IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process. The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus with (...) clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information but significantly reduced for potential complications and biopsy description. Only 17.8% of the candidates mentioned the legal perspective of ICP. Death was omitted by the majority of the candidates ; after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior, but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

The current study analyzed how trait emotional intelligence (trait EI) mediates the relationship between self-esteem and state anxiety (STAI-S) and trait anxiety (STAI-T). The sample was composed of 153 undergraduate students from the University of Cádiz, Spain (71.9% women and 28.1% men). Students completed measures of self-esteem, STAI-S, STAI-T, and trait EI. Mediation analyses were completed with three trait EI dimensions (emotional attention -EA-, emotional clarity -EC-, and mood repair -MR) as mediating variables, self-esteem as the independent variable, and STAI-S (...) and STAI-T as the dependent ones. Our results confirmed that self-esteem scores explained and predicted both, STAI-S and STAI-T values (13% for STAI-S and 21% for STAI-T). This explanatory capacity is increased by 8% when accounting for all trait EI dimensions. Considering STAI-S, the results of the direct effects showed that a decrease in their levels is predicted through the increases in the levels of both, self-esteem and MR. Regarding STAI-T, the results of the direct effects showed that a decrease in their levels is predicted, in addition to an increment of self-esteem and MR values, by an increase of EC and a decrease of EA. Conversely, indirect effects revealed that higher levels of self-esteem were associated with worse scores in EA and worse MR, which in turn would enhance both STAI-S and STAI-T levels. Moreover, regarding STAI-T higher levels of self-esteem were associated with worse scores in EA and worse EC, therefore increasing STAI-T levels. As shown, the negative association found between self-esteem and EA becomes a key element. The effect of self-esteem on EA and the influence that the latter had on EC and MR exerts an indirect mediated effect with the power to invert the influence that self-esteem wields on both types of anxiety. In this sense, the apparent protective role of self-esteem changed, turning into a risk factor that promotes higher anxiety values. (shrink)

La trata de personas con fines de explotación sexual es un delito que se encuentra vinculado a las desigualdades de género y sociales, a la alta demanda de prostitución, a la inmigración irregular y al crimen organizado internacional. Es una vulneración de los derechos de las personas víctimas, especialmente la dignidad y la libertad. Aunque se han realizado importantes avances normativos y políticos tanto en el ámbito internacional como nacional, en los países donde las mujeres son explotadas, suelen ser detenidas, (...) tratadas como delincuentes y deportadas, sin que hubiesen sido identificadas, protegidas y atendidas, tal como lo establecen la normativas internacionales. En este sentido, el artículo presenta la experiencia de APRAMP como organización especializada en la lucha contra la trata, que ofrece un modelo de atención integral desde una perspectiva de derechos humanos y género para garantizar los derechos de las mujeres y menores de edad víctimas de este delito. This article analyzes the sculptural depiction of two nonhuman animals, Greyfriars Bobby in Edinburgh, Scotland and the Brown Dog in Battersea, South London, England. It explores the ways in which both these cultural depictions transgress the norm of nineteenth century dog sculpture. It also raises questions about the nature of these constructions and the way in which the memorials became incorporated within particular human political spaces. The article concludes by analyzing the modern "replacement" of the destroyed early twentieth century statue (...) of the Brown Dog and suggests that the original meaning of the statue has been significantly altered. Grounding refers to expressions that establish a connection between the ground and the content evoked by a nominal or finite clause. In this paper we report on two grammatical implementations of nominal grounding in Argentine Sign Language: pointing and placing. For pointing constructions, we also examine distal-proximal pointing and directive force. We introduce the concept of placing, in which a sign is produced at a specific meaningful location in space. Two types of placing are discussed: Placing-for-Creating, in which a new (...) meaningful location is created, and Placing-by-Recruiting, which recruits an existing meaningful location. We suggest that our analysis of pointing and placing provides an account of nominal grounding unified by general cognitive principles as described within the theory of Cognitive Grammar. Pointing is known to occur in all signed languages studied to date. Although previously undocumented, we suggest that placing is also common to many, perhaps all, signed languages. (shrink)

