Background: Freezing of gait is a common symptom in Parkinson’s disease and can be difficult to treat with dopaminergic medications or with deep brain stimulation. Novel stimulation paradigms have been proposed to address suboptimal responses to conventional DBS programming methods. Burst-cycling deep brain stimulation delivers current in various frequencies of bursts, while maintaining an intra-burst frequency identical to conventional DBS.Objective: To evaluate the safety and tolerability of BCDBS in PD patients with FOG.Methods: Ten PD subjects with STN or GPi DBS (...) and complaints of FOG were recruited for this single center, single blinded within-subject crossover study. For each subject, we compared 4, 10, and 15 Hz BCDBS to conventional DBS during the PD medication-OFF state.Results: There were no serious adverse events with BCDBS. It was feasible and straightforward to program BCDBS in the clinic setting. The benefit was comparable to conventional DBS in measures of FOG, functional mobility and in PD motor symptoms. BCDBS had lower battery consumption when compared to conventional DBS.Conclusions: BCDBS was feasible, safe and well tolerated and it has potential to be a viable future DBS programming strategy. IntroductionDeep brain stimulation is an effective treatment for advanced Parkinson’s disease with the targeting bilateral subthalamic nucleus or globus pallidus internus. So far, detailed studies on the efficacy of unilateral STN-DBS for motor symptoms have been reported, but few studies have been conducted on unilateral GPi-DBS.Materials and MethodsSeventeen patients with Parkinson’s disease who underwent unilateral GPi-DBS were selected. We conducted comparison analyses between scores obtained 6–42 months pre- and postoperatively using the following measurement tools: the Movement Disorder Society Unified Parkinson’s (...) Disease Rating Scale part III, the Hoehn and Yahr stage, the presence/absence of dyskinesia, Mini-Mental State Examination, Frontal Assessment Battery, Geriatric Depression Scale, levodopa equivalent dose, and cerebral blood flow by single photon emission computed tomography. Patient backgrounds were compared between four cohorts with favorable and unfavorable postoperative outcome.ResultsSignificant improvement was observed postoperatively in the following: total MDS-UPDRS Part III scores during the off period, contralateral scores, ipsilateral scores, and axial scores. Similarly, the Hoehn and Yahr stages during the off period, and GDS also showed significant decrease. In contrast, LED, MMSE, and FAB remained unchanged while the number of patients who scored positive for dyskinesia decreased by 40%. Abnormal cerebral blood flow preoperatively seen in the cerebral cortex had normalized in the total score-based good responder cohort. In the ipsilateral score-based good responder cohort, cerebral blood flow increased in the contralateral frontal lobe including in the premotor cortex, contralateral to the DBS. Compared to the poor responders, postoperative good responders demonstrated significantly higher preoperative MMSE scores.DiscussionUnilateral GPi-DBS therapy was effective in improving contralateral, ipsilateral, and axial motor symptoms of patients with advanced PD; in particular, it was found to be especially beneficial in PwPD whose cognitive function was unimpaired; the treatment efficacy rivaled that of bilateral counterparts up till at least 6 months postoperatively. Finally, normalization of preoperative abnormalities in cerebral blood flow and increased cerebral blood flow in the contralateral frontal lobe indicated the beneficial potential of this therapy on ipsilateral motor symptoms. When originally published, this historical and textual study was the first to examine in detail the line of continuity between Dogen and his Chinese predecessors, through his Chinese master, Ju-ching.

Approximately thirty years ago, Barbara H. Partee tried to think of counterexamples to David Lewis’s observation that no intransitive verbs appeared to have intensional subject positions. She came up with such verbs as ‘rise,’ ‘change,’ and ‘increase.’ Lewis agreed that they were indeed counterexamples to his observation. He mentioned it to Richard Montague, who incorporated these verbs into his now famous grammatical theory for English.

In this paper we define the notion of -linearity for and discuss interpretability (and noninterpretability) of -linear orders in structures and theories.

In any nonstandard model of Peano arithmetic, the standard part is not first-order definable. But we show that in some model the standard part is definable as the unique solution of a formula , where P is a unary predicate variable.

