Introduction: Clinical policies control several aspects of clinical practice, including individual treatment and care, resource management and healthcare professionals’ etiquette. This article presents Clinical Policy Ethics Assessment Tool, an ethical assessment tool for clinical policies that could be used not only by clinical ethics committees but also by policy committees or other relevant groups. Aim: The aim of this study was to find or create a tool to identify ethical issues and/or confirm ethical validity in nursing practice policies, protocols and (...) guidelines. Methodology: The development of Clinical Policy Ethics Assessment Tool involved first a literature review, followed by modification of the Research Protocol Ethics Assessment Tool, which was created to identify research protocols’ ethical issues, and finally, a trial of Clinical Policy Ethics Assessment Tool to ensure its reliability and validity. Ethical consideration: The policies analysed trialling Clinical Policy Ethics Assessment Tool were in the public domain and did not contain any confidential information. Despite that, Clinical Policy Ethics Assessment Tool also had the approval of a research ethics committee. Results: Research Protocol Ethics Assessment Tool was chosen as the template for a Clinical Policy Ethics Assessment Tool, to which several modifications were added to adapt it to work within a nursing practice context. Clinical Policy Ethics Assessment Tool was tested twice, which resulted in a general test–retest reliability coefficient = 0.86, r = 0.84, α1 = 0.817, α2 = 0.824 and interclass correlation coefficient = 0.874. Discussion: Contemporary nursing practice in a developed country is often ruled by clinical policies. The use of Clinical Policy Ethics Assessment Tool could confirm the ethical validity of those clinical practice policies, impacting on nurses’ education, values and quality of care. Conclusion: Clinical Policy Ethics Assessment Tool has the potential to detect ethical issues and facilitate the correction and improvement of clinical policies and guidelines in a structured way. This is especially so as it has shown reliability in detecting issues in clinical policies involving human participants and in encouraging policymakers to consider common ethical dilemmas in nursing practice. Introduction: Nurses who work in an emergency department regularly care for acute patients in a fast-paced environment, being at risk of suffering high levels of burnout. This situation makes them especially vulnerable to be accountable for decisions they did not have time to consider or have been pressured into. Research objective: The objective of this study was to find which factors influence ethical, legal and professional accountability in nursing practice in an emergency department. Research design: Data were analysed, codified and (...) triangulated using qualitative ethnographic content analysis. Participants and research context: This research is set in a large emergency department in the Midlands area of England. Data were collected from 186 nurses using participant observation, 34 semi-structured interviews with nurses and ethical analysis of 54 applicable clinical policies. Ethical considerations: Ethical approval was granted by two research ethics committees and the National Health Service Health Research Authority. Results: The main result was the clinical nursing accountability cycle model, which showed accountability as a subjective concept that flows between the nurse and the healthcare institution. Moreover, the relations among the clinical nursing accountability factors are also analysed to understand which factors affect decision-making. Discussion: The retrospective understanding of the factors that regulate nursing accountability is essential to promote that both the nurse and the healthcare institution take responsibility not only for the direct consequences of their actions but also for the indirect consequences derived from previous decisions. Conclusion: The decision-making process and the accountability linked to it are affected by several factors that represent the holistic nature of both entities, which are organised and interconnected in a complex grid. This pragmatic interpretation of nursing accountability allows the nurse to comprehend how their decisions are affected, while the healthcare institution could act proactively to avoid any problems before they happen. In this 2017 conversation, Gianni Vattimo discusses with Paolo Diego Bubbio the core themes of his own philosophical journey. Vattimo first comments on the legacy of his mentor Luigi Pareyson and on the differences between Pareyson’s conception of the relation between truth and interpretation and his own. Vattimo and Bubbio then elaborate on the return to Hegel and the