Durch den Kontrast zu psychischen Erkrankungen zeigt sich die enge Verwobenheit zwischen Zeitlichkeit und gutem Leben. Im Gegensatz zur Depression und Schizophrenie ist die Posttraumatische Belastungsstörung bisher wenig unter einer zeitpsychopathologischen Perspektive untersucht worden. Tatsächlich kann diese aber mit ausgeprägten Störungen des Zeiterlebens einhergehen: Diese soll als unintegrierbare Zeitlichkeit beschrieben werden und lässt sich durch eine sich-aufdrängende, gegenwärtige Vergangenheit, eine unsichere Gegenwart und eine verkürzte Zukunftsperspektive charakterisieren.

Current moral philosophy has serious problems with the concept of human dignity. Although it seems to be an almost inevitable ingredient of every day moral judgments, philosophers have difficulties to find an analysis of the concept that could support this central role. One way out of these difficulties consists in a closer look at the various areas where the concept is used so widely and naturally, in the attempt to extract inductively an adequate understanding of human dignity from these contexts. In the article, this strategy is used to glean features of human dignity from the history of psychiatry, condense them into a plausible understanding of human dignity and finally sketch some practical implications for modern psychiatric ethics. Der Beitrag soll dreierlei leisten. Erstens gibt er einen Einstieg in das Thema Menschenwürde in der modernen Philosophie und beschreibt das Dilemma, in dem sich die Philosophie zurzeit bei der Beschäftigung mit der Menschenwürde befindet. Zweitens zeigt er auf der Basis eines kursorischen Durchgangs durch die Geschichte der Psychiatrie, wie die Psychiatrie zur Lösung dieses Dilemmas beitragen kann. Und drittens zieht er aus dem so gewonnenen Menschenwürdeverständnis Konsequenzen für die psychiatrische Ethik. (shrink)

« Cet article a pour but d’interroger, selon une perspective à la fois historique et épistémologique, le lien que le courant phénoménologique de la psychopathologie a tissé avec la recherche typologique en psychologie et psychiatrie à partir des années vingt. Tout en évoquant les problématiques spécifiquement philosophiques inhérentes aux démarches de typification, j’analyse ces démarches en me concentrant sur les enjeux et les desiderata caractérisant certains des projets les plus influents qui, dans le contexte de la psychopathologie germanophone, se (...) sont rapprochés de la phénoménologie afin de trouver un point de repère méthodologique. En particulier, j’insiste sur le modèle théorique élaboré par Ludwig Binswanger, puisqu’il a été parmi les premiers à poser de manière systématique la question de la rationalité intrinsèque de la connaissance clinique, et à affronter le problème de la relation entre général et singulier qui se présente lorsqu’un “cas” est décrit et analysé dans son “exemplarité”. ». L'importance que les défenseurs du Research Domain Criteria (RDoC) accordent aux circuits du cerveau dans la compréhension des désordres mentaux a conduit certains chercheurs à suspecter qu'on avait affaire à un programme de recherche foncièrement réductionniste. Puisque le RDoC devrait éventuellement affecter la psychiatrie clinique, on a craint qu'elle ne se transforme en une science neuro-comportementale appliquée sans tenir compte de l'esprit (mindless). Cet article montre en quoi le projet du RDoC s'éloigne du réductionnisme classique et comment il en (...) évite les écueils. Pour cette raison, du moins en principe, le RDoC pourrait enrichir plutôt qu'appauvrir la psychiatrie clinique. Pour conclure, les effets néfastes produits par le réductionnisme de façade des défenseurs du RDoC seront soulignés. (shrink)

