The thesis of this article is that engagement and suffering are essential aspects of responsible caregiving. The sense of medical responsibility engendered by engaged caregiving is referred to herein as clinical phronesis, i.e. practical wisdom in health care, or, simply, practical health care wisdom. The idea of clinical phronesis calls to mind a relational or communicative sense of medical responsibility which can best be understood as a kind of virtue ethics, yet one that is informed by the exigencies of moral (...) discourse and dialogue, as well as by the technical rigors of formal reasoning. The ideal of clinical phronesis is not (necessarily) contrary to the more common understandings of medical responsibility as either beneficence or patient autonomy — except, of course, when these notions are taken in their disengaged form (reflecting the malaise of modern medicine). Clinical phronesis, which gives rise to a deeper, broader, and richer, yet also to a more complex, sense than these other notions connote, holds the promise both of expanding, correcting, and perhaps completing what it currently means to be a fully responsible health care provider. In engaged caregiving, providers appropriately suffer with the patient, that is, they suffer the exigencies of the patient's affliction (though not his or her actual loss) by consenting to its inescapability. In disengaged caregiving — that ruse Katz has described as the silent world of doctor and patient — provides may deny or refuse any given connection with the patient, especially the inevitability of the patient's affliction and suffering (and, by parody of reasoning, the inevitability of their own. When, however, responsibility is construed qualitatively as an evaluative feature of medical rationality, rather than quantitatively as a form of calculative reasoning only, responsibility can be viewed more broadly as not only a matter of science and will, but of language and communication as well — in particular, as the task of responsibly narrating and interpreting the patient's story of illness. In summary, the question is not whether phronesis can save the life of medical ethics — only responsible humans can do that! Instead, the question should be whether phronesis, as an ethical requirement of health care delivery, can prevent the death of medical ethics. (shrink)

Neuropalliative care developed to address the needs of patients living with life‐limiting neurologic disease. One critical consideration is that disease‐related changes to cognition, communication, and function challenge illness experiences and care practices. We conducted an ethnography to understand neuropalliative care as a phenomenon; how it was experienced, provided, conceptualized. Personhood served as our conceptual framework; with its long philosophical history and important place in nursing theory, we examined the extent to which it captured neuropalliative experiences and concerns. Personhood contextualized complex (...) losses, aligning the impact of functional and relational changes. Cognition, communication, and functional alterations stretched conceptions of personhood, insinuating it can be relational, fluid, adaptive. Although normative conceptions of personhood guided research and decision‐making, ethical considerations suggested personhood could be transformed, remade. We consider the implications of our findings through three themes. First, we examine how literature on illness experience fails to integrate the realities of people living with and dying from neurologic disease; we counter this by interrogating the concept of experience. Second, we turn to Ricoeur's work on recognition to illuminate relational conceptions of personhood to inform care practices. Finally, we reflect on how personhood can bridge the gap left by functional changes, enhance relational engagement, and promote dignity at the end of life. (shrink)

