There are opportunities in practice for practitioners to sustain the citizenship of the people with whom they work. These opportunities arise as a matter of everyday decision-making, in the ways that service users and their families are facilitated to participate in decisions affecting their lives. Citizenship also hinges on the organisation of services to meet the needs of service users and carers. In this article, a care situation which fails to meet the needs of one family is examined using an (...) ethics of care. A social worker reflects on her role in the decision-making and the eventual lack of commitment from a wider team to provide good care. To answer yes to the question of whether others matter, we need to consider whether the moral acheivement has been accomplished; that is to show in practice, that others do matter. (shrink)

Ethical research practice requires inclusionary approaches that enable people to contribute as fully as possible. Not enough is yet known about the impacts of dementia on daily life, however, people with dementia may find inclusion in research challenging, as the ‘cognitive load’ required may be overwhelming. When responding is difficult, others may contribute and the voice of people with dementia may be diminished. In this paper, the method of walking interviews is reflected on following a study that examined the acceptability (...) and usefulness of Global Positions Systems (GPS). Attention is drawn to an observation of the contributions people with dementia made whilst out walking with the researchers. When out walking, people with dementia used the environment as sensory prompts to start conversations, and these discussions shaped research data, and enabled people with dementia to raise concerns about the impacts of dementia, their futures and what they feared for themselves and their families. The challenges that people with dementia faced in negotiating everyday practices were visible. The person with dementia showed the researcher around their neighbourhood, and this significantly changed the interview dynamic and positioned people with dementia as leading the interaction. Social death is apparent when people are considered unworthy of social participation and deemed to be dead when they are alive. Some marginalized groups are more susceptible to this treatment than others, and one such group is people with dementia. Studies into discrimination towards older people are well documented and serve as a source of motivation of older people’s social movements worldwide. Concurrently, theories of ageing and care have been forthcoming in a bid to improve the quality of responses to (...) older people in times of need. Included in this theorizing has been the analysis of values and approaches that paid carers convey to citizens who require their help. In this article, the values and approaches of social workers and mental health nurses bring to people with dementia are considered within the context of social life and social death. It is based on a small study that undertook to critically examine how participation of people with dementia was facilitated. A thanatological lens was used to interpret inclusive and exclusive practices which potentially create opportunity for participation or reinforce the loss of citizenship for older people with dementia. This article draws on two studies that have used an ethic of care analysis to explore lay, nursing and social work care for people with dementia. It discusses the political as well as the practice application of ethic of care principles and highlights the necessity to understand both what people do and the meanings with which such practices are imbued in order to identify `good care' and the relationship between this and social justice. Examples of care for people with dementia (...) are discussed by reference to core principles of an ethic of care: attentiveness, responsibility, competence, responsiveness and trust. These illustrate the potential for the development of a shared language within which different disciplines, lay carers and people with dementia can communicate about how needs could best be met in complex and difficult circumstances. In crisis situations, the authority of the nurse is legitimised by legal powers and professional knowledge. Crisis stakeholders include those who directly use services and their families, and a wide range of health, social service and justice agencies. Alternative strategies such as therapeutic risk taking from the perspective of socially inclusive recovery policy coexist in a sometimes uneasy relationship with mental health legislation. A critical discourse analysis was undertaken to examine mental health policies and guidelines, and we interviewed service users, (...) families, nurses and the police about experiences of accessing services. For those who attempt to access services early in crisis, as is suggested to lead to a better outcome, provision of services and rights appear to be reversed by an attempt to exclude them through practices that screen them out, rather than prioritising a choice in access. In this paper, we explore the idea of digital technologies as truth‐bearers in health care and argue that devices like SenseCam, which facilitate reflection and memory recall, have a potentially vital role in healthcare situations when questions of veracity are at stake (e.g., when best interest decisions are being made). We discuss the role of digital technologies as truth‐bearers in the context of nursing people with dementia, as this is one area of health care in which the topic of truth‐telling (...) has been hotly debated. People with dementia have been excluded from research studies and decisions that affect their lives because they are not regarded as truth‐bearers—that is, as being capable of giving truthful accounts of their experiences. Also, considerable research has focused on the ethics of lying to and deceiving people with dementia. Given their increasing prominence in healthcare settings, there has been surprisingly little discussion of what role digital technologies might play in relation to these questions of truth and deception. Drawing on theories from science and technology studies (STS), we explore their possible future role in some of the truth‐making processes of health care. In particular, we discuss the potential value of constraints on use of SenseCam to support the accounts of people with dementia as part of their care. (shrink)

