A growing body of research has demonstrated significant heterogeneity of hospital ethics committee (HEC) size, membership and training requirements, length of appointment, institutional support, clinical and policy roles, and predictors of self identified success. Because these studies have focused on HECs at a single point in time, however, little is known about how the composition of HECs changes over time and what impact these changes have on committee utilization. The current study presents 20 years of data on the evolution of (...) the Massachusetts General Hospital HEC. Between 1993 and 2012, the average number of committee members per year was 38 ± 3 and the average length of membership was 4.8 ± 0.4 years. During that time, the committee performed 934 consults, averaging 47 ± 3 per year. Attendance rates fell from 61.5 to 23.8 % over the study period and were inversely correlated with the total number of members. Between 1993 and 2012, the committee saw substantial growth in the diversity of the professional backgrounds of its members. Multivariate analysis, however, suggests that substantial changes in committee composition did not impact its utilization and that other factors are more likely to explain fluctuations in consultation volume. We describe the structure, operation, and experience of the Massachusetts General Hospital ethics committee, formally called the Edwin H. Cassem Optimum Care Committee, from January 2007 through December 2013. Founded in 1974 as one of the nation’s first hospital ethics committees, this committee has primarily focused on the optimum use of life-sustaining treatments. We outline specific sociodemographic and clinical characteristics of consult patients during this period, demographic differences between the adult inpatient population and patients for whom the ethics committee was (...) consulted, and salient features of the consults themselves. We include three case studies that illustrate important consult themes during this period. Our findings expand knowledge about the structure and workings of hospital ethics committees and illustrate how one ethics committee has developed and utilized policies on end-of-life care. More generally, we model a sociological approach to the study of clinical ethics consultation that could be utilized to contextualize institutional practices over time. (shrink)

Roch Kereszty long enjoyed thinking about how, and how much, we can discover the truth about Jesus of Nazareth through historical research into his earthly life. Fr. Roch also often enjoyed indicating that at least part of the answer is that research about a human being can never be content with (...) descriptions of any number of empirical facts or ideas from that person's life. The true goal of one who writes or reads about the life of another is insight into the meaning of that person's life. Such insight is possible some extent while the person's life is ongoing, but it calls for a more decisive kind of assessment after the person's death. The meaning of a life also cannot truly reveal itself when viewed through the lens of an inadequate philosophy or theology. Indeed, Fr. Roch tended to think that philosophy itself is adequate to show that any quest to understand another human being cries out to be structured by love, or at least some form of empathy: I can truly understand another only in my proportion to my ability to discover communion or affinity with that person, to find in him or her another self who is in some deep way similar to my own self.1 [End Page 781] Although Fr. Roch elaborated such ideas to guide historians seeking to write a "biography" of Jesus, and although they make great demands of many kinds, it nevertheless seems fitting to consider them when beginning to memorialize Fr. Roch himself.Last December, God granted Fr. Roch the gift of a passage from this life that was surrounded by the love of many people who had known him on earth. Among them were many who had known him through his theological writings and had found them to offer a special combination of intellectual vigor and personal relevance. It is a blessing now to be able to offer a short memorial for the benefit of his sympathetic readers of the past and the future. Attempting to explain the golden thread that unites his many books and articles seems unreasonable; the writings themselves remain their own best explanation. Such an approach could also perhaps suggest that we are most loved in our productivity rather than in the ways we are dependent on God and others. For both of these reasons, one good way to serve love and communion with a theologian of interest would be to portray him in some of the ways that he himself was loved, fathered forth and shaped, by the One from whom all fatherhood is named. In this essay, then, I offer a simple and very fragmented portrait of the sources of a theologian, the fontes theologi: the different ways that God fathered in Roch Kereszty a theological writer who was a blessing to people who read periodicals such as this one. The new National Institutes of Health (NIH) Policy on the Use of a Single Institutional Review Board (sIRB) for Multi-Site Research was adopted primarily to simplify and speed the review of complex multisite clinical trials. However, speeding review requires overcoming a number of obstacles. Perhaps the most substantial obstacle is the time and effort needed to develop reliance agreements among the participating sites. We conducted 102 semistructured interviews with sIRB personnel, including directors, chairs, reviewers, and staff, from 20 IRBs that (...) acted as sIRBs for multisite research, including 6 commercial/independent sIRBs, and 10 university-based academic and 4 federal sIRBs. Almost without exception, the interviewees agreed that reliance agreements were complex, difficult to develop, and time-consuming. A major problem for relying sites was that different agreements specified different responsibilities for the relying sites. Attitudes differed about whether these problems will be resolved as IRB staff and managers become more experienced with sIRBs. However it is clear that the process of developing reliance agreements must be simplified. Federal assistance in standardizing at least some sections of reliance agreements might reduce the difficulties involved. (shrink)

