Our families are our first and most important ethical training grounds. But what is the family? And what are our ethical commitments to our family members and to the broader moral community? After a brief introductory chapter on basic ethical concepts and theories, the essays in this volume provide readers with ethical analyses of issues ranging from same-sex marriage to a controversial proposal to "license" parents. The chapters cover love, sex, marriage, parents and children, the relationship between the family and (...) the larger moral community, and the influence of emerging technologies on the ethical issues inherent in family life. Cognition has traditionally been understood in terms of internal mental representations, and computational operations carried out on internal mental representations. Radical approaches propose to reconceive cognition in terms of agent-environment dynamics. An outstanding challenge for such a philosophical project is how to scale-up from perception and action to cases of what is typically called ‘higher-order’ cognition such as linguistic thought, the case we focus on in this paper. Perception and action are naturally described in terms of agent-environment dynamics, but can (...) a person’s thoughts about absent, abstract or counterfactual states of affairs also be accounted for in such terms? We argue such a question will seem pressing so long as one fails to appreciate how richly resourceful the human ecological niche is in terms of the affordances it provides. The explanatory work that is supposedly done by mental representations in a philosophical analysis of cognition, can instead be done by looking outside of the head to the environment structured by sociomaterial practices, and the affordances it makes available. Once one recognizes how much of the human ecological niche has become structured by activities of talking and writing, this should take away at least some of the motivation for understanding linguistic thinking in terms of content-bearing internal representations. We’ll argue that people can think about absent, abstract or counterfactual because of their skills for engaging with what we will call “enlanguaged affordances”. We make use of the phenomenological analysis of speech in Merleau-Ponty to show how the multiple affordances an individual is ready to engage with in a particular situation will typically include enlanguaged affordances. Predictive Processing (PP) is an increasingly influential neurocognitive-computational framework. PP research has so far focused predominantly on lower level perceptual, motor, and various psychological phenomena. But PP seems to face a “scale-up challenge”: How can it be extended to conceptual thought, language, and other higher cognitive competencies? Compositionality, arguably a central feature of conceptual thought, cannot easily be accounted for in PP because it is not couched in terms of classical symbol processing. I argue, using the example of language, that (...) there is no strong reason to think that PP cannot be scaled up to higher cognition. I suggest that the tacitly assumed common-sense conception of language as Generative Grammar (“folk linguistics”) and its notion of composition leads to the scale-up concerns. Fodor’s Language of Thought Hypothesis (LOTH) plays the role of a cognitive computational paradigm for folk linguistics. Therefore, we do not take LOTH as facing problems with higher cognition, at least with regard to compositionality. But PP can plausibly play the role of a cognitive-computational paradigm for an alternative conception of language, namely Construction Grammar. If Construction Grammar is a plausible alternative to folk linguistics, then PP is not in a worse position than LOTH. (shrink)

As demand for locally grown food increases there have been calls to ‘scale-up’ local food production to regionally distribute food and to sell into more mainstream grocery and retail venues where consumers are already shopping. Growing research and practice focusing on how to improve, expand and conceptualize regional distribution systems includes strategies such as value chain development using the Agriculture of the Middle framework. When the Ohio Food Policy Advisory Council asked how they could scale-up the distribution of Ohio fresh (...) fruits and vegetables to Ohioans, we decided to use this practical opportunity to not only provide recommendations to this council, but to simultaneously contribute to the literature on AOTM, value-based and spatially–proximate relationships, and conceptualizations of food system hybridity. We do this while examining an entire sub-sector of the Ohio agricultural economy, namely fruit and vegetables and applying the AOTM framework beyond the farm, namely to distributors and retailers. Through interviews with Ohio retailers and a survey of all fresh fruit and vegetable distributors Ohio we: Describe current distribution systems within the state; Identify firms interested in scaling-up distribution, and; Inform state-level policy efforts by identifying opportunities to better target any state-level policy and program efforts. We demonstrate support for the concept of AOTM applied beyond the farm, for value chain development strategies that can transmit ‘quality’ via spatially proximate supply chains, and support for considering hybrid solutions, such as piggybacking for scaling-up local food systems. This work highlights the role a statewide food policy council can have in facilitating market development and their unique position to provide public sector and institutional support to facilitate meaningful connections in the food system. The Theory of Event Coding deals with brief events but has implications for longer, complex events, particularly goal-directed activities. Two of the theory's central claims are consistent with or assumed by theories of complex events. However, the claim that event codes arise from the rapid activation and integration of features presents challenges for scaling up to larger events.