ZusammenfassungBei gerichtlich angeordneten Unterbringungen aufgrund von Eigen- oder Fremdgefährdung können Patientenverfügungen zu klinisch und ethisch schwierigen Situationen führen. Dies gilt vor allem dann, wenn darin medizinisch indizierte psychiatrische Behandlungsmaßnahmen abgelehnt werden. In solchen Situationen können Patienten zunächst weder aus dem psychiatrischen Krankenhaus entlassen noch psychiatrisch behandelt werden. Der vorliegende Beitrag erörtert ethische Herausforderungen im Zusammenhang mit Therapieablehnungen durch Patientenverfügungen bei psychischen Erkrankungen aus interdisziplinärer Perspektive. Dabei werden die rechtlichen Grundlagen und Grenzen einer Patientenverfügung aufgezeigt. Mit Blick auf die klinische Praxis (...) werden die normativen Voraussetzungen einer wirksamen Vorausverfügung analysiert. Darüber hinaus werden ethische Argumente für und gegen die Anwendung von Zwangsmaßnahmen und -behandlungen in Situationen von Eigen- oder Fremdgefährdung diskutiert. Die Identifizierung normativ relevanter Aspekte soll dazu beitragen, in der klinisch-psychiatrischen Praxis zu ethisch begründeten Entscheidungen im Kontext von vorausverfügten Therapieablehnungen zu gelangen.In der Schlussbetrachtung wird aufgezeigt, welche Implikationen sich aus der rechtlichen Bindungskraft von Patientenverfügungen für die klinische Praxis ergeben. Auf einer übergeordneten Ebene wird das ethische Spannungsverhältnis, das sich durch den bestehenden gesellschaftlichen Sicherungsauftrag der Psychiatrie in Situationen von Eigen- oder Fremdgefährdung ergibt, problematisiert. Auf der Ebene des individuellen Arzt-Patienten-Verhältnisses stellen wir die Bedeutung von Beratungs- und Unterstützungsangeboten für die Abfassung von Vorausverfügungen dar und plädieren dafür, in Beratungsgesprächen auch die weitreichenden und komplexen Konsequenzen Therapieablehnungen zu erörtern. Résumé Cet article expose les tendances actuelles de ce nouveau domaine de recherche qu’est la « philosophie de la psychiatrie » à travers son évolution dans la dernière décade. La première partie se concentre sur le passage d’une approche strictement conceptuelle des thèses issues du savoir psychiatrique à une approche qui prend en compte également les dimensions sociales, pratiques et cliniques de cette discipline. La seconde partie de l’article montre comment l’exigence d’un engagement mutuel de la philosophie et de (...) la psychiatrie a été diversement perçue selon les pays engagés dans le programme de la philosophie de la psychiatrie. L’article s’intéresse en particulier à la France, où l’enthousiasme pour la « nouvelle philosophie de la psychiatrie » n’a pas eu le même retentissement sur la scène philosophique que dans les pays anglophones. Enfin cette contribution montre comment le domaine de la philosophie de la psychiatrie se présente comme un terrain fécond pour de nouvelles formes d’interaction entre la tradition analytique et la tradition continentale, en particulier à partir de la réflexion sur les thèmes de la normativité, du langage et de l’interprétation. Maxime F. Giguère Cet article propose une nouvelle stratégie pour écarter la conclusion sceptique, mise de l’avant par Thomas Szasz, selon laquelle la psychiatrie est illégitime. La conclusion sceptique repose sur une démarcation radicale entre troubles mentaux et somatiques. Afin de minimiser cette démarcation, Jerome Wakefield emploie une analyse conceptuelle stipulant que les troubles mentaux et somatiques sont tous les deux des dysfonctions préjudiciables. De récentes critiques ont toutefois montré que son analyse bute sur la difficulté pratique de distinguer (...) les fonctions normales des dysfonctions. En raison de ce problème, l’analyse de Wakefield ne permet pas d’écarter la conclusion sceptique. Une nouvelle stratégie ne nécessitant pas de résoudre le problème reconnu par ces critiques s’annonce alors préférable pour écarter la conclusion sceptique. (shrink)

Does the reference to a mental realm in using the notion of mental disorder lead to a dilemma that consists in either implying a Cartesian account of the mind-body relation or in the need to give up a notion of mental disorder in its own right? Many psychiatrists seem to believe that denying substance dualism requires a purely neurophysiological stance for explaining mental disorder. However, this conviction is based on a limited awareness of the philosophical debate on the mind-body problem. (...) This article discusses the reasonableness of the concept of mental disorder in relation to reductionist and eliminativist strategies in the philosophy of mind. It is concluded that we need a psychological level of explanation that cannot be reduced to neurophysiological findings in order to make sense of mental disorder. (shrink)

