ones. Altruism, hoping to improve or the perception of continuing to "fight" the disease are aspects to bear in mind when evaluating respect for the autonomy of the persons involved. (shrink)

Healthcare professionals often face ethical conflicts and challenges related to decision-making that have necessitated consideration of the use of conscientious objection (CO). No current guidelines exist within Spain’s healthcare system regarding acceptable rationales for CO, the appropriate application of CO, or practical means to support healthcare professionals who wish to become conscientious objectors. As such, a procedural framework is needed that not only assures the appropriate use of CO by healthcare professionals but also demonstrates its ethical validity, legislative compliance through (...) protection of moral freedoms and patients’ rights to receive health care. Our proposal consists of prerequisites of eligibility for CO (individual reference, specific clinical context, ethical justification, assurance of non-discrimination, professional consistency, attitude of mutual respect, assurance of patient rights and safety) and a procedural process (notification and preparation, documentation and confidentiality, evaluation of prerequisites, non-abandonment, transparency, allowance for unforeseen objection, compensatory responsibilities, access to guidance and/or consultative advice, and organizational guarantee of professional substitution). We illustrate the real-world utility of the proposed framework through a case discussion in which our guidelines are applied. (shrink)

Running marathons is an increasingly popular activity with an ever-increasing number of events and participants. Many participants declare that they pursue a variety of goals by running, namely, the maintenance of good health, the development of strength and improvement of fitness, the management of emotions, and the achievement of resilience and psychological wellbeing. The research has examined marathon running, like many other sports, and has studied various factors that reduce athletic performance, such as the experience of anxiety, and that enhance (...) such performance, such as an increase in general wellbeing. This article reports the results of a study on the experience of competitive anxiety among 238 male marathon runners who participated in Seville’s 26th Marathon race on February 23, 2020, and investigates the relationship between anxiety and key dimensions of wellbeing as measured by the Spanish-adapted 20-item PWB Scale. We hypothesized that participating athletes who rated high on the dimensions of PWB would experience lower levels of competitive anxiety with respect to this race. We also proposed that PWB would function as a mediating factor with respect to the experience of anxiety. The results show, as hypothesized, that marathon running enhances wellbeing and reduces anxiety. The data showed significant negative correlations between four of five wellbeing dimensions and the three types of anxiety measured, namely, somatic anxiety, worry, and concentration-impairing anxiety. Other findings supported our hypothesis that wellbeing, as measured, functions as a mediating factor for the moderation of competitive anxiety. Generalization of these findings is limited by the fact that the low number of female participants recruited did not permit valid statistical analyses in this respect. It is known that both anxiety and wellbeing are subjects to variation by gender. The future inclusion of male and female subjects in equivalent studies will undoubtedly add valuable information concerning the dynamics of anxiety and wellbeing. The implications of these findings and the limitations of the study will be discussed. (shrink)