ABSTRACT Historians have been slow to incorporate the ocean as a focus of study, in part because we have viewed it as standing mostly apart from human societies and activities. Whether that was ever truly the case is arguable, but it is certainly no longer true today. Global climate change and ocean acidification point to the now-pervasive impact of humans on the ocean environment and, conversely, the crucial importance of the ocean in the development of human affairs. Understanding the human (...) effects on the ocean will remain mainly the preserve of natural scientists, but understanding the origins, development, and repercussions of those impacts is a job for historians and other social scientists. Alternative Food Networks (AFNs), which include local food and Fair Trade, work to mitigate some of the many shortcomings of mainstream food systems. If AFNs have the potential to make the world’s food systems more just and sustainable (and otherwise virtuous) then we may have good reasons to scale them up. Unfortunately, it may not be possible to increase the market share of AFNs while preserving their current forms. Among other reasons, this is because there are limits to both the (...) productive capacities of small owner-operated farms and to the distribution capacities of Farmers Markets and Community Supported Agriculture (CSA). These limits tell in favor of AFN partnerships with larger producers and distributors. But some advocates of AFNs have worried that these partnerships would sacrifice too much. (shrink)

What are we trying to explain when we explain behavior? How is behavior conceptualized as an object of study and how else might it be conceptualized? Longino urges pluralism with respect to causes and explanations of behavior. This paper extends Longino’s analysis to the object of those explanations and urges pluralism with respect to behavior itself. The paper proposes that there are three ways in which behavior can be conceptualized, each of which opens to different kinds of research questions.

Chemical engineering - as a recognised skill in the workplace, as an academic discipline, and as an acknowledged profession - is scarcely a century old. Yet from a contested existence before the First World War, chemical engineering had become one of the 'big four' engineering professions in Britain, and a major contributor to Western economies, by the end of the twentieth century. The subject had distinct national trajectories. In Britain - too long seen as shaped by American experiences - the (...) emergence of recognised chemical engineers was the result of professional aspirations and contingency, and shaped by a shifting ecology of institutions, firms and government. Drawing upon extensive archival research, this book examines the evolution of technical practice, working environment and social interactions of chemical engineering. This article reports on a case-based, longitudinal study of the micro-foundations of business sustainability development in the Royal Bank of Scotland (RBS) in the turbulent years between 2002 and 2012. The study proposes an emerging 3-i process model, mapping the role of bounded, shared, and embedded intentionality; operational, functional, and strategic integration; and constraining, accelerating, and stabilizing institutionality as they relate to the micro-foundations underpinning the development of corporate sustainability from an operational capability to as a dynamic capability as it (...) evolved across multiple levels of context and over time. The research extends extant literature exploring transformations toward sustainability as part of the strategic change process, the micro-foundations of capabilities as well as discussions on sustainability and temporality. This paper reports on a relationship between the University of Toronto and a non-profit, non-governmental (“third party”) certifying organization called Local Flavour Plus (LFP). The University as of August 2006 requires its corporate caterers to use local and sustainable farm products for a small but increasing portion of meals for most of its 60,000 students. LFP is the certifying body, whose officers and consultants have strong relations of trust with sustainable farmers. It redefines standards and verification to create ladders for (...) farmers, Aramark and Chartwells (the corporations that won the bid), and the University, to continuously raise standards of sustainability. After years of frustrated efforts, other Ontario institutions are expressing interest, opening the possibility that a virtuous circle could lead to rapid growth in local, sustainable supply chains. The paper examines the specificities of the LFP approach and of the Toronto and Canadian context. Individuals in LFP acquired crucial skills, insights, experience, resources, and relationships of trust over 20 years within the Toronto “community of food practice,” located in a supportive municipal, NGO and social movement context. The neural blackboard architecture is a localist structured connectionist model that employs a novel connection matrix to implement dynamic bindings without requiring propagation of temporal synchrony. Here I note the apparent need for many distinct matrices and the effect this might have for scale-up to semantic processing. I also comment on the authors' initial foray into the symbol grounding problem.