possibility of a “hermeneuticized” Hegelianism. The participants also discuss Vattimo’s view of religion and the role that the Christian notion of (...) caritas plays in his “weak hermeneutics.” Finally, Vattimo comments on his recent political writings and on his view of a “hermeneutic communism,” arguing that revolution is possible only as a collective inner transformation. The Baldwin effect is a process by which learnt traits become gradually incorporated into the genome through a Darwinian mechanism. From its inception, the Baldwin effect has been regarded with skepticism. The objective of this paper is to relativize this assessment. Our contribution is two-fold. To begin with, we provide a taxonomy of the different arguments that have been advocated in its defense, and distinguish between three justificatory dimensions—feasibility, explanatory relevance and likelihood—that have been unduly conflated. Second, we sharpen the (...) debate by providing an evolutionary game theoretic perspective that is able to generalize previous results. The upshot of this paper is that the mechanism envisaged by Baldwin is less puzzling than commonly thought. (shrink)

Multiple realization occurs when a natural kind is variably realized at more basic levels and the common physical structure of the realizers is not essential for supporting nomological statements. It has been suggested that this phenomenon may be an outcome of natural selection acting over multiple realizers that perform an adaptive function. In this paper, we make the following contributions. First, we present a revision of this model, characterized by stricter equilibrium conditions and superior explanatory power. Second, we present a (...) typology of multiple realization that provides a plausible account of the differences between across- and within-species multiple realization. In diesem Band deckt Diego D'Angelo semiotische Strukturen in der Husserl’schen Phänomenologie der Wahrnehmung auf. Ist es der Phänomenologie darum zu tun, die Erfahrung von Dingen in unserer Umwelt zu beschreiben, so ist dabei der Begriff des Horizontes von zentraler Bedeutung: Was wir unmittelbar wahrnehmen, verweist immer schon auf anderes, was nur „mitgegeben“ ist. Wenn wir Dinge wahrnehmen, haben wir nur eine bestimmte Perspektive, d.h. wir sehen lediglich einen Aspekt. Aber wir nehmen immer ganze Gegenstände wahr (wir sehen Tische (...) und Stühle und andere Menschen). Jeder dieser Gegenstände erscheint in einem Feld weiterer Gegenstände, und es ist der Horizontbegriff, der es erlaubt, das Verhältnis zwischen Selbstgegebenheit und Mitgegebenheit zu explizieren. Dieses Buch stellt den ersten detaillierten Versuch dar, die Ursprünge solcher horizontaler Felder in semiotischen Strukturen zu suchen. Aus der Verbindung zwischen Husserls eigener Semiotik und seiner Phänomenologie der Wahrnehmung ergibt sich, dass das wahrgenommene Phänomen als Zeichen verstanden werden muss. Das Zeichen wiederum bezeichnet etwas, was in leiblicher Bewegung eingeholt werden kann. Durante la última década del siglo XX y los primeros años del nuevo milenio, las instituciones públicas y/o de gobierno han pasado por un proceso de ajuste y cambio debido al arribo de las técnicas gerenciales provenientes del sector privado conocidas como Nueva Gestión Pública. Dichas técnicas han generado, por un lado, una modificación sustantiva en la forma de organizar, estructurar y operar los servicios públicos y, por otro, algunos cambios en la conducta de los servidores públicos pero desviándose de (...) la esencia de servicio público original. Suicidal men feel the need to be self-reliant and that they cannot find another way out of relationship or socioeconomic issues. Suicide prevention is of crucial importance worldwide. The much higher rate of suicide in men engenders action. The prelude is a subjective experience that can be very isolating and severely distressing. Men may not realize a change in their thinking and behaviors, which makes it more difficult to seek and get help, thereby interrupting a “downward spiral”. Stoicism often prevents (...) men from admitting to their personal struggle. The lack of “quality” connections and “non-tailored” therapies has led to a high number of men “walking out” on traditional clinical approaches. But there are complicated relationships in motivations and formative behaviors of suicide with regards to emotional state, psychiatric disorders, interpersonal life events and suicidal behavior method selection. Middle-aged and older men have alternated as the most at-risk of suicide. There is no one solution that applies to all men, but digital tools may be of assistance. Digital interventions require higher levels of effectiveness for distress and suicidality but self-guided approaches may be the most suitable for men especially where linked with an integrated online suicide prevention platform. Furthermore, technology-enabled models of care offer promise to advance appropriate linking to mental health services through better and faster understanding of the specific needs of individuals and the type and level of suicidality experienced. Long-term evidence for suicidality and its evaluation may benefit from progressing human computer-interaction and providing impetus for an eminent integrated digital platform. (shrink)