Psychiatrisches Handeln sieht sich in besonderem Maße dem Risiko dogmatischer Einengung ausgesetzt. Schon dies stellt eine Parallele zu Fichte dar, dessen Wissenschaftslehre es ja auch wesentlich um die Kritik an unbegründeten erkenntnistheoretischen Vorurteilen, also an dogmatischen Fassungen von Theorien, ging. Seine - und auch schon vor ihm Kants - Transzendentalphilosophie wandte sich gegen das idealistische Dogma, nur der Geist als absolut freies Wirken sei von Bedeutung, und gegen das realistische Dogma, die Welt bestehe aus »real« außer uns und unabhängig von (...) uns existierenden Dingen, die wir zwar erkennend abbilden können und sollen, die wir aber dadurch nicht beeinflussen und schon gar nicht wesentlich mit konstituieren. Wie ist es philosophisch zu erklären, dass Menschen frei und selbstbestimmt Entscheidungen treffen, die durch das klassische Begründungsmuster der Eigennützlichkeit und Selbsterhaltung unverständlich bleiben? Der Autor rekonstruiert und erweitert mit seiner neuartigen Konzeption – der autoeidetischen Struktur – Möglichkeitsbedingungen der praktischen Vernunft. Im Mittelpunkt steht dabei die eigene Existenz. Das Streben nach Selbsttreue bedingt das Wollen des Gesollten. Urteil und Praxis werden hierdurch nachvollziehbar zu Bedingungen gelingenden Lebens.

This is the second of three articles on the relationship between science, religion and professional practice in psychology and psychiatry. The first article pointed out that the weakness of the integration debate consists of lack of awareness of the epistemological distinction between everyday knowledge, professional knowledge, scientific knowledge and philosophical assumptions. This article shows how the integration debate gains new impetus by relating knowledge to practices and by investigating the intrinsic normative nature of professional practices. In the first part of (...) the article I will analyze the structure of professional practices (psychiatry; psychotherapy), in order to gain insight into the role of norms and values in these practices. In the second part I show how the notion of integration can be elaborated upon along the dimensions of structure (levels; functions) and direction (opening-up; disclosure). It appears that the I-self relationship plays a crucial role in the conceptualization of the relation between these two dimensions. (shrink)

This is the last of three articles on the relationship between science, religion, and professional practice in psychology and psychiatry. The first article highlighted the importance of the distinction between four types of knowledge. In the second article the scope was broadened and amounted to an analysis of the normative structure of professional practices. I showed the relevance of this analysis by investigating its meaning for the notion of restoration, along the dimensions of structure and direction. The I-self relationship played (...) an important role in the conceptualization of the relationship between these two dimensions. In this article I will apply the notion of (normative) practice to science andphilosophy. After a discussion of the views on integration of Nancey Murphy and Ian Barbour (both Christian philosophers) I will briefly introduce some insights of the Dutch (Christian) philosopher Herman Dooyeweerd, who emphasized the primacy of everyday knowledge and experience. In the latter part of the article I will discuss and reject constructivist objections to both the idea of the primacy of everyday knowledge and the normative practice model. (shrink)

This is the first of three articles on the relationship between science, religion and professional practice in psychology and psychiatry. This article describes the current debate. I begin with the observation that increasing professionalism of the health care professions seems to lead, almost inevitably, to secularism within these professions. I describe four ways to respond to this secularism. I give a summary of five major approaches to the issue of integration of psychology and religion, as they have emerged in the (...) North-American literature on the subject. I conclude that all these attempts fail at a conceptual level: neither of them provides a sufficient account of the conceptual differences between everyday knowledge, professional knowledge, scientific knowledge and philosophical insight. These differences are important, since they determine the way the relationship between psychology/psychiatry and religion should be conceptualized. (shrink)
