BackgroundRespect for confidentiality is important to safeguard the well-being of patients and ensure the confidence of society in the doctor-patient relationship. The aim of our study is to examine real situations in which there has been a breach of confidentiality, by means of direct observation in clinical practice.MethodsBy means of direct observation, our study examines real situations in which there has been a breach of confidentiality in a tertiary hospital. To observe and collect data on these situations, we recruited students (...) enrolled in the Medical Degree Program at the University of Cordoba. The observers recorded their entries on standardized templates during clinical internships in different departments: Internal Medicine; Gynecology and Obstetrics; Pediatrics; Emergency Medicine; General and Digestive Surgery; Maxillofacial Surgery; Plastic Surgery; Orthopedics and Traumatology; Digestive; Dermatology; Rheumatology; Mental Health; Nephrology; Pneumology; Neurology; and Ophthalmology.ResultsFollowing 7138 days and 33157 h of observation, we found an estimated Frequency Index of one breach per 62.5 h. As regards the typology of the observed breaches, the most frequent were related to the consultation and/or disclosure of clinical and/or personal data to medical personnel not involved in the patient’s clinical care, as well as people external to the hospital. As regards their severity, severe breaches were the most frequent, accounting for 46.7 % of all incidents. Most of the reported incidents were observed in public areas, such as corridors, elevators, the cafeteria, stairs, and locker rooms.ConclusionsIn addition to aspects related to hospital organization or infrastructure, we have shown that all healthcare personnel are involved in confidentiality breaches, especially physicians. While most are committed unintentionally, a non-negligible number are severe, repeated breaches, thus suggesting a certain carelessness, perhaps through ignorance about certain behaviors that can jeopardize patient confidentiality. The physician’s duty of confidentiality is based on the observance of the patient’s privacy and intimacy and on the importance of respecting both of these rights, thus creating a relationship of confidence and collaboration between doctor and patient. The main objective of this work consists of analyzing the aspects that are related to the confidentiality of patients’ data with respect to the training, conduct and opinions of doctors from different Clinical Management Units of a third-level hospital via a questionnaire. The (...) present study aimed to define the problem and determine whether the opinions of these professionals correspond to those observed in a previous work conducted at the same center. Of the 200 questionnaires that were collected, 62.5% were from consultants and the rest were from residents with an average of 14.4 ± 12.5 years in professional practice. The respondents noted habitual situations in which confidentiality was breached in the reference hospital. The section on their attitudes and behaviors towards situations related to confidentiality showed a slightly lower average score than that of their medical knowledge; significant differences in these scores were observed between the consultants and residents as well as between the extreme age groups and years of professional practice, thus more inadequate attitudes were consistently noted in younger doctors who had fewer years of experience. Finally, the respondents answered that the training of doctors in the aspects of healthcare law and ethics was the most important measure that the hospital could adopt regarding confidentiality practices. The purpose of professional education programs is to prepare aspiring professionals for the challenges of practice within a particular profession. These programs typically seek to ensure the acquisition of necessary knowledge and skills, as well as providing opportunities for their application. While not denying the importance of knowledge and skills, this paper reconfigures professional education as a process of becoming. Learning to become a professional involves not only what we know and can do, but also who we are (becoming). It (...) involves integration of knowing, acting, and being in the form of professional ways of being that unfold over time. When a professional education program focuses on the acquisition and application of knowledge and skills, it falls short of facilitating their integration into professional ways of being. In addition, through such a focus on epistemology (or theory of knowing), ontology (or theory of being) is overlooked. This paper explores what it means to develop professional ways of being where the focus is becoming, not simply knowing as an end in itself. (shrink)