La información se obtuvo a partir de talleres, diarios de campo y autorrelatos de docentes participantes, representantes de 19 programas de pregrado de la Universidad Mariana, Pasto. El análisis se realizó mediante categorías deductivas e inductivas, de donde se extrajo las proposiciones que dan consistencia a la comprensión de la pedagogía franciscana desde la teoría de los campos de Bourdieu. El aporte logrado se resume en una mirada integradora y novedosa de la pedagogía franciscana, (...) la resignificación de las prácticas pedagógicas de los docentes, a partir de relatos y experiencias que configuran el corpus teórico de la constitución del campo, capital y habitus, desde la perspectiva franciscana para la formación en capacidades humanas. La actitud del diseño es una estrategia interpretativa propuesta por Dennett, que consiste en tratar al sistema, cuyo “comportamiento” se quiere predecir, bajo el supuesto de que sus partes cumplen funciones que obedecen a un diseño satisfactorio. Sin embargo, estudios recientes sobre atribución funcional en artefactos técnicos consideran que la actitud del diseño supone una estrategia inadecuada para entender qué hace un artefacto técnico. En particular, dos críticas han ganado visibilidad. Por una parte, Vermaas et al. (2013) señalan una inconsistencia (...) en la propuesta dennettiana de la actitud del diseño: las intenciones del diseñador a veces desempeñan un papel en la interpretación de los artefactos y a veces no lo hacen, sin tener finalmente una intuición clara sobre su rol. Por otra parte, Vaesen y Van Amerongen (2008) sostienen que la actitud del diseño admite dos interpretaciones, una descriptiva y una normativa, y que ambas son implausibles. No obstante, en este trabajo argumentamos que estas críticas son rebatibles y que el problema reside en que ambas acusaciones, i.e. inconsistencia e implausibilidad, suponen una interpretación incorrecta de la posición de Dennett. The design attitude is an interpretive strategy proposed by Dennett, which consists of treating the system, whose “behavior” is to be predicted, under the assumption that its parts fulfill functions that accomplish a satisfactory design. Recent studies on technical artifacts' functional attribution claim that the design attitude is an inadequate strategy to elicit what a technical artifact does. In particular, two criticisms have gained visibility. On the one hand, Vermas et al (2013) remark an inconsistency in Dennett’s design stance. The very designer's intentions sometimes play a role in the interpretation of artifacts and sometimes they don´t, without finally having a clear intuition about their role. On the other hand, Vaesen and Van Amerongen (2008) argue that the design stance admits two interpretations, a descriptive and a normative one and that both are implausible. However, we argue that these criticisms miss the point given that both accusations, i.e. of inconsistency and of implausibility, result from a wrong interpretation of Dennett's position. (shrink)

Zonulin is a physiological modulator of intercellular tight junctions, which upregulation is involved in several diseases like celiac disease (CeD). The polyQ gliadin fragment binds to the CXCR3 chemokine receptor that activates zonulin upregulation, leading to increased intestinal permeability in humans. Here, we report a general hypothesis based on the structural connection between the polyQ sequence of the immunogenic CeD protein, gliadin, and enteric coccidian parasites proteins. Firstly, a novel interaction pathway between the parasites and the host is described based (...) on the structural similarities between polyQ gliadin fragments and the parasite proteins. Secondly, a potential connection between coccidial infections as a novel environmental trigger of CeD is hypothesized. Therefore, this report represents a promising breakthrough for coccidian research and points out the potential role of coccidian parasites as a novel trigger of CeD that might define a preventive strategy for gluten‐related disorders in general. Resumen: El artículo aborda uno de los modos de localización de entidades en situaciones espaciales estáticas: la deíxis espacial. Nuestro objetivo principal es describir la estructura y uso del sistema demostrativo en espacio de escala menor en la