In March 2021, the Spanish Congress approved the law regulating euthanasia, that regulates both euthanasia and physician-assisted suicide (PAS). In this article, we analyse the Spanish law regulating euthanasia and PAS, comparing it with the rest of the European laws on euthanasia and PAS (Netherlands, Belgium and Luxembourg). Identified strengths of the Spanish law, with respect to other norms, are that it is a law with many safeguards, which broadly recognises professionals’ right to conscientious objection and the specification that it (...) makes on the prior comprehensive care of the patient, including the approach to care dependency. Regarding its shortcomings, the law does not differentiate well between euthanasia and PAS; it barely assigns a role to the healthcare team as a whole (similar to other regulations); it does not clarify the functions of the different professionals involved; it does not detail the specific composition and duration of theevaluation commission; it has not been accompanied by a prior or simultaneous regulation of palliative care; and, lastly, the period of time to implement the law is too short. Objectives To develop and test two educational programmes (interactive and passive) aimed at improving family doctors' (FD) prescribing practices and patient's knowledge and use of non-opioid analgesics (NOA).Methods The educational programmes were conducted in two family medicine clinics by using a three-stage approach: baseline evaluation, design, and implementation of educational activities, and post-programme evaluation. An interactive educational programme (IEP) was compared with a passive educational programme (PEP); both were participated by FDs and patients. The IEP for FDs comprised of workshops, (...) discussion groups, in-service training and guidelines, while for patients the IEP consisted of an interactive session with a video, leaflets and a discussion. The PEP consisted in delivering the guidelines to the FDs and the leaflets to patients. The effect of the programmes on the FDs was measured through the appropriateness of prescriptions and analysed using the differences-in-differences estimator (D-in-D), and on patients through changes in self-medication and in their knowledge about the proper use and adverse events by analysing the inter- and intra-group differences before and after the programmes.Results The IEP obtained better results to improve appropriate FDs prescription of NOA than PEP (D-in-D = 15%). Regarding the patients, the PEP group reached higher reduction of self-medication than the IEP group (13.4% vs. 9.1%); the knowledge of proper NOA use increased by 8.5% in both groups, whereas knowledge of NOA-related adverse events was better in the IEP (39.6%) than in the PEP group (9.2%).Conclusions The IEP was better to improve the doctors' abilities to prescribe NOAs, and both programmes improved patients' knowledge. This paper explains conceptual modeling within the framework of Frame-Based Terminology (Faber, 2012; 2015; 2022), as applied to EcoLexicon (ecolexicon.ugr.es), a specialized knowledge base on the environment (León-Araúz, Reimerink &, Faber, 2019; Faber & León-Araúz, 2021). It describes how a frame-based terminological resource is currently being restructured and reengineered as an initial step towards its formalization and subsequent transformation into an ontology. It also explains how the information in EcoLexicon can be integrated in environmental ontologies such as ENVO (...) (Buttigieg, Morrison, Smith, Mungall & Lewis, 2013; Buttigieg, Pafilis, Lewis, Schildhauer, Walls & Mungall, 2016), particularly at the bottom tiers of the Ontology Learning Layer Cake (Cimiano, 2006; Cimiano, Maedche, Staab & Volker, 2009). El aprendizaje de la Historia en Educación Secundaria se relaciona con un proceso memorístico y conceptual que dificulta la formación del pensamiento crítico y reflexivo en el alumnado de esta etapa educativa. Por este motivo, con esta experiencia y desde un enfoque investigativo, hemos fomentado la curiosidad y el interés de los alumnos de 1º de Secundaria por las prácticas religiosas y funerarias que profesaron los romanos. El método de enseñanza-aprendizaje fue activo y participativo, lo que facilitó la indagación y (...) la experimentación del taller “Funus romanorum”. Los resultados mostraron las ventajas de trabajar la arqueología experimental para comprender mejor los hechos acaecidos en la Antigüedad y adquirir contenidos históricos a través de talleres experienciales. Background:The first wave of the COVID-19 pandemic caused a shortage of qualified nurses in Spain. As a result, the government authorized the hiring of senior students.Objectives:To explore the ethical dilemmas and ethical conflicts experienced by final-year nursing students who worked during the first outbreak of the COVID-19 pandemic in Spain.Research design:A qualitative exploratory study was conducted using purposive sampling. Semi-structured interviews were carried out using a question guide. Interviews took place via a private video chat room platform. A thematic, inductive (...) analysis was performed of the information gathered.Participants and research context:Eighteen nursing students were recruited from two universities of Madrid, aged between 18 and 65 years old, enrolled in the fourth year of nursing studies and who were hired under a relief contract for health professionals during the pandemic.Ethical considerations:The present study was carried out in accordance with the Declaration of Helsinki, and the study was approved by the Local Ethics Committee of Universidad Rey Juan Carlos.Results:Three specific themes emerged: (a) coping with patient triage, (b) difficulties in providing end-of-life care, and (c) coping with patient death. Nursing students participated in the process of patient selection for resource allocation and ICU bed occupancy. They were shown how to care for patients who were not admitted to the ICU, in their last moments and were faced with the difficulties of applying end-of-life care. Finally, the nursing students were confronted with the death of their patients, in overwhelming numbers and under adverse conditions.Conclusions:These findings can help shed light on the ethical dilemmas and ethical conflicts faced by novice nursing students, incorporated into the workforce during the COVID-19 pandemic. Regularization may be used as an alternative to dimensionality reduction when the number of variables in a model is much larger than the number of available observations. In a recent study from our group regularized regression was employed to quantify brain functional connectivity in a sample of healthy controls using a brain parcellation and resting state fMRI images. Here regularization is applied to evaluate resting state connectivity abnormalities at the voxel level in a sample of patients with schizophrenia. Specifically, ridge (...) regression is implemented with different degrees of regularization. Results are compared to those delivered by the weighted global brain connectivity method, which is based on averaged bivariate correlations and from the non-redundant connectivity method, a dimensionality reduction approach that applies supervised principal component regressions. Ridge regression is able to detect a larger set of abnormally connected regions than both GBC and NRC methods, including schizophrenia related connectivity reductions in fronto-medial, somatosensory and occipital structures. Due to its multivariate nature, the proposed method is much more sensitive to group abnormalities than the GBC, but it also outperforms the NRC, which is multivariate too. Voxel based regularized regression is a simple and sensitive alternative for quantifying brain functional connectivity. (shrink)