This study tests the hypothesis that university professors (lecturers) (in the U.S. and Spain) with different money profiles (based on Factors Success, Budget, Motivator, Equity, and Evil of the Love of Money Scale) will differ in work-related attitudes and satisfaction. Results suggested that Achieving Money Worshipers (with high scores on Factors Success, Motivator, Equity, and Budget) had high income, Work Ethic, and high satisfaction with pay level, pay administration, and internal equity comparison but low satisfaction with external equity comparison. Careless (...) Money Admirers (high Success but low Budget) had low intrinsic job satisfaction and low satisfaction with pay level and life. Apathetic Money Managers (low Evil and low Motivator) had the highest intrinsic job satisfaction and life satisfaction. Money Repellent Individuals (high Evil and low Success) had low income, work experience, Work Ethic, and low satisfaction with pay administration. Money does not provide the same motivation for people in all four money profiles. Results are discussed in light of the effectiveness of using money to reward people with different money profiles, intrinsic motivation, and unethical behavior. (shrink)

Physical forces regulate numerous biological processes during development, physiology, and pathology. Forces between the external environment and intracellular actin cytoskeleton are primarily transmitted through integrin‐containing focal adhesions and cadherin‐containing adherens junctions. Crosstalk between these complexes is well established and modulates the mechanical landscape of the cell. However, integrins and cadherins constitute large families of adhesion receptors and form multiple complexes by interacting with different ligands, adaptor proteins, and cytoskeletal filaments. Recent findings indicate that integrin‐containing hemidesmosomes oppose force transduction and traction (...) force generation by focal adhesions. The cytolinker plectin mediates this crosstalk by coupling intermediate filaments to the actin cytoskeleton. Similarly, cadherins in desmosomes might modulate force generation by adherens junctions. Moreover, mechanotransduction can be influenced by podosomes, clathrin lattices, and tetraspanin‐enriched microdomains. This review discusses mechanotransduction by multiple integrin‐ and cadherin‐based cell adhesion complexes, which together with the associated cytoskeleton form an integrated network that allows cells to sense, process, and respond to their physical environment. El Problema de la Adopción de Reglas de Inferencia constituye un desafío para todo aquel que proponga que podemos seguir nuevos patrones inferenciales a partir del reconocimiento de reglas. En esta sección temática se exploran diversos asuntos conectados a si podemos seguir un nuevo patrón inferencial en virtud de una regla.

En 1993 y 2012 la poeta irlandesa Mary O’Malley publicó Valparaiso y Where the Rocks Float que edificó alrededor de la figura del mar y de la zona costera irlandesa donde nació. En ambas colecciones, la figura del mar se transforma en un potente agente activo que denuncia el rol de la mujer en la sociedad irlandesa y la explotación de los espacios azules con fines de consumo. In higher education, the conventional design of educational programs emphasises imparting knowledge and skills, in line with traditional Western epistemology. This emphasis is particularly evident in the design and implementation of many undergraduate programs in which bodies of knowledge and skills are decontextualised from the practices to which they belong. In contrast, the notion of knowledge as foundational and absolute has been extensively challenged. A transformation and pluralisation has occurred: knowledge has come to be seen as situated and localized into (...) various 'knowledges', and the status of the body has taken on renewed significance in epistemological debates. Rather than thinking of knowledge as transcending the body, the embodiment of knowledge has become a key factor in understanding the nature of knowledge and what it means to know. In this paper, we adopt a phenomenological perspective in exploring the notion of embodied knowing as it relates to higher education programs and, more specifically, the ways in which information and communication technologies (ICTs) are used in these programs. Surveys were conducted at two metta meditation retreats in order to examine the psychological effects of metta meditation. Participants were invited to complete the survey at the beginning of the retreat, at the end of the retreat, and two weeks after the end of the retreat. Participants completed the same scales at each time phase, which included measures of happiness, compassionate love, revenge and avoidance motivation, gratitude, and a depression, anxiety and stress scale. Contra la opinión generalizada de que la "mímesis" deja de ser un paradigma del arte a partir del siglo XVIII, este artículo, inspirado en una idea general de Gadamer, intenta mostrar su vigencia apoyándose para ello en una revisión de los textos griegos fundadores de la teoría y en otras fuentes que dan fe de la evolución del concepto en la estética occidental. Muy lejos de la desfiguración moderna, que lo redujo a la simple copia de apariencias sensibles, el concepto (...) original de "mímesis" alude a un hacer paralelo con la naturaleza: se fundamenta, por un lado, en la noción pitagórica del orden universal presente en el movimiento de los cuerpos celestes, así como en el alma