This discussion paper aims to explore potential ethical and moral implications of (patient) centredness in nursing and healthcare. Healthcare is experiencing a philosophical shift from a perspective where the health professional is positioned as the expert to one that re‐centres care and service provision central to the needs and desires of the persons served. This centred approach to healthcare delivery has gained a moral authority as the right thing to do. However, little attention has been given to its moral and (...) ethical theoretical grounding and potential implications for nurses, persons served and the healthcare system. Based upon a review of academic and grey literature, centredness is proposed as a value‐laden concept in nursing inquiry. Potential moral and ethical implications of centredness on nurses/healthcare providers, persons served and the healthcare system are discussed. These challenges are then considered within the context of normative and relational ethical theories. These perspectives may offer guidance relative to how one should act in those circumstances as well as an understanding as to how interdependency and engagement with the other person(s) can help navigate the challenges of a centred care approach. Viewing centredness through an ethical theoretical lens provides a valuable discourse to nursing in efforts to expand the knowledge base and integrate centred approaches into practice and policy. As nursing professionals, we believe access to healthcare is fundamental to health and that it is a determinant of health. Therefore, evidence suggesting access to healthcare is problematic for many Indigenous peoples is concerning. While biomedical perspectives underlie our current understanding of access, considering alternate perspectives could expand our awareness of and ability to address this issue. In this paper, we critique how access to healthcare is understood through a biomedical lens, how a postcolonial theoretical lens can extend that understanding, (...) and the subsequent implications this alternative view raises for the nursing profession. Drawing on peer‐reviewed published and gray literature concerning healthcare access and Indigenous peoples to inform this critique, we focus on the underlying theoretical lens shaping our current understanding of access. A postcolonial analysis provides a way of understanding healthcare as a social space and social relationship, presenting a unique perspective on access to healthcare. The novelty of this finding is of particular importance for the profession of nursing, as we are well situated to influence these social aspects, improving access to healthcare services broadly, and among Indigenous peoples specifically. Digital Pills are an innovative drug-device technology that permits to combine traditional medications with a monitoring system that automatically records data about medication adherence as well as patients’ physiological data. Although DP are a promising innovation in the field of digital medicine, their use has also raised a number of ethical concerns. These ethical concerns, however, have been expressed principally from a theoretical perspective, whereas an ethical analysis with a more empirically oriented approach is lacking. There is also a lack (...) of clarity about the empirical evidence available concerning the application of this innovative digital medicine. To map the studies where DP have been tested on patients and discuss the ethically relevant issues evident therein, we performed a scoping review of the empirical literature concerning DP. Our search allowed us to identify 18 papers reporting on studies where DP were tested on patients. These included studies with different designs and involving patients with a variety of conditions. In the empirical literature, a number of issues with ethical relevance were evident. At the patient level, the ethical issues include users’ interaction with DP, personal sphere, health-related risks and patients’ benefits. At the provider level, ethically relevant issues touch upon the doctor-patient relationship and the question of data access. At the societal level, they concern the benefits to society, the quality of evidence and the dichotomy device-medicine. We conclude that evidence concerning DP is not robust and that more research should be performed and study results made available to evaluate this digital medicine. Moreover, our analysis of the ethically relevant aspects within empirical literature underscores that there are concrete and specific open questions that should be tackled in the ethical discussion about this new technological solution. Recent studies of semantic memory have investigated two theories of optimal search adopted from the animal foraging literature: Lévy flights and marginal value theorem. Each theory makes different simplifying assumptions and addresses different findings in search behaviors. In this study, an experiment is conducted to test whether clustering in semantic memory may play a role in evidence for both theories. Labeled magnets and a whiteboard were used to elicit spatial representations of semantic knowledge about animals. Category recall sequences from a (...) separate experiment were used to trace search paths over the spatial representations of animal knowledge. Results showed that spatial distances between animal names arranged on the whiteboard were correlated with inter-response intervals during category recall, and distributions of both dependent measures approximated inverse power laws associated with Lévy flights. In addition, IRIs were relatively shorter when paths first entered animal clusters, and longer when they exited clusters, which is consistent with marginal value theorem. In conclusion, area-restricted searches over clustered semantic spaces may account for two different patterns of results interpreted as supporting two different theories of optimal memory foraging. ABSTRACTRumination and worry are two perseverative, negatively valenced thought processes that characterise depressive and anxiety disorders. Despite significant research interest, little is known about the everyday precipitants and consequences of rumination and worry. Using an experience sampling methodology, we examined and compared rumination and worry with respect to their relations to daily events and affective experience. Participants diagnosed with Major Depressive Disorder, Generalized Anxiety Disorder, co-occurring MDD–GAD, or no diagnosis carried an electronic device for one week and reported on rumination, (...) worry, significant events, positive affect, and negative affect. Across the clinical groups, occurrences of everyday events predicted subsequent increases in rumination, but not worry. Further, higher momentary levels of rumination, but not worry, predicted subsequent decreases in PA and increases in NA. Thus, in these clinical groups, rumination was more susceptible to... This commentary focuses on four major points: (1) “Tacit knowledge” is not a complete explanation for imagery phenomena, if it is an explanation at all. (2) Similarities and dissimilarities between imagery and perception are entirely consistent with the depictive view. (3) Knowledge about the brain is crucial for settling the debate. (4) It is not clear what sort of theory Pylyshyn advocates.

As scholars have shown, acceptance is key to the success of Payment for Ecosystem Services (PES) scheme. While many studies adopt a static cost-benefit perspective, few address the social process leading to acceptance. Drawing on Suchman (1995), this article examines the legitimacy process underlying the acceptance of a PES in agriculture. In particular, the role of collective action in the legitimisation process is analysed, using a combination of qualitative and quantitative methods of discourse analysis. Data from an agro-environmental PES scheme (...) in France on water quality shows that acceptance depends on the normative and cognitive legitimacy that actors confer upon a public policy. (shrink)

The cause of the problems is an underlying lack of clarity about the relation between the operation of the subpersonal vision module and visual perception at the level of the subject or person.