El duelo anticipado es un tema al que la filosofía ha prestado muy poca atención, a diferencia de lo que ocurre con el duelo que sigue a la muerte. Una peculiaridad obvia del primero es que sucede mientras todavía existe una relación interpersonal entre el doliente y aquel por quien se duele. En este artículo examino el impacto que el duelo anticipado puede tener en dicha relación, particularmente con respecto a las actitudes reactivas de sanción, propias del conflicto interpersonal.

A reforma trabalhista vem sendo normalmente apresentada como uma ruptura neoliberal com a tradição de protecionismo da Justiça do Trabalho e da legislação referente ao tema. Existe, todavia, um elemento de continuidade, que é a persistência na promessa da paz perpétua. A CLT apresenta a fraternidade entre as classes como imposição cristã. A Reforma Trabalhista insiste na superação dos conflitos como imposição da democracia, uma vez removida a intervenção estatal representada pela justiça do trabalho. The sequencing of the human genome has opened the way for using bioinformatics to identify sets of genes controlled by specific regulatory signals. Here, we review the unexpected diversity of DNA response elements mediating transcriptional regulation by estrogen receptors (ERs), which control the broad physiological effects of estrogens. Consensus palindromic estrogen response elements are found in only a few known estrogen target genes, whereas most responsive genes contain only low‐affinity half palindromes, which may also control regulation by other nuclear receptors. ERs can also regulate gene expression in the absence of direct interaction with DNA, via protein–protein interactions with other transcription factors or by modulating the activity of upstream signaling components, thereby significantly expanding the repertoire of estrogen‐responsive genes. These diverse mechanisms of action must be taken into account in screening for potential estrogen‐responsive sequences in the genome or in regulatory regions of target genes identified by expression profiling. In this article, the dominant legitimating discursive strategies around the ever-increasing labour demands and ever-declining standards of safety and well-being at work in Spain are analysed through the lens of Boltanski and Thévenot’s (2006) view of the transformation of capitalism. A qualitative methodology based on semi-structured interviews is used. Results show that the justificatory arguments most commonly put forward by managers and experts are the logic of supply and demand, and values related to flexibility, resilience and adaptability to change. In (...) contrast, the stability and protection established in the twentieth century are widely cherished values, particularly among union representatives and some human resources specialists. We found that discourse practices map two specific legitimating categories (market values and a projects orientation), and one critical category (Civic World), but the critical category has no practical impact, as the predominant feeling is resignation. Our research describes the common attitudes to legitimacy in the job market and points to a deep discontent in some of its key agents. (shrink)

Cancer is a chronic disease that causes the most deaths in the world, being a public health problem nowadays. Even though breast cancer affects the daily lives of patients, many women become resilient after the disease, decreasing the impact of the diagnosis. Based on a positive psychology approach, the concept of co-vitality arises understood as a set of socio-emotional competencies that enhance psychological adaptation. In this sense, emotional intelligence is one of the main protective factors associated with resilience. However, it (...) is not always as beneficial as it seems, and can lead to collateral effects on psychological adjustment. Given this controversy, this study aims to find the specific processes through which the dimensions of Perceived Emotional Intelligence can act as a risk or protective factor in the development of resilience. The total sample was 167 women, 46.7% were breast cancer survivors, and 53.3% were healthy controls. The selection of women with breast cancer carries out randomly, recruited through the Oncology Units. The sample completed measures of resilience and PEI, through Resilience Scale and TMMS-24. The results showed that breast cancer survivors showed higher age and greater levels of resilience and mood repair than healthy women. The mediation analysis revealed that breast cancer survival and PEI predicted 28% of the variance of resilience. The direct effects showed that emotional clarity and mood repair increased resilience levels. Although breast cancer did not predict resilience directly, it does through mood repair by an indirect process. Besides, the analysis showed that emotional attention played a role in vulnerability, decreasing mood repair, and resilience. These research support theories that point to a possible dark side of PEI, thus, a great level of emotional attention makes dark the positive effect of mood repair and personal growth if a clear perception of emotions does not complement it. These results provide empirical support concerning the need to work complementary each dimension of PEI to avoid unwanted effects on intrapersonal adjustment. (shrink)