Una búsqueda en la historia de las publicaciones de la provincia de Camagüey, fundamentalmente revistas y periódicos que incluyeron artículos de carácter científico entre las décadas del treinta y el cincuenta del siglo XX, es propósito esencial de este trabajo. El sondeo realizado permitió la valoración de la figura de la primera doctora camagüeyana en Medicina Gertrudis Aguilera Céspedes y su actuación descollante en este período. Para ello se revisaron los documentos que atesoran la Sala de Fondos Raros y Valiosos (...) de la Biblioteca Provincial “Julio Antonio Mella”, el Departamento de Investigaciones del Museo Provincial “Ignacio Agramonte” y bibliotecas particulares de algunos médicos y profesionales del territorio. La búsqueda condujo a la demostración del impacto que en el orden científico representó esta galena y además, una contribución para preservar la memoria cultural de nuestra ciudad A search in the Camagüey´s publications, mainly articles with scientific purposes during the thirty's up to the fifty's in the XX Century, is covered under the scope of this work. The study was carried out to value the figure of the first female doctor in Medicine Gertrudis Aguilera Cespedes and her outstanding performance at that time. Some journals and documents from different libraries were revised. The search led to the demonstration of the impact that Tula Aguilera represented for this scientific field, at the same time this is a contribution to preserve the cultural memory of Camagüey city. En el artículo se describe el tránsito filosófico-existencial de Tula, protagonista de la novela La tía Tula de Miguel de Unamuno, por los tres estadios vitales del filósofo danés Søren Kierkegaard. A lo largo del texto, se muestra que las dudas existenciales que se plantea la protagonista son extensivas para todo el género humano. Además, el texto busca mostrar cómo la narrativa del pensador español está plagada de reflexiones filosóficas y está fuertemente influida por el existencialismo cristiano de (...) Kierkegaard. Debatiéndose entre la fe y la razón, Tula transita por los estadios tomando decisiones que entrañan sufrimiento. En esta novela, se plasman los afanes del ser humano por la perpetuación, inmortalidad y salvación frente al rostro descarnado de la nada. (shrink)

La intención de este trabajo consiste en perfilar algunos de los fundamentos filosóficos que envuelven la agonía existencial que se percibe a lo largo de la obra La Tía Tula de Miguel de Unamuno. Para ello hemos comparado el pensamiento de este autor con el de Kierkegaard, siendo la idea de inmortalidad la que sirve de vínculo entre ambos. Desde esta perspectiva de análisis la lucha entre fe y razón, expuesta por Unamuno, deja claro que el anhelo de inmortalidad (...) se enfrenta a categorías universales, mientras que para Kierkegaard la fe está por encima de la razón ante toda decisión por absurda que parezca. Se realizó una investigación en sistemas y servicios de salud de tipo descriptiva transversal, con el objetivo de elaborar un sistema de acciones de capacitación para el profesional de la enfermería comunitaria en la prevención del embarazo en la adolescencia. Se aplicaron métodos teóricos y empíricos propios de la investigación científica. El universo lo constituyeron 20 profesionales de enfermería que laboran en consultorios del Área Salud "Tula Aguilera". La muestra quedó conformada por los 12 profesionales que aceptaron participar en (...) el estudio. El criterio de exclusión fue la libre decisión de cada individuo para participar o retirarse de la investigación. Se aplicó una encuesta estandarizada, previo consentimiento informado, para determinar el nivel de conocimientos sobre salud reproductiva en la adolescencia. Los resultados constatados reflejaron que la mayoría de los encuestados tenían criterios estrechos, no fundamentados científicamente ni actualizados sobre el tema, por lo que se diseñó un sistema de acciones de capacitación basado en conocimientos científicos actualizados, pertinentes y contextualizados. Cada actividad se propuso demostrar la factibilidad e importancia de utilizar técnicas participativas y la fundamentación de las ventajas de su uso en diferentes actividades comunitarias, encaminadas a enfatizar en la organización, movilización, concientización y capacitación a los diferentes sujetos sociales involucrados en la prevención del embarazo en la adolescencia, para que estos se conviertan en transformadores de su propia realidad, en correspondencia con sus necesidades y demandas sociales. A descriptive transversal study was carried out on health systems and services aiming at designing a system of training actions for the community professional nurses in order to prevent pregnancy in adolescence. Theorical and empirical methods were applied. The universe was made up of twenty professionals who consented to participate in the study. The inclusion criterion was the free decision of each individual on either participating or not in the investigation. A standard survey was applied -with a previous informed consent , to determine the knowledge about reproductive health in adolescence. The results revealed that most of the surveyed people had narrow criteria with no updated scientific grounds on the issue, thus a system of training actions based on updated scientific pertinent contextualized information was designed Each activity aimed at demonstrating the feasibility study of using participative techniques as well as the reasons for the advantages of their use in a variety of community activities to emphasize the organization and understanding of the different social subjects involved in the prevention of pregnancy in adolescence.