Overdiagnosis is an emerging problem in health policy and practice: we address its definition and ethical implications. We argue that the definition of overdiagnosis should be expressed at the level of populations. Consider a condition prevalent in a population, customarily labelled with diagnosis A. We propose that overdiagnosis is occurring in respect of that condition in that population when the condition is being identified and labelled with diagnosis A in that population ; this identification and labelling would be accepted as (...) correct in a relevant professional community; but the resulting label and/or intervention carries an unfavourable balance between benefits and harms. We identify challenges in determining and weighting relevant harms, then propose three central ethical considerations in overdiagnosis: the extent of harm done, whether harm is avoidable and whether the primary goal of the actor/s concerned is to benefit themselves or the patient, citizen or society. This distinguishes predatory, misdirected and tragic overdiagnosis; the degree of harm moderates the justifiability of each type. We end with four normative challenges: methods for adjudicating between professional standards and identifying relevant harms and benefits should be procedurally just; individuals, organisations and states are differently responsible for addressing overdiagnosis; overdiagnosis is a matter for distributive justice: the burdens of both overdiagnosis and its prevention could fall on the least-well-off; and communicating about overdiagnosis risks harming those unaware that they may have been overdiagnosed. Viewing workplace deviance within a victim precipitation framework, we explore how abusive supervisors target subordinates low in core self-evaluations to explain when such employees respond by engaging in workplace deviance. We theorize that employees who are lower in CSE receive more abusive supervision, which generates subsequent harmful reactions toward supervisors, peers, and the organization. This occurs primarily when employees lack sufficient cognitive resources in dealing with supervisor abuse. We test, replicate, and extend our theoretical model in three empirical studies. Results (...) demonstrate that lower employee CSE drew more abusive supervision and led low-CSE employees to exhibit workplace deviance. This abusive supervision mediation effect was stronger for employees with comparatively lower cognitive ability levels. PurposePrevious studies examining the relationship between health-related quality of life and speech perception ability in cochlear implant users have yielded variable results, due to a range of factors, such as a variety of different HRQoL questionnaires and CI speech testing materials in addition to CI configuration. In order to decrease inherent variability and better understand the relationship between these measures in CI users, we administered a commonly used clinical CI speech testing battery as well as two popular HRQoL questionnaires in (...) bimodal and bilateral CI users.MethodsThe Glasgow Benefit Inventory, a modified five-factor version of the GBI, and the Nijmegen Cochlear Implant Questionnaire were administered to 25 CI users. Speech perception abilities were measured with the AzBio sentence test in several conditions.ResultsHigher performance scores on the GBI general subscore were related to greater binaural speech perception ability in noise. There were no other relationships between the GBI or NCIQ and speech perception ability under any condition. Scores on many of the GBI-5F factors were substantially skewed and asymmetrical; therefore, correlational analyses could not be applied. Across all participants, binaural speech perception scores were greater than first-ear CI only scores.ConclusionThe GBI general subscore was related to binaural speech perception, which is considered the everyday listening condition of bimodal and bilateral CI users, in noise; while the more CI-specific NCIQ did not relate to speech perception ability in any listening condition. Future research exploring the relationships between the GBI, GBI-5F, and NCIQ considering bimodal and bilateral CI configurations separately is warranted. (shrink)