lengua hablada por los gitanos de Chile: el rromané jorajané. Para la obtención de los datos se utilizó The 1999 Demonstrative Questionnaire: ‘THIS’ and ‘THAT’ in comparative perspective. Los resultados del análisis destacan que los demostrativos espaciales del rromané jorajané constituyen un grupo (...) altamente simétrico, que combina un componente semántico deíctico, junto con contenidos gramaticales. Se trata de un sistema demostrativo binario en cuanto a la categoría de distancia respecto del centro deíctico. Se concluye también que la selección del demostrativo apropiado obedece al siguiente patrón: el demostrativo proximal tiene su área focal en la zona próxima al hablante, mientras que el demostrativo distal incluye en su campo de aplicación tanto la zona próxima al oyente como la zona distante al hablante.: This paper addresses one way of locating entities in static spatial situations: the spatial deixis. Our main purpose is to describe the structure and use of the demonstrative system in small-scale space in the language spoken by Gypsies in Chile: the rromané jorajané. Data collection was performed with “The 1999 Demonstrative Questionnaire: ‘THIS’ and ‘THAT’ in comparative perspective”. The analysis results highlight that spatial demonstratives in rromané jorajané are a highly symmetrical group combining a deictic semantic component with grammatical contents. It is a binary demonstrative system in terms of the category of distance from the deictic center. Additionally, it is concluded that choice of the suitable demonstrative proceeds according to the following pattern: the proximal demonstrative has its focal area in the speaker’s zone of proximity, while the distal demonstrative includes both the addressee’s zone of proximity and the speaker’s distal zone within its application field. In this paper we study the semantic data complexity of several controlled fragments of English designed for natural language front-ends to OWL (Web Ontology Language) and description logic ontology-based systems. Controlled languages are fragments of natural languages, obtained by restricting natural language syntax, vocabulary and semantics with the goal of eliminating ambiguity. Semantic complexity arises from the formal logic modelling of meaning in natural language and fragments thereof. It can be characterized as the computational complexity of the reasoning problems associated (...) to their semantic representations. Data complexity (the complexity of answering a question over an ontology, stated in terms of the data items stored therein), in particular, provides a measure of the scalability of controlled languages to ontologies, since tractable data complexity implies scalability of data access. Más aún, en la medida en que toda ‘declaración’ demanda una ejecución, su pretendido estatuto trascendental queda puesto en tensión. There is abundant empirical evidence on the negative effects of discrimination on psychological well-being. However, little research has focused on exploring the factors that can mitigate this effect. Within this framework, the present study examined the mediating role of positive and negative affects in the relationship between ethnic and racial discrimination and psychological well-being in the migrant population. About 919 Colombians, first-generation migrants, residing in Chile were evaluated, of which 50.5% were women, and the participants’ average age was 35 years. (...) Krieger’s discrimination questionnaires, Watson’s Positive and Negative Affect Schedule, and Ryff’s Psychological Well-Being Scale were applied. The measurement models of each variable were estimated, and then the structural equation models were used. The results of the hypothesized multiple mediation model showed that the main mediator in the relationship between ethnic-racial discrimination and psychological well-being was positive affects over negative ones. This paper concerns the Aristotelian inquiry on φαντασία’ in De Anima iii 3. I argue for a systematic interpretation of the chapter, according to which iii 3 neatly instantiates what David Charles has called the Three Stage View on scientific inquiry. The first stage establishes the meaning of the term φαντασία so it provides a nominal definition of the object, the second stage dialectically confirms the existence of φαντασία as something different from other already known cognitive powers (perception, thought), the (...) third stage posits a hypothetical essence (real definition), then it tests its power of enabling us to derive from such an essence those manifest and apparent features of φαντασία, that had been previously individuated. On the basis of such reading, some general suggestions on the status and role of φαντασία are also proposed, on the basis of which we can take the definition of iii 