humana y en la belleza de las obras de arte, y, por otro, en la noción aristotélica de "reconocimiento" como un volver a conocer que deja de lado todo lo inesencial. En el imaginario del anime contemporáneo de ciencia ficción encontramos algunos ejemplos claros de una reinterpretación de la relación entre ser humano y tecnología en términos no-clásicos. La filosofía de la individuación de Gilbert Simondon, con su crítica radical al individualismo y al sustancialismo, y en especial su filosofía de la técnica, centrada en la denuncia del olvido cultural de los aspectos humanos de la técnica, ofrecen una clave filosófica idónea para la exploración de ese imaginario. Mediante la aplicación de (...) los conceptos desarrollados por Simondon en sus análisis del modo de existencia de los objetos técnico a los animes Akira, Evangelion, Ghost in The Shell y Serial Experiments Lain, muestro que existe en ellos una visión coherente e innovadora de la relación entre humanidad y técnica, que coincide en muchos aspectos con las críticas y propuestas elaboradas por el mismo Simondon. Los más importantes de estos aspectos se relacionan con la idea de la extensión tecnológica de lo humano y con los grados de integración o separación existentes entre el dominio de lo técnico y el de la espiritualidad y la religión. During the last decades the question of defining life has gained increased interest but, at the same time, the difficulty in reaching consensus on a possible answer has led many to skeptical positions. This, in turn, has raised a wider debate about why defining life is so hard and controversial. Such a debate introduces additional aspects to be considered, like the role and nature of a definition of life itself. In this paper, we will focus on those aspects, arguing that (...) progress can be made if we conceive definitions of life as open heuristic tools that contribute to develop specific research strategies in the biological sciences and, more generally, to increase our understanding of life’s complexity. In contrast with pragmatic or operationalist approaches, we will defend that definitions of life comprise a set of ontological assumptions, together with an inherent unifying vocation, so they should be subject to comparison and critical assessment, closely related to the success or failure of the corresponding research programs, but also to the success or failure in establishing well-grounded interconnections among the latter. We consider that the search for a more coherent, integrated and generalized theory of biology cannot be pursued without keeping an empirical standpoint, and the exercise of defining life should not be taken as an obstacle but as a valuable instrument to achieve that goal. This contribution aims to highlight the ambivalent condition of the concepts of transparency and acceleration, considered as key concepts for the understanding of the changes experienced by the conceptions of time from Modernity to Postmodernity. In order to enlight this double value, we will show the difference between a centrifugal acceleration, due to modernity, tied to modernity, which time structure is articulated as an asymptotical approach to the focal point toward it is oriented, and a centripetal acceleration, where the transparency (...) is so perfect that in absence of both bottoms of retraction and a center of gravity providing an order to the time sequence, it expounds all to the visibility, as the concept of freedom unfortunately remains linked to the selfcoercion. This article offers a philosophical-empirical account of embodied skilful performance in the practice of plant biotechnology. Drawing on the work of Merleau-Ponty and others, we elaborate how skilful performance emerges from and through reciprocal relations encompassing the body-in-the-world and the world-in-the-body. The contribution of this article lies in offering an account of skilful performance that is attentive to a perceiving, motile, feeling body entwined with world. In genetically modifying plants, scientists direct their senses of touch and vision to manipulating plant (...) materials and matter, ‘reading’ subtle changes in tissue cultures, plantlets and so on. In rhythmic movements, they extend their bodies through skilful use of complex equipment, while calling on ‘intuition’ to guide their work. Skilful performance, then, relies on our lived body that is not merely a physical object among other objects, nor a passive instrument for the mind, but dynamic and inescapably entwined with people and things in a world in flux. Bringing to the fore how perceiving, motile, feeling bodies are implicated in skilful performance has significant implications for education and workplace training. (shrink)