Aim Facebook is an increasingly popular online social networking site. The purpose of this study was to describe the Facebook activity of residents and fellows and their opinions regarding the impact of Facebook on the doctor–patient relationship. Methods An anonymous questionnaire was emailed to 405 residents and fellows at the Rouen University Hospital, France, in October 2009. Results Of the 202 participants who returned the questionnaire (50%), 147 (73%) had a Facebook profile. Among responders, 138 (99%) displayed their real name (...) on their profile, 136 (97%) their birthdates, 128 (91%) a personal photograph, 83 (59%) their current university and 76 (55%) their current position. Default privacy settings were changed by 61% of users, more frequently if they were registered for >1 year (p=0.02). If a patient requested them as a ‘friend’, 152 (85%) participants would automatically decline the request, 26 (15%) would decide on an individual basis and none would automatically accept the request. Eighty-eight participants (48%) believed that the doctor–patient relationship would be altered if patients discovered that their doctor had a Facebook account, but 139 (76%) considered that it would change only if the patient had open access to their doctor's profile, independent of its content. Conclusions Residents and fellows frequently use Facebook and display personal information on their profiles. Insufficient privacy protection might have an impact the doctor–patient relationship. Sua (...) teoria que posteriormente seria chamada de materialismo histórico dialético pelos seus interpretes, seria uma síntese da dialética hegeliana e do materialismo de Feuerbach, sendo concebida como um conjunto de leis que explicam a evolução da natureza como uma relação determinada por elementos da economia, sociedade e história. Essa idéia utópica compartilhada por Marx, Engels e seus sucessores foi refutada por desenvolvimentos subsequentes da filosofia. Recently, two apparently quite different duality-based approaches to automata minimisation have appeared. One is based on ideas that originated from the controllability-observability duality from systems theory, and the other is based on ideas derived from Stone-type dualities specifically linking coalgebras with algebraic structures derived from modal logics. In the present paper, we develop a more abstract view and unify the two approaches. We show that dualities, or more generally dual adjunctions, between categories can be lifted to dual adjunctions between categories (...) of coalgebras and algebras, and from there to automata with initial as well as final states. As in the Stone-duality approach, algebras are essentially logics for reasoning about the automata. By exploiting the ability to pass between these categories, we show that one can minimize the corresponding automata. We give an abstract minimisation algorithm that has several instances, including the celebrated Brzozowski minimisation algorithm. We further develop three examples that have been treated in previous works: deterministic Kripke frames based on a Stone-type duality, weighted automata based on the self-duality of semimodules, and topological automata based on Gelfand duality. As a new example, we develop alternating automata based on the discrete duality between sets and complete atomic Boolean algebras. (shrink)

Recent evidence suggests that probabilistic grammars may be modulated by communication mode and genre. Accordingly, the question arises how complex language users’ lectal competence is, where complexity is proportional to the extent to which choice-making processes depend on the situation of language use. Do probabilistic constraints vary when we talk to a friend compared to when we give a speech? Are differences between spoken and written language larger than those within each mode? In the present study, we aim to approach (...) these questions systematically. Guided by theorizing in cognitive (socio)linguistics and using logistic regression based on corpus materials, we analyzed the dative alternation with give (The government gives farmers money vs. The government gives money to farmers) in four broad registers of English: spoken informal, spoken formal, written informal, and written formal. Corpus analysis was supplemented with a scalar rating experiment. Results suggest that language users’ probabilistic grammars vary as a function of register. (shrink)

