We report on the deliberations of an interdisciplinary group of experts in science, law, and philosophy who convened to discuss novel ethical and policy challenges in stem cell research. In this report we discuss the ethical and policy implications of safety concerns in the transition from basic laboratory research to clinical applications of cell-based therapies derived from stem cells. Although many features of this transition from lab to clinic are common to other therapies, three aspects of stem cell biology pose (...) unique challenges. First, tension regarding the use of human embryos may complicate the scientific development of safe and effective cell lines. Second, because human stem cells were not developed in the laboratory until 1998, few safety questions relating to human applications have been addressed in animal research. Third, preclinical and clinical testing of biologic agents, particularly those as inherently complex as mammalian cells, present formidable challenges, such as the need to develop suitable standardized assays and the difficulty of selecting appropriate patient populations for early phase trials. We recommend that scientists, policy makers, and the public discuss these issues responsibly, and further, that a national advisory committee to oversee human trials of cell therapies be established. **NB we did not reccommend a NAC, we think it might be appropriate**. If stem cell-based therapies are developed, we will likely confront a difficult problem of justice: for biological reasons alone, the new therapies might benefit only a limited range of patients. In fact, they might benefit primarily white Americans, thereby exacerbating long-standing differences in health and health care.

ABSTRACT A number of philosophers have argued that alcoholics should receive lower priority for liver transplantations because they are morally responsible for their medical conditions. In this paper, I argue that this conclusion is false. Moral responsibility should not be used as a criterion for the allocation of medical resources. The reason I advance goes further than the technical problem of assessing moral responsibility. The deeper problem is that using moral responsibility as an allocation criterion undermines the functioning of medicine.

In recent times Wittgenstein's work in logic has had an influence on other branches of philosophy. I am thinking, in particular, of social philosophy and the philosophy of religion. In these branches, Wittgenstein's followers have made much use of his notion of a language game. It has been argued, for example, that religion forms a language game of its own, having its own standards of reason, and is therefore not subject to criticism from outside. This argument has given rise to (...) controversy, some seeing it as a subtle attempt by the religious to evade criticism. I have come myself to feel that the notion of a language game has been put to uses with which Wittgenstein himself might not have agreed, or, if he had, would have been wrong to do so. In order to explain what I mean I should like to look closely at the opening section of Peter Winch's article ‘Understanding a Primitive Society’. (shrink)

Nocebo effects occur when an individual experiences undesirable physiological reactions caused by doxastic states that are not a treatment’s core or characteristic features.1 As Scott Gelfand2 points out, there are numerous studies that have shown that the disclosure of a treatment’s side effects to a patient increases the risk of the side effects. From an ethical point of view, nocebo effects caused by the disclosures of side effects present a challenging problem. On the one hand, clinicians’ duty to inform patients (...) of the consequences (including possible side effects) of their treatments is critical in ensuring that patients’ autonomy is respected. Patients cannot act autonomously if relevant information is withheld from them (without their consent, perhaps). On the other hand, clinicians also ought to minimize harm to patients. (shrink)

We are dealing with the Dirac Hamiltonian H = H0 + V with no magnetic field and radially symmetric electrostatic potential V = V(r), preferably the Coulomb potential. While the observable H is precisely predictable, its components H0 (relativistic mass) and V (potential energy) are not. However they both possess precisely predictable approximations H0 ∼ and V∼ which approximate accurately if the particle is not near its nucleus. On the other hand, near 0, H0 and V are practically unpredictable, perhaps (...) in agreement with the fact, that a neutrino also should be in the game. [We have not yet studied the corresponding observables for the (≥ 12-dimensional) problem of electro-weak interaction.] Mathematically we are focusing on the spectral theory of the unbounded self-adjoint operators H0 ∼ and V ∼. We can prove that V ∼ is unitarily equivalent to V(r) again, by a unitary map given as Wiener-Hopf-type singular integral operator in the standard separation of variables for radially symmetric Dirac Hamiltonians. [This is, as far as the continuous spectrum is concerned.] Very similar unitary equivalence holds for H 0 ∼ and H 0. We are tempted to regard this as a form of “renormalization”. (shrink)