Biological forms are very complex, and mechanisms of pattern formation are not well understood. Although developmental biology deals with the mechanistic explanation of patterns, currently we do not know how to understand the mechanisms of pattern formation from huge amounts of molecular information. In this article, I present one useful tool, mathematical modeling, to obtain a mechanistic understanding of biological pattern formation, and show an actual example in lung branching morphogenesis. In this example, mathematical modeling plays an indispensable role in (...) understanding the biological phenomena. The model successfully reproduces the basic features of morphogenesis in vitro, but mechanism in vivo still remains to be elucidated. (shrink)

This paper centers on Takashi Yagisawa’s book Worlds and Individuals, Possible and Otherwise (Oxford: 2010), which provides a novel and systematic analysis of modality and time. I consider the overall structure of Yagisawa’s treatment of modality and time, and discuss in detail the following three topics: (i) Possible worlds as modal indices, (ii) Trans-world identity, (iii) The claim that existence, unlike reality, is relative. My main conclusion is that Yagisawa's view of modality is driven by a strong primitivism, leading (...) to the endorsement of possibility, actuality, and presentness, but also possible worlds and times, as irreducible. The resulting view is saddled with the typical problems of primitivism on the one hand, and modal realism on the other. I criticize the view that existence is relative. (shrink)

Was konnen Rechtsvergleich und Rechtsgeschichte leisten? Der Autor beantwortet diese Frage mit Blick auf die Gesetzgebungslehre des deutschen Juristen und Philosophen Christian Thomasius (1655-1728). A complete list of Finsler, Scott and Boffa sets whose transitive closures contain 1, 2 and 3 elements is given. An algorithm for deciding the identity of hereditarily finite Scott sets is presented. Anti-well-founded sets, i. e., non-well-founded sets whose all maximal ∈-paths are circular, are studied. For example they form transitive inner models of ZFC minus foundation and empty set, and they include uncountably many hereditarily finite awf sets. A complete list of Finsler and Boffa awf sets with 2 (...) and 3 elements in their transitive closure is given. Next the existence of infinite descending ∈-sequences in Aczel universes is shown. Finally a theorem of Ballard and Hrbáček concerning nonstandard Boffa universes of sets is considerably extended. (shrink)

Deep theorizing about possibility requires theorizing about possible objects. One popular approach regards the notion of a possible object as intertwined with the notion of a possible world. There are two widely discussed types of theory concerning the nature of possible worlds: actualist representationism and possibilist realism. They support two opposing views about possible objects. Examination of the ways in which they do so reveals difficulties on both sides. There is another popular approach, which has been influenced by the philosophy (...) of Alexius Meinong. The Meinongian approach is relevant to theorizing about possible objects because it attempts to construct a general theory of objects other than ordinary concrete existing objects. Independently of the debate about the nature of possible worlds or about Meinongianism, it is not always as straightforward as it may at first appear to determine whether putative possible objects are indeed possible. Another category of object similar to that of a possible object is the category of a fictional object. Although initially attractive, the idea that fictional objects are possible objects should not be accepted blindly. An important instance of theoretical usefulness of possible objects is their central role in the validation of two controversial theorems of a simple quantified modal logic. (shrink)

I argue that Aristotle’s arguments in passages regarding chance in the Physics and in passages about ignorance in action in the Nicomachean and Eudemian Ethics presuppose two different uses of ‘for the sake of something’, which are able to explain respectively the wish or thought of agents and the type or nature of what they actually do. In my view, however, this does not commit Aristotle, in the ‘ignorance’ passages from the two Ethics, to holding that the type or nature (...) of what the agents actually do is for the sake of killing or wounding. Mark Jago’s four arguments against Takashi Yagisawa’s extended modal realism are examined and shown to be ineffective. Yagisawa’s device of modal tense renders three of Jago’s arguments harmless, and the correct understanding of predications of modal properties of world stages blocks the fourth one.

In this paper, I show that Aristotle’s refutation of the Eleatic argument in Physics I.8 is based on the idea that a thing at the starting point of coming to be is composite and is made up of what underlies and a priva­tion. In doing so, I clarify how the concept of accidentality as used in his solution should be understood in relation to the composite nature of what comes to be. I also suggest an explanation of why Aristotle’s discus­sion (...) of the Eleatic dilemma in Physics I.8, unlike his discussion in the previous chapter, is not clear. (shrink)
