The term “Electronic Health Records” means something different to each of the stakeholders in health care, but it always seems to carry a degree of emotional baggage. Increasingly, EHRs are advertized as a nearly unmitigated good that will transform medical care, improve safety and efficiency, allow better patient engagement, and open the door to an era of cheap, effective, timely, and patient-centered care. Indeed, for some EHR proponents the benefits of adopting them are so obvious that adoption has become an (...) end in itself. But for others — and especially for a number of skeptical practitioners and patients — EHR is a code word that portends the corporate transformation of health care delivery, the loss of patient privacy, the demand that patients bear more responsibility in health care, and the unreflective takeover of the health care system by people who do not understand medical care or how health care relationships unfold. According to nearly all theorists writing on the subject, a certain derogatory content is regularly and systematically communicated by slurs. So united, the theorists disagree sharply on the elements of this content, on its provenance, and on its mechanism. I argue that the basic premiss of all these views, that there is any such derogatory content conveyed with the use of slurs, is highly dubious.

A number of EU institutions and government committees across Europe have expressed interest in developing methods and decision-support tools to facilitate consideration of the ethical dimensions of biotechnology assessment. As part of the work conducted in the EC supported project on ethical tools (Ethical Bio-TA Tools), a number of ethical frameworks with the potential to support the work of public policy decision-makers has been characterized and evaluated. One of these potential tools is the Delphi method. The Delphi method was originally (...) developed to assess variables that are intangible and/or shrouded in uncertainty by drawing on the knowledge and abilities of a diverse group of experts through a form of anonymous and iterative consultation. The method has hitherto been used by a diversity of practitioners to explore issues such as technology assessment, environmental planning, and public health measures. From the original (classical) Delphi, a family of Delphi-related processes has emerged. As a result of the evaluation of the various Delphi processes, it is proposed that the classical method can be further developed and applied as a form of ethical framework to assist policy-makers. Through a series of exercises and trials, an Ethical Delphi has been developed as a potential approach for characterizing ethical issues raised by the use of novel biotechnologies. Advantages and disadvantages of the method are discussed. Further work is needed to develop the procedural aspects of the Ethical Delphi method and to test its use in different cultural contexts. However, utilizing an ethical framework of this type combines the advantages of a methodical approach to capture ethical aspects with the democratic virtues of transparency and openness to criticism. Ethical frameworks such as the Ethical Delphi should contribute to better understanding of and decision-making on issues that involve decisive ethical dimensions. (shrink)

Individuals frequently make use of the body and environment when engaged in a cognitive task. For example, individuals will often spontaneously physically rotate when faced with rotated objects, such as an array of words, to putatively offload the performance costs associated with stimulus rotation. We looked to further examine this idea by independently manipulating the costs associated with both word rotation and array frame rotation. Surprisingly, we found that individuals’ patterns of spontaneous physical rotations did not follow patterns of performance (...) costs or benefits associated with being physically rotated, findings difficult to reconcile with existing theories of strategy selection involving external resources. Individuals’ subjective ratings of perceived benefits, rather, provided an excellent match to the patterns of physical rotations, suggesting that the critical variable when deciding on-the-fly whether to incorporate an external resource is the participant's metacognitive beliefs regarding expected performance or the effort required for each approach. Implications for metacognition's future in theories of cognitive offloading are discussed. (shrink)

In this paper we introduce canonical extensions of partially ordered sets and monotone maps and a corresponding discrete duality. We then use these to give a uniform treatment of completeness of relational semantics for various substructural logics with implication as the residual(s) of fusion.