When scientists consider the interaction of science and value judgments, debates often occur. When public policy grows out of science, disagreements between scientists can become even more spirited. This paper examines the case of nutrition policy in the United States, which has been both at the interface between agriculture and medicine and the object of serious discord concerned with the strength and validity of the scientific evidence and the responsibility for action. The development of indirect intervention policies, designed to educate (...) and inform the public on diet and health, is traced as a practical demonstration of the effects of involvements of nutritional scientists of different disciplines and philosophic bents. Controversies centered mainly on the issues of diet and coronary heart disease and of diet and cancer in nutritional guidelines for Americans and the recommended dietary allowances. But the arguments turned on a complex web of values and interests as well as scientific questions. A remarkable turnaround by animal agriculture and its scientific support occurred, changing from a defensive, rightist stance to one that appears to recognize a moral responsibility to the public health. The convincing point was likely the changing market. Nutritional scientists, however, remained divided on the issue of whether a public health strategy keyed to public education should replace a strategy to identify persons of high risk and modify the risk by treatment. Our analysis suggests that the tension between libertarian and utilitarian social values of scientists is at least as important as disagreements relative to validity and strengths of the scientific evidence. This essay examines the growing concerns about disciplinary narrowing occurring in agricultural research and the prospects of ameliorating the detrimental effects of disciplinary compartmentalization while capitalizing on its positive effects. The general model for agricultural science is that disciplinary groupings set the logic and standards for research; the disciplinary sciences are set in a hierarchical arrangement which allows communication from the relevant basic sciences through applied research into technology development and use and problem-solving. But agricultural research throughout most of its (...) history has been goal-oriented and, therefore, is subject to ethical judgements of its worth and consequences. Also, strategic aspects of agricultural research have been subject to the evaluations and criticisms of both scientists and critics with differing interests at stake. Goals can change and organizations can be set to enable multidisciplinary and interdisciplinary research, but both goals and organizations come up against values associated with the disciplinary quality of the research, the social setting of academic science, the competition for resources, and the scientific reward system. However, there are changes underway in the agricultural scientific community which may recast the impacts of disciplinary structuring: (1) changes in the disciplinary components of subject areas and departments, (2) evolution and introductions of integrative and systems sciences into the system, (3) infusion of the same new powerful tools into most of the sciences, and (4) increased networking among scientists of different disciplines. Given that scientists' values and personalities intrude in agricultural science and research strategies, the future of agricultural research may rest on the scientists' intellectual vision and philosophical awareness that go beyond the expected disciplinary limits. The human subjects researchmodel is increasingly invoked in discussions ofethics for Internet research. Here we seek toquestion the widespread application of thismodel, critiquing it through the two themes ofspace and textual form. Drawing on ourexperience of a previous piece ofresearch, we highlightthe implications of re-considering thetextuality of the Internet in addition to thespatial metaphors that are more commonlydeployed to describe Internet activity. Weargue that the use of spatial metaphors indescriptions of the Internet has shaped theadoption of the human subjects research (...) model.Whilst this model is appropriate in some areasof Internet research such as emailcommunication, we feel that researchers, whennavigating the complex terrain of Internetresearch ethics, need also to consider theInternet as cultural production of texts. In 1954 F. R. Leavis wrote to the Times Literary Supplement taking issue with one of its reviewers. The reviewer had contrasted Leavis's approach to Shakespeare with that of Empson and Bradley. The latter, the reviewer had said, ‘like the plain man, or the audience in a theatre, cannot help considering the situation [in one of Shakespeare's plays] as “actual” and the characters as “real”’. Leavis, the reviewer had implied, treats the situation and characters somewhat differently.

My title has been taken from the following passage in Wittgenstein's Philosophical Investigations:Describe the aroma of coffee—why can't it be done? Do we lack the words? And for what are words lacking?—But how do we get the idea that such a description must after all be possible? Have you ever felt the lack of such a description? Have you tried to describe the aroma and not succeeded?.

Evidence for the association of ‘soluble’ enzymes in vivo is extensive and compelling. These associations occur in all compartments of the cell of both prokaryotes and eukaryotes. Several factors present in vivo promote these associations among enzymes whose association in vitro is often too weak to detect. Several physiological advantages of the associated enzyme complexes can be identified, most (but not all) of which are the consequence of microcompartmentation of metabolites (substrate channeling). Substrate channeling of intermediates by either a ‘direct (...) transfer’ process or ‘proximity effects’ can occur. The latter mechanism does not require the special molecular features needed for the direct transfer mechanism and may, therefore, exist in more general situations in the cell. Criticisms of these views are discussed. We argue that these criticisms have been largely answered by experiment and theory in recent years. Studies on simple systems in vitro, nevertheless, contribute important insights concerning the more complex phenomena in vivo. (shrink)