To understand lying, we naturally focus on small scale lies involving one speaker, one listener, one assertion. This methodology confers artificial plausibility upon the requirement that liars intend to deceive. For it excludes principal-agent conflicts that emerge from linguistic division of labor. When an employee lies for her boss, she need not inherit his motive to deceive. She displays loyalty even if her lie does not deceive. Focus on a single lie in isolation also blinds us to tactical deceptions such (...) as telling a lie which is intended to be caught to advance another lie. Many of these complexities arise from situations that approximate common knowledge without quite crystallizing into this uniform transparency. (shrink)

This paper advances the conception of social entrepreneurs as caring entrepreneurs. We argue that the care ethics of social entrepreneurs, implying the pursuit of caring goals through caring processes, can be challenged when they engage in the process of scaling up their ventures. We propose that social entrepreneurs can sustain their care ethics as the essential dimension of their venture only if they are able to build a caring enterprise. Organizational care designates the set of organizing principles that facilitate (...) the embedding of care ethics at an organizational level, beyond the imprinting induced by social entrepreneurs’ personal ethics. The ‘scaling up’ objection says non-representational ecological-enactive accounts will be unable to explain ‘representation hungry’ cognition. Obsessive-compulsive disorder presents a paradigmatic instance of this objection, marked as it is by ‘representation hungry’ obsessive thoughts and compulsive behavior organized around them. In this paper I provide an ecological-enactive account of OCD, thereby demonstrating non-representational frameworks can ‘scale up’ to explain ‘representation hungry’ cognition. First, I outline a non-representational account of mind— a predictive processing operationalization of Sean Kelly’s theory of perception. (...) This account explains the ‘tensions’ and ‘pulls’ which guide and constrain our action-oriented and affect-laden perceptual ‘grip’ upon the world to be underwritten by an imperative to minimize prediction error. I then argue that OCD is best understood as ‘grip gone awry’— malformed predictive models signal inappropriately high error, and this results in extremely strong ‘tensions’ and ‘pulls’ which prescribe actions. Thus, I arrive at the idea that OCD is primarily constituted by ‘not just right’ feelings caused by high error signaling. Finally, I explain that this account provides a causal explanation of the non-representational existential feeling fundamental to OCD. Compulsions are considered manifestations of non-representational grip responsive to this feeling, whilst obsessions are explained to play a meta-role in the condition, being formulated by subjects in a bid to make their ‘not just right’ feeling intelligible. I explain that the meta-role obsessions play makes their representational status largely irrelevant to OCD, and so leave this an open question. Consequently, I provide a non-representational account of OCD, thereby demonstrating that ecological-enactive approaches can respond to the ‘scaling up’ objection. (shrink)

How does a social enterprise pursue its ethical mandate of social impact growth while navigating the perils of the most vulnerable stage in a venture’s life—scaling up? We observe a small inclusivity social enterprise attempting to scale up rapidly to create equality for people with disabilities throughout the world. Our embedded, ethnographic study is terminated with the venture’s unfortunate demise after their dramatic effort to scale up failed. By examining scaling decision-making and conflicts around creation reasoning longitudinally, our (...) study identifies over-use of effectual logic—a creation reasoning type considered more ethical and more appropriate for high-innovativeness contexts than causal logic—as a major factor in the venture’s failure. From this insight, we extend the parameters of effectuation theory to scaling up and dimensionalize its ethical implications. Guidance for social entrepreneurs to scale up successfully while maintaining ethical integrity is also provided. (shrink)

Can groups of people possess epistemic virtues? There has been some attention to this question in recent years in social epistemology and ethics. Interestingly, most defenses and criticisms of collective virtues so far have focused on proving or disproving that groups can have collective versions of individual virtues. Trying to or disprove that a group can have a good motivation that supervenes motivations of individuals, for example, is a typical strategy. But maybe group virtues are a completely different kind of (...) animal. Does it make sense to demand that they resemble individual virtues? I propose that group virtues can be something different that individual virtues, and whether this is plausible or not depends on the theory we adopt about how virtues are grounded. (shrink)