Over the last decades, changes within higher education have created increased pressure and uncertainty for academics, increasing their risk for cognitive, behavioral, physical, as well as psychological issues due to high job demands. Specifically, for new academics in teaching roles, their lack of knowledge and skills can contribute to a negative effect of these job demands on their well-being. This study therefore explored how teaching-related professional development programs can enhance new university teachers’ well-being, through semi-structured interviews with 10 university teachers (...) participating in such a program at a mid-sized Dutch university. We pay special attention to the relationship between specific learning activities integrated in the program and various dimensions of the psychological model of well-being by Ryff and Keyes. Using co-occurrence analysis and content analysis, we found that different learning activities had distinct relationships with different well-being facets. For example, formal workshops were mainly related to environmental mastery, a purpose in life and personal growth, while reflecting seemed to be especially connected to teachers’ self-acceptance, and participating in a learning community was mainly related to positive relations with others and personal growth. Our findings have implications for research on teacher well-being as well as for the design of professional development programs for higher education teaching staff. (shrink)

Framed within theories of fairness and stress, the current paper examines bystanders’ intervention intention to workplace bullying across two studies based on international employee samples (N = 578). Using a vignette-based design, we examined the role of bullying mode (offline vs. online), bullying type (personal vs. work-related) and target closeness (friend vs. work colleague) on bystanders’ behavioural intentions to respond, to sympathise with the victim (defender role), to reinforce the perpetrator (prosecutor role) or to be ambivalent (commuter role). Results illustrated (...) a pattern of the influence of mode and type on bystander intentions. Bystanders were least likely to support the victim and more likely to agree with perpetrator actions for cyberbullying and work-related acts. Tentatively, support emerged for the effect of target closeness on bystander intentions. Although effect sizes were small, when the target was a friend, bystanders tended to be more likely to act and defend the victim and less likely to reinforce the perpetrator. Implications for research and the potential for bystander education are discussed. (shrink)

Cet article propose la première publication complète d’un monument consacré par les Thespiens aux Muses dans leur sanctuaire au pied de l’Hélicon, dont les éléments constitutifs sont connus depuis le xixe s., et notamment depuis les fouilles de P. Jamot au Val des Muses. Les statues en bronze de petite taille des neuf Muses se dressaient sur une base en arc de cercle d’une ampleur de ca 10 m, placée dans un écrin architectural largement ouvert sur l’extérieur. Un nouveau lustre (...) est donné à ce groupe statuaire hellénistique exceptionnel à l’époque d’Auguste par la gravure, sous chacune des figures, d’une épigramme descriptive et érudite signée par le poète corinthien Honestos. Est ainsi conservé l’un des rares exemples archéologiques de recueil de poèmes décrivant des œuvres d’art figuré. L’analyse des épigrammes et l’observation détaillée des mortaises de fixation visibles à la face supérieure des blocs de la base permettent d’émettre des hypothèses sur les types utilisés pour la représentation des Muses, mais aussi d’assurer, au‑delà de tout doute, que les statues prenaient place au milieu d’ornements végétaux en bronze qui évoquaient le paysage du Val des Muses. La représentation de la nature comme élément non plus secondaire, mais essentiel d’un groupe sculpté correspond aux recherches nouvelles de la basse époque hellénistique. Faute de fouilles exhaustives du sanctuaire des Muses, l’emplacement du monument ne peut être déterminé avec certitude. Des lettres de remontage nous apprennent en revanche qu’il a été déplacé, peut‑être à l’époque où furent gravés les poèmes d’Honestos. (shrink)

This study examined the extent to which concussion management plans at National Collegiate Athletic Association member schools were in line with NCAA Concussion Policy and best practice recommendations in absence of any process to ensure compliance. Most schools' concussion management plans were in compliance with 3 or 4 of the NCAA's 4 required components. Annual athlete education and acknowledgement was the requirement least often included, representing an area for improvement. Further, schools tended to more often include best practices that were (...) more medically-oriented, compared to best practices that were less medical in nature. (shrink)