Este artículo argumenta que los debates actuales en torno a la movilidad sostenible se centran en lo urbano, relegando a las áreas rurales a una posición secundaria o periférica. Se indaga en los límites que pueden tener las lógicas de movilidad urbana sostenible aplicadas (...) sobre entornos rurales altamente dependientes del vehículo privado y con déficit de servicios públicos de transporte. Para ello se analizan, por un lado, las principales políticas de movilidad y transporte europeas y españolas y, por otro, los discursos y las prácticas de la población local en una ruralidad remota. Una línea de investigación que necesita ser reforzada para diseñar políticas de movilidad ajustadas a las necesidades de las poblaciones rurales. (shrink)

What does woman mean? According to two competing views, it can be seen as a sex term or as a gender term. Recently, Jennifer Saul has put forward a contextualist view, according to which woman can have different meanings in different contexts. The main motivation for this view seems to involve moral and political considerations, namely, that this view can do justice to the claims of trans women. Unfortunately, Saul argues, on further reflection the contextualist view fails to do justice (...) to those moral and political claims that motivated the view in the first place. In this article I argue that there is a version of the contextualist view that can indeed capture those moral and political aims, and in addition, I use this case to illustrate an important and more general claim, namely, that moral and political considerations can be relevant to the descriptive project of finding out what certain politically significant terms actually mean. The logician Kurt Godel in 1951 established a disjunctive thesis about the scope and limits of mathematical knowledge: either the mathematical mind is equivalent to a Turing machine (i.e., a computer), or there are absolutely undecidable mathematical problems. In the second half of the twentieth century, attempts have been made to arrive at a stronger conclusion. In particular, arguments have been produced by the philosopher J.R. Lucas and by the physicist and mathematician Roger Penrose that intend to show that the (...) mathematical mind is more powerful than any computer. These arguments, and counterarguments to them, have not convinced the logical and philosophical community. The reason for this is an insufficiency if rigour in the debate. The contributions in this volume move the debate forward by formulating rigorous frameworks and formally spelling out and evaluating arguments that bear on Godel's disjunction in these frameworks. Telic monism in logic is the thesis that there is one single philosophically primary goal to logic. A different way to put it is that there is only one canonical application to logic. This thesis is widely present—implicitly or more explicitly—in the literature on the philosophy of logic, yet has not been examined nor argued for extensively. In this paper I will present and critically examine telic monism. One prominent candidate for the canonical application of logic, namely the formal codification (...) of logical consequence in natural language, will be challenged directly. The critique will then be generalized by appealing to a practice-based philosophy of logic. The general argument I put forth here claims that we have no principled reason to assume there to be just one primary goal to logic. Rather, I claim that it is more plausible that there actually is a variety of goals, purposes and applications to logic, without one of them enjoying primacy over the others. If this argument succeeds, at least one of the consequences is that it allows for a kind of logical pluralism that has previously been deemed insignificant, namely logical instrumentalism. (shrink)