The “scaling-up” problem concerns radical embodied cognition’s (REC) supposed inability to extend its explanatory reach beyond simple cognitive phenomena. This paper questions the problem’s main assumption, that is, the possibility of sorting phenomena according to their inherent cognitive complexity or representation-hunger. To do so, I focus on a class of phenomena whose degree of representation-hunger has been much debated: social phenomena (and their cognitive correlates). Some, like Clark and Toribio (1994), consider social phenomena the absolute top of representation-hunger, while (...) others, like radical enactivists, conceive of sociality as a non-representational resource; two opposite views which demonstrate how hard it is to place sociality somewhere in a representational hierarchy. An exploration into two social disciplines provides reasons to question the tenability of the scaling-up problem in the case of social phenomena and hence in general. The long debate in social psychology over dual-process models of cognition suggests distinguishing between the complexity of a task and its putative cognitive requirements, therefore rejecting that form of cognitive-task essentialism inherent in the idea of representation-hunger. The recent encounter of radical enactivism with social theory provides, on the other hand, a non-representational explanation of institutional phenomena. In the end, the paper suggests that one of the following must hold: either the scaling-up problem is untenable in its traditional (hierarchical) understanding or radical enactivism has already scaled up to social phenomena, understood as the pinnacle of representation-hunger. Bumped from a flight? Relax with this defense of the big business practice of deliberately promising more services than one will provide. On a small scale, over‐promising yields a toxic moral dilemma and a lie. At a large scale, the dilemma becomes dilute, and the lie completely disappears. Overbooking is honest because there is a sufficiently high probability of fulfilling each promise. Overbooking is socially beneficial because the promised resources are used more efficiently. There are fewer wasted seats on jumbo (...) jets and hence cheaper tickets with less pollution. Widespread disapproval of overbooking is a fallacious scaling error. Verbal autopsy presents the opportunity to understand the disease burden in many low-income countries where vital registration systems are underdeveloped and most deaths occur in the community. Advances in technology have led to the development of software that can provide probable cause of death information in real time, and research considering the ethical implications of these advances is necessary to inform policy. Our research explores these ethical issues in rural Nepal using a public health ethics framework. We considered the burdens (...) and benefits of VA and giving cause of death information to families of the deceased through qualitative research with VA interviewers, community members, national policy stakeholders and international academics. Burdens can be experienced differently, and it is important to balance the emotional burden of VA with utilization of the data to inform planning and increased access to health services. The training, support and supervision of VA interviewers should be prioritized if VA is taken to scale. Initial and ongoing community engagement is recommended in addition to engaging ethical, legal, health and policy personnel in developing protocols and systems. Integrating rigorous research while cautiously moving forward is recommended to ensure systems and responses to concerns are relevant to contexts. (shrink)

Despite the evidence showing the promise of HIV treatment as prevention (TasP) in reducing HIV incidence, a variety of ethical questions surrounding the implementation and “scaling up” of TasP have been articulated by a variety of stakeholders including scientists, community activists and government officials. Given the high profile and potential promise of TasP in combatting the global HIV epidemic, an explicit and transparent research priority-setting process is critical to inform ongoing ethical discussions pertaining to TasP.

The Consciousness Reframed proposition is exciting because it is both interdisciplinary and offers the possibility of a macrocosmic perspective on the world – a view of the world as simultaneously a whole in itself, the sum of its parts, and as part of a wider whole – or as Koestler put it, ‘After a period of some 500 years when the western eye has been focussed primarily on the microcosmic, scaling up from the micro to the macro does not (...) at first come easily’. Global civilization has too often been misled by grand narratives – never more so than in the twentieth century, when grand narratives of race and class slugged it out bringing death to millions. On the other hand the planetary ‘big picture’ nowadays forces itself upon us through global information networks that all of us simultaneously share in a time-compact world. As such we are creatures of the macrocosmos, whether we care to acknowledge it or not. The Planetary Collegium, this article suggests, is a beach-head on the shores of a macrocosmos awaiting both discovery and re-discovery. (shrink)