3 to be consistent with any other explanatory uses of φαντασία in the corpus. Partiendo de un nuevo documento que fecha la conversión de la antigua mezquita de Bāb al-Mardūm en capilla en 1183, un siglo después de la conquista de Toledo por Alfonso VI, se ha tratado de indagar en el proceso de cristianización de las mezquitas urbanas y rurales de Toledo entre los siglos XI y XIII: el papel desempeñado en su reparto y el de sus habices por el Rey, la Iglesia y los diferentes grupos de población, el destino de (...) aquellas mezquitas que no se consagraron, o las semejanzas y las particularidades del proceso toledano respecto al llevado a cabo en otras regiones andalusíes. Para acabar, se plantean algunas preguntas e hipótesis sobre la función y el carácter de la propia mezquita, fundada por Aḥmad ibn al-Ḥadīdī en el año 390/999-1000. (shrink)

Esa temporalidad es la de las formas de un poder destituyente articulado en distintas organizaciones e iniciativas, como el movimiento piquetero, las asambleas, las formas de justicia de los escraches, las ollas populares, etc. Las preguntas que recorren el texto son, entre otras, qué pasó con las figuras comunitarias en los años de los gobiernos kirchneristas, cuáles (...) fueron las memorias del 2001, y cómo se activa la política “por abajo” ante el escenario de una nueva crisis y la reaparición del “que se vayan todos”, pero esta vez como marco del ascenso de la derecha. Durante la última década, en Argentina ha sido posible ubicar al formatoblog como un disparador de pruebas, polémicas y análisis en torno a autores y lectores dentro del campo literario nacional. La repercusión crítica llevó a revisitar algunas prácticas, tanto en lo que respecta a la producción de obras como a las funciones de autor y lector, y sus modos de interacción en un momento histórico determinado. El objetivo que proponemos, a partir de la aparición y usos del blog por (...) parte de escritores argentinos,se orienta a tratar de conocer cómo se ha insertado el formato en el campo literario y, en otro nivel, qué cambios o continuidades han marcado su aparición en relación a los formatos convencionales de la cultura impresa, y a la producción de ficción en las generaciones emergentes de autores. El objeto de análisis es el trabajo de escritores consagrados o en vías de consagración que comenzaron a administrar espacios personales y llevan a cabo un mantenimiento periódico de sus sitios en la web. Nos preguntamos: ¿por qué un escritor consagrado o en vías de consagración, con una obra editada, elige mantener un espacio virtual? La respuesta a este interrogante exige un abordaje de numerosas cuestiones de campo. Aquí nos dedicamos a una de esas aristas, concentrada en las reflexiones sobre las formas que han admitido las llamadas escrituras del yo, íntimas, y las narrativas vivenciales o biográficas. This book examines how Shanghai aims to improve public service provision by accelerating the use of artificial intelligence (AI) in the public sector. After clarifying the technical and social factors that shape the use of AI in this area, the book delves into the AI policy environment and AI ecosystem of Shanghai to gauge the city’s capacity to implement public sector AI applications. Then it examines how this capacity translates into real-world policy initiatives through the investigation of case studies. By using the findings on Shanghai to gain a deeper understanding of key topics in public sector AI research, this book also contributes new knowledge about the use of AI to improve the provision of public services. En este artículo se realiza una relectura de las inscripciones de carácter religioso de las puertas de la Mezquita de Córdoba en época califal. A pesar de que no todas se han conservado, el estudio de conjunto reveló suficientes elementos singulares como para sugerir nuevas hipótesis en torno al sentido y la intencionalidad de los textos coránicos escogidos. Las inscripciones, tanto interiores como exteriores, conforman un programa epigráfico perfectamente coherente y dotado de un complejo significado ligado al contexto político, social (...) y religioso de la época. El programa incluye asimismo alusiones a la polémica anti-cristiana en el siglo x. En ese sentido, la mezquita cordobesa constituye un valioso documento histórico del Califato omeya de al-Andalus. The purpose of the present paper is twofold. First, to examine what beliefs, if any, underlie (a) the Pyrrhonist’s desire for ataraxia and his account of how this state may be attained, and (b) his philanthropic therapy, which seeks to induce, by argument, ejpochv and ataraxia in the Dogmatists. Second, to determine whether the Pyrrhonist’s philanthropy and his search for and attainment of ataraxia are, as scholars have generally believed, essential aspects of his stance.