BackgroundAn editorial expression of concern (EEoC) is issued by editors or publishers to draw attention to potential problems in a publication, without itself constituting a retraction or correction.MethodsWe searched PubMed, PubMed Central (PMC), and Google Scholar to identify EEoCs issued for publications in PubMed and PMC up to 22 August 2016. We also searched the archives of the Retraction Watch blog, some journal and publisher websites, and studies of EEoCs. In addition, we searched for retractions of EEoCs and affected articles (...) in PubMed up to 8 December 2016. We analyzed overall historical trends, as well as reported reasons and subsequent editorial actions related to EEoCs issued between August 2014 and August 2016.ResultsAfter screening 5076 records, we identified 230 EEoCs that affect 300 publications indexed in PubMed, the earliest issued in 1985. Half of the primary EEoCs were issued between 2014 and 2016 (52%). We found evidence of some EEoCs that had been removed by the publisher without leaving a record, and some were not submitted for PubMed or PMC indexing. A minority of publications affected by EEoCs had been retracted by early December 2016 (25%). For the subset of 92 EEoCs issued between August 2014 and August 2016, affecting 99 publications, the rate of retraction was similar (29%). The majority of EEoCs were issued because of concerns with validity of data, methods, or interpretation of the publication (68%), and 31% of cases remained open. Issues with images were raised in 40% of affected publications. Ongoing monitoring after the study identified another 17 EEoCs to year’s end in 2016, increasing the number of EEoCs to 247 and publications in PubMed known to be affected by EEoCs to 320 at the end of 2016.ConclusionsEEoCs have been rare publishing events in the biomedical literature, but their use has been increasing. Most have not led to retractions, and many remain unresolved. Lack of prominence and inconsistencies in management of EEoCs reduce the ability of these notices to alert the scientific community to potentially serious problems in publications. EEoCs will be made identifiable in PubMed in 2017. (shrink)

Drawing on contemporary coverage, particularly in The Field and Country Life, this article considers the construction of rabbits and squirrels as images of the past in England. By the 1930s, the red squirrel had become increasingly rare in the English countryside. Particularly in towns and suburbs, the population of the grey squirrel was growing rapidly. Those who saw themselves as the custodians of the countryside depicted the grey squirrel as a foreign force inimical to a mythical English way of life (...) as epitomized by the red squirrel. In the post war period, the debate resurfaced about the nature of the countryside and who had a right to defend it. The focus then was upon the spread of myxomatosis from France, which was depicted as a foreign disease. Wild rabbits, who died in the thousands from this infection, became appropriated, as red squirrels before them, as symbols of a lyrical and ordered past in the countryside. (shrink)