Corporate carbon footprint data has become ubiquitous. This data is also highly promissory. But as this paper argues, such data fails both consumers and citizens. The governance of climate change seemingly requires a strong foundation of data on emission sources. Economists approach climate change as a market failure, where the optimisation of the atmosphere is to be evidence based and data driven. Citizens or consumers, state or private agents of control, all require deep access to information to judge emission realities. (...) Whether we are interested in state-led or in neoliberal ‘solutions’ for either democratic participatory decision-making or for preventing market failure, companies’ emissions need to be known. This paper draws on 20 months of ethnographic fieldwork in a Fortune 50 company’s environmental accounting unit to show how carbon reporting interferes with information symmetry requirements, which further troubles possibilities for contesting data. A material-semiotic analysis of the data practices and infrastructures employed in the context of corporate emissions disclosure details the situated political economies of data labour along the data processing chain. The explicit consideration of how information asymmetries are socially and computationally shaped, how contexts are shifted and how data is systematically straightened out informs a reflexive engagement with Big Data. The paper argues that attempts to automatise environmental accounting’s veracity management by means of computing metadata or to ensure that data quality meets requirements through third-party control are not satisfactory. The crossover of Big Data with corporate environmental governance does not promise to trouble the political economy that hitherto sustained unsustainability. (shrink)

Autonomous lifelong development and learning are fundamental capabilities of humans, differentiating them from current deep learning systems. However, other branches of artificial intelligence have designed crucial ingredients towards autonomous learning: curiosity and intrinsic motivation, social learning and natural interaction with peers, and embodiment. These mechanisms guide exploration and autonomous choice of goals, and integrating them with deep learning opens stimulating perspectives.

Genome‐wide identification of transcription factor binding sites with the ChIP‐seq method is an extremely important scientific endeavor − one that should ideally be performed for every transcription factor in as many cell types as possible. A major hurdle on the way to this goal is the necessity for a specific, ChIP‐grade antibody for each transcription factor of interest, which is often not available. Here, we describe CETCh‐seq, a recently published method utilizing genome engineering with the CRISPR/Cas9 system to circumvent the (...) need for a specific antibody. Using the CETCh‐seq method, targeted genomic editing results in an epitope‐tagged transcription factor, which is recognized by a well‐characterized, standard antibody, efficacious for ChIP‐seq. We have used CETCh‐seq in human cancer cell lines as well as mouse embryonic stem cells. We find that roughly 60% of transcription factors tagged using CETCh‐seq produce a high quality ChIP‐seq map, a significant improvement over traditional antibody‐based methods. (shrink)

From one of the most influential scientists of our time, a dazzling exploration of the hidden laws that govern the life cycle of everything from plants and animals to the cities we live in. The former head of the Sante Fe Institute, visionary physicist Geoffrey West is a pioneer in the field of complexity science, the science of emergent systems and networks. The term "complexity" can be misleading, however, because what makes West's discoveries so beautiful is that he has found (...) an underlying simplicity that unites the seemingly complex and diverse phenomena of living systems, including our bodies, our cities and our businesses. Fascinated by issues of aging and mortality, West applied the rigor of a physicist to the biological question of why we live as long as we do and no longer. The result was astonishing, and changed science, creating a new understanding of energy use and metabolism: West found that despite the riotous diversity in the sizes of mammals, they are all, to a large degree, scaled versions of each other. If you know the size of a mammal, you can use scaling laws to learn everything from how much food it eats per day, what its heart-rate is, how long it will take to mature, its lifespan, and so on. Furthermore, the efficiency of the mammal's circulatory systems scales up precisely based on weight: if you compare a mouse, a human and an elephant on a logarithmic graph, you find with every doubling of average weight, a species gets 25% more efficient--and lives 25% longer. This speaks to everything from how long we can expect to live to how many hours of sleep we need. The experience of grace is currently attracting increased empirical interest. Validated instruments to assess aspects of the experience of grace in a secularized context are scarce which limits current psychological research on grace in other than highly religious populations. We present a bottom-up construction of the Experiencing Grace Scale (EGS) and evidence supporting its reliability and validity. Confirmatory factor analysis showed a good fit for the four subscales: Appraising Grace, Giving Grace, Receiving Grace, and Divine Grace. Reliability analysis showed overall (...) good internal consistency and test–retest stability. Convergent validity was demonstrated by means of expected associations between subscales of the EGS and other relevant measures. Predictive validity was confirmed by associations between subscales of the EGS at measurement Wave 1 with mental well-being scales at Wave 2. The EGS contributes to further research on the experience of grace and its relationship to other psychosocial variables in the field of positive psychology. (shrink)