Interoception can be broadly defined as the sense of signals originating within the body. As such, interoception is critical for our sense of embodiment, motivation, and well-being. And yet, despite its importance, interoception remains poorly understood within modern science. This paper reviews interdisciplinary perspectives on interoception, with the goal of presenting a unified perspective from diverse fields such as neuroscience, clinical practice, and contemplative studies. It is hoped that this integrative effort will advance our understanding of how interoception determines well-being, (...) and identify the central challenges to such understanding. To this end, we introduce an expanded taxonomy of interoceptive processes, arguing that many of these processes can be understood through an emerging predictive coding model for mind–body integration. The model, which describes the tension between expected and felt body sensation, parallels contemplative theories, and implicates interoception in a variety of affective and psychosomatic disorders. We conclude that maladaptive construal of bodily sensations may lie at the heart of many contemporary maladies, and that contemplative practices may attenuate these interpretative biases, restoring a person’s sense of presence and agency in the world. The implicational fragment of the logic of relevant implication, $R_{\to}$ is one of the oldest relevance logics and in 1959 was shown by Kripke to be decidable. The proof is based on $LR_{\to}$ , a Gentzen-style calculus. In this paper, we add the truth constant $\mathbf{t}$ to $LR_{\to}$ , but more importantly we show how to reshape the sequent calculus as a consecution calculus containing a binary structural connective, in which permutation is replaced by two structural rules that involve $\mathbf{t}$ (...) . This calculus, $LT_\to^{\text{\textcircled{$\mathbf{t}$}}}$ , extends the consecution calculus $LT_{\to}^{\mathbf{t}}$ formalizing the implicational fragment of ticket entailment . We introduce two other new calculi as alternative formulations of $R_{\to}^{\mathbf{t}}$ . For each new calculus, we prove the cut theorem as well as the equivalence to the original Hilbert-style axiomatization of $R_{\to}^{\mathbf{t}}$ . These results serve as a basis for our positive solution to the long open problem of the decidability of $T_{\to}$ , which we present in another paper. (shrink)

Bioethics is gradually becoming an important part of the drive to increase quality healthcare delivery in sub‐Saharan African countries. Yet many healthcare service‐users in Africa are familiar with incidences of questionable health policies and poor healthcare delivery, leading to severe consequences for patients. We argue that the overarching rights‐based ethical administrative framework recently employed by healthcare authorities contributes to the poor uptake and enforcement of current normative tools. Taking Ghana as a case study, we focus on the cultural ethical context (...) and we tease out the concepts of the good and the ethical among the Akan and Bulsa ethnic groups. We point out three tenets towards building a normative framework that can resonate with service‐users and practitioners: ontological communitarianism; empathic humanism; and virtuous character. Finally, we indicate how these core tenets can be dovetailed into building an effective normative framework and into the training of healthcare providers. (shrink)

One central and unfortunately unavoidable characteristic of the aging process is its association with chronic physiological deterioration. Frailty, cognitive impairment, and physical conditions such as cardiovascular disease and vision and hearing loss are more frequent in this phase of life, and these conditions translate into an increasing need for care and support of multiple kinds. In traditional bioethical scholarship, these distinctive features of aging have been examined predominantly through a health‐focused lens. My main contention in this essay, however, is that (...) viewing aging within bioethics as primarily a health problem, to be addressed through frameworks for decision‐making or for resource allocation, is inadequate. My aim is to consider how the health conditions associated with aging affect older people’s lives in a much more expansive way than has typically been acknowledged. Just as importantly, I intend to show how shifting our bioethical imagination in this way raises different and challenging questions about what a good life in late life consists in and about what is owed to older people, in their personal and social lives, as a matter of justice. Surrogate (proxy) decision makers must make research decisions for people with dementia who lack decision-making capacity. Proxies’ decision-making processes are minimally understood. We randomly assigned 82 proxies of AD patients to informed consent for one of three hypothetical protocols with differing levels of risk and benefit. Proxies answered questions about potential benefits of the described research to the patient and society, as well as about whether they would enroll their relative and why or why not. Proxies interested in enrolling their (...) relative cited the potential for direct benefit to their relative, altruism, and trust in researchers. Those declining cited risks, inconvenience, and stage of illness. Proxies weighed numerous factors, incorporating both substituted judgment and best interests standards in their decision-making processes. Although further empirical work is needed to understand the influences on and adequacy of proxies’ decision making regarding research, these findings can help inform policy regarding surrogate consent. Feminist transitional justice has greatly contributed to the study of justice in the ruins of war, notably around prosecuting wartime rape. At the same time, scholars have observed limitations to this research agenda such as externally-driven definitions gendered harms and how to address them. This paper explores two novel areas for feminist TJ research: ‘everyday gendered harms’ and customary justice. Based on a three month field study of baraza, a customary justice mechanism in parts of South Kivu, Democratic Republic of (...) Congo, I explore three cases of ‘everyday’ harms against women: domestic violence, polygynous relationships and witchcraft. Through a substantive examination of these baraza cases, I highlight how studying the contextualised functioning of customary justice mechanisms provides new insights into different areas of feminist TJ scholarship, including women’s participation in the transition, justice for harms against women, and advancing gender equality. Additionally, this paper adds to the broader TJ literature by engaging with local TJ needs as they pertain to people’s everyday life in transition. (shrink)

Urquhart works in several areas of logic where he has proved important results. Our paper outlines his topological lattice representation and attempts to relate it to other lattice representations. We show that there are different ways to generalize Priestley’s representation of distributive lattices—Urquhart’s being one of them, which tries to keep prime filters in the representation. Along the way, we also mention how semi-lattices and lattices figured into Urquhart’s work.