According to the BD-model of mindreading, we primarily understand others in terms of beliefs and desires. In this article we review a number of objections against explicit versions of the BD-model, and discuss the prospects of using its implicit counterpart as an explanatory model of early emerging socio-cognitive abilities. Focusing on recent findings on so-called ‘implicit’ false belief understanding, we put forward a number of considerations against the adoption of an implicit BD-model. Growing powers to manipulate human bodies and minds, not merely to heal disease but to satisfy desires, control deviant behavior, and to change human nature, make urgent questions of whether and how to regulate their use, not merely to assure safety and efficacy but also to safeguard our humanity. Oversight in democratic societies rightly belongs to the polity, not merely to self-appointed experts, scientific or ethical. In this article ideas from Kit Fine’s theory of arbitrary objects are applied to questions regarding mathematical structuralism. I discuss how sui generis mathematical structures can be viewed as generic systems of mathematical objects, where mathematical objects are conceived of as arbitrary objects in Fine’s sense.

The first part of the paper distinguishes between a real past which has nothing to do with historical events and an historical past made up of hypothetical events introduced for the purpose of explaining historical evidence. Attention is next paid to those so-called ancillary historical disciplines which study historical evidence, and it is noted that the historical event is brought in to explain the particular constellation of different kinds of historical evidence which are judged to belong together. The problem of (...) explaining events is then taken up, and an attempt is made to defend the view that such explanation must presuppose general laws. And this is followed by a discussion, partly speculative, of social-historical laws. The final section of the paper tries to argue that the subjective intentions of individuals are irrelevant to historical explanation. (shrink)

New epistemic principles are formulated in the language of Shapiro's system of Epistemic Arithmetic. It is argued that some plausibility can be attributed to these principles. The relations between these principles and variants of controversial constructivistic principles are investigated. Special attention is given to variants of the intuitionistic version of Church's thesis and to variants of Markov's principle.

This article contains an overview of the main problems, themes and theories relating to the semantic paradoxes in the twentieth century. From this historical overview I tentatively draw some lessons about the way in which the field may evolve in the next decade.

The activism of institutional investors tends more and more toward the supervision and control of the behavior of the managers of big companies. In this article, we present a model based on the creation of an activism index that lets us evaluate such activism's effect on the sensitivity of the investment policies of a company in the face of financial variables and market variables . To test our assertions, we analyze firm-level data for United Kingdom , Germany, France, Denmark, and (...) Spain during the period 1995-2004. Our results point to a significant reduction in the sensitivity of company investment decisions in the face of these variables, especially relative to intangible capital, as a result of the neutralizing effect of activism on the high agency costs of free cash flow and on the information asymmetries of the market. This article is concerned with the role of moral principles, specifically the Fundamental Principles of Olympism, in the judgements of the International Olympic Committee (IOC) on matters of performance enhancement. The article begins with two pairs of distinctions, that between moral judgements and morally-laden judgements, and that between the moral judgement of cases and the ethical environment of a society. The article is concerned with working through the implications of those distinctions in the context of the IOC's judgements on performance (...) enhancement. The article favours a particularist account of the moral judgement of cases, while preserving a place for meaningful general moral statements as contributions to the ethical environment of a society, but not as general action-guiding statements or principles that can be applied to judgements in specific cases. The article illustrates the implications of this conclusion in the context of the decision-making of the IOC on performance enhancement (the case of Alain Baxter is considered). It is argued that there is a danger of the decision-making of the IOC suffering deep confusion over the difference between general and particular statements, the nature of reasons and the logic of what it is to apply a general action-guiding statement.