Abstract“Scaling-up” is the next hurdle facing the local food movement. In order to effect broader systemic impacts, local food systems (LFS) will have to grow, and engage either more or larger consumers and producers. Encouraging the involvement of mid-sized farms looks to be an elegant solution, by broadening the accessibility of local food while providing alternative revenue streams for troubled family farms. Logistical, structural and regulatory barriers to increased scale in LFS are well known. Less is understood about the (...) way in which scale developments affect the perception and legitimacy of LFS. This value-added opportunity begs the question: Is the value that adheres to local food scalable? Many familiar with local food discourse might suggest that important pieces of added value within LFS are generated by the reconnection of producer and consumer, the direct exchange through which this occurs, and the shared goals and values that provide the basis for reconnection. However, these assertions are based on tenuous assumptions about how interactions within the direct exchange produce value, and how LFS are governed. Examination shows that existing assumptions do not properly acknowledge the hybridity, diversity, and flexibility inherent in LFS. A clear analysis of the potential of scale in LFS will depend on understanding both how value is determined within LFS, and the processes through which these systems are governed. Such an analysis shows that, while scaled-up LFS will be challenged to maintain legitimacy and an identity as “alternative”, the establishment of an open governance process—based on a “negotiation of accommodations”—is likely to enhance their viability. This VSD nugget considers how the small-scale effects of practice-based design activities can be sustained and scaled up to contribute to global societal change. Examining in particular the character of social networking activity, we discuss how the sustainability of an initiative is related to its ability to: be scaled up and linked with like-minded initiatives; navigate diverse values; establish organizational structures beyond the scientific context; establish a voice in public discourse; and identify and involve relevant policy makers.

Nineteenth-century optical toys that showcase illusions of motion such as the phenakistoscope, zoetrope, and praxinoscope, have enjoyed active “afterlives” in the twentieth and twenty-first centuries. Contemporary incarnations of the zoetrope are frequently found in the realms of fine art and advertising, and they are often much larger than their nineteenth-century counterparts. This article argues that modern-day optical toys are able to conjure feelings of wonder and spectacle equivalent to their nineteenth-century antecedents because of their adjustment in scale. Exploring a range (...) of contemporary philosophical toys found in arts, entertainment, and advertising contexts, the article discusses various technical adjustments made to successfully “scale up” optical toys, including the replacement of hand-spun mechanisms with larger sources of motion and the use of various means such as architectural features and stroboscopic lights to replace traditional shutter mechanisms such as the zoetrope’s dark slots. Critical consideration of scale as a central feature of these installations reconfigures the relationship between audience and device. Large-scale adaptations of optical toys revise the traditional conception of the user, who is able to tactilely manipulate and interact with the apparatus, instead positing a viewer who has less control over the illusion’s operation and is instead a captive audience surrounded by the animation. It is primarily through their adaptation of scale that contemporary zoetropes successfully elicit wonder as scientific spectacles from their audiences today. (shrink)