A ternary relation is often used nowadays to interpret an implication connective of a logic, a practice that became dominant in the semantics of relevance logics. This paper examines two early manuscripts --- one by Routley, another by Meyer --- in which they were developing set-theoretic semantics for various relevance logics. A standard presentation of a ternary relational semantics for, let us say, the logic of relevant implication R is quite illuminating, yet the invention of this semantics was fraught with (...) false starts. Meyer's manuscript, in which he builds on some ideas from Routley's manuscript, essentially contains a relational semantics for which R^{ot} is sound and complete. (shrink)

Values are inescapable. They pervade and shape our psychology, our agency and our lives as reflective and self-knowing subjects. This book explores the crucial ways in which values figure within reflection and thereby shape our theoretical and practical lives, against the backdrop of an expressivist moral psychology that is sensitive to the vicissitudes of valuing.

In this paper, we examine the interactive effects of positive affect and perspective-taking on workplace incivility and family incivility, through moral disengagement. We draw from broaden-and-build and moral disengagement theories to suggest a potential negative consequence of positive affect. Specifically, we argue that positive affect increases incivility toward coworkers and spouses through moral disengagement among employees with low, but not high perspective-taking. Data from two time-lagged field studies and one online experiment provide support for our hypotheses. These findings suggest that (...) the beneficial effects of positive feelings are not universal, and the fostering of positive feelings at work might have unintended negative consequences, namely moral disengagement, and increased incivility at work and at home. Relevance logics are known to be sound and complete for relational semantics with a ternary accessibility relation. This paper investigates the problem of adequacy with respect to special kinds of dynamic semantics (i.e., proper relation algebras and relevant families of relations). We prove several soundness results here. We also prove the completeness of a certain positive fragment of R as well as of the first-degree fragment of relevance logics. These results show that some core ideas are shared between relevance logics (...) and relation algebras. Some details of certain incompleteness results, however, pinpoint where relevance logics and relation algebras diverge. To carry out these semantic investigations, we define a new tableaux formalization and new sequent calculi (with the single cut rule admissible) for various relevance logics. (shrink)

The world of the Na'vi is much more feminine. Na'vi women are equal partners with their men and are just as capable as their male counterparts. And as the tsahìk (spiritual leader) of the Omaticaya clan, Neytiri's mother Mo'at exercises an unrivalled degree of power and influence due to her ability to interpret the will of Eywa, the Na'vi's female deity. Historically, women are the ones who have had the most intimate experience of care, since they have traditionally been the (...) ones tasked with providing it for children, the sick, the disabled, and the elderly. The philosopher Sara Ruddick (1935–2011), one of the pioneers of care ethics, identified three activities at the core of what she calls “maternal work”: preserving the life of the child, fostering the growth of the child, and training the child for social acceptability. Sometimes we are interested in how groups are doing epistemically in aggregate. For instance, we may want to know the epistemic impact of a change in school curriculum or the epistemic impact of abolishing peer review in the sciences. Being able to say something about how groups are doing epistemically is especially important if one is interested in pursuing a consequentialist approach to social epistemology of the sort championed by Goldman. According to this approach we evaluate social practices and institutions (...) from an epistemic perspective based on how well they promote the aggregate level of epistemic value across a community. The aim of this paper is to investigate this concept of group epistemic value and defend a particular way of measuring it. (shrink)

Medical advances in newborn care have raised ethical dilemmas as to the level of care which is appropriate when an infant is severely damaged and incapable of a reasonable quality of life in the future. Some of the considerations, philosophical and practical, are discussed. It is concluded that within the framework of the law and after due consultation, decisions are best reached in privacy by the parents and their medical advisor and should be based on the perceived interests of the (...) child. As my time writing scripture reflections for this journal has drawn to an end, it is a good opportunity to reflect more theoretically about the nature of homiletic preaching today. My first peer-reviewed publication was on this topic. Since then I have returned on occasion to investigate preaching in the early Christian centuries both on its own terms qua preaching and as source material for theological expression. It is a matter worthy of fresh reflection, because in the twenty years since (...) the publication of the article in 'Worship' there have been new documents issued from Rome, which in turn have given rise to fresh statements from bishops' conferences. The question is: has there been much change in the way the nature of a homily is understood over those past twenty years, and how does that align with the 'Worship' article? The argument advanced here is that these recent documents reveal a multiplicity of understandings of the purpose of the Sunday liturgical homily. (shrink)
