Obese people consume significantly greater amounts of health resources. This study set out to determine if health resource utilization by obese people decreases after losing weight in a comprehensive medically supervized weight management programme. Four hundred and fifty-six patients enrolled in a single-centred, multifaceted weight loss programme in a universal health care system were studied. Patient information was anonymously linked with administrative databases to measure health resource utilization for 1 year before and after the programme. Mean body mass index (BMI) (...) decreased by more than 15%. The mean annual physician visits (pre = 9.6, post = 9.4) did not change significantly after the programme. However, patients saw a significantly fewer number of different physicians per year following the programme (pre = 4.5, post = 3.9; P < 0.001). Mean annual number of emergency visits (pre = 0.2; post = 0.2) and hospital admissions (pre = 0.05; post = 0.08) did not change. Neither baseline BMI, nor its change during the programme, influenced changes in health resource utilization. Our study suggests that weight loss in a supervized weight management programme does not necessarily decrease short-term health resource utilization. Further study is required to determine if patients who maintain their weight loss experience a decrease in health utilization. (shrink)

According to a typical skeptical hypothesis, the evidence of your senses has been massively deceptive. Venerable skeptical hypotheses include the hypotheses that you have been deceived by a powerful evil demon, that you are now having an incredibly detailed dream, and that you are a brain in a vat. It is obviously reasonable for you now to be confident that neither of the above hypotheses is true. Epistemologists have proposed many stories to explain why that is reasonable. One theory (...) is that those hypotheses are inherently much less plausible than the hypothesis that your senses are basically reliable. Another theory is that you currently don’t believe any of those hypotheses, and that you need no justification to continue in that disbelief, given that your current beliefs cohere properly. A third theory is that —given that your senses are working properly—your sensory experiences themselves justify you in believing propositions such as the proposition that you have hands. Neo is given very good evidence that some skeptical hypothesis is true. He rightly becomes doubtful that his senses are or have been trustworthy. In fact, he becomes confident of a particular hypothesis: that AI’s created the Matrix, etc. But this is just one of many types of hypotheses that might account for his experiences. According to familiar accounts, Rousseau held that humans are actuated by two distinct kinds of self love: amour de soi, a benign concern for one's self-preservation and well-being; and amour-propre, a malign concern to stand above other people, delighting in their despite. I argue that although amour-propre can (and often does) assume this malign form, this is not intrinsic to its character. The first and best rank among men that amour-propre directs us to claim for ourselves is that of occupying (...) 'man's estate'. This does not require, indeed it precludes, subjection of others. Puesto que los actuales niveles y modos de consumo no son ambientalmente sostenibles, los consumidores necesitan concienciarse de las cuestiones medioambientales y de las implicaciones políticas de sus prácticas. Este artículo rastrea algunos elementos que afectan al denominado “consumo verde” en varias áreas: vestido, transporte, alimento y gestión doméstica. El consumo ecológico es un fenómeno complejo, con influencias en el ámbito societal de movimientos sociales, boicots o legislación gubernamental, y en el plano individual presenta influencias de la clase social, los (...) ingresos, la educación o el estilo de vida. La mayoría de los estudios que han examinado la relación entre actitudes específicas ante este tipo de consumo y el comportamiento real de los consumidores, revelan inconsistencias entre tales actitudes y las conductas. Esta discrepancia entre actitud y comportamiento puede explicarse por el hecho de que el consumo verde está fuertemente asociado a un específico estilo de vida, el de las clases medias, más que tratarse de un fenómeno de difusión generalizada. In 1962 a team of scientists conducted their first joint fieldwork in a Xavante village in Central Brazil. Recycling long-standing notions that living Indigenous people represented human prehistory, the scientists saw Indigenous people as useful subjects of study not only due to their closeness to nature, but also due to their sociocultural and political realities. The geneticists’ vision crystalized around one subject – the famous chief Apöwẽ. Through Apöwẽ, the geneticists fixated on what they perceived as the political prowess, impressive (...) physique, and masculine reproductive aptitude of Xavante men. These constructions of charismatic masculinity came at the expense of recognizing how profoundly colonial expansion into Mato Grosso had destabilized Xavante communities, stripping them of their land and introducing epidemic disease. The geneticists’ theorizing prefigured debates to come in sociobiology, and set up an enduring research programme that Apöwẽ continues to animate even four decades after his death. (shrink)