In this paper, I argue that the term kinêsis bears different connotations when associated with two different Plotinian hypostases in the Enneads: Intellect and Soul1. I propose an interpretation of this term as intellectual movement when it is associated with the Intellect and spatial movement when it is associated with the Soul.In the first section, I evaluate the meaning of kinêsis in reference to the hypostasis Intellect. In the second section, I turn to a critical examination of kinêsis associated with (...) the hypostasis Soul, emphasising the conceptual complexity of the term. I develop my interpretation on the distinction between kinêsis tês ousias and kinêsis pros ousias, translating the former as ‘movement of the hypostasis’ and the latter as ‘movement from the hypostasis.’ On this revised reading, I conclude that kinêsis in Plotinus is a context-dependent concept that alters its significance according to the hypostasis and introductory locution (‘tês’ or ‘pros’) with which it is associated. I conclude by linking Plotinian henology with Platonic and Aristotelian doctrines to illuminate an ongoing conversation between Plotinian metaphysics and Platonic and Aristotelian concepts of movement. There is a peak in peer victimization during middle childhood, with multiple negative consequences. Parental use of corporal punishment and child aggression are the most widely studied predictors of this phenomenon. The aim of the present study was to analyze whether parental use of corporal punishment affects peer victimization through child aggression. This mediation model was explored for both mothers and fathers and for both physical and relational forms of aggression and peer victimization. Furthermore, we also analyzed whether the mediation (...) models were moderated by the sex of the child. Participants were 234 third graders. Child aggression and victimization were measured by peers using the Mini Direct Indirect Aggression Inventory. Independent measures of mother’s and father’s use of corporal punishment were obtained from a PCA of items from the Parental Styles and Dimensions Questionnaire. Conditional process modeling was carried out using a macro for SPSS developed by Hayes. Results indicated that aggression mediated the relation of parental corporal punishment to peer victimization. Some interesting moderating effects of sex in this mediation model were found; specifically, physical, and relational aggression mediated the relation of maternal corporal punishment to peer victimization only in boys. Few studies to date have addressed the connection between aggressive behavior and peer victimization as outcomes of corporal punishment, taking into consideration the role of parent’s and child’s sex, and both physical and relational forms of aggression and victimization during childhood. En este artículo, abordo la disputa entre normativismo y descriptivismo centrándome en el problema del arbitraje. Para esto, propongo acercar a esta disputa la discusión sobre el problema del colapso que se ha dado recientemente en el marco de la filosofía de la lógica. Mi propuesta sostiene, por un lado, que el razonamiento humano, en cuanto práctica social, es una actividad esencialmente normada y, por otro lado, que los sistemas formales que se toman de referencia en la psicología del razonamiento (...) son el resultado de diversos procesos de equilibrio reflexivo. Esto permite defender el rol normativo de los sistemas formales como estándar para el razonamiento humano. Además, sostengo que la adecuación de un sistema formal está signada por los contextos específicos de razonamiento. Así, defiendo un pluralismo normativo respecto de los sistemas formales de inferencia en el marco de los estudios empíricos sobre el razonamiento humano. (shrink)

Las fake news se suelen caracterizar como contenidos, de modo que prevalece el planteamiento semántico. Aquí se propone un enfoque pragmático, donde son actos comunicativos intencionales. Esto permite entender las fake news desde los actos locucionarios, ilocucionarios y perlocucionarios. Esta triple versión de intencionalidad comunicativa permite una mejor intelección de la mala información (misinformation) y de la desinformación (disinformation), que se refleja con claridad en el caso económico.Para mitigar o neutralizar los daños causados por las fake news en el tejido (...) social, se suele recomendar la contrastación de hechos (fact checking). La contribución de la Economía Experimental —y, en concreto, la aportación de Alvin Roth (Premio Nobel de Economía 2012)— puede ser especialmente relevante en dos direcciones: (a) la actividad económica y (b) la Economía como actividad entrelazada con otras. Esto supone el contrastar hechos partiendo de la intencionalidad en la actividad económica, en cuanto tal y en su nexo con otras actividades humanas. A este respecto, las fake news convergen con la toma de decisiones económicas, en cuanto que ambas están surcadas por la intencionalidad y, además, tienen lugar en un contexto donde adquieren un perfil orientado a fines. Emotions are an integral part of human cognition and they guide not only our understanding of the world but also our actions within it. As such, whether we soothe or flame an emotion is not inconsequential. Recent work in conversational AI has focused on responding empathetically to users, validating and soothing their emotions without a real basis. This AI-aided emotional regulation can have negative consequences for users and society, tending towards a one-noted happiness defined as only the absence of "negative" (...) emotions.