Partiendo de un nuevo documento que fecha la conversión de la antigua mezquita de Bāb al-Mardūm en capilla en 1183, un siglo después de la conquista de Toledo por Alfonso VI, se ha tratado de indagar en el proceso de cristianización de las mezquitas urbanas y rurales de Toledo entre los siglos XI y XIII: el papel desempeñado en su reparto y el de sus habices por el Rey, la Iglesia y los diferentes grupos de población, el destino de (...) aquellas mezquitas que no se consagraron, o las semejanzas y las particularidades del proceso toledano respecto al llevado a cabo en otras regiones andalusíes. This paper explores the notion of reciprocity in the context of active pulmonary and laryngeal tuberculosis treatment and related control policies and practices. We seek to do three things: First, we sketch the background to contemporary global TB care and suggest that poverty is a key feature when considering the treatment of TB patients. We use two examples from TB care to explore the role of reciprocity: isolation and the use of novel TB drugs. Second, we explore alternative means of (...) justifying the use of reciprocity through appeal to different moral and political theoretical traditions. We suggest that each theory can be used to provide reasons to take reciprocity seriously as an independent moral concept, despite any other differences. Third, we explore general meanings and uses of the concept of reciprocity, with the primary intention of demonstrating that it cannot be simply reduced to other more frequently invoked moral concepts such as beneficence or justice. We argue that reciprocity can function as a mid-level principle in public health, and generally, captures a core social obligation arising once an individual or group is burdened as a result of acting for the benefit of others. We conclude that while more needs to be explored in relation to the theoretical justification and application of reciprocity, sufficient arguments can be made for it to be taken more seriously as a key principle within public health ethics and bioethics more generally. Suppose that a team of neurosurgeons and bioengineers were able to remove your brain from your body, suspend it in a life-sustaining vat of liquid nutrients, and connect its neurons and nerve terminals by wires to a supercomputer that would stimulate it with electrical impulses exactly like those it normally receives when embodied. According to this brain-in-a-vat thought experiment, your envatted brain and your embodied brain would have subjectively indistinguishable mental lives. For all you know—so one argument goes—you could be (...) such a brain in a vat right now.1 Daniel Dennett calls this sort of philosophical thought experiment an “intuition pump” (Dennett 1995). An intuition pump is designed to elicit certain intuitive convictions, but is not itself a proper argument: “intuition pumps are fine if they’re used correctly, but they can also be misused. They’re not arguments, they’re stories. Instead of having a conclusion, they pump an intuition. They get you to say ‘Aha! Oh, I get it!’ (Dennett 1995, p. 182). Philosophers have used the brain-in-a-vat story mainly to raise the problem of radical skepticism and to elicit various intuitions about meaning and knowledge (Putnam 1981). The basic intuition the story tries to pump is that the envatted brain, though fully conscious, has systematically false beliefs about the world, including itself. The concept of dignity has occasioned a robust conversation in recent healthcare scholarship. When viewed as a whole, research on dignity in healthcare has engaged each of the four bioethical principles popularized by Beauchamp and Childress, but has paid the least attention to beneficence. In this paper, we look at dignity and beneficence. We focus on the dignity promotion component of a model of dignity derived from a grounded theory study. After describing the study and presenting a précis of the (...) resulting model, we review the principle of beneficence and look at the ways in which the notion of dignity promotion can be used to complement our understanding of this principle. Specifically, we explore what we can learn from dignity promotion about the relational nature of beneficence in healthcare and how dignity promotion can be marshaled to help address the epistemological quandary of soft paternalism. South Africa shared with the world the warning of a new strain of SARS-CoV2, Omicron, in November 2021. As a result, many high-income countries (HICs) instituted complete travel bans on persons leaving South Africa and other neighbouring countries. These bans were unnecessary from a scientific standpoint, and they ran counter to the International Health Regulations. In short, South Africa was penalized for sharing data. Data sharing during pandemics is commonly justified by appeals to solidarity. In this paper, we argue that (...) solidarity is, at best, an aspirational ideal to work toward but that it cannot ground an obligation to share data. Instead, low-and-middle income countries (LIMCs) should be guided by the principle of reciprocity, which states that we ought to return good for good received. Reciprocity is necessarily a conditional principle. LMICs, we argue, should only share data during future pandemics on the condition that HICs provide enforceable assurances that the benefits of data sharing will be equitably distributed and that LMICs won’t be penalized for sharing information.