Tissue plasminogen activator (tPA) is administered to patients with suspected ischemic stroke to improve blood flow to the brain In rare cases, patients present with complaints of stroke symptoms that appear to be non-organic due to malingering, factitious disorder, or conversion disorder (psychogenic stroke mimics). Deciding whether or not to administer tPA to these patients can be challenging. The risk of hemorrhage after administration of tPA is low, but not zero. The ethical principles of beneficence and nonmaleficence need to be (...) weighed carefully in these situations. We present two cases of patients with suspected psychogenic stroke mimics to illustrate the ethical challenges faced in identifying and managing psychogenic stroke mimics. Further research is needed to demonstrate effective treatment strategies for patients with acute stroke symptoms of psychogenic etiology. (shrink)

Aristotle's endeavor, at least insofar as we can judge from the way it has been transmitted to us and from the titles of the lost works, is often presented as the first work of an encyclopedic nature, as it seems to embrace and order all of the elements of knowledge. Does Aristotle not advance a classification of sciences, in Metaphysics, E, 1, as well as a systematic outline of the “sciences of nature” in his Meteorologica, I, 1? And again, although (...) logic is often presented as not belonging to the system of sciences since it is not counted as a science in Metaphysics, E, 1, is Aristotle not generally considered the inventor of this discipline? The impression is all the stronger in that Andronicus of Rhodes, in the first century b.c., edited Aristotle's works by roughly adopting the threefold division of philosophy into Logic-Physics-Ethics, a division that had become common during the Hellenistic period though more particularly proper to the Stoic system. Background The first COVID-19 wave started in February 2020 in France. The influx of patients requiring emergency care and high-level technicity led healthcare professionals to fear saturation of available care. In that context, the multidisciplinary Ethics- Support Cell (EST) was created to help medical teams consider the decisions that could potentially be sources of ethical dilemmas. Objectives The primary objective was to prospectively collect information on requests for EST assistance from 23 March to 9 May 2020. The secondary aim was (...) to describe the Cell’s functions during that period. Research design This observational, real-time study of requests for Cell consultations concerned ethical dilemmas arising during a public health crisis. The EST created a grid to collect relevant information (clinical, patient’s/designated representative’s preferences and ethical principles strained by the situation), thereby assuring that each EST asked the same questions, in the same order. Participants and research context Only our university hospital’s clinicians could request EST intervention. Ethical considerations The hospital Research Ethics Committee approved this study (no. CER-2020-107). The patient, his/her family, or designated representative was informed of this ethics consultation and most met with EST members, which enabled them to express their preferences and/or opposition. Findings/results 33 requests (patients’ mean age: 80.8 years; 29 had COVID-19: 24 with dyspnea, 30 with comorbidities). 17 Emergency Department solicitations concerned ICU admission, without reference to resource constraints; others addressed therapeutic proportionality dilemmas. Discussion Intervention-request motives concerned limited resources and treatment intensity. Management revolved around three axes: the treatment option most appropriate for the patient, the feasibility of implementation, and dignified care for the patient. Conclusions COVID-19 crisis forced hospitals to envisage prioritization of ICU access. Established decision-making criteria and protocols do not enable healthcare professionals to escape ethical dilemmas. That acknowledgement highlights ethical risks, enhances the added-value of nursing and encourages all players to be vigilant to pursue collective deliberations to achieve clear and transparent decisions. Previous research shows that females use more exclamation marks than males, often to establish rapport. The purpose of the present studies was to test whether people associate texters’ use of exclamation marks with friendliness and femaleness. If this association is due to normative expectations, we hypothesized that females would appear less friendly if they did not use an exclamation mark in texting. In Study 1, participants rated a texter using an exclamation mark to be highly female and highly friendly. The (...) gender results disappeared when friendliness was controlled for. In Study 2, we tested whether friendliness ratings decreased if texters violated gender-associated punctuation. Participants rated a texter with a gendered name on friendliness. Regardless of gender, participants inferred greater friendliness to texters using an exclamation mark. That is, there was no evidence of a cost for violating this gender expectation. We conclude that people predict that a texter using an exclamation mark is likely to be female, but do not penalize females for not using an exclamation mark. (shrink)