This paper deals with the methodological role played by the term «motivation» in young Heidegger’s early hermeneutic transformation of phenomenology. To that effect, I shall start analyzing the concept of motivation in Husserl’s phenomenology so as to better understand its hermeneutical variation in young Heidegger’s philosophy. Subsequently, I will pay special attention to the relevance exhibited by motivation in the emergence of the most important methodological notions of hermeneutical phenomenology as «destruction», «formal indication» and «preconception». To conclude, I shall explore (...) the possibility of reshaping the phenomenological problem of the motivation to reduction in hermeneutical terms. That is to say: a motivation to reduction in factical life experience is always needed to access to the primordial sphere of meaning. Accordingly, I will finally suggest that the philosophical basic experience of radical questioning can be read as a hermeneutical epoche, which is, however, directly linked to the concern for one’s own existence. O artigo tem por objetivo examinar a relação intrínseca entre o teatro e a filosofia a partir do estudo do verbo théorein, revelador de instigantes elementos constitutivos da arte dramática e da maestria especulativa. Cuida-se de um aprendizado do olhar que, no teatro e na filosofia, irá encontrar as condições propícias para o seu pleno exercício nas manifestações artísticas e filosóficas do século V a.C. em Atenas. A invenção do olhar representa um traço peculiar e profícuo da cultura grega, que (...) escreverá uma história artística e filosófica paradigmática para a civilização ocidental. Analisa a querela suscitada com a expulsão dos poetas e tragediógrafos da República de Platão. Destaca-se a importância da interdisciplinaridade como apreensão do humano e desenvolvimento de uma consciência crítica dos problemas artísticos e filosóficos. (shrink)

The idea of man in José Gaos could be explained taking in account two essential axes that make an inter-relationship between them and define man substantially: 1) antinomy love-hate-emotions and motions-expression and 2) reason-thought-language. The first is “the condition of possibility” of the second, being the element more suitable and radical that allows man to think concepts. The emotions and motions impose conditions over his will and determine the moral being that is. The man is a unity, but at the (...) same time, is an oscillation between opposites that make him take consciousness of his individuality, from the others, his historicity and intersubjectivity. The Gaos proposal is an important contribution to the anthropological reflection of the moment, because of the analysis categories that offers, of undeniable value, to the better understanding of suitable aspects of the being and acting of the Latino American man. In response to recent work on the nature of human aggression, and to shed light on the proximate, as opposed to ultimate, causes of tribal warfare, we present a record of events leading to a fatal Waorani raid on a family from another tribe, followed by a detailed first-person observation of the behavior of the raiders as they prepared themselves for war, and upon their return. We contrast this attack with other Waorani aggressions and speculate on evidence regarding their hormonal (...) underpinnings. On-the-ground ethnographic observations are examined in light of the neuroendocrinological literature. The evidence suggests a chain of causality in launching lethal violence, beginning with a perceived injury, culminating in a massacre, and terminating in rejoicing. Although no blood or saliva samples were taken at the time of this raid, the behaviors were consistent with a deliberate initiation of the hormonal cascade characterizing the “fight-or-flight” response, along with other hormonal changes. We conclude with observations on the stratified interrelationships of the cognitive, social, emotional, and neuroendocrinological causes of aggression leading to coalitional male homicide. (shrink)

Some mutual funds not only apply the usual asset management and custodial fees, but also front loads and redemption fees as a kind of ‘toll charge’ payable on entering and/or leaving the fund. The aim of this work is to examine the implications of the different loads and fees applied to mutual fund investors in the Spanish market. The results show that there is a relationship between the various charges and fees. The fact that load fund companies charge higher management (...) and custody fees proves the potential of the fund companies to impose higher fees on a segment of the clientele. The investors in load funds, which tend to be large in number of shareholders and belonging to banks and savings banks, are small investors who show a low cost sensitivity. A lower level of financial sophistication may be the reason for the apparent lower price awareness. The problem is that the investors in load funds are not financially compensated for the extra cost represented by the front‐load and redemption fees. The only beneficiary seems to be the financial institution itself. On this view, the survival of load funds seems to depend on the lack of financial sophistication of their clientele, combined with market inefficiencies. It is worth asking about the ethics of a situation of market segmentation that allows managing institutions to benefit from the segment of the least sophisticated investors. (shrink)

Cet article analyse quatre photographies qui révèlent différents modes de vie et de résistance combattante des femmes indigènes au Chiapas. On les voit ainsi s’organiser et transformer leur propre image. En même temps, elles construisent leur autonomie à la lumière du genre et à travers la conscience de leur statut social et de leur appartenance au peuple indigène de Mexico. « Personne ne peut nous prendre ce que nous sommes » (Commandante Esther, EZLN).