Objectives To conduct an independent evaluation of the first phase of the Health Foundation’s Safer Patients Initiative (SPI), and to identify the net additional effect of SPI and any differences in changes in participating and non-participating NHS hospitals. Design Mixed method evaluation involving five substudies, before and after design. Setting NHS hospitals in the United Kingdom. Participants Four hospitals (one in each country in the UK) participating in the first phase of the SPI (SPI1); 18 control hospitals. Intervention The SPI1 (...) was a compound (multi-component) organisational intervention delivered over 18 months that focused on improving the reliability of specific frontline care processes in designated clinical specialties and promoting organisational and cultural change. Results Senior staff members were knowledgeable and enthusiastic about SPI1. There was a small (0.08 points on a 5 point scale) but significant (P<0.01) effect in favour of the SPI1 hospitals in one of 11 dimensions of the staff questionnaire (organisational climate). Qualitative evidence showed only modest penetration of SPI1 at medical ward level. Although SPI1 was designed to engage staff from the bottom up, it did not usually feel like this to those working on the wards, and questions about legitimacy of some aspects of SPI1 were raised. Of the five components to identify patients at risk of deterioration—monitoring of vital signs (14 items); routine tests (three items); evidence based standards specific to certain diseases (three items); prescribing errors (multiple items from the British National Formulary); and medical history taking (11 items)—there was little net difference between control and SPI1 hospitals, except in relation to quality of monitoring of acute medical patients, which improved on average over time across all hospitals. Recording of respiratory rate increased to a greater degree in SPI1 than in control hospitals; in the second six hours after admission recording increased from 40% (93) to 69% (165) in control hospitals and from 37% (141) to 78% (296) in SPI1 hospitals (odds ratio for “difference in difference” 2.1, 99% confidence interval 1.0 to 4.3; P=0.008). Use of a formal scoring system for patients with pneumonia also increased over time (from 2% (102) to 23% (111) in control hospitals and from 2% (170) to 9% (189) in SPI1 hospitals), which favoured controls and was not significant (0.3, 0.02 to 3.4; P=0.173). There were no improvements in the proportion of prescription errors and no effects that could be attributed to SPI1 in non-targeted generic areas (such as enhanced safety culture). On some measures, the lack of effect could be because compliance was already high at baseline (such as use of steroids in over 85% of cases where indicated), but even when there was more room for improvement (such as in quality of medical history taking), there was no significant additional net effect of SPI1. There were no changes over time or between control and SPI1 hospitals in errors or rates of adverse events in patients in medical wards. Mortality increased from 11% (27) to 16% (39) among controls and decreased from 17% (63) to 13% (49) among SPI1 hospitals, but the risk adjusted difference was not significant (0.5, 0.2 to 1.4; P=0.085). Poor care was a contributing factor in four of the 178 deaths identified by review of case notes. The survey of patients showed no significant differences apart from an increase in perception of cleanliness in favour of SPI1 hospitals. Conclusions The introduction of SPI1 was associated with improvements in one of the types of clinical process studied (monitoring of vital signs) and one measure of staff perceptions of organisational climate. There was no additional effect of SPI1 on other targeted issues nor on other measures of generic organisational strengthening. (shrink)

A number of research projects have recently taken up the challenge of formulating large-scale models of brain mechanisms at unprecedented levels of detail. These research enterprises have raised lively debates in the press and in the scientific and philosophical literature, some of them revolving around the question whether the incorporation of so many details in a theoretical model and in a computer simulations of it is really needed for the model to be explanatory. Is there a “right” level of detail? (...) In this article I analyse the claim, made by two leading neuroscientists, according to which the content of the why-question addressed and the amount of computational resources available constrains the choice of the most appropriate level of detail in brain modelling. Based on the recent philosophical literature on scientific explanation, I distinguish between two kinds of details, called here mechanistic decomposition and property details, and argue that the nature of the why-question provides only partial constraints to the choice of the most appropriate level of detail under the two interpretations of the term considered here. Greek culture matters because its unique pluralistic debate shaped modern discourses. This ground-breaking book explains this feature by retelling the history of ancient literary culture through the lenses of canon, space and scale. It proceeds from the invention of the performative 'author' in the archaic symposium through the 'polis of letters' enabled by Athenian democracy and into the Hellenistic era, where one's space mattered and culture became bifurcated between Athens and Alexandria. This duality was reconfigured into an eclectic variety consumed (...) by Roman patrons and predicated on scale, with about a thousand authors active at any given moment. As patronage dried up in the third century CE, scale collapsed and literary culture was reduced to the teaching of a narrower field of authors, paving the way for the Middle Ages. The result is a new history of ancient culture which is sociological, quantitative, and all-encompassing, cutting through eras and genres. (shrink)