Our goal is to prove that the golden section, however important, is not the only key to understand a mathematical-formalizing approach to the idea of beauty. Having developed, from this point of view, reading keys linked to the post-modern, it is necessary to link together the multiple rivulets of knowledge that gather in this direction. Moreover the canons of the approaches presented up to now are very indicative for the understanding of many aspects of beauty, which however depends on the (...) historical moment and the cultures created in the various civilizations. Therefore we can affirm that there is no effective definition of "beauty" that can be codified through fixed canons, but that the concept is expressed by a series of stratifications and interpretations that tend to link several major variations, expressing the various answers given by man to the question: what is the beauty? The present work introduces the neuropsychological paradigm as a new approach to studying ancient literature. In the first part of the article, an epistemological framework for the proper use of neuropsychology in relation to ancient literature is presented. The article then discusses neuropsychological methods of studying different human experiences and dimensions already addressed by ancient literatures. The experiences of human encounters with gods among ancient cultures are first considered, through the contributions of Julian Jaynes and Eric R. Dodds. The concepts (...) of right and left in the Bible, and that of soul are then discussed. Ecstatic experience in Paul of Tarsus is also presented, with a particular focus on glossolalia. Neuroscientific differences between mindful and unitive meditative practices are then described referring to ancient Buddhist literature, and finally a brief description of dreams in ancient Greek literature is proposed. Neuropsychology variously enables a more profound understanding of themes characterizing human experiences that ancient literature has already explored; these investigations prove that the collaboration of neuroscience and humanistic studies can return fruitful and interesting results. Background: The COVID-19 pandemic severely strained the already unprepared Italian healthcare system. This had repercussions on healthcare workers, stemming, in particular, from a lack of clear guidelines, adequate protective equipment, and professional preparedness. Such conditions were especially prevalent in Northern Italy.Objectives: This study aimed to examine COVID-19-related professional and psychoemotional stress among nurses and doctors in two hospitals in Northern Italy, along with the worst critical incidents affecting healthcare personnel. A parallel objective was to elicit healthcare professionals' opinions about what (...) changes are needed in the healthcare system's operations, as well as about the relational/emotional skills that are needed to better manage relationships with patients in emergency situations.Participants: Snowball sampling was used to recruit participants and yielded 17 hospital professionals: six nurses and 11 doctors. Three of these professionals worked in intensive care and the others in different wards. All had close contact with COVID-19 patients.Methods: The study employed a qualitative research design, using in-depth interviews of ~60 min each that were conducted via Skype video calls. The interviews were recorded and transcribed, then analysed. The qualitative analysis employed mixed methods to identify the most relevant and recursive themes from the interviews.Results: Four fundamental themes emerged from our analysis of the interview texts: disorganisation and psychoemotional stress; urgency and critical incidents; everything surreal; and disruptions in empathetic relationships with patients.Conclusions: Through our analysis of the interview narratives, we found that systematic and in-depth psychological training is needed to prepare professionals for altered relationships with patients in emergencies; use of exceptional medical equipment; elaboration of new bioethical models suitable for disasters and pandemics; and engagement with the themes of death and dying. Spinal muscular atrophy (SMA) type 1 is a genetic neuromuscular disease in children that leads to degeneration of spinal cord motor neurons. This sometimes results in severe muscular paralysis requiring mechanical ventilation to sustain the child’s life. The onset of SMA type 1, the most severe form of the disease, is during the first year of life. These children become severely paralysed, but retain their intellectual capacity. Ethical concerns arise when mechanical ventilation becomes necessary for survival. When professionals assess the (...) resulting life for the child and family, they sometimes fear it will result in unreasonably excessive care. The aim of this article is to present an analysis of ethical arguments that could support or oppose the provision of invasive ventilation in this population. This examination is particularly relevant as France is one of the few countries performing tracheotomies and mechanical ventilation for this condition. (shrink)

In this paper guidelines are addressed to internalize strategic thinking in managers based on thephilosophy of autonomous institutions in the public sector. Taking into account, it is essential that thestrategic thinking of leaders to deal with the organizational culture to be clear on who they are, what youthink, what their requirements as well as, meet the commitments and responsibilities. All this is based on themission, vision, values and strategy tend to be intuitive elements. Therefore, the study was located in adescriptive and information was obtained through an instrument Likert. The results show atthe managerial level, have not entirely a management philosophy, which constitutes a guide for the conduct ofsuch institutions effectively.Keywords. Recent descendants of David Lewis, such as Stephen Torre, Dilip Ninan, and Dirk Kindermann have utilized multi-centered propositions, which are roughly sets of possible worlds centered on a sequence of individuals, to characterize the content of attitudes. In an attempt to explain counterfactual attitudes such as wishing and imagining, Ninan (2012, 2013) developed a more fine-grained characterization of multi-centered propositions than others in the multi-centered camp. While Ninan provides a systematic explanation of the nature of de se attitudes (attitudes one (...) expresses with first-person pronouns like ‘I’ or ‘me’), the nature of other types of indexical attitudes (attitudes one expresses with indexical terms like ‘you,’ ‘today,’ ‘now,’) is not explored. This paper does two things. First, it demonstrates how we can account for indexical attitudes within Ninan’s framework. This, however, has the consequence of generating propositions of limited accessibility of sorts, which in turn conflict with a received and simple model of linguistic communication. According to this simple model, successful communication consists of the transmission of mental content from speaker to hearer. Second, in light of this conflict, the paper proposes an adjustment to Ninan’s theory in order to avoid commitment to propositions of limited accessibility, and thus preserve the shareability of attitudinal content from speaker to hearer. ABSTRACTImmigrant origin children and youth are now, and will continue to be, a diverse and demographically important segment of all post-industrial nations’ populations. In order to realize their potential, receiving contexts will need to find effective ways to integrate them into the fabric of their society. Using an ethic of care approach, we must begin by taking a comprehensive perspective on integration, which incorporates both a risk and resilience framework and an ecological perspective. A number of practices have emerged that (...) have promise for positive implications for the schooling performance and educational integration of immigrant students These practices include 1 – short-term newcomer programs; 2 – specialized services for Students with Interrupted Formal Education; 3 – sustained services focused on new language development; 4 – services focused on heritage language development; 5 – providing explicit college pathway knowledge; 6 – summer and after-school academic enrichment programs; 7 – facilitating parental involvement ; and 8 – advisory groups, after-school academic supports, mentoring and health services, and postsecondary supports. Providing the kinds of ecologies of care outlined here will go a long way toward helping these children and youth thrive and become the kinds of contributing members to their new society that will serve our collective future well.