En este artículo, emergente de pensadores (...) ambientales latinoamericanos y pensadores-otros que han sospechado radicalmente del edificio de la ética ambiental emergente de la conferencia del Club de Roma o del Informe Burndtland, intentamos desplegar lo ético, como ethos con e larga, que en griego significa casa, y desde este lugar de enunciación, intentamos salirnos del sujeto y del objeto en la variación ética ambiental eurocentrista, y proponer con las voces de la tierra sur, abya yala, diseños del ethos, ligado óntica-relacionalmente con el cuerpo-tierra. Diseñamos, inspirados en Husserl, Heidegger, Deleuze, Guattari, Ángel-Maya, Enrique Leff, Noguera, Echeverri, pensadores-otros y voces de montañas, animales, ríos, y plantas, la vida, territorios de libertad entendida ésta como expansión de los cuerpos en la naturaleza y de paz integral y completa, entendiendo que no puede haber paz, si no amamos ni respetamos la tierra que somos. Los primeros con el resurgimiento del marxismo que a partir de la tesis humanista de la filosofía de la praxis revive desde Latinoamérica el legado de Mariátegui, el Che Guevara y Camilo Torres R. El artículo se pregunta si el paradigma del marxismo humanista arroga (...) los fundamentos del buen vivir o vivir bien para enfrentar al capitalismo neoliberal y su proyecto desarrollista. La aproximación al tema se hace de manera cualitativa y descriptiva. A través del método de análisis del discurso político expuesto en la teoría crítica al desarrollismo se aborda la caracterización del capitalismo neoliberal y el modelo de desarrollo, continuando con las tesis que desde el marxismo humanista y la estrategia de revolución cultural se estructura para la emancipación en Latinoamérica. Finalmente, se exponen los planteamientos del buen vivir a partir de la apropiación que hacen los neomarxistas para dar la batalla contra el capitalismo neoliberal. (shrink)

We argue that planning and control may not be separable entities, either at the behavioural level or at the neurophysiological level. We review studies that show the involvement of superior and inferior parietal cortex in both planning and control. We propose an alternative view to the localization theory put forth by Glover.

The late antique and early medieval Mediterranean was characterized by wide-ranging cultural and linguistic diversity. Yet, under the influence of Christianity, communities in the Mediterranean world were bound together by common concepts of good rulership, which were also shaped by Greco-Roman, Persian, Caucasian, and other traditions. This collection of essays examines ideas of good Christian rulership and the debates surrounding them in diverse cultures and linguistic communities. It grants special attention to communities on the periphery, such as the Caucasus and (...) Nubia, and some essays examine non-Christian concepts of good rulership to offer a comparative perspective. As a whole, the studies in this volume reveal not only the entanglement and affinity of communities around the Mediterranean but also areas of conflict among Christians and between Christians and other cultural traditions. By gathering various specialized studies on the overarching question of good rulership, this volume highlights the possibilities of placing research on classical antiquity and early medieval Europe into conversation with the study of eastern Christianity. In contrast to the history of science and to science and technology studies, the value discourse in the philosophy of science has not provided a thorough analysis of the material culture of science. Instruments in science have a special characteristic, namely that they explicitly and clearly emerge from and remain embedded in social contexts, and are thus imbued with values. We argue that the materials (in most cases they are artifacts) used in science are necessarily influenced by both epistemic and (...) non-epistemic considerations. A consequence of this is that a descriptive term cannot give sufficient information whether an artifact is performing in an acceptable way. Instead of the prevailing descriptive approach, we therefore advocate a normative notion of values in the material culture of science. To this end, we connect the material culture of science to the so-called “new demarcation problem”, in order to lay the foundations for a value-sensitive approach to the analysis of instruments. By assessing the five approaches of demarcation concerning value-influences, it will be shown that they break down at various points if the material aspects of science are taken seriously. (shrink